Enhancement of the intracranial arterial wall at MR imaging: relationship to cerebral atherosclerosis.

PURPOSE: To investigate the relationship between intracranial arterial wall enhancement and atherosclerosis. MATERIALS AND METHODS: Intracranial vertebral arteries of 30 patients and carotid arteries of 62 patients were studied with spin-echo magnetic resonance imaging with contrast enhancement and spatial presaturation. Arterial wall enhancement was graded as follows: stage 1, no substantial enhancement; stage 2, faint or thin area of enhancement; stage 3, definite and thick area of enhancement. RESULTS: In vertebral arteries, stage 3 enhancement was seen in 11 patients (mean age, 73.7 year

Relation of coronary atherosclerosis and apolipoprotein E genotypes in Alzheimer patients.

BACKGROUND AND PURPOSE: Apolipoprotein E (apoE) epsilon 4 allele has been associated with a high risk for coronary heart disease. Increased frequency of the epsilon 4 allele has also been reported in patients with late-onset familial and sporadic Alzheimer's disease (AD). The aim of this study was to investigate the degree of coronary and cerebral atherosclerosis in a neuropathologically verified series of AD patients with different apoE genotypes. In addition, we studied the relationship between the degree of coronary and cerebral atherosclerosis and the extent of beta-amyloid (A beta) accumu

[Relationships among obesity, atherosclerotic disorders, and longevity in the elderly. An autopsy study].

To evaluate the effect of obesity on atherosclerosis, health, and longevity in the elderly, we studied the relationship between body mass index, atherosclerosis, and the duration of disability by multivariate analyses. We analyzed data from 521 residents of the Yokufu-kai home for the aged. They underwent a 50 g oral glucose tolerance test, and autopsies were done after they died. All functional evaluations were done retrospectively from the hospital records. The mean age of the time of death was significantly higher in the overweight group than in the lean group. The incidence of marked cereb

[The efficacy of treatment with the tranquilizer tranxene in elderly persons with psychoemotional disorders against a background of initial and moderately pronounced atherosclerotic circulatory encephalopathy].

Therapeutic effects of a minor tranquilizer (from the group of benzodiazepines) tranxene were studied in patients presenting with psychoemotional disorders in the presence of the onset moderately severe atherosclerotic dyscirculatory encephalopathy. The patients derived apparent benefit from the treatment administered as per subjective as well as objective evidence: the levels of hypochondria, depression, hysterioidness, psychoasthenicity, got significantly decreased, with positive changes being recordable in EEG as well as REG. The results obtained allow tranxene to be recommended for use in

[Alkaline blood serum proteins in patients with the initial manifestations of cerebral arteriosclerosis and the late sequelae of mild closed craniocerebral trauma (comparative data)].

Alkaline proteins play their pathogenetic part in the course of cerebral atherosclerosis the development of which is accompanied by decline in their content. It has been ascertained that in patients with early cerebral atherosclerosis and remote sequelae of mild closed craniocerebral injury (RS MCCCI) the content of alkaline proteins is below the established norm. The absence of significant differences between the patients presenting with early manifestation of cerebral circulatory insufficiency and those with RS MCCCI may suggest similarity of mechanisms of development of vascular disorders o

Can transcranial Doppler really detect reduced cerebral perfusion states?

BACKGROUND AND PURPOSE: This study was designed to determine whether transcranial Doppler ultrasonography (TCD) may detect reduced perfusion states of the brain in patients with hypertension or diabetes mellitus with suspected cerebral atherosclerosis and arteriolosclerosis. METHODS: We determined blood flow velocity with TCD in the middle cerebral artery and cerebrovascular vasodilator responses to carbon dioxide in 22 patients with or without carotid artery occlusive disease and minor stroke; we compared the results with the measurements of cerebral blood flow and oxygen metabolism by posit

[Ultrasonic diagnosis of patients with circulatory encephalopathy].

The paper describes the results of examining intracerebral blood flow in patient with different stages of dyscirculatory encephalopathies. Transcranial Doppler was applied. Ultrasound examination of the great arteries of the head was made by determining the linear and volumetric velocities of blood flow. Progressive cerebral atherosclerosis was found to result in an earlier decrease in cerebrovascular blood flow.

Apolipoprotein(a) deposition in atherosclerotic plaques of cerebral vessels. A potential role for endothelial cells in lesion formation.

Atherosclerosis is the leading cause of death and serious morbidity in economically developed societies through its sequelae of coronary artery and cerebrovascular disease. The causes and mechanisms of atherosclerosis are still largely unknown. Serum levels of a lipoprotein, Lp(a), have been shown, in retrospective and some prospective clinical studies, to be associated with increased risk of myocardial and cerebral infarction. The active part of Lp(a), apo(a), has > 80% homology with plasminogen; thus it may competitively inhibit the thrombolytic action of plasminogen and enhance thrombogenes

Cerebral arteritis associated with oral use of phenylpropanolamine: report of a case.

Phenylpropanolamine (PPA) is the major ingredient of many over-the-counter cold remedies and diet pills. Use or abuse of PPA may cause hemorrhagic stroke or cerebral vasculitis similar to the clinical and angiographic picture associated with amphetamine use or abuse. We report a 32-year-old Taiwanese women who developed sudden onset of severe headache, nausea and vomiting on the seventh day of oral ingestion of 75 mg PPA per day. Cerebral angiograms showed multiple areas of alternating focal constriction and dilatation ("beading" appearance) in the anterior and posterior cerebral arteries cons

Mechanisms involved in platelet vessel wall interaction.

The molecular mechanisms underlying formation of platelet thrombi constitute one of the central problems in vascular biology and medicine. Fibrinogen binding to its platelet integrin receptor alpha IIb beta 3 (glycoproteins IIb-IIIa complex) plays a pivotal role in formation of platelet thrombi by providing molecular bridges spanning platelets and by contributing to its receptor-mediated outside-in signaling. Conversely, fibrinogen binding to integrin alpha IIb beta 3 requires inside-out signaling mediated by intraplatelet signal transducers activated in response to external signals generated

Cerebral hemodynamics and "re-build-up" phenomenon on electroencephalogram in children with moyamoya disease.

"Re-build-up" phenomenon, induced by hyperventilation, is a characteristic finding on electroencephalogram (EEG) in children with moyamoya disease, and suggests close correlation with cerebral ischemic attack. Its mechanism, however, remains obscure. In this study, we examined the relationship between the cerebral hemodynamics and this phenomenon before and after vascular reconstructive surgery. Regional cerebral blood flow and its reactivity to acetazolamide were analyzed using the xenon-133 inhalation method and single photon emission computed tomography in six children with moyamoya disease

Cerebral hemodynamics in pre-eclampsia/eclampsia syndrome.

Pre-eclampsia and eclampsia represent a syndrome with progressive pregnancy-induced hypertension. They are associated with considerable morbidity and mortality. The pre-eclampsia/eclampsia syndrome has been hypothesized to reflect a subtype of hypertensive encephalopathy. Additionally, an endothelial dysfunction with impaired cerebral autoregulation has been proposed. Neurological symptoms in hypertensive encephalopathy have been attributed to marked vasospasm or forced vasodilatation. This study was undertaken to evaluate the changes in cerebral hemodynamics due to this syndrome. Twelve patie

Localized necrotizing arteritis of the central nervous system.

We describe a case of necrotizing arteritis of the brain in a 26-year-old man who presented as organic brain syndrome. The lesion has been confused clinically with acute encephalitis. At necropsy, no other abnormalities suggestive of systemic arteritis were found. Such CNS-restricted necrotizing arteritis of medium and small arteries in the absence of granuloma is extremely uncommon.

[A case of ruptured cerebral aneurysm associated with fenestrated vertebral artery in osteogenesis imperfecta].

An extremely rare case of ruptured cerebral aneurysm associated with a fenestrated vertebral artery in osteogenesis imperfecta (OI) is presented. A 33-year-old female suffering from OI was admitted to our hospital with severe headache and vomiting. A CT scan revealed subarachnoid hemorrhage. Cerebral angiography with four vessel study showed a fenestration in the V3 portion of the left vertebral artery and a dilatation in its V4 portion, but no cerebral aneurysm was detected. After conservative treatment for three weeks, repeated angiography demonstrated an aneurysm of the anterior communicati

Amoxapine overdose in a young man: a transient mitochondrial abnormality?

Amoxapine is a second-generation tricyclic antidepressant structurally related to the neuroleptic loxapine. It was previously marketed as an alternative to traditional tricyclic antidepressants because of alleged shorter onset of action and fewer cardiotoxic effects. However, various adverse reactions, including cardiac dysrhythmias, renal failure, coma, seizures, and neuroleptic malignant syndrome, were reported during therapy or after acute overdose. A 14-year-old boy ingested 1900 mg of amoxapine and developed seizures, hypertension, hyperpyrexia, altered mental status, myoglobinuria, renal

Fanconi anemia and moyamoya: evidence for an association.

We report two patients with Fanconi anemia (FA) and moyamoya disease taken from a clinical database composed of 434 FA patients. Both are compound heterozygotes for the 322delG and R185X mutations in the FA complementation group C (FACC) gene. This combination of mutations is not found in any other of the 174 FA families screened. Either the 322delG or R185X mutation alone or in combination may predispose to primary, possibly congenital, vascular anomalies.

[Association of Takayasu's arteritis, pregnancy and Still's disease].

Two pregnant patients presented with Takayasu's disease predominating in the major branches of the aorta. There was no extention below the diaphragm and no complications. One of the patients had Still's disease which is a predominantly cutaneous form without chronic arthritis. Both pregnancies were uneventful excepting dysgravidia in one case. Two eutrophic infants were born at term. The risk of Takayasu's arteritis associated with pregnancy, as reported in the literature, is mainly due to the consequences of arterial hypertension with pre-eclampsia (60%), heart failure and cerebral vascular e

Corticosteroids in the treatment of rheumatologic diseases.

Over the past year, a number of important advances have been made in understanding the pathobiology and clinical utility of corticosteroid agents for rheumatic disease patients. Unfortunately, the new information did not always confirm our preconceived notions. Pulse or intermittent high-dose steroid administration for rheumatoid arthritis as yet does not have a clear role. Observational analyses of populations of rheumatoid arthritis subjects, although based on data from the most severely ill patients, turn us away from using prednisone as background therapy in rheumatoid arthritis. Optic neuropathy, headache, and diplopia with MRI suggestive of cerebral arteritis in relapsing polychondritis.

The pathogenesis of central nervous system disease in relapsing polychondritis (RPC) is unknown but may be related to cerebral arteritis. Previous reports have described clinical and histopathologic evidence of cerebral vasculitis in RPC; however, a neuroimaging correlate has not been reported. We present a 36-year-old man with neuro-ophthalmic features of RPC whose magnetic resonance imaging revealed multifocal gray- and white-matter high intensities. This pattern is consistent with cerebral arteritis as described in other systemic vasculitides.

Stroke-induced cardiovascular changes: a rare cause of death from polyarteritis nodosa.

Polyarteritis nodosa (PAN) is an uncommon but well documented cause of subarachnoid haemorrhage (SAH). We report a case of SAH in a patient with PAN causing death indirectly from the effects of resultant cardiovascular changes upon other manifestations of PAN. Clinicians should be aware of the systemic effects of SAH in patients with known vasculitides.
