Vascular dementia (VD) is one of the most prevalent forms of dementia, yet effective treatments remain limited. Our previous research identified hippocampal neural stem cells (hNSCs) senescence as a key contributor to VD progression and suggested that reducing hNSC senescence could help reverse cognitive impairment. In this study, we investigated whether Oleracein E (OE), a phenolic antioxidant alkaloid, could alleviate hNSC senescence and improve cognitive function in VD. Using a two-vessel occlusion mouse model of VD, we found that OE treatment significantly reduced hNSCs senescence, restore

Vascular cognitive impairment (CI) is an urgent medical problem due to its high prevalence, progressive course, and high degree of disability. CI may be associated not only with the direct effects of stroke, but also with the presence of premorbid cognitive decline of both vascular and neurodegenerative nature. In addition to vascular risk factors, there are a number of factors leading to dementia, and the heterogeneity of the nature of CI is noted. The management of patients with CI is complicated due to the comorbidity of most of them, as well as low adherence to medical recommendations. The

BACKGROUND: As a key inflammatory factor, the nucleotide-binding oligomerization domain (NOD)-like receptor protein 3 (NLRP3) inflammasome plays a crucial role in neuroinflammation and the progression of neurodegenerative diseases. Dysregulation of NLRP3 signaling can trigger various inflammatory responses in the brain, contributing to the development of neurodegenerative diseases such as ischemic stroke, vascular dementia (VaD), Alzheimer's disease (AD), Parkinson's disease (PD), and amyotrophic lateral sclerosis (ALS). Therefore, the NLRP3 signaling pathway is a promising therapeutic target

Adverse pregnancy consequences such as preeclampsia, diabetes and gestational hypertension can contribute to maternal vascular cognitive impairment and dementia (VCID) risk via vascular endothelial dysfunction and neuroinflammation, resulting in progressive cognitive health decline. Emerging evidence suggests that flavonoids, a diverse group of phytonutrients found in fruits, vegetables and certain beverages, exhibit potential anti-inflammatory and antioxidant properties that may protect against vascular damage. This review explores the potential of flavonoids to mitigate pregnancy-induced vas

Vascular dementia (VaD), the second-leading cause of dementia, is primarily a white matter ischemic disease with no direct therapies. Cell-cell interactions within lesion sites dictate disease progression or repair. To elucidate key intercellular pathways, we employ a VaD mouse model with focal ischemia replicating many elements of the complex pathophysiology of human VaD combined with transcriptomic and functional analyses. By integrating cell-type-specific mouse VaD transcriptomes and human VaD single-nucleus RNA sequencing (snRNA-seq) data plus a custom ligand-receptor database (4,053 human

Vascular dementia is the second most common form of dementia. Yet, the mechanisms by which cerebrovascular damage progresses are insufficiently understood. Here, we create bilateral common carotid artery stenosis in mice, which effectively impairs blood flow to the brain, a major cause of the disease. Through imaging and single-cell transcriptomics of the mouse cortex, we uncover that blood vessel venous cells undergo maladaptive structural changes associated with increased Epas1 expression and activation of developmental angiogenic pathways. In a human cell model comparing arterial and venous

White matter disease, a broad-spectrum term that covers various types of white matter lesions and degeneration, is strongly related to age-related neurodegenerative disorders including Alzheimer's disease (AD), and vascular contributions to cognitive impairment and dementia (VCID) and Alzheimer's related dementias (ADRD). There is no specific treatment for white matter disease. Therefore, basic research and clinical studies are essential for future outcomes. Since its formation in 2020, the Albert Research Institute for White Matter and Cognition (ARIWMC) mission has been to support white matt

Laminin is a major component of the vascular basement membrane and transgenic mice deficient in astrocyte or pericyte laminin manifest blood-brain barrier (BBB) disruption, implying an important stabilizing role for laminin in BBB integrity. As the laminin receptor α6β4 integrin is strongly induced on CNS blood vessels in the neuroinflammatory animal model experimental autoimmune encephalomyelitis (EAE), and mice deficient in endothelial β4 integrin (β4-EC-KO) display worse EAE, here we tested in a novel transgenic knock-in mouse, whether constitutive overexpression of endothelial β4 integrin

RATIONALE: Posterior reversible encephalopathy syndrome (PRES) is a relatively rare neurological disorder in obstetrics. Clinically, the most common precipitating factors are hypertension, immunosuppressive agents, or chemotherapy drugs. It is extremely rare for PRES to be induced by the entry of local anesthetic into the subdural space.
PATIENT CONCERNS: In this case report, we present an instance where a patient experienced blurred vision, and even blindness, following an accidental subdural block administered by the anesthetist, and was subsequently diagnosed with PRES. The patient was a 3

BACKGROUND: Mitochondrial neurogastrointestinal encephalopathy (MNGIE) is a rare autosomal recessive disorder caused by variants in the TYMP gene, which encodes thymidine phosphorylase (TP). It is characterized by multisystem involvement, with prominent gastrointestinal, neurological, and systemic manifestations that typically exhibit progressive worsening over time.
METHODS: We characterized a multigenerational MNGIE family through comprehensive proband analysis, identifying compound heterozygous TYMP variants (c.131G > C, p.Arg44Pro and c.1268T>G, p.Leu423Arg in trans) as the molecular basi

Cerebral small vessel diseases (cSVDs) contribute significantly to stroke and dementia. Advanced 7 T MRI techniques have revealed small vessel dysfunction in cSVD patients, linked to global white matter damage cross-sectionally. However, it remains unclear whether these vascular deficits predict progressive tissue damage. This longitudinal study examined the spatial relationship between local vascular function and white matter damage progression in patients with Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL). Twenty-two patients underwent b

Rationale: Vascular dementia (VaD), driven by chronic cerebral hypoperfusion (CCH), leads to synaptic degeneration and cognitive decline, yet mechanisms linking vascular dysfunction to synaptic loss remain unclear. Intermittent fasting (IF) has emerged as a potential intervention, but its effects on synaptic integrity in VaD are unknown. This study aims to investigate the effects of IF against synaptic degeneration and cognitive impairment induced by CCH. Methods: Bilateral common carotid artery stenosis (BCAS) was employed to induce chronic CCH by placing 0.18 mm micro-coils around each commo

Cognitive impairment represents a progressive neurodegenerative condition with severity ranging from mild cognitive impairment (MCI) to dementia and exerts significant burdens on both individuals and healthcare systems. Vascular cognitive impairment (VCI) represents a heterogeneous clinical continuum, spanning a spectrum from subcortical ischemic VCI (featuring small vessel disease, white matter lesions, and lacunar infarcts) to mixed dementia, where vascular and Alzheimer's-type pathologies coexist. While traditionally linked to macro- and microvascular dysfunction, the mechanisms underlying

Coats plus (Cp) is a pleiotropic disorder with multisystemic manifestations including retinal vascular disease and leukoencephalopathy with intracranial calcifications and cysts. It is an autosomal recessive telomere biology disorder mostly associated with compound heterozygous defects in CTC1 gene. Cp typically presents in early childhood with poor long-term prognosis. In this study, we describe the case of a 46-year-old woman presenting with a first unprovoked seizure in the setting of 2 years of progressive behavioral and cognitive deterioration. Her medical history was notable for retinal

BACKGROUND: Studies consistently demonstrate that vascular cognitive impairment and dementia (VCID) onset and progression are associated with diminished activity in the Wnt/β-catenin signaling pathway, crucial for maintaining blood-brain barrier (BBB) integrity and promoting angiogenesis in the central nervous system. Transcutaneous auricular vagal nerve stimulation (taVNS) has shown potential to enhance cognitive function by reducing BBB permeability and stimulating angiogenesis, yet its direct linkage to the Wnt7/β-catenin pathway activation remains uncertain and requires mechanistic validat

BACKGROUND: Vascular dementia (VaD) is the second most common type of dementia after Alzheimer's disease, imposing a substantial burden on global health. Accumulating evidence indicates that oxidative stress, characterized by an imbalance between reactive oxygen species (ROS) production and antioxidant defense, plays a pivotal role in the pathogenesis of VaD.
OBJECTIVE: This review aims to summarize the latest research progress on the involvement of oxidative stress in the pathogenesis of VaD and explore emerging therapeutic strategies targeting oxidative stress.
RESULTS: Oxidative stress is

Vascular contributions to cognitive impairment and dementia (VCID) are one of the leading causes of dementia, where reactive astrogliosis, blood-brain barrier (BBB) disruption, and white matter lesions (WML) are the key features. However, the molecular and cellular mechanisms underlying VCID are not well understood. Na-K-Cl cotransporter 1 (NKCC1) activation via its upstream regulatory kinase SPAK (STE20/SPS1-related proline/alanine-rich kinase) causes intracellular Na+ overload, hypertrophy, and astrogliosis, a cascade that has been implicated in VCID. In this study, we investigated whether t
