OBJECTIVE: Endovascular revascularization is increasingly used to treat symptomatic acute intracranial vertebral artery (ICVA) occlusion, with reported clinical benefit. In contrast, chronic total occlusion (CTO) is managed primarily with conservative therapy, and the indications for and efficacy of endovascular revascularization remain uncertain.
CASE PRESENTATION: A 74-year-old man with right vertebral artery (VA) occlusion and left VA stenosis was followed at our outpatient clinic. He presented with a 40-day history of dizziness and gait disturbance. MRI showed no new acute infarction, whe

Klippel-Feil syndrome (KFS) is a rare congenital disorder characterised by the fusion of the cervical vertebrae. We report the case of an early adolescent patient who presented with acute dizziness, vomiting, slurred speech and left-sided hemiparesis following neck movements. Imaging revealed craniocervical anomalies (including atlanto-occipital assimilation and C2-C3 fusion) and severe vertebrobasilar artery stenosis/occlusion, which led to acute ischaemic infarcts in the cerebellum and pons. Notably, the patient lacked the classical short-neck phenotype. The patient was diagnosed with KFS an

BACKGROUND: Dizziness is a frequent complaint among elderly patients with acute ischemic stroke. However, its association with different grades and locations of arterial stenosis remains unclear. This study aimed to assess the links between stenosis severity, posterior circulation involvement, dizziness, and short-term neurological outcomes.
METHODS: A retrospective analysis was performed on 134 elderly patients with acute ischemic stroke admitted from January 2024 to May 2025. All patients underwent Computed Tomography Angiography (CTA) or MRA of the intracranial and extracranial arteries, i

Interventional trials with high-density lipoprotein cholesterol (HDL-C)-raising drugs have generally failed to demonstrate a beneficial effect on cardiovascular outcomes, although low HDL-C levels confer the risk of cerebrocardiovascular diseases. Previous experimental studies indicate that HDL-C promotes angiogenesis/arteriogenesis. Therefore, we aimed to clinically investigate whether high blood HDL-C levels could clinically predict cerebral hemodynamic improvements in patients with asymptomatic carotid artery stenosis/occlusion showing cerebral hypoperfusion. This longitudinal retrospective

Large, wide-necked basilar artery (BA) apex aneurysms remain challenging to manage despite advances in endovascular techniques, with persistent risks of recurrence and complications, such as parent artery stenosis, perforator occlusion, and in-stent thrombosis. This report introduces a novel endovascular technique, named ''ↃC (double-C) stenting'', for complex BA aneurysms involving the proximal posterior cerebral arteries (PCAs) or superior cerebellar arteries (SCAs). This technique combines bidirectional stenting with coil embolization. From the anterior circulation, a stent or a flow divert

BACKGROUND: In the endovascular treatment of symptomatic severe vertebral artery (VA) stenosis without collateral blood flow, intraprocedural acute occlusion due to thrombus rupture or lesion dissection can result in catastrophic ischemic complications in the posterior circulation. In this report, the authors describe an intentional staged angioplasty approach in which bypass surgery was performed before percutaneous transluminal angioplasty (PTA) at the stenotic lesion. This "safety bypass" was expected to provide distal collateral flow for inadvertent occlusion of the basilar artery due to d

Posterior circulation stroke, particularly basilar artery occlusion, represents a neurologic emergency with high morbidity and mortality, often complicated by diagnostic ambiguity and rapid systemic deterioration. We present the case of a 56-year-old male with multiple vascular risk factors who presented with acute altered mental status, dysarthria, and focal neurologic deficits. Initial evaluation considered a broad differential diagnosis, including intracranial hemorrhage, seizure, and metabolic derangements; however, rapid multimodal assessment excluded these etiologies and supported an acu

BACKGROUND: Bow Hunter's syndrome (BHS) is positional vertebrobasilar ischemia caused by dynamic vertebral artery compromise during head rotation, usually attributed to degenerative compression at C1-2 and typically presenting in midlife. Very elderly cases are seldom recognized.
CASE DESCRIPTION: A 92-year-old woman presented with a right posterior inferior cerebellar artery infarct. Despite empirical anticoagulation for suspected cardioembolism, she developed recurrent infarcts confined to multiple bilateral posterior circulation territories on day 121. BackgroundBow Hunter's Syndrome (BHS) is a rare condition in which dynamic vertebral artery (VA) compression during neck rotation induces transient symptoms, such as vertigo, whereas major arterial occlusion is extremely uncommon. There is no standardized treatment for BHS, and management includes conservative, surgical, and rarely endovascular approaches. We report a unique case of BHS with acute VA occlusion successfully treated by mechanical thrombectomy followed by elective stent placement.Case ReportAn 87-year-old woman presented with right gaze deviation, dysarthria, right hemiparesis, a

BACKGROUND Top-of-the-basilar syndrome (TOBS) is a rare but clinically heterogeneous posterior circulation disorder characterized by acute ischemia affecting 2 or more territories supplied by the distal basilar artery, including the thalami, midbrain, occipital lobes, medial temporal lobes, and cerebellum. Intravenous thrombolysis with recombinant tissue plasminogen activator (rt-PA) is an established treatment for eligible patients with TOBS, and reports of transient coma induced during rt-PA infusion remain exceedingly scarce. BACKGROUND: In the event of clivus exhibiting longitudinal fractures, the basilar artery (BA) may become entrapped at the fracture site, although this occurrence is infrequent. Occlusion of bilateral BA perforators, accompanied by entrapment of the BA, can result in brainstem ischemia, which often leads to unfavorable outcomes. We report here the usefulness of slab maximum intensity projection (MIP) images of rotational angiography in evaluating the BA perforators.

Background and Clinical Significance: Marfan syndrome (MFS) is an autosomal-dominant connective-tissue disorder caused by pathogenic FBN1 variants. Aortic-root dilation and dissection are the canonical complications, whereas spontaneous vertebral artery dissection (VAD) is described only sporadically. Yet, cerebrovascular events are several-fold more common in MFS, and up to 74% of patients exhibit increased vertebral artery tortuosity, a validated predictor of dissection; Case Presentation: A 32-year-old African man with hypertension, type 2 diabetes mellitus, tobacco use, and headaches label

BACKGROUND: Cerebrovascular reactivity(CVR), a key indicator of cerebrovascular reserve, is crucial for evaluating cerebrovascular pathophysiology. This study employed resting-state MRI (rs-MRI) to assess CVR alteration in patients with unilateral middle cerebral artery stenosis or occlusion (MCA-S) under non-hypercapnic conditions, comparing them with healthy controls.
METHODS: A total of 41 patients with unilateral MCA-S and 50 age-, sex-, and education-matched normal controls (NC). All underwent rs-MRI and neuropsychological assessments. CVR was derived from rs-fMRI frequency band signals,

BACKGROUND: Futile recanalization (FR) after endovascular therapy (EVT) is common in basilar artery occlusion (BAO). The purpose of this study was to investigate the predictors of FR in the posterior circulation with an emphasis on the role of perfusion imaging and its relation to the underlying etiology.
METHODS: We included BAO patients who had pretreatment perfusion imaging and successful recanalization after EVT. Patients were dichotomized into futile and non-futile groups according to the favorable functional outcome at 90 days (modified Rankin Scale (mRS) 0-3). Perfusion abnormalities w

The treatment for vertebral artery stump syndrome (VASS) remains controversial. Here, we report a case of VASS in which cervical vertebral artery (VA) occlusion was performed. A 35-year-old man was admitted for left VA cerebellar infarction caused by left cervical VA dissection with severe stenosis on angiography, and was administered aspirin (100 mg/day). One month after discharge, the patient experienced recurrent stroke with cerebellar infarction. Digital subtraction angiography revealed that the origin of the left VA was occluded and that the VA received antegrade and retrograde flow via t

Flow diverters (FDs) have been employed in the treatment of unruptured intracranial vertebral artery dissecting aneurysms (IVADAs), yielding seemingly favorable outcomes. Despite FD treatment, aneurysm persistence (incomplete occlusion post-initial treatment) can occur in some patients, potentially leading to recurrent symptoms or complications. This study aims to explore the risk factors associated with the persistence of unruptured IVADAs that have undergone treatment with FDs. The study encompassed 77 unruptured IVADAs from 75 consecutive patients who received treatment with FDs. At a media

Vertebral artery occlusion is a condition that can reduce blood flow to the vertebrobasilar system, which may cause various neurological symptoms. We report the case of a 34-year-old female patient who had a cesarean delivery under spinal anesthesia 19 days before presentation. She had no identifiable risk factors. The patient presented to the Family Health Center with complaints of dizziness. She exhibited numbness on the right side of the face, right arm, and right side of the tongue, as well as weakness in her right arm and leg, and was subsequently referred to the emergency department via

BACKGROUND: An occipital artery-vertebral artery (OA-VA) anastomosis is rarely observed on cerebral angiography. The authors present a case of posterior circulation infarction during carotid artery stenting (CAS) due to an OA-VA anastomosis.
OBSERVATIONS: A 73-year-old male presented with mild right internal carotid artery (ICA) stenosis, which progressed to near occlusion over 18 months. Cerebral angiography revealed anterograde flow from the external carotid artery (ECA) to the vertebrobasilar system via the OA-VA anastomosis, which was not recognized by the surgeons before treatment.
