[Pre-occlusive stenosis of the internal carotid artery. Haemodynamic study by Doppler effect, phonoangiography-oculoplethysmography and arteriography (author's transl)].

The diagnosis of near total occlusion of the internal carotid artery was made on a group of 122 patients (90 symptomatic - 32 asymptomatic) by Doppler examination on one hand and carotid phonoangiography and oculoplethysmography, used together, on the other hand. The angiographic examination, including the study of the extra cranial arteries and the intra cerebral vessels, was performed. It has shown that a stenosis (greater than or equal to 90% of an internal carotid artery was the only lesion on 65 patients (53.3%) and that in 57 cases (46.7%) there were multiple lesions. 96 patients (76 sym

Persistent hypoglossal artery in association with advanced atherosclerosis.

The persistent hypoglossal artery is an anomaly of great interest which probably becomes clinically significant only when it is associated with occlusive cerebrovascular disease or some other life-threatening vascular abnormality such as intracranial aneurysm on the same side in the carotid-middle cerebral artery territory. In the case reported here it was apparently of no clinical importance in spite of associated atherosclerosis and thrombosis.

[Transoral ultrasonic flow metering of the vertebral arteries].

Using the method of ultrasonic flowmetering based on Doppler's effect with transoral positioning of the sensor occluding lesions of the vertebral arteries were diagnosed in 32 patients, in 22 of whom also the carotid arteries were involved. The method has proved to be effective in diagosing the subclavicular steal syndrome due to occlusions of the subclavicular arteries in 13 other patients. The agreement between the data of the transoral measurements of the blood flow in the vertebral arteries on the one hand, and the angiographic findings on the other was 81.1%. Disagreements were observed m

Mechanisms of acute carotid stroke.

In a three-year study, the clinical course and results of intracranial angiography were compared in patients having an acute stroke in the carotid artery territory combined with angiographic abnormalities indicating severe extracranial carotid stenosis or occlusion. Two major mechanisms of stroke were delineated. In one group, the angiographic intracranial abnormalities strongly suggested the presence of embolism in the cerebral vessels supplied by the stenotic or occluded carotid artery; many of these patients had no obvious transient ischemic attacks prior to their stroke and experienced a m

Cerebrovascular evaluation: assessment of Doppler scanning of carotid arteries, ophthalmic Doppler flow and cervical bruits.

In 38 patients who underwent cerebrovascular evaluation followed by angiography the Doppler ultrasound scanning technique was found to be an excellent screening procedure for detecting marked stenosis (greater than 50%) or occlusion of the internal carotid artery (93% correlation). It is noninvasive, easily reproducible and can be performed by a qualified technician. The major problems are: the inability to detect ulcerated plaques without marked stenosis, the requirement for patient cooperation (lying still for periods of 15 minutes), and the fact that it assesses only the extracranial circul

[The combination of STA-MCA anastomosis with another operation for the occlusive cerebrovascular disease (author's transl)].

Case 1. A 65 year old male had left hemiparesis with sudden onset since 8 years ago, which gradually aggravated for these 2 years. On Sept. 27, 1973, he was admitted to the Department of Neurosurgery, Kitano Hospital. There was left spastic hemiparesis with hemisensory disturbance and he could not walk without help for the maked spasticity. Left carotid angiogram revealed the complete occlusion of the internal carotid artery and marked stenosis of the external carotid artery at the common carotid bifurcation. External carotid endarterectomy was performed on Nov. 19, 1973, which was followed by

[The validity of degree and variety of carotid-artery-stenosis (author's transl)].

In cerebro-vascular deficiency the significance of carotoid artery-stenosis is determined by local and general risk factors. Indications for surgical treatment depend both on the degree and variety of stenosis. Clinical findings and measuring of blood pressure during operation have shown that cardiac efficiency, blood pressure, sympathetic affections of other brain-supplying arteries and above all function of arterious circle of Willis constitute further important risk factors.

[Unilateral blindness as a complication of carotid angiography (author's transl)].

In a patient with stenosis of the left internal carotid artery acute amaurosis of the left eye was observed after carotid arteriography. The macular branches of the central retinal artery and of the cilioretinal artery were obliterated by embolic material. Two further patients with carotid occlusion and stenosis, in whom spontaneous loss of vision occured, are presented. In such cases the diagnosis may be made with comparable accuracy by other methods, such as ophthalmodynamometry or angioscintigraphy, which entail risk to the patient.

[Bulbar infarcts. Vascular lesions in 26 observations].

The pattern of arterial occlusion and related lesions have been studied in 26 cases with 38 medullary infarcts. In 10 of these cases, only the intracranial vessels were examined. In the 16 other cases, the entire cerebral arterial supply was dissected. Medial infarcts were more often related to occlusion of the vertebral artery involving the termination of the artery (7/10 cases). In 6 cases a thrombosis on an atheromatous plaque and, once, an embolus of cardiac origin, were found. Lateral infarcts were usually due either to occlusion of the vertebral artery (14/23 cases), reaching, in 10 case

Induced seizures as therapy of experimental strokes in dogs.

Intracerebral pO2, as measured in normal dog brains by a modified mass spectrometer, was found to increase following seizure activity and remain elevated at least 2 hours. These results were found with both drug- and electrically-induced seizures. The pO2 increased to a greater degree in brain tissue rendered ischemic by middle cerebral artery occlusion. A transient reflex hypertension was observed with seizure activity, but hypertension alone failed to produce significant pO2 changes. Since oxidative metabolism has been shown by other investigators to proceed at an elevated rate during seizur

Abnormalities of the vertebrobasilar circulation due to subclavian artery disease.

The studies of Hutchinson and Yates on caroticovertebral stenosis have stimulated further interest in cerebrovascular disease. As a consequence, investigation of the four vessels, supplying the brain is now routine in the assessment of patients being considered for surgery for occlusive disease of the vessels. This has led to some interesting and unexpected findings. The effect of subclavian artery disease on the vertebrobasilar circulation was studied in two patients. Angiography showed the vertebral arteries to function as a collateral pathway when an occlusion of the subclavian artery was pr
