A collaborative framework for uncovering molecular and cellular drivers of VCID: Foundations for future interventions in dementia.

Vascular-related factors are now considered major contributors to most forms of dementia, including Alzheimer's disease. However, the degree to which vascular deficits contribute to risk, onset, and progression of cognitive impairment and dementia has only recently been appreciated. Our understanding of the mechanisms by which vascular deficits drive cognitive decline in dementia is still limited. Further, the testing of therapeutic approaches to prevent vascular deficits to treat dementia are few. These factors motivated the establishment of the Vascular Contributions to Cognitive Impairment

Exosomes: Mechanisms, Diagnosis, and Therapeutic Progress in Vascular Cognitive Impairment.

Vascular cognitive impairment and dementia (VCID) is a cognitive impairment syndrome triggered by cerebrovascular lesions and their associated risk factors, representing the second most prevalent cognitive disorder after Alzheimer's disease (AD). Currently, there are no disease-modifying therapies available for VCID. In recent years, significant progress has been made in the study of exosomes in the context of neurodegenerative diseases (NDs). Given the shared pathophysiological features and underlying heterogeneity between VCID and neurodegenerative disorders, it is hypothesized that exosomes

Proteomic-guided targeted treatment of leukoencephalopathy with calcification and cysts.

Precision therapeutic approaches for rare neurological diseases are often hampered by a limited molecular understanding of the disease. Here, we report the first application of spatially-resolved proteomics to guide targeted treatment in a monogenic leukoencephalopathy. We investigated leukoencephalopathy with calcifications and cysts (LCC), a rare ribosomopathy caused by biallelic mutations in SNORD118 that leads to progressive cerebral white matter degeneration and vascular proliferation. LCC is currently incurable, though understanding the pathogenesis at the proteomic level can offer novel

Compound Congrong Yizhi capsule partially remodels the fronto-retrosplenial-hippocampal memory circuit in vascular dementia rats.

BACKGROUND: Progressive cognitive impairment in vascular dementia (VaD) is increasingly linked to brain network disruption, or "disconnection syndrome." Although Compound Congrong Yizhi Capsule (CCYC), a marketed Chinese patent medicine approved in China for vascular cognitive disorders, has been studied in terms of therapeutic efficacy and molecular targets, these approaches do not directly explain how this multi-target intervention modulates brain functional organization. Resting-state functional magnetic resonance imaging (rs-fMRI) enables in vivo characterization of BOLD-based brain networ

A novel ABCD1 frameshift mutation detected in a Chinese male with adrenomyeloneuropathy.

BACKGROUND: Adrenomyeloneuropathy (AMN), an adult-onset form of X-linked adrenoleukodystrophy (X-ALD), is caused by pathogenic mutations in ABCD1 gene. CASE: We reported a 60-year-old Chinese male with 15-year chronic progressive spastic paraplegia without symptoms suggesting adrenal insufficiency. RESULTS: Critical findings included abnormal plasma very long-chain fatty acids (VLCFAs), slightly elevated ACTH, and a novel pathogenic ABCD1 mutation (c.172_198delinsACGCAGGA, p.Ser58Thrfs*4) confirmed by genetic testing, which established the AMN diagnosis. CONCLUSION: Our case underscores tha

Longitudinal association of chronic periodontitis with all-cause dementia, Alzheimer disease, vascular dementia, and mild cognitive impairment: a distributed network analysis.

Chronic periodontitis (CPO) is prevalent among older adults and may elevate dementia risk. Evidence regarding its association with dementia subtypes and the effect of disease progression remains unclear. This study aimed to assess the relationship between CPO and dementia development, including subtypes, and evaluated whether the clinical course modifies this risk. Electronic health records of patients aged ≥ 60 years from six institutions were analyzed. A 10-year longitudinal analysis assessed the incidence of all-cause dementia, Alzheimer disease (AD), vascular dementia (VaD), and a composit

Transcriptomic integration reveals immune heterogeneity in vascular dementia and identifies ELF2 as a candidate therapeutic target.

Vascular dementia (VaD) is the second most prevalent form of dementia after Alzheimer's disease and is primarily driven by chronic cerebral hypoperfusion and neurovascular dysfunction. Growing evidence indicates that immune dysregulation and neuroglial-vascular injury play critical roles in disease progression; however, the molecular regulators underlying these processes remain insufficiently characterized. This study aimed to delineate immune heterogeneity and neuroglial-vascular alterations in VaD and to identify key regulatory targets involved in disease pathogenesis. Bulk RNA sequencing da

Wernicke Encephalopathy Complicating a Distinctive POLG Phenotype With MNGIE-Like Features.

BACKGROUND: Mitochondrial neurogastrointestinal encephalomyopathy (MNGIE) is an extremely rare autosomal recessive disease caused by variants in the thymidine phosphorylase gene (TYMP), primarily characterized by severe gastrointestinal and neurological symptoms. The complete phenotype of MNGIE has not been linked to any gene other than TYMP. METHODS: We describe two identical twins who exhibited delayed psychomotor development, infantile bilateral cataract, congenital demyelinating polyneuropathy, and severe progressive gastrointestinal dysmotility with recurrent pseudo-obstruction episodes,

Vascular Cognitive Impairment and Dementia: Clinical Features, Neuropathology, and Biomarkers.

Vascular cognitive impairment and dementia (VCID), ie, cognitive impairment secondary to cerebrovascular disease (CeVD), is the second most common form of dementia after Alzheimer's disease (AD), accounting for 15% to 20% of all cases. CeVD, in fact, contributes to dementia alongside other neuropathologies in up to 75% of dementia cases. CeVD and AD not only frequently co-occur in the brain, but they may also interact, and some VCID risk factors (midlife hypertension and diabetes) also increase AD risk. Because CeVD and cardiovascular disease share risk factors and pathophysiology, the cardiov

Binswanger's Disease: A Diagnostic Challenge in a Middle-aged Male.

Binswanger's disease, a rare form of subcortical vascular dementia, presents with a progressive decline in cognitive and motor functions due to chronic small vessel ischemic changes in the brain. Its clinical presentation often overlaps with other neurological conditions, particularly Wernicke's encephalopathy, leading to potential diagnostic delays. We report the case of a 45-year-old male who presented with altered mental status, gait abnormalities, memory impairment and ocular pain. Given his significant history of alcohol consumption, an imperial diagnosis of Wernicke's encephalopathy was

Hepatic Manifestations of STAT1 Gain-of-Function Variants.

Primary immune regulatory disorders (PIRDs) are inborn errors of immunity characterized by immune dysregulation and multisystem involvement, whose hepatic manifestations can mimic autoimmune, cholestatic, or microvascular liver disease. Among these, heterozygous signal transducer and activator of transcription 1 gain-of-function pathogenic variants are predominantly associated with autoimmune hepatitis-like liver injury whereas vascular and portal hypertensive phenotypes are rarely reported and likely underrecognized. We describe a young adult with recurrent infections and progressive liver di

Investigating the Frequency and Outcome of Central Vein Sign and Paramagnetic Rim Lesions in Children With MOGAD.

BACKGROUND AND OBJECTIVES: Central vein sign (CVS) is a common feature in multiple sclerosis (MS) lesions, but its frequency in myelin oligodendrocyte glycoprotein antibody-associated disease (MOGAD) varies significantly across studies. Paramagnetic rim lesions (PRLs) are described in MS, but not in MOGAD. Our goals were to evaluate the prevalence of PRLs and CVS in a large multicenter cohort of pediatric MOGAD. We compared CVS frequencies between acute vs remission phases, as well as with longitudinal dynamics of lesion evolution. METHODS: In this longitudinal retrospective multicenter study

Astrocyte CB1 receptors drive blood-brain barrier disruption in central nervous system inflammatory disease.

Reactive astrocytes shape central nervous system (CNS) inflammation and participate in myelin damage and repair mechanisms in multiple sclerosis (MS). Through the activation of cannabinoid CB1 receptors (CB1R) expressed by neurons and oligodendrocyte lineage cells, endocannabinoid signaling restricts neurodegeneration and promotes remyelination in preclinical MS models. However, despite accumulating evidence that supports cell-specific roles for CB1R in brain physiology and pathology, the implications of astrocyte CB1R signaling in MS initiation and progression remain uncertain. Using compleme

Natalizumab exacerbates astrocytopathy in NMOSD via blockade of endothelial VCAM1-astrocytic integrin α4 interaction.

Neuromyelitis optica spectrum disorder (NMOSD) is a rare autoimmune inflammatory disorder of the central nervous system (CNS) that shares clinical features with multiple sclerosis (MS) but typically manifests with more severe symptoms. The presence of pathogenic IgG autoantibodies targeting aquaporin-4 (AQP4) channels on astrocytes serves as a highly specific biomarker that distinguishes NMOSD from MS. Unlike MS, NMOSD is characterized by profound astrocytic destruction and exhibits a distinct response to therapies. Notably, disease-modifying therapies (DMTs) effective in MS, including nataliz

Alzheimer's Disease and Vascular Dementia, Connecting and Differentiating Features.

Alzheimer's disease (AD) and vascular dementia (VD) are the leading causes of dementia, presenting a significant challenge in differential diagnosis. While their clinical presentations can overlap, their underlying pathologies are distinct. AD is characterized by the accumulation of amyloid plaques and neurofibrillary tangles, leading to progressive neurodegeneration. VD, on the other hand, arises from cerebrovascular insults that disrupt blood flow to the brain, causing neuronal injury and cognitive decline. Despite distinct etiologies, AD and VD share common risk factors such as hypertension

Retinal vasculopathy with cerebral leukoencephalopathy: a rare mimic of CNS vasculitis.

Retinal vasculopathy with cerebral leukoencephalopathy is a rare autosomal dominant genetic disorder due to mutation in the TREX1 gene and presents with both central nervous system (CNS) and other organ dysfunction. It is often misdiagnosed as demyelination or vasculitis based on imaging features, often with potentially harmful immunotherapy given unnecessarily. This report describes two sisters with progressive hemiparesis, retinal vasculopathy and hepatic dysfunction, one of whom was initially misdiagnosed and treated for cerebral vasculitis. Imaging showed extensive and asymmetric white mat

Disease Severity Staging System for NOTCH3-Associated Small Vessel Disease, Including CADASIL.

IMPORTANCE: Typical cysteine-altering NOTCH3 (NOTCH3cys) variants are highly prevalent (approximately 1 in 300 individuals) and are associated with a broad spectrum of small vessel disease (SVD), ranging from early-onset stroke and dementia (cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy [CADASIL]) to nonpenetrance. A staging system that captures the full NOTCH3-SVD severity spectrum is needed and currently lacking. OBJECTIVE: To design a simple disease severity staging system that captures the broad clinicoradiological NOTCH3-SVD severity spectrum.
