Vascular cognitive impairment (VCI) describes cerebrovascular disease (CeVD)-associated cognitive disorders regardless of pathogenesis, ranging from a prodrome to dementia. Heterogeneity in the etiology and severity of CeVD, and significant co-occurrence with Alzheimer's disease (AD) pathology has hampered investigations. Research into VCI is especially relevant in Asia, where cognitive impairment and dementia, often due to VCI, grows due to rapidly aging populations and high prevalence of vascular risk factors. This manuscript reviewed the rationale, unique positioning, design, methodology, a

Vascular cognitive impairment and dementia (VCID) is common in the ageing population. VCID is often the cause of alterations in vascular perfusion or function, reflected for example in strokes, haemorrhages, and small vessel disease. The risk of developing VCID is accelerated by the presence of cardiovascular risk factors like hypertension, hyperlipidemia, diabetes mellitus, smoking, and atrial fibrillation. Symptoms of VCID vary depending on the brain regions affected and the extent of blood flow disruption. Common early signs include slowed thinking, difficulty planning or organizing, troubl

Vascular dementia (VaD) is the second most common cognitive disease, and chronic cerebral hypoperfusion (CCH) is one of the causes of VaD. In CCH, neuroinflammation by immune cells in the brain is a key pathological feature in the development of VaD. However, it is unclear whether the cells contributing to disease pathogenesis during CCH are resident microglia (MG) or infiltrating monocyte-derived macrophages (MΦs) owing to blood-brain barrier disruption. Therefore, we applied bilateral common carotid artery stenosis (BCAS) to Ccr2RFP/+; Cx3cr1GFP/+ mice capable of distinguishing between MG an

Cerebral autosomal dominant arteriopathy with subcortical infarcts and leukoencephalopathy (CADASIL) is caused by mutations in the NOTCH3 gene. We reported a novel mutation in the exon 10 of the NOTCH3 gene (NOTCH3 p.C533S) in a Chinese family with CADASIL. This study aimed to identify NOTCH3 p.C533S-induced CADASIL-like pathogenic mechanisms and to observe the therapeutic effects of edaravone dexborneol (EDB) on CADASIL, which is clinically used for acute ischemic stroke in China. We generated human cerebral microvascular endothelial cells (hCMEC) carrying NOTCH3 p.C533S mutation (NOTCH3 p.C5

The involvement of the oral microbiome (OM) in the pathophysiology of Alzheimer's disease and vascular dementia has been recognized epidemiologically, but the molecular mechanisms remain elusive. In this study, we uncovered the presence of OM-derived proteins (OMdPs) in brain extracellular vesicles (bEVs) from post-mortem Alzheimer's disease and vascular dementia subjects using unbiased metaproteomics. OMdP circulation in blood EVs was also confirmed in an independent cohort. Our findings also reveal that specific OMdPs are present in bEVs, with their levels varying with disease progression. P

The study by Lu et al. highlights progressive structural changes in cerebral small vessel disease, spanning subcortical vascular mild cognitive impairment to vascular dementia. Using voxel-based morphometry, diffusion tensor imaging, and fixel-based analysis, the authors demonstrate converging gray and white matter alterations associated with both cognitive and motor decline. This commentary discusses the significance of their multimodal approach, its clinical implications for early diagnosis and disease monitoring, and the methodological challenges of generalizability and mixed pathology. Retinal vasculopathy with cerebral leukoencephalopathy and systemic manifestations (RVCL-S) is a disease that causes deterioration of small vessels, affecting various organs: eyes, brain, liver, and others. The RVCL-S carriers have lower life expectancy. There is no cure available to date. The disease has been linked to mutations in TREX1 gene disrupting its cytoplasmic localization. To facilitate the disease mechanism investigation, we employed model organism D. melanogaster, identified human TREX1 ortholog cg3165, and confirmed its vital significance to flies. Then, we expressed human TREX1

BACKGROUND: Cerebral small vessel disease can manifest as vascular dementia, typically associated with vascular risk factors or cerebral amyloid angiopathy, and more rarely associated with hereditary conditions. We present a case of late-onset dementia, associated with a likely pathogenic COL4A2 variant.
CASE PRESENTATION: A 74-year-old woman presented with cognitive decline and behavioral changes, without personal history of cerebral infarcts or intracerebral hemorrhage. Brain MRI revealed an extensive leukoencephalopathy affecting periventricular and deep white matter, including subcortical

Chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) is an inflammatory neuropathy, characterized by a slowly progressive onset and symmetrical, sensorimotor involvement. Several diseases, nevertheless, may present similar features as CIDP. It is of paramount importance to recognize these mimics to yield an optimal therapeutic effect. Neuronal intranuclear inclusion disease (NIID) is a rare, progressive neurodegenerative disorder known for its diverse clinical manifestations. Since it may present as sporadic as well as inherited manner and may only involve the peripheral nerve syst

Brain border-associated macrophages (BAMs) are resident immune cells at the border of the central nervous system (CNS), and their physiological functions and roles in neurological diseases have been widely reported. However, the specific mechanisms by which BAMs contribute to vascular cognitive impairment and dementia (VCID) remain unclear. This article systematically reviews the subsets, origin and differentiation, molecular markers of BAMs, and their research progress in various brain diseases such as hypertension, Alzheimer's disease (AD), and stroke. On this basis, this article deeply anal

Dementia is a neurodegenerative condition marked by progressive cognitive decline, which affects people worldwide. Studies on dementia typically continue over years of uncertainty. Different types of dementia, like Alzheimer's disease dementia, Lewy body dementia, frontotemporal dementia, and vascular dementia, exhibit different pathological features, yet their downstream inflammatory pathways involve similar inflammatory mediators. As an initial trigger, microglial cells and astrocytes become activated by protein aggregates, mutations, or any other cause, and release pro-inflammatory cytokine

Cognitive disorders present significant medical and social challenges nowadays, due to their high prevalence, progressive course and a lack of effective methods for treatment of neurodegenerative diseases and comorbid pathologies. An important area of research is the identification of molecular biomarkers that reflect early pathophysiological changes and facilitate a more accurate biological characterization of cognitive impairment. This study provides an overview of the most relevant signaling molecules for diagnosing cognitive disorders. It presents data on the effectiveness of using compreh

OBJECTIVES: While depression is a risk factor for dementia, few studies examining the association between selective serotonin reuptake inhibitors (SSRIs) and dementia risk have controlled for confounding major depression and none to our knowledge have examined neuropathological outcomes. We examined the association between continuation of SSRIs in older adults with a lifetime history of depression and progression of cognitive and functional impairment, depression, and neuropathologic correlates of dementia.
DESIGN: This retrospective cohort study analyzed data from the National Alzheimer's Co

Advances in biomarkers and pathophysiology of vascular contributions to cognitive impairment and dementia (VCID) are expected to bring greater mechanistic insights, more targeted treatments, and potentially disease-modifying therapies. The 2025 Annual Workshop of the Albert Research Institute for White Matter and Cognition, sponsored by the Leo and Anne Albert Charitable Trust since 2015, focused on novel biomarkers for VCID. The meeting highlighted the complexity of dementia, emphasizing that the majority of cases involve multiple brain pathologies, with vascular pathology typically present.

Collagen IV is a basement membrane component that is encoded by six genes in mammals (COL4Α1-COL4A6). The α-chains encoded by these genes assemble into three known heterotrimers - collagen α1α1α2(IV), α3α4α5(IV) and α5α5α6(IV) - that provide structure and act as multifunctional signalling platforms. The ancestral collagen superfamily members collagen alpha-1(IV) chain (COL4Α1) and collagen alpha-2(IV) chain (COL4Α2) are present throughout the animal kingdom and in all developing and most mature mammalian tissues. Consistent with this broad distribution, variants in COL4A1 and COL4A2 cause a co

Vascular dementia (VaD) and post-stroke dementia (PSD) are two leading subtypes of vascular cognitive impairment (VCI), each arising from distinct cerebrovascular pathologies. VaD typically results from chronic cerebral hypoperfusion and small vessel disease, leading to progressive executive dysfunction and white matter degradation. In contrast, PSD occurs following acute ischemic events and is frequently associated with hippocampal damage and episodic memory deficits. This review delineates the pathophysiological divergence between VaD and PSD by integrating findings from human clinical studi

BACKGROUND: Small vessels have the pivotal role for the brain's autoregulation. The arteriosclerosis-dependent alteration of the brain perfusion is one of the major determinants in small vessel disease. Endothelium distress can potentiate the flow dysregulation and lead to subcortical vascular dementia (sVAD). sVAD increases morbidity and disability. Epidemiological studies have shown that sVAD shares with cerebrovascular disease most of the common risk factors. The molecular basis of this pathology remains controversial.
PURPOSE: To detect the possible mechanisms between small vessel disease

Apathy is a frequently overlooked yet disabling neuropsychiatric symptom in dementia syndromes, often misdiagnosed as depression. We describe a diagnostically challenging presentation of subcortical ischaemic vascular dementia in a 48-year-old Middle Eastern man, whose primary symptom was progressive apathy rather than overt cognitive decline or focal neurological deficits. Despite repeated emergency visits and antidepressant trials, his condition deteriorated until detailed neuropsychiatric evaluation and neuroimaging revealed right temporal gliosis/encephalomalacia, left frontal periventricu

JC Polyomavirus is the causative agent of progressive multifocal leukoencephalopathy (PML), an often-fatal demyelinating disease. Unfortunately, a diagnosis of PML occurs only after patients have suffered irreversible neuropathologies. The first step in the initiation of PML is viral entry to the brain, but the route and mechanisms responsible for neuroinvasion have not been well established. To gain a better understanding of this, we asked whether purified virus or virus associated with extracellular vesicles (EVs) could penetrate two different cell culture models of the blood-brain barrier.

Progressive multifocal leukoencephalopathy (PML), an often-fatal neurodegenerative disease, is caused by the neuroinvasive polyomavirus JCPyV. Peripheral organs, including the kidney, are the site of lifelong persistent infections that are asymptomatic. In a subset of immunosuppressed or immunomodulated patients, the virus invades the central nervous system infecting oligodendrocytes, which leads to the multifocal white matter disease known as PML. The mechanisms that lead to neuroinvasion by JCPyV have not been well described. The brain is protected from viruses and other pathogens by physiol
