Early neurological deterioration before endovascular treatment in patients with mild stroke. MINORCAT-END-EVT study.

BACKGROUND AND PURPOSE: The management of patients with anterior circulation large vessel occlusion (LVO) presenting with mild neurological symptoms remains a matter of debate. Early neurological deterioration (END) may influence the decision to perform endovascular treatment (EVT); however, evidence regarding its impact on clinical outcomes is limited. METHODS: We conducted a retrospective analysis of prospectively collected data from the Catalan Stroke Registry (2016-2021). Patients with anterior circulation LVO, baseline NIHSS ≤5, baseline mRS score 0-1, and treatment with EVT were include

Upper Brachial Artery Access Using a 4-French Sheath for Diagnostic Cerebral Angiography Prior to Transradial Neurointerventional Therapy: A Single-Center Retrospective Study.

PURPOSE: The transradial access (TRA) is often preferred for neurointerventional therapy; however, radial artery occlusion after diagnostic angiography may limit subsequent TRA. To preserve radial access for neurointerventional therapy, this study proposed using the right transbrachial access (TBA) with a 4-French (Fr) sheath as a method for diagnostic angiography. MATERIALS AND METHODS: A retrospective analysis was performed on 51 patients who underwent diagnostic cerebral angiography via the right TBA and subsequently received neurointerventional therapy using right-sided TRA between April

The Cerebrovascular Reactivity Adjusted Fractional Amplitude of Low-Frequency Fluctuations Abnormalities in Middle Cerebral Artery Stenosis and Occlusive Disease.

This study investigated the variation characteristics of static fractional amplitude of low-frequency fluctuations (sfALFF) and dynamic fALFF (dfALFF) in Middle cerebral artery stenosis or occlusion(MCA-S) patients, and explored these two indicators alterations with cerebrovascular reactivity (CVR) correction. A total of 41 MCA-S patients and 50 matched controls underwent resting-state functional MRI and neuropsychological testing. Group differences in sfALFF/dfALFF were compared between the two groups, and the brain regions with differences in sfALFF/dfALFF with and without CVR correction wer

Angioscopy Visualized a Residual Free-Floating Thrombus in Mild Carotid Artery Stenosis Following Acute MCA Occlusion: A Case Report.

Carotid free-floating thrombus (CFFT) is a rare yet severe cause of the ischemic stroke. Early identification and appropriate management are crucial to prevent recurrent embolic events. A 76-year-old man experienced sudden left-sided hemiparesis and dysarthria. Magnetic resonance imaging revealed a high-intensity diffusion area in the right frontal and temporal cortices and occlusion of the right middle cerebral artery (MCA). Digital subtraction angiography (DSA) confirmed the right MCA occlusion with suspected CFFT at the internal carotid artery bifurcation. Mechanical thrombectomy with an as

Carotid Revascularization Protocol for Symptomatic Stenosis.

Carotid endarterectomy (CEA) has been considered the standard of care for symptomatic carotid stenosis (sCS). Carotid artery stenting (CAS) warrants reevaluation for equivalence to CEA given advances in embolic protection and stent technology. Our patients with sCS undergo computed tomography angiography and perfusion, magnetic resonance imaging, and carotid duplex ultrasonography to guide selection for CEA, CAS, or transcarotid artery revascularization (TCAR). Preoperative management includes dual antiplatelet therapy and maximal-dose statin therapy, comorbidity optimization, smoking cessatio

Tandem Occlusion to Thrombolysis in Cerebral Infarction (TICI) 3: A Case of Perioperative Tirofiban for Carotid Artery Stenting.

Large vessel occlusions associated with ipsilateral high-grade extracranial internal carotid artery stenosis, also called tandem occlusions, remain under investigation for optimal management. The risk of in-stent thrombosis is heightened for those who undergo carotid stenting in conjunction with mechanical thrombectomy (MT). Tirofiban, an intravenous antiplatelet agent functioning by binding the GIIb/IIIa receptors, inhibits platelet aggregation and has been used in cases of tandem occlusions to reduce the risk of in-stent thrombosis. Unfortunately, the bleeding risk is elevated in patients wh

Clinical and Angiographic Outcomes in Patients With Tandem Occlusion Thrombectomy: Analysis From the ASSIST International Registry.

BACKGROUND: Tandem lesions (TLs) are a subtype of acute ischemic stroke, which involve an extracranial carotid lesion (ie, severe carotid stenosis ≥70%) with large vessel occlusion. While large vessel occlusion has a standard approach with mechanical thrombectomy (MT), the best practice for intervention of the carotid lesion is not well-established. Utilizing data from the ASSIST registry, we investigated the differences in clinical and procedural outcomes between acute versus nonacute stenting. METHODS: The ASSIST registry (an international acute ischemic stroke MT prospective study) was uti

A Bilateral Carotid Web Presenting as Recurrent Transient Ischemic Attacks With Negative Initial Stroke Imaging: A Case Report.

Carotid webs (CWs) are an increasingly recognized cause of ischemic stroke and transient ischemic attacks (TIAs), particularly in patients without an alternative clearly identified etiology, although they likely remain underdiagnosed because of their subtle radiographic appearance and the absence of significant carotid stenosis. The lesion is considered an intimal variant of fibromuscular dysplasia that may promote local blood stasis, thrombus formation, and distal cerebral embolization. We present the case of a 59-year-old woman with recurrent transient neurological deficits and initially neg

Computed Tomography Angiography Recognition of a Hemorrhagic Moyamoya Pattern May Alter Acute Blood Pressure Management.

Moyamoya disease (MMD) is a rare progressive cerebrovascular occlusive disorder characterized by stenosis of the terminal internal carotid arteries and the development of fragile lenticulostriate collateral vessels. In adults, MMD can manifest as intracranial hemorrhage (ICH), carrying significant morbidity and a high risk of recurrent bleeding. When hemorrhagic MMD coexists with bilateral large vessel occlusion, acute management becomes exceptionally complex, as the standard imperative to aggressively lower blood pressure following ICH directly conflicts with the hemodynamic requirements of c

Wake-Up Stroke Due to Middle Cerebral Artery Occlusion and the Decision Between Medical Management and Stenting: A Case Report.

Intracranial arterial dissections (IAD) are rare but high-morbidity entities, especially in young adults. Optimal management remains controversial, ranging from conservative medical therapy to emergent endovascular intervention. A 27-year-old female presented with a "wake-up stroke" (National Institutes of Health Stroke Scale (NIHSS) 21). Initial angiography revealed a supraclinoid right internal carotid artery (ICA) dissection with 40% stenosis without hemodynamic compromise. Conservative management with dual antiplatelet therapy (DAPT) was initiated, given the patient's young age, leading to

Correlation between vascular stenosis severity and dizziness symptoms and neurological prognosis in elderly patients with acute ischemic stroke.

BACKGROUND: Dizziness is a frequent complaint among elderly patients with acute ischemic stroke. However, its association with different grades and locations of arterial stenosis remains unclear. This study aimed to assess the links between stenosis severity, posterior circulation involvement, dizziness, and short-term neurological outcomes. METHODS: A retrospective analysis was performed on 134 elderly patients with acute ischemic stroke admitted from January 2024 to May 2025. All patients underwent Computed Tomography Angiography (CTA) or MRA of the intracranial and extracranial arteries, i

Five-Year Delayed Rupture of a Suspected Infectious Intracranial Aneurysm: A Case Report.

Intracranial infectious aneurysms are uncommon cerebrovascular lesions caused by infection-related injury of the arterial wall and may rupture with devastating neurological consequences. A 57-year-old man had developed Streptococcus mitis infective endocarditis five years earlier, which was complicated by embolic infarction in the left posterior cerebral artery (PCA) territory and renal infarction, and he had subsequently undergone mitral valve replacement. He presented with sudden impaired consciousness and right hemiplegia while receiving warfarin therapy. Computed tomography (CT) demonstrat

A Metallic Screw Gun-Traumatic Vertebral Artery Dissection Injury: A Case Report and Literature Review.

Traumatic vertebral artery dissection (VAD) is a tear of the intimal layer, which may extend into the tunica media, resulting in blood tracking within the arterial wall and consequent luminal stenosis. It presents complex challenges for surgical and neurovascular management. The most commonly affected vertebral artery segments are V2 and V3, due to their anatomical vulnerability to mechanical stress during neck motion. Potential complications include ischemic stroke, thromboembolism, subarachnoid hemorrhage, and pseudoaneurysm, which necessitate early diagnosis and timely intervention using en

A Novel and Simple Score to Predict Embolic or Atherosclerotic Middle Cerebral Artery Occlusion Before Mechanical Thrombectomy: AHOC Score.

OBJECTIVE: The mechanical thrombectomy (MT) strategy obviously differs for acute middle cerebral artery occlusion (MCAO) stroke caused by embolism or atherosclerosis. Our study aimed to develop and validate a simple and universally applicable score for predicting etiology [embolism or intracranial arteriosclerosis (ICAS)] before MT in patients with acute MCAO stroke. METHODS: Between November 2019 and September 2022, we retrospectively enrolled eligible patients in our hospital as the training cohort. Additionally, consecutive patients between July 2023 and April 2024 were recruited as the va

INTRODUCTION: Convexity subarachnoid hemorrhage (cSAH) is a rare subtype of nontraumatic subarachnoid hemorrhage. Large artery atherosclerotic stenosis or occlusion has emerged as a prevalent etiology in China. The diagnosis of cSAH is primarily based on imaging examinations. However, there are no established guidelines or consensus for its treatment. CASE REPORT: This report presented 3 cases of cSAH associated with atherosclerotic stenosis or occlusion. All 3 patients had significant large-artery stenosis or occlusion. In 2 cases (cases 1 and 3), computed tomography (CT) angiography reveale
