Use of a Balloon Guide Catheter during Endovascular Treatment of Acute Ischaemic Stroke due to Large Vessel Occlusion in the Anterior Circulation in Patients with an Atherosclerotic Tandem Lesion.

OBJECTIVE: Approximately 15 - 20% of patients with anterior circulation large vessel occlusion (LVO) acute ischaemic stroke (AIS) have a tandem carotid lesion, usually caused by atherosclerosis. These lesions are associated with worse outcomes compared with isolated LVO AIS. During endovascular thrombectomy (EVT), balloon guide catheters (BGCs) may reduce distal embolisation and subsequently improve outcomes, but evidence in this population is limited. This study aimed to evaluate the impact of BGC use on EVT outcomes in patients with anterior circulation LVO AIS and atherosclerotic tandem les

Progressive Unilateral Moyamoya-like Vasculopathy After Head Trauma with Chronic Subdural Hematoma: A Case Demonstrating the Utility of Anterior Circulation Basi-Parallel Anatomical Scanning.

Background: Moyamoya syndrome is a moyamoya-like cerebrovascular condition associated with an identifiable underlying condition. Although head trauma has historically been considered a possible contributing factor, it is currently excluded from the Japanese diagnostic criteria. We report a rare case of progressive unilateral moyamoya-like vasculopathy that developed on the ipsilateral chronic subdural hematoma (CSDH) following head trauma, with a decade-long imaging follow-up. Anterior circulation basi-parallel anatomical scanning (BPAS) provided unique insights into the progressive vessel nar

Comparison of Tubridge and Pipeline Embolisation Devices in intracranial aneurysms: a multicentre, propensity-matching analysis on the patency of branch vessels.

To compare the impact of two flow diverter devices (Pipeline Embolization Device [PED] and Tubridge Embolization Device [TED]) on branch vessels patency in intracranial aneurysm treatment, with specific focus on branch vessel outcomes and clinical safety. A retrospective analysis of 320 patients with 430 aneurysms and 602 covered branches treated with PED or TED was conducted. Statistical analyses included descriptive statistics and group comparisons. Potential predictors of aneurysm and branch occlusion were identified through univariate analysis followed by multivariate logistic regression.

Thrombectomy in basilar stroke via persistent trigeminal artery secondary to internal carotid artery occlusion.

Internal carotid artery (ICA) occlusion typically affects the anterior circulation. However, in rare cases, it may present exclusively as posterior circulation ischaemia, particularly in the presence of anatomical variants such as a persistent trigeminal artery (PTA), an abnormal embryonic connection between the ICA and the basilar artery (BA).We report the case of a patient with ICA and PTA occlusion who presented clinically with a posterior circulation stroke. The initial National Institutes of Health Stroke Scale score was 20. CT angiography revealed occlusion of the ICA and distal BA. Foll

Hemoglobin SC Sickle Cell Disease Presenting as Acute Ischemic Stroke With Moyamoya Syndrome in an Adult: A Case Report.

Moyamoya disease is a rare condition where blood vessels at the base of the brain become narrow or blocked. This leads to the growth of small, fragile vessels that look like a "puff of smoke" on imaging tests like CT or MRI scans. These vascular changes predominantly involve the terminal portion of the internal carotid arteries and the proximal segments of the anterior and middle cerebral arteries, leading to progressive stenosis and the development of collateral networks. Although the exact cause of moyamoya disease is unknown, it increases the risk of having a stroke. In this disease, there

Ischemic Stroke Secondary to Arterial Tunica Media Embolism Following Percutaneous Coronary Intervention: An Uncommon Etiology.

Ischemic stroke following percutaneous coronary intervention (PCI) is a rare complication, with an overall incidence of 0.56%. Most embolic strokes result from the dislodgement of atherosclerotic plaques, thrombi formed on catheter surfaces, procedural maneuvers, or, less commonly, air or metallic emboli originating from fractured guidewires. We present a unique case of stroke following PCI due to a previously unreported mechanism-arterial tunica media embolization associated with arterial access. A 57-year-old female presented with chest pain at rest and with exertion, accompanied by episodes

Rescue intracranial permanent stenting for refractory occlusion following thrombectomy: a propensity matched analysis.

BACKGROUND: Rescue intracranial stenting (RIS) can be used in refractory large vessel occlusion (LVO) after mechanical thrombectomy (MT). We aimed to assess the safety and efficacy of RIS versus a propensity matched sample of patients with persistent LVO. METHODS: We retrospectively analysed a multicenter retrospective pooled cohort of patients with anterior LVO (2015-2021) treated with MT, and identified patients with at least three passes and a modified Thrombolysis In Cerebral Infarction (mTICI) score of 0 to 2a. Propensity score matching was used to account for determinants of outcome in

Endovascular therapy in patients with internal carotid artery occlusion and patent circle of Willis.

BACKGROUND: Occlusion of the internal carotid artery (ICA) may extend into the middle or anterior cerebral artery (ICA-T) or be confined to the intracranial (ICA-I) or extracranial segment (ICA-E). While there is excellent evidence for endovascular therapy (EVT) in ICA-T occlusions, studies on EVT in non-tandem ICA-I or ICA-E occlusions are scarce. OBJECTIVE: To characterize EVT-treated patients with ICA-I- and ICA-E occlusion by comparing them with ICA-T occlusions. METHODS: The German Stroke Registry (GSR), a national, multicenter, prospective registry was searched for EVT-treated patients

Assessment of Collateral Flow in Patients with Carotid Stenosis Using Random Vessel-Encoded Arterial Spin-Labeling: Comparison with Digital Subtraction Angiography.

BACKGROUND AND PURPOSE: Collateral circulation plays an important role in steno-occlusive internal carotid artery disease (ICAD) to reduce the risk of stroke. We aimed to investigate the utility of planning-free random vessel-encoded arterial spin-labeling (rVE-ASL) in assessing collateral flows in patients with ICAD. MATERIALS AND METHODS: Forty patients with ICAD were prospectively recruited. The presence and extent of collateral flow were assessed and compared between rVE-ASL and DSA by using Contingency (C) and Cramer V (V) coefficients. The differences in flow territory alterations strat

Future of Endovascular and Surgical Treatments of Atherosclerotic Intracranial Stenosis.

Intracranial atherosclerotic disease and resultant intracranial stenosis is a global leading cause of stroke, and poses an ongoing treatment challenge. Among patients with intracranial stenosis, those with hemodynamic compromise are at high risk for recurrent stroke despite medical therapy and risk factor modification. Revascularization of the hypoperfused territory is the most plausible treatment strategy for these high-risk patients, yet surgical and endovascular therapies have not yet shown to be sufficiently safe and effective in randomized controlled trials. Advances in diagnostic and the

Clinico-radiological features of intracranial atherosclerosis-related large vessel occlusion prior to endovascular treatment.

The identification of large vessel occlusion with underlying intracranial atherosclerotic disease (ICAS-LVO) before endovascular treatment (EVT) continues to be a challenge. We aimed to analyze baseline clinical-radiological features associated with ICAS-LVO that could lead to a prompt identification. We performed a retrospective cross-sectional study of consecutive patients with stroke treated with EVT from January 2020 to April 2022. We included anterior LVO involving intracranial internal carotid artery and middle cerebral artery. We analyzed baseline clinical and radiological variables ass

Preliminary findings on diagnostic performance of computed tomography perfusion images for intracranial arterial stenosis: a retrospective study.

OBJECTIVES: Computed tomographic perfusion (CTP) can play an auxiliary role in the selection of patients with acute ischemic stroke for endovascular treatment. However, data on CTP in non-stroke patients with intracranial arterial stenosis are scarce. We aimed to investigate images in patients with asymptomatic intracranial arterial stenosis to determine the detection accuracy and interpretation time of large/medium-artery stenosis or occlusion when combining computed tomographic angiography (CTA) and CTP images. METHODS: We retrospectively reviewed 39 patients with asymptomatic intracranial

Predictors of In-Stent Stenosis Following the Implantation of Pipeline Embolization Devices for the Treatment of Aneurysms Located at or beyond the Circle of Willis in the Anterior Circulation.

BACKGROUND AND PURPOSE: In-stent stenosis is commonly observed after stent implantation. There is no consensus on the contributing factors for in-stent stenosis, especially for aneurysms located at or beyond the circle of Willis in the anterior circulation. This study aimed to investigate the morbidity and determinants of in-stent stenosis in distal anterior circulation aneurysms following the implantation of Pipeline Embolization Devices. MATERIALS AND METHODS: Patients who underwent Pipeline Embolization Device treatment at our center between January 1, 2018, and June 15, 2023, were enrolle

Cerebral hyperperfusion syndrome after endovascular reperfusion therapy for medium vessel occlusion: A case report.

Cerebral hyperperfusion syndrome is a rare but serious complication after revascularization procedures for cerebrovascular diseases. Cerebral hyperperfusion syndrome can develop after treatment of acute ischemic stroke, including intravenous thrombolysis and endovascular treatment of large vessel occlusion. However, to the best of our knowledge, there are no previous reports describing cerebral hyperperfusion syndrome after endovascular treatment of medium vessel occlusion (eg, anterior cerebral artery A2/3 segment). We report a case of cerebral hyperperfusion syndrome after endovascular reper

Trivial Neck Bite Resulting in a Stroke in an 18-Month-Old Child: A Case Report.

Stroke is often viewed as a diagnosis found In the elderly with or without comorbidities, but it is vital to not rule it out in a pediatric patient presenting with signs and symptoms of stroke. Here, we present a case of an 18-month-old boy who arrived at the emergency department with left arm weakness and left-sided seizures a few minutes after a right-sided trivial neck bite that was initially overlooked by the parents until symptoms occurred. Urgent imaging further with a computed tomography scan of the brain revealed a hypodense lesion in the area covering the lateral part of the frontal l
