Thrombectomy in basilar stroke via persistent trigeminal artery secondary to internal carotid artery occlusion.

Internal carotid artery (ICA) occlusion typically affects the anterior circulation. However, in rare cases, it may present exclusively as posterior circulation ischaemia, particularly in the presence of anatomical variants such as a persistent trigeminal artery (PTA), an abnormal embryonic connection between the ICA and the basilar artery (BA).We report the case of a patient with ICA and PTA occlusion who presented clinically with a posterior circulation stroke. The initial National Institutes of Health Stroke Scale score was 20. CT angiography revealed occlusion of the ICA and distal BA. Foll

Adoptive NK cell transfer confers neuroprotection by attenuating neuroinflammation and alpha-synuclein pathology in a mouse model of synucleinopathy.

UNLABELLED: Clevidipine (Cleviprex), a late-generation dihydropyridine calcium channel antagonist available as a lipid emulsion for intravenous infusion, is approved in the US for the reduction of blood pressure (BP) when oral therapy is not feasible or desirable. Intravenous clevidipine is effective in the treatment of both acute preoperative and postoperative hypertension in adult cardiac surgery patients, and with a rapid onset and short duration of action the drug can be easily titrated for predictable BP control. Moreover, in terms of controlling acutely elevated BP in this patient popula

Case report: Lateral medullary syndrome with eight-and-a-half syndromes.

RATIONALE: Lateral medullary syndrome is caused by atherosclerosis or embolism of the vertebral artery and its branches or the posterior inferior cerebellar artery (PICA).The eight-and-a-half syndrome is a rare pontocerebellar nerve-ocular syndrome presenting as a one-and-a-half syndrome plus ipsilateral seventh cerebral nerve palsy. The dorsolateral medullary syndrome combined with the eight-and-a-half syndromes is even rarer, so it is important to recognize the features of the classical brainstem syndrome and the eight-and-a-half syndromes. PATIENT CONCERNS: Most patients with dorsolateral

End-to-end revascularization between the occipital Artery(OA) and the p1 segment of posterior inferior cerebellar Artery(PICA) for a patient with posterior circulation ischemia via a far-lateral approach:2-Dimensional Operative video.

Vertebral artery (VA)Aneurysms involving the origin of the posterior inferior cerebellar artery (PICA) ,occasionally, induce cerebellum and brainstem infarction due to intraluminal thrombus and calcific VA stenosis. At times, vessel occlusion and revascularization is necessary for successful obliteration of these aneurysms.2 The occipital artery (OA) is often the preferred donor graft for lesions of the posterior fossa. Although most OA-PICA bypasses can be performed using the p3 segment as the recipient site for an end-to-side anastomosis, a more feasible alternative to conventional OA-p3 PIC

Ruptured aneurysm in collateral arteries with blood-flow impairment of posterior inferior cerebellar artery: a report of three cases.

Herein, we report three cases of cerebellar hemorrhage due to a ruptured small aneurysm located on a collateral artery compensating for one or more stenotic or occluded major cerebellar arteries. In each case, endovascular distant parent artery occlusion of both the collateral artery and aneurysm was performed to prevent rebleeding. A ruptured small aneurysm in a collateral artery may be observed in patients with hemorrhage in an atypical cerebellar region, especially in cases of stenosis or occlusion of the vertebral artery or posterior inferior cerebellar artery. Thus, cerebral angiography i

BACKGROUND: Flow diversion (FD) has emerged as an effective treatment option for intracranial aneurysms. However, there is limited evidence regarding its safety and efficacy specifically for distal and small-artery aneurysms of the posterior circulation. This study aimed to investigate the outcomes of FD for aneurysms arising from the posterior inferior cerebellar artery, anterior inferior cerebellar artery, superior cerebellar artery, and the P2 and P3 segments of the posterior cerebral artery. METHODS: This is a subanalysis of the Post-FD (Posterior Circulation Aneurysms Treated With Flow D

Emergency Revascularization for Acute Vertebral Artery Occlusion Accompanied by Systemic Lupus Erythematosus: A Case Report.

The efficacy of mechanical thrombectomy (MT) for acute ischemic stroke has been established, but there are few reports on the effectiveness of MT for stroke patients with collagen disease. We report the case of a systemic lupus erythematosus (SLE) patient with cerebral infarction who underwent MT. A 48-year-old woman had been diagnosed with SLE for 30 years. She visited our hospital because of dizziness from the day before, but when she arrived at the hospital parking lot, she developed vomiting and impaired consciousness. An MRI revealed increased cerebellar hemisphere infarction and magnetic

Stroke pattern in giant-cell arteritis mostly involves watershed areas.

CONTEXT: Stroke related to giant cell arteritis (GCA) is rare and is associated with a poor outcome. One of the putative ischemic mechanisms is narrowing of the arterial lumen due to wall infiltration by inflammation and intimal proliferation, leading to reduced distal blood flow. It was hypothesized that GCA-related stroke could predominate in watershed areas (WA). METHODS: Literature review including all cases of GCA-related stroke with brain images. RESULTS: Among 75 cases of GCA-related stroke, the anterior and posterior territories were involved in 48 % and 62.6 %, respectively. Up to 8

High-resolution magnetic resonance imaging in the diagnosis and management of vertebral artery dissection: A case report.

BACKGROUND: Vertebral artery dissection (VAD) is a rare but life-threatening condition characterized by tearing of the intimal layer of the vertebral artery, leading to stenosis, occlusion or rupture. The clinical presentation of VAD can be heterogeneous, with common symptoms including headache, dizziness and balance problems. Timely diagnosis and treatment are crucial for favorable outcomes; however, VAD is often missed due to its variable clinical presentation and lack of robust diagnostic guidelines. High-resolution magnetic resonance imaging (HRMRI) has emerged as a reliable diagnostic too

Bow Hunter's Stroke Caused by Cryptic Recanalization of an Occluded Vertebral Artery: A Case Report.

An 80-year-old man was admitted to our hospital with acute cerebellar infarction. Conventional magnetic resonance angiography and computed tomography angiography (CTA) showed occlusion of the right vertebral artery (VA). Carotid ultrasonography revealed that the right VA was narrowed at its entry point into the transverse foramen near C6. Given the location of the VA stenosis, the blockage may have been due to compression from the cervical spine. Therefore, we assessed the right VA blood flow by moving the neck. The cervical spine was rotated left and right for evaluation; however, no blood fl

Bow-Hunter Syndrome Due to Tandem Lesions; A Case Report.

Bow-Hunter syndrome (BHS) includes a form of stroke caused by dynamic factors affecting the extracranial vertebral artery (VA), such as rotation-induced flow disturbance at the atlantoaxial level. BHS is a rare condition that can result from a variety of factors, including abnormal bone compression and vertebral instability. So far, there have been no reports in which both vertebral instability and osteophytes compress the VA at different sites, leading to stroke. Herein, we present a rare case of stroke attributed to BHS involving thrombosis formation due to osteophyte compression at C3/4 and

Bilateral bow-hunter's stroke caused by sleeping in prone position: A case of recurrent posterior circulation infarction.

Objectives Bow hunter's stroke (BHS) is a condition characterized by temporary vertebrobasilar insufficiency caused by mechanical compression or occlusion of the vertebral artery when the head is rotated, leading to transient neurological symptoms such as dizziness, vertigo, nystagmus, dysarthria, and potentially stroke. Therefore, a situation in which the head remains rotated for an extended period is a significant risk factor for BHS. Materials and Methods We present a case of a 25-year-old woman who experienced three recurrent cerebral infarctions in the posterior circulation area after sle
