Multidomain perioperative analysis of neurological deficits following adult Moyamoya revascularisation: A retrospective cohort study.

BACKGROUND: Moyamoya disease (MMD) presents distinctive perioperative challenges owing to impaired cerebrovascular autoregulation and exhausted vascular reserve. Perioperative predictors of neurological deficit remain undercharacterized. METHODS: This single-center retrospective cohort study analyzed 24 consecutive MMD patients undergoing revascularization (2017-2022). A multidomain dataset encompassing Suzuki stage, surgical technique, intraoperative hemodynamics, perioperative laboratories, and anesthetic variables was analyzed using ROC analysis and exploratory penalized logistic regressio

Cerebral venous sinus thrombosis in pregnancy and the postpartum period: epidemiology, management, and maternal mortality-a systematic review and meta-analysis.

BACKGROUND: Cerebral venous sinus thrombosis is a rare cerebrovascular disease of importance both during pregnancy and in the puerperium. However, the existing cohort literature relates to two distinct clinical populations: women with acute onset of pregnancy-/puerperium-associated cerebral venous thrombosis (CVST), and women with known previous CVST who were assessed in connection with a subsequent pregnancy. As these populations address distinct clinical questions and have distinct denominators and follow-up structures, this review has distinguished outcomes from the two populations and synt

Aberrant signals in vascular dementia: Pleiotrophin/laminin pathway and glutamate metabolic dysregulation.

Vascular dementia (VaD), the second most common cause of dementia, is closely associated with chronic cerebral hypoperfusion and progressive cognitive decline; however, its molecular and cellular basis remains incompletely understood. Dysregulated intercellular communication within the neurovascular unit may contribute to the cellular and pathological alterations observed in VaD. To identify candidate signaling pathways associated with VaD, we integrated single-cell transcriptomic analysis, microarray data analysis, pseudotemporal trajectory analysis, and experimental validation. Pleiotrophin

Endovascular treatment for Moyamoya-associated intracranial aneurysms: a systematic review and meta-analysis.

BACKGROUND: Moyamoya disease (MMD) is a rare cerebrovascular disorder characterized by progressive stenosis of the distal internal carotid arteries, leading to increased collateral flow and the development of an abnormal vascular network, which imposes shear stress on small vessels and increases the risk of aneurysm formation. Endovascular treatment (EVT) has emerged as a potential approach for managing MMD-associated intracranial aneurysms (MMD-IA), but its safety and efficacy remain unclear. OBJECTIVE: This systematic review and meta-analysis aimed to evaluate the safety, efficacy, and long

Machine Learning-Enhanced TCAB Score for Predicting Postoperative Ischemic Stroke After CABG.

BACKGROUND: Postoperative acute ischemic stroke remains a critical complication of coronary artery bypass grafting. This study aimed to develop a novel Total Cerebral Atherosclerosis Burden (TCAB) score for predicting the risk of AIS post-coronary artery bypass grafting. METHODS: A prospective cohort of patients undergoing coronary artery bypass grafting was enrolled. The TCAB score was calculated by summing stenosis severity grades (0: <50%, 1: 50-69%, 2: 70-99%, 3: 100%) across all intracranial and extracranial artery segments. Primary outcome was in-hospital ischemic stroke. Multivariable

Biochemical Rationale for the Development of a Rapid Spectrophotometric Assay for Diagnosis of Mineral Homeostasis Disorders in Patients with Cardiovascular Disease.

Patients with coronary and cerebral atherosclerosis are characterized by increased levels of total serum calcium, ionized calcium, and phosphate, against a background of reduced levels of total serum protein and albumin. Here we aimed to develop a rapid diagnostic assay for mineral homeostasis disorders, based on assessing capacity of the acidic plasma proteins to bind excess calcium and phosphate ions. Plasma from bony fish, amphibians, reptiles, birds, mice, and patients with myocardial infarction was incubated with excess concentrations of calcium and phosphate at 37°C for varying time peri

Molecular and multimodal biomarkers in Moyamoya disease: from pathogenic mechanisms to clinical translation.

Moyamoya disease (MMD) is a chronic, progressive cerebrovascular condition marked by narrowing or blockage of the terminal segments of the internal carotid arteries, resulting in ischemic and hemorrhagic strokes. Its pathogenesis involves a multifactorial interplay between genetic susceptibility, immune-inflammatory dysregulation, endothelial dysfunction, and aberrant vascular remodeling, influenced by non-genetic and environmental factors. Despite considerable research progress, clinically useful biomarkers remain limited, lacking sufficient sensitivity and specificity for predicting disease

The role of artificial intelligence in estimating stroke events in Moyamoya patients: A systematic review and meta-analysis of diagnostic test accuracy.

INTRODUCTION: Moyamoya disease (MMD) and syndrome (MMS) are rare cerebrovascular arteriopathies marked by progressive internal carotid stenosis, fragile collateral networks, and a five-year stroke risk near 10% despite optimal care. Artificial-intelligence (AI) models integrating angiographic, perfusion, and clinical data show promise for risk stratification, but their diagnostic accuracy and clinical readiness remain uncertain. METHOD: We conducted a systematic review of AI algorithms for predicting ischemic or hemorrhagic stroke events in angiographically or magnetic resonance imaging (MRI)

Evaluating the role of graft angle on cerebral hemodynamics following direct cerebral bypass for moyamoya disease.

Direct cerebral bypass is a key treatment for moyamoya disease (MMD). This surgery grafts a donor vessel onto a recipient cerebral artery to boost blood flow to hypoperfused brain regions. Unlike coronary bypass, which restores downstream flow around a blockage, cerebral bypass for MMD reverses flow in the recipient vessel to perfuse the upstream network. Surgical decisions-such as donor vessel choice and anastomosis angle-significantly affect graft hemodynamics and outcomes. Yet these choices still rely on neurosurgeons' experience, lacking quantitative guidance. This study addresses that gap

Organoid Modeling and Single-Cell Profiling Reveal Smooth Muscle Cell Migration in Moyamoya Disease.

Moyamoya disease (MMD) is a cerebrovascular disorder characterized by progressive intracranial arterial stenosis, which can lead to ischemic or hemorrhagic stroke. However, its pathogenesis remains poorly understood, limiting advances in the diagnosis and treatment of MMD. We perform data-independent acquisition (DIA) proteomics on serum samples from 40 MMD patients and 20 healthy controls, followed by ELISA validation of an additional 45 cohort. Vascular organoids are generated from induced pluripotent stem cells (iPSCs) derived from the peripheral blood, and analyzed using histological stain

Cilostazol in a Child With Moyamoya Disease.

Moyamoya disease is characterized by progressive stenosis of the terminal internal carotid arteries and their branches with secondary angiogenesis of lenticulostriate collateral vessels. Although surgical revascularization decreases the risk of ischemic and hemorrhagic strokes, there remains little evidence demonstrating safety and efficacy of nonsurgical interventions in pediatric moyamoya. Recent data support the use of cilostazol in adult moyamoya, but cilostazol safety and efficacy in pediatric moyamoya remains unknown. We report the case of a 9-year-old girl diagnosed with MMD following a

Postoperative seizure, stroke, hemorrhage, and cerebral hyperperfusion syndrome rates in adult patients with moyamoya disease undergoing revascularization: a systematic review and proportional meta-analysis.

To compare the postoperative incidence rate of seizure, stroke, hemorrhage, and cerebral hyperperfusion syndrome (CHP) in patients undergoing revascularization for Moyamoya disease (MMD) across direct, indirect, or combined revascularization approaches. MMD is a rare, progressive cerebrovascular disorder characterized by critical stenosis of the carotid terminus leading to chronic hypoperfusion and risk for cerebral ischemia. Revascularization in MMD may lead to a risk of perioperative seizures; however, this has not been rigorously studied. PubMed, Medline, Embase, Scopus, and Web of Science
