[Blunt injury of the vertebral artery: report of three cases].

The authors reported three cases, whose vertebral arteries had been injured by blunt trauma to the neck which was followed by cerebello-brainstem infarctions. Case 1: a 32-year-old man, who developed severe vertigo and nausea 7 days after a traffic accident. He showed neck pain and horizontal nystagmus on admission. Three days later, he became drowsy. CT scan of the head demonstrated right-side cerebellar infarction, and the angiography revealed an occlusion of the right vertebral artery at C4-5 level. After the removal of the right cerebellar hemisphere, he recovered neurologically and was di

Analysis of the angiographic findings in cases of childhood moyamoya disease.

Cerebral angiograms of 34 patients with childhood moyamoya disease have been analysed, this analysis having revealed the following findings. The blood flow to abnormal net-like vessels (ANV) in the base of the brain is mainly supplied from the internal carotid artery (ICA) in the early stage of this disease. In a later stage, however, the blood supply is mainly from the posterior cerebral artery (PCA). Though, no remarkable change was found in the volumes of ANV from the early to later stages of the disease, the leptomeningeal collaterals, the most prominent anastomoses in moyamoya disease, ha

Resting and hypercapnic rCBF in patients with unilateral occlusive disease of the internal carotid artery.

Regional cerebral blood flow was measured by the 133Xe inhalation technique in 15 patients with severe unilateral internal carotid artery stenosis (75%) or occlusion, and in the absence of evidence of any sign of occlusive disease in other main afferent cerebral arteries. A comparison with normal subjects showed that lowered resting flow in both hemispheres was a common finding in all patients. Interhemispheric asymmetry was present only in patients with occlusion and the precentral, posterior temporal, and occipital regions were the most seriously affected. The CO2 reactivity was substantiall

Cerebrovascular disorders associated with von Recklinghausen's neurofibromatosis: a case report.

A 28-year-old woman with von Recklinghausen's neurofibromatosis (NF-1) had a huge hematoma in the left posterior nuchal region. Carotid and vertebral angiograms revealed marked stenosis at the C3 portion of the left internal carotid artery, slight moyamoya staining, occlusion of the left vertebral artery at the atlas level, and a right internal carotid artery aneurysm. The radiographic, clinical, and histological features of this case are discussed together with a review of 42 similar cases found in the literature.

[Cases similar to cerebrovascular moyamoya disease--investigation by angiography and computed tomography].

The criteria of the cerebrovascular Moyamoya disease is defined by the characteristic findings of its cerebral angiograms, as follows; 1) The internal carotid siphon is narrowed or obstructed bilaterally. 2) The "Moyamoya vessels" are observed at the base of the brain or the basal ganglionic regions. 3) Main trunks of the cerebral arteries such as the anterior, the middle, and/or the posterior cerebral arteries are often not or poorly visualized. 4) Its etiology is unknown. It has been known that the occlusion of the internal carotid fork with Moyamoya vessels is not infrequently seen in patie

Reduced hypercapnic vasoreactivity in moyamoya disease.

We measured cerebral perfusion at rest and in response to CO2 in eight patients with moyamoya disease (MMD), using the 133xenon inhalation method to determine the effect of large-vessel occlusive disease on vasoreactivity. We studied three other groups for comparison, including four with bilateral internal carotid artery occlusions (BICAO), 11 with unilateral carotid occlusion (UICAO), and six with unilateral middle cerebral artery stem occlusion (UMCAO). Resting flows appeared to correlate with the severity of occlusive disease overall. Normocapnic perfusion was lowest in the group with BICAO

Three-dimensional transcranial Doppler blood flow mapping in patients with cerebrovascular disorders.

We investigated 60 patients with cerebrovascular disorders using a three-dimensional transcranial Doppler blood flow mapping system. A composite display of the circle of Willis is created with computer assistance, allowing accurate vessel identification and optimal data documentation of blood flow velocity and direction in the basal cerebral arteries. The basilar artery was insonated in every patient; the middle cerebral artery and the most distal internal carotid artery were found in 95% of the patients, the anterior cerebral artery in 85%, and the posterior cerebral artery in 84%. Insonation

Pulsed Doppler assessment of innominate artery obstructive diseases.

In 14 patients with severe stenosis or occlusion of the innominate artery a new high-energy, low-frequency (2 MHz) pulsed Doppler ultrasound method was used to investigate blood flow velocity patterns of both intrathoracic and intracranial cerebral arteries. Direct acquisition and evaluation of the innominate artery at its origin near the aortic arch enabled separation of it from adjacent arteries and reliable differentiation of stenosis from occlusion. Transcranial recordings from the basal cerebral arteries showed abnormal Doppler signals in 12 patients (86%). Among these abnormalities, obse

Posterior cerebral artery stenosis.

We analyzed the clinical features of symptomatic posterior cerebral artery (PCA) stenosis in 6 patients selected from 15 patients with angiographically documented PCA atherostenosis occurring during a 7-year period. Transient ischemic attacks (TIAs) were the major presentation in 5 patients. A homonymous visual field defect was present in 2 patients. TIA symptoms were predominantly visual or sensory, or both. The most common visual symptom was difficulty seeing to one side. One patient saw flashing lights. Sensory spells were always paresthetic, usually involving the arm and hand and occasiona

Vertebrobasilar insufficiency. Part 2. Microsurgical treatment of intracranial vertebrobasilar disease.

Posterior circulation transient ischemic attacks have an associated risk of subsequent infarction of approximately 5% per year. Intracranial vertebrobasilar thrombo-occlusive lesions appear particularly likely to result in repetitive ischemic symptoms and in infarction due to hemodynamic insufficiency. The authors present their experience with 45 patients with symptomatic intracranial vertebrobasilar vascular disease despite maximal medical therapy. The specific operative approaches for intracranial vertebral artery endarterectomy and extracranial to intracranial posterior circulation revascul

A case of TIAs due to proximal common carotid artery stenosis which was successfully treated with autogenous saphenous vein graft between the subclavian artery and the external carotid artery is presented. A 57-year-old, right handed female was admitted to our hospital for the treatment of left common carotid artery stenosis which was pointed out at a local hospital. She had a 7-years' history of repeated transient right hemiparesis and/or left amaurosis fugax. No neurological deficit was revealed on admission. Angiography showed an 80% irregular stenosis of the left common carotid artery at i

Basilar artery stenosis: middle and distal segments.

We report the clinical features and prognosis in nine patients with angiographically documented basilar artery stenosis of the middle and distal segments. Six patients had transient ischemic attacks (TIAs), and in two this was their only clinical manifestation. The TIAs in four patients included two or more of the following symptoms: dizziness, diplopia, perioral numbness, dysphagia, weakness, or loss of consciousness. Two other patients had isolated symptoms of transient dizziness and unilateral weakness. Seven patients had posterior circulation strokes, preceded by TIAs in four. Basilar arte

The arterial patterns associated with internal carotid disease and cerebral infarcts.

In 20 necropsies with 15 stenosed and 17 thrombotic occluded internal carotid arteries there were 46 cerebral infarcts larger than 1 cm diameter. Using portmortem arteriographic and pathological techniques the patterns of the neck and brain artery systems were correlated with the situation and extent of the brain infarcts. Massive infarcts involving two major cerebral artery territories were associated with distal internal carotid artery occlusion and grossly ineffective cervical and circle of Willis anastomoses. Isolated middle cerebral artery territory infarcts were associated with internal

This case was 36-year-old female with the past history of pulmonary tuberculosis on her 10 years old. During the treatment for pulmonary tuberculosis, transient tetraparesis appeared, and improved completely. She was admitted to our hospital, complaining sensory disturbance of left face and arm on July 25, 1983. Plain skull roentogenogram showed calcification at the base of brain. Computerized tomography (CT scan) showed dilatation of lateral and third ventricles. Cerebral angiograms revealed an occlusion of the right internal carotid artery and the left middle cerebral artery, the stenosis of

Quantitative analysis of intracranial circulation using rapid-sequence DSA.

By using a high-frame-rate technique, intravenous digital subtraction angiography (IVDSA) of the intracranial circulation was performed in patients with transient ischemic attacks and asymptomatic bruits. Twenty patients with normal carotid arteries or mild stenosis of no hemodynamic significance were selected as a control group to evaluate the effect of carotid stenosis on the difference between hemispheres in the peak arrival time (delta TMAX) of the contrast bolus. Data were obtained for the anterior (ACA), middle (MCA), and posterior (PCA) cerebral artery distributions. Raw data of the tim

Balloon embolization of a large distal basilar artery aneurysm. Case report.

Interventional neurovascular techniques have advanced to a level where treatment of intracranial aneurysms by intravascular detachable balloon embolization therapy is now possible. A patient is presented who had a spontaneous subarachnoid hemorrhage from a large aneurysm of the distal basilar artery. The aneurysm arose at the bifurcation of the posterior cerebral arteries and measured 15 X 9 X 9 mm. With the patient fully awake, a detachable silicone balloon was passed into the basilar artery by a transfemoral arterial approach. Stenosis (greater than 60%) of the mid-section of the basilar art

Influence of anatomic origin on intracranial distribution of micro-emboli in the baboon.

The purpose of this study was to investigate whether the anatomic origin of micro-emboli influences their intracranial distribution. In twenty-two baboons, we examined the distribution of 99-Technetium labelled albumin aggregates (5 to 40 microns in size) after injection into the circulation at the left atrium (LA), carotid trifurcation (CA), and anterior and posterior common carotid artery (CCI). In a further subgroup, the emboli were introduced at the carotid trifurcation with the contralateral carotid artery ligated (CA + L). The results of this study demonstrated that aggregates introduced

What determines the symptoms associated with subclavian artery occlusive disease?

Symptoms associated with subclavian artery stenosis are related to reduced cerebral or arm blood flow. A large difference in blood pressure between the two arms is associated with symptoms of arm ischemia alone and is usually caused by an anatomic variant. The presence or absence of a radiologic steal alone did not seem to determine the type or presence of symptoms. The type of cerebral symptoms seems to be determined by the location of other extracranial vascular stenosis. In patients with hemispheric symptoms there was a higher incidence of anterior circulation insufficiency and a greater re

Vertebrobasilar transient ischemic attacks in internal carotid artery occlusion or tight stenosis.

We studied 12 patients with internal carotid artery (ICA) occlusion or tight stenosis but without vertebrobasilar and subclavian atherosclerosis who suffered vertebrobasilar insufficiency (VBI). The patients with ICA occlusion were compared with a sex- and age-matched control group that had ICA occlusion but no VBI. Visible infarct on computed tomographic scan, greater size of visible infarct, weak collateral circulation, and bilateral atherosclerosis of the ICA significantly correlated with the occurrence of VBI. No significant difference was demonstrated for emboligenic lesions, but posterio
