Dual-center development and validation of a LDL-C and TC based nomogram for preoperative hemorrhagic risk in adult moyamoya disease.

INTRODUCTION: Moyamoya disease (MMD) is a chronic cerebrovascular disease characterized by progressive stenosis of intracranial arteries and compensatory formation of abnormal collateral vessels. Preoperative intracranial hemorrhage represents a severe and catastrophic complication in adult patients with MMD, while reliable strategies for precise risk stratification are still lacking to date. Furthermore, the potential association of lipid metabolic indices with preoperative hemorrhagic risk in MMD remains poorly clarified and insufficiently explored. METHODS: A retrospective multicenter stud

Postoperative Basal Ganglia-Dominant Perfusion Pattern Predicts Lower Risk of Transient Neurological Events After Direct Revascularization in Moyamoya Disease.

BACKGROUND AND PURPOSE: Early postoperative transient neurological events are common after direct revascularization for moyamoya disease (MMD). We investigated whether the redistribution of perfusion between the basal ganglia and cortical territories on early postoperative SPECT is associated with transient neurological events. MATERIALS AND METHODS: We retrospectively analyzed 78 hemispheres from 61 patients with MMD who underwent superficial temporal artery-MCA bypass. N-Isopropyl-p-[123I]iodoamphetamine SPECT was performed within 3 months preoperatively and on postoperative day 1. Relative

AI-Guided Patient-Derived Endothelial Modeling Identifies a Reversible Moyamoya-Specific miRNA Deficiency Corrected by Stem Cell-Extracellular Vesicles.

Moyamoya disease (MMD) is a progressive cerebrovascular disorder characterized by abnormal vascular remodeling and impaired angiogenesis. We profiled plasma extracellular vesicle (EV)-derived microRNAs (miRNAs) from MMD patients and healthy controls and generated patient-specific induced pluripotent stem cell-derived endothelial cells (iPSC-ECs) to investigate disease-associated molecular alterations and therapeutic reversibility. Both plasma EVs and iPSC-ECs from MMD patients showed significant depletion of angiogenesis-related miRNAs. Small RNA sequencing data revealed mesenchymal stem cell-

RNF213 deficiency in human iPSC-derived vascular organoids captures key feature of moyamoya disease vasculopathy.

OBJECTIVES: This study aimed to establish a human iPSC-derived model of Moyamoya disease (MMD) by generating RNF213-knockdown vascular organoids to capture key disease-relevant vascular features. METHODS: RNF213-knockdown iPSC lines were established using lentiviral shRNA and selected with puromycin. Knockdown efficiency was validated by qRT-PCR and Western blot. Vascular organoids were differentiated from the engineered iPSCs using the STEMdiff™ Blood Vessel Organoid Kit according to the manufacturer's protocol. Organoids were analyzed by immunofluorescence for vascular markers (CD31, COL4A1

Late-Onset Moyamoya Disease Presenting With Acute Hemiparesis in a 77-Year-Old Patient: A Case Report.

Moyamoya is a progressive cerebrovascular condition leading to narrowing of the distal internal carotid artery (ICA) and its branches with subsequent development of abnormal collateral vessels around the ischemic brain area. Moyamoya disease is mostly encountered in children and young adults. It is rare in older patients with acute ischemic stroke and requires maintaining a broad differential diagnosis despite the presence of a rare cerebrovascular condition. A 77-year-old woman presented with left-sided weakness outside the thrombolysis window. Imaging revealed moyamoya vasculopathy and an ac

Implications for a Deeper Clinical Understanding of Moyamoya Disease: Temporal Evolution of Suzuki Staging.

BACKGROUND AND OBJECTIVES: Moyamoya disease (MMD) is a rare cerebrovascular disorder discovered in Japan, characterized by progressive internal carotid artery stenosis and fragile collateral vessel formation. Diagnosis relies on imaging, with Suzuki staging commonly used to classify disease severity. There is limited literature on MMD in non-Asian, non-White populations. This study evaluated clinical factors associated with disease presentation, severity, treatment, and progression, with a novel focus on longitudinal changes in Suzuki stage. METHODS: A retrospective cohort study was conducted

The effectiveness of indirect revascularization on haemorrhagic moyamoya disease and the activities that trigger intracranial haemorrhage: a comprehensive study.

PURPOSE: Revascularization surgery is the standard treatment for moyamoya disease (MMD); however, data on the effectiveness of indirect revascularization (IR) in haemorrhagic MMD (hMMD) are limited. This study aims to analyse the effectiveness of IR on hMMD. The activities that triggered the intracranial haemorrhage (ICH) were also identified to prevent the catastrophic haemorrhage in moyamoya patients. MATERIALS AND METHODS: From February 2016 to May 2025, 189 IR procedures were performed on 137 moyamoya patients by a single neurosurgeon. A standardized time-to-peak (TTP) scoring system, ran

The Influence of Dual Revascularization Techniques on Middle Meningeal Artery Collateral Networks in Moyamoya Angiopathy.

BACKGROUND AND OBJECTIVES: Moyamoya disease is characterized by spontaneous and progressive occlusion of the intracranial internal carotid arteries and their proximal branches. Combined surgical revascularization uses a superficial temporal artery to middle cerebral artery direct bypass with concomitant indirect revascularization, most commonly encephaloduroarteriosynangiosis. The role and evolution of collaterals, particularly from the middle meningeal artery (MMA), is incompletely understood in combined revascularization. METHODS: Patients undergoing combined revascularization for Moyamoya

Moyamoya disease and angiogenesis: a quantitative analysis of key angiogenic markers.

Moyamoya disease (MMD) is a cerebrovascular condition characterized by progressive narrowing of the internal carotid arteries and their adjacent branches. Aberrant angiogenesis plays a crucial role in its pathogenesis, contributing to both ischemic and hemorrhagic complications. This study aimed to identify potential biomarkers with prognostic value by investigating angiogenic markers in MMD patients. Paired blood and dura mater tissue samples were collected from MMD patients and healthy controls. Using quantitative reverse transcription-polymerase chain reaction, we analysed the expression le

ALIGNED Network for rare cerebrovascular diseases: methodology and preliminary results.

INTRODUCTION: Cerebrovascular diseases are a major cause of morbidity/mortality worldwide, yet more than 30% of strokes remain of undetermined origin. Rare cerebrovascular diseases (rCVDs) contribute to this burden but are often underdiagnosed due to limited awareness, clinical heterogeneity, and fragmented diagnostic access. The ALIGNED project was established to address these gaps through a nationwide multidisciplinary network. METHODS: ALIGNED was aimed to improve clinical/molecular characterization of rCVDs through standardized data collection, assess level of care of rCVDs by an online/o

LPIN3 emerges as a diagnostic biomarker in Moyamoya disease revealing immune-lipid metabolic crosstalk.

BACKGROUND: Moyamoya disease (MMD) is a progressive cerebrovascular disorder characterized by stenosis or occlusion of the terminal portions of the internal carotid arteries and their proximal branches, accompanied by the formation of abnormal collateral vessel networks. It represents a leading cause of ischemic and hemorrhagic stroke in both pediatric and adult populations. However, a comprehensive understanding of the molecular drivers underlying the hallmark vascular pathology of MMD remains elusive. Emerging evidence indicates that dysregulated lipid metabolism significantly contributes to
