Differential Blood-Brain Barrier Permeability Driven by Asymmetric Hypoperfusion in a Vascular Dementia Model.

INTRODUCTION: Blood-brain barrier (BBB) dysfunction has been associated with vascular dementia (VaD). However, the underlying mechanisms causing BBB dysfunction remain unclear, especially regarding cerebral hypoperfusion. This study aimed to investigate the effects of asymmetric hypoperfusion on BBB permeability using a mouse model of VaD. METHODS: Asymmetric bilateral common carotid artery stenosis (ACAS) was induced using ligature rings. BBB integrity was assessed on 3 days post-surgery using Evans blue, IgG, and albumin extravasation. A modified tissue processing protocol optimized endogen

Imaging features of adult-onset leukoencephalopathy with axonal spheroids and pigmented glia: a case report.

BACKGROUND: Adult-onset leukoencephalopathy with axonal spheroids and pigmented glia (ALSP) is a rare, progressive white matter disorder caused by pathogenic variants in the CSF1R gene. It typically presents in mid-adulthood with progressive cognitive, motor, and neuropsychiatric decline, but its phenotypic variability often leads to delayed diagnosis. CASE PRESENTATION: We describe a 41-year-old woman with no prior neurological history or relevant psychiatric tendencies who developed progressive cognitive impairment, urinary incontinence, gait disturbance, and sleep disruption over 3-4 years

Aberrant signals in vascular dementia: Pleiotrophin/laminin pathway and glutamate metabolic dysregulation.

Vascular dementia (VaD), the second most common cause of dementia, is closely associated with chronic cerebral hypoperfusion and progressive cognitive decline; however, its molecular and cellular basis remains incompletely understood. Dysregulated intercellular communication within the neurovascular unit may contribute to the cellular and pathological alterations observed in VaD. To identify candidate signaling pathways associated with VaD, we integrated single-cell transcriptomic analysis, microarray data analysis, pseudotemporal trajectory analysis, and experimental validation. Pleiotrophin

Transcriptomic and experimental validation identifies SOX6 and LDLRAD3 as key factors in vascular dementia.

Vascular dementia (VaD) is a leading cause of cognitive decline, yet its underlying molecular mechanisms remain incompletely understood. This study aims to investigate the alterations associated with VaD in neuronal and endothelial cells by integrating single-nucleus RNA sequencing (snRNA-seq) with microarray data from postmortem VaD brain tissues. Using high-dimensional weighted gene co-expression network analysis (hdWGCNA) and machine learning approaches, we identified SOX6 in neurons and LDLRAD3 in endothelial cells as key factors associated with VaD pathology. Functional enrichment analyse

Dynamic Spatial Metabolomics Reveals Region-Specific Metabolic Reprogramming and Markers during Vascular Cognitive Impairment Progression.

Vascular cognitive impairment (VCI) significantly contributes to dementia; however, the precise metabolic mechanisms underlying its region-specific pathological progression remain poorly understood. We hypothesized that spatially resolved metabolomics could uncover detailed spatiotemporal metabolic disruptions and key therapeutic windows. Using matrix-assisted laser desorption/ionization mass spectrometry imaging (MALDI-MSI), we systematically mapped metabolite alterations in hippocampal subregions (CA1, CA3, dentate gyrus) and cortical areas of a rat model of chronic cerebral hypoperfusion (2

Exploring targeted therapy in retinal vasculopathy with cerebral leukoencephalopathy: a case report and review of literature.

Retinal vasculopathy with cerebral leukoencephalopathy and systemic manifestations (RVCL-S) is a rare autosomal dominant microvascular disorder caused by C-terminal truncating mutations in TREX1 gene, which impair protein localization and lead to multisystem involvement. We report a patient carrying the pathogenic TREX1 variant NM_033629.6:c.703dup (p.Val235fs), the most frequently described mutation in RVCL-S, whose clinical course was consistent with the classic phenotype but with simultaneous pulmonary granulomatous lesions compatible with sarcoidosis. Neuroimaging Biomarkers of Disease Progression and Cognitive Change in Patients With Retinal Vasculopathy With Cerebral Leukoencephalopathy.

BACKGROUND AND OBJECTIVES: A monogenic, age-related cerebral small vessel disease (cSVD), retinal vasculopathy with cerebral leukoencephalopathy and systemic manifestations (RVCL-S), causes accelerated vascular dementia, vision loss, and premature death. Given knowledge is limited regarding its pathophysiology, we examined the natural history of progression in imaging and cognitive endpoints to define the rate and variability of progression in a prospective RVCL-S cohort. We hypothesized that cerebral blood flow (CBF) and oxygen extraction fraction (OEF), as metrics of cerebral hypoxia-ischemi

NOTCH3 Mutations in CADASIL: a multicellular perspective on neurodegeneration.

Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL) is a hereditary cerebral small-vessel disease caused by NOTCH3 cysteine-altering mutations. Clinical evidence, including early-onset migraines, psychiatric symptoms, and diffuse white-matter hyperintensities preceding strokes, suggests a nonvascular pathology. NOTCH3 is broadly expressed in neural progenitors, neurons, astrocytes, oligodendrocytes, and brain microvascular endothelial cells (BMECs), and is responsible for neurodevelopment, blood-brain barrier (BBB) homeostasis, glymphatic functi

Protocol for mouse vascular dementia model and evaluation of progressive tissue damage in subcortical white matter and adjacent cortex.

Here, we present a protocol for generating a vascular dementia (VaD) mouse model through stereotaxic microinjection of a vasoconstrictor into the subcortical white matter (WM). We describe steps for preparing the N5-(1-iminoethyl)-L-ornithine (L-NIO) solution, surgical procedures, and immunohistochemistry. We then detail procedures for confocal imaging and image analysis using ImageJ and Imaris software to evaluate key pathological features of human VaD, including vessel dilation, demyelination, glial reactivity, neuronal damage, subcortical WM atrophy, and ventriculomegaly. Microemboli model of vascular cognitive impairment/dementia (VCID) presents with long-term brain tissue hypoxia: relevance to the endothelin (ET) system.

Diabetes increases the risk of vascular contributions to cognitive impairment and dementia (VCID). Using a microemboli (ME) model, we found that diabetic rats experience more severe white matter damage and progressive cognitive decline than controls. Moreover, the restoration of endothelial function before ME injection prevents VCID in diabetes. Given that diabetes mediates early endothelial dysfunction, activates the endothelin (ET) system, and the post-mortem brain (b) ET-1 levels correlate with tissue hypoxia in dementia patients, this study investigated the relationship between ET system,

Body mass index levels and changes before and after dementia diagnosis and risk of all-cause mortality: a nationwide cohort study.

BACKGROUND: Dementia is a progressive disease associated with various health risks, including increased mortality. Although weight changes have been linked to adverse outcomes in older adults, limited research has examined the impact of body weight status after dementia diagnosis and its changes before and after diagnosis on mortality. This study aimed to address these gaps. METHODS: Using nationwide data from the Korean National Health Insurance Service, we included 37,717 individuals newly diagnosed with dementia between 2010 and 2016, comprising 29,982 with Alzheimer’s disease (AD) and 3,22

LDL oxidation and cerebrovascular aging: mechanisms of endothelial dysfunction, inflammation, and vascular cognitive impairment and dementia.

Vascular cognitive impairment and dementia (VCID) is increasingly recognized as one of the leading causes of cognitive decline in aging populations. VCID is driven by the interplay of endothelial dysfunction, chronic inflammation, oxidative stress, and metabolic dysregulation. Aging exacerbates cerebrovascular damage by impairing endothelial function, compromising the blood-brain barrier (BBB), and promoting neuroinflammation, all of which contribute to the progression of small vessel disease (SVD) and white matter hyperintensities (WMHs), which are hallmarks of VCID. Epidemiological evidence

Impact of early glymphatic disorders on the development of vascular dementia.

Early treatment can effectively ameliorate cognitive decline and slow disease progression in vascular cognitive impairment (VCI). Glymphatic system dysfunction may significantly contribute to the onset and progression of diverse neurodegenerative diseases. To investigate its influence on VCI, we used dynamic contrast-enhanced magnetic resonance imaging to observe the exchange of cerebrospinal fluid (CSF) and interstitial fluid (ISF) in the brains of vascular dementia (VD) model and SHAM rats, and used the Morris water maze and hematoxylin–eosin staining to confirm the predictive value of the g

Carotid revascularization and hemispheric white matter disease progression.

BACKGROUND: Carotid revascularization prevents ipsilateral ischemic stroke in high-grade stenosis, but its effect on hemispheric white matter disease (WMD) progression is not known. METHODS: We conducted a longitudinal study of patients ≥40 years with carotid stenosis (moderate50-69%, high-grade 70-99% or occluded) seen at Mayo Clinic (Florida, Rochester, Arizona) between 2011 and 2015 with serial brain MRIs ≥1 year apart and clinical follow-up through 2020. Hemispheric WMD volumes were quantified from axial T2-FLAIR using automated segmentation. Patients were stratified by revascularization
