[Arteriosclerotic changes in the carotid arteries in patients with hypersensitive carotid sinus reflex].

Thirty-seven patients (15 female, 22 male; aged 46-89 years, mean age 68.8 years) with III or IV grade carotid sinus reflex carotid arteries were investigated by sonography (B-scan and CW-Doppler). It was demonstrated that stenoses or arteriosclerotic plaques, localized in the region of the carotid bulb or bifurcation, were present on both sides in 27 patients and on one side only in four patients. Five patients did not show these changes and four of them had thickening of common carotid artery endothelium. Thus, vascular changes occurred in 35 patients. Occlusions and stenoses greater than 50

[Transcranial sonographic monitoring of the blood flow of the middle cerebral artery in recanalizing operations of the extracranial internal carotid artery].

During endarterectomy of the internal carotid artery (ICA) blood flow velocity of the ipsilateral medial cerebral artery (MCA) was continuously monitored in twelve patients with the help of a new transcranial pulsed Doppler system. Additionally, the basal cerebral arteries where examined pre- und postoperatively in order to evaluate criteria for selective intraoperative shunting and to document flow improvement following ICA reconstruction. All patients had symptoms, either of TIA or of minor stroke. Five of them had in addition a contralateral ICA occlusion. During intraoperative carotid cros

Resolving occlusive lesions of the basilar artery.

Progression of severe basilar artery stenosis to occlusion is a well-recognized phenomenon, but regression of a severe basilar artery atherothrombotic lesion has not been described previously. In this report, two patients with a symptomatic severe basilar artery occlusive lesion showed substantial angiographic improvement after 3 months of anticoagulation in one case and 4 months after superficial temporal artery to superior cerebellar artery bypass in the other case. The findings in these cases indicate that clinical improvement in similar cases may reflect recanalization of a basilar artery

Intravenous digital subtraction angiography of arteriosclerotic vertebrobasilar disease.

Intravenous digital subtraction angiography (DSA) was performed in 111 patients with vertebrobasilar ischemia. Ninety percent of the vertebral images were of diagnostic quality; 23% of the basilar images were good quality and 53% fair quality; and 58% of the posterior cerebral images were poor. Compared with selective film arteriography in 23 patients, DSA tended to underestimate the degree of atheromatous disease. Segments of the basilar artery were often poorly seen, which could result in false-negative errors. DSA can provide a general assessment of atheromatous disease of the brachiocephal

Cerebrovascular Moyamoya disease.

Clinical features of ten Japanese children with cerebrovascular Moyamoya disease are reported and non-Japanese paediatric cases of the disease are reviewed from the world literature. The most common initial manifestations were headache in four cases (40%), motor deficit and convulsion in three cases (30%). As their recurrent and/or residual symptoms, eight children (80%) developed motor deficit consisting of hemiplegia in five cases and paresis or weakness of the extremities in three cases, and four (40%) had headaches. The mode of presentation in our cases was similar to that of non-Japanese

[Multiple cerebrovascular occlusive disease associated with neurofibromatosis].

Multiple cerebrovascular occlusive disease is rarely seen in patients with neurofibromatosis. Two cases of such lesions are presented and literatures dealing with the clinical and angiographical aspects of this occlusive disease are reviewed. Case 1; A 38-year-old normotensive man had sudden onset of vomiting, left hemiparesis and disturbance of consciousness, one day before the admission. He had family history of neurofibromatosis, and examination showed café au lait spots over the body. CT scans revealed a subcortical hematoma in the right temporal lobe. Angiogram revealed multiple occlusive

Surgical reconstruction of the proximal vertebral artery.

The authors have reviewed their experience in the management of 55 patients admitted to Henry Ford Hospital with symptoms of vertebrobasilar insufficiency and associated proximal vertebral artery stenosis or occlusion. In 48 patients, the symptoms occurred as multiple repeated events, five of which resulted in permanent deficits. The remaining seven patients had single events, four of which caused permanent deficit. These patients had been treated unsuccessfully with antiplatelet agents (37 cases) and with anticoagulant drugs (15 cases) before surgery. Most patients had multiple angiographic a

Complete bilateral internal carotid artery occlusion in a young man.

Partial or complete occlusion of the internal carotid artery is a familiar consequence of severe atherosclerosis seen in the elderly. Complete obstruction of both internal carotids is rare, particularly in the young or middle-aged. The rapid onset of bilateral internal carotid occlusion would be expected to produce devastating neurological sequelae and probably not be compatible with survival. We present a case of a young man with complete obstruction of both internal carotid arteries whose presenting symptoms were those of a visual field cut. The history suggests that the carotid occlusion oc

Can a patent extracranial-intracranial bypass provoke the conversion of an intracranial arterial stenosis to a symptomatic occlusion?

Although extracranial-intracranial (EC-IC) artery anastomosis seems to result in symptomatic improvement in certain types of cerebrovascular ischemic disease, this procedure can also be associated with significant morbidity, some of which paradoxically may be the direct result of a patent bypass. A review of the last 51/2 years at the Oregon Health Sciences University shows that 50 patients underwent 51 superficial temporal artery to middle cerebral artery bypass procedures with an angiographic patency rate of 91%. Of the 50 patients, 17 had intracranial stenotic lesions of either the middle c

Axilloaxillary bypass for symptomatic stenosis of the subclavian artery.

Since the association has been made between stenosis of the subclavian artery and neurologic symptoms, controversy has existed over the preferred surgical procedure for bypass. In addition, concern has been raised regarding the long-term patency and effectiveness of this extraanatomic procedure in relieving neurologic symptoms. Twenty-seven patients underwent this operation for posterior cerebral symptoms between 1973 and 1982; 25 were followed for up to 77 months (mean 26 months). Twenty-two patients had complete relief of symptoms, although 3 of them required a subsequent carotid endarterect

Short vein grafts for cerebral revascularization.

Experience with the use of a short saphenous vein bypass graft for cerebral revascularization is reviewed. Twenty patients with symptomatic atherosclerotic occlusive disease underwent a total of 21 bypass procedures. Cerebral revascularization was performed using a short (5 to 10 cm) saphenous vein graft (SVG) extending from the superficial temporal artery (STA) trunk anterior to the ear in 19 bypasses, or from the occipital artery (OA) immediately behind the mastoid process to the posterior temporal or angular branch of the middle cerebral artery (MCA) in two bypasses. The early patency rate

Carotid Doppler evaluation in cerebrovascular disease.

The Doppler technique has proven to be a useful noninvasive technique for evaluating the patency of the carotid artery in patients at risk of stroke. The data obtained from 246 carotid Doppler examinations were compared with the angiographic findings in the same patients. The sensitivity, specificity and accuracy were high when the degree of stenosis was greater than 50%, but occlusions were less reliably detected, with 8 (33%) of the 24 being misdiagnosed as high-grade stenoses. Carotid Doppler evaluation guides and accelerates decisions regarding further investigations, such as cerebral angi

Regional cerebral blood flow in patients with vertebrobasilar disease.

We have investigated the changes in regional cerebral blood flow (rCBF) in patients with occlusive vertebrobasilar disease. Fifty patients who showed severe stenosis or occlusion of the vertebrobasilar artery territory were studied by angiography. Lesions in 13 of these patients were limited to the vertebrobasilar artery (limited VB group), and 37 patients showed combined vertebrobasilar/carotid artery lesions (combined VB-C group). Measurements of rCBF in the group of 50 patients using the intravenous xenon 133 technique showed that there was a significant decrease in the mean rCBF (47.5 +/-

[Occlusion of the basilar artery. A review with 17 personal cases (author's transl)].

Occlusion of the basilar artery is mainly of atherosclerotic origin; embolic occlusion, dissecting aneurysm, trauma and arteritis are less frequent. Pathologic and angiographic findings allow to classify basilar artery occlusions in three types: segmental (superior, medial or inferior), plurisegmental and extensive. The infarcted areas involve brain stem, especially pons and cerebellum, also diencephalon and cerebral hemispheres, in various combinations. Clinically, there is typically a prodromic stage, with transient ischemic attacks (vertigo, headaches, visual disturbances, motor deficit). F

Cerebral infarction secondary to sickle cell disease: arteriographic findings.

Cerebral angiograms were performed in 14 children with sickle cell disease and clinical findings of cerebral infarction. The angiogram was normal in four patients. Ten patients had abnormalities in the carotid artery or its branches. The internal carotid artery was stenosed or occluded usually with bilateral disease. The anterior and middle cerebral arteries were involved in nine patients and branch occlusion was frequent. The posterior fossa arteries were not involved in these patients. The risk of angiography is low if appropriate precautions are taken. In these patients the indications for

[Role of vertebral artery hypoplasia in the pathogenesis of cerebral circulatory disorders].

Clinico-angiographic comparison of 30 cases of vertebral artery hypoplasia was carried out. It was found that the artery hypoplasia was often combined with a lateral shift of the artery opening, convolute shape of the major brain vessels, and functional reconstruction of the Villisian circle, a transitional type of the posterior trifurcation. It was ascertained that the vertebral artery hypoplasia, not being a pathogenetic factor itself, can play a negative role in cases of occlusion of a major brain vessel, since it limits the potentialities of compensatory blood circulation.

Brainstem lesion with coma for five years following manipulation of the cervical spine.

A woman, aged 38, lost consciousness immediately after manipulation of the cervical spine and remained in coma for nearly 5 years in a combined decorticate and decerebrate state. Cerebral angiography revealed impaired circulation in the vertebrobasilar system. The EEG initially showed generalized cerebral dysrhythmia but tended, over the years, to become more normal with desynchronized fast activity of low voltage. Neuropathological examination postmortem revealed a large cystic lesion in the pressure equalization area of the carotid and basilar circulation in the upper pons, mesencephalon, po

External carotid-vertebral artery anastomosis for vertebrobasilar insufficiency.

Two patients with previous brain stem infarction and current symptoms of vascular insufficiency in the basilar circulation had evidence of vertebral artery occlusive disease. Angiography demonstrated ostial stenosis of the dominant right vertebral artery and retrograde flow down the left vertebral artery to the level of the transverse process of C2 in the first case and to the level of a severely stenosed origin in the second case. Carotid circulation was patent in both cases. The blood flow in the posterior circulation was patent in both cases. The blood flow in the posterior circulation was

[Bulbar infarcts. Vascular lesions in 26 observations].

The pattern of arterial occlusion and related lesions have been studied in 26 cases with 38 medullary infarcts. In 10 of these cases, only the intracranial vessels were examined. In the 16 other cases, the entire cerebral arterial supply was dissected. Medial infarcts were more often related to occlusion of the vertebral artery involving the termination of the artery (7/10 cases). In 6 cases a thrombosis on an atheromatous plaque and, once, an embolus of cardiac origin, were found. Lateral infarcts were usually due either to occlusion of the vertebral artery (14/23 cases), reaching, in 10 case
