Indirect revascularization in Moyamoya vasculopathy: clinical and radiological outcomes of multiple burr hole surgery.

PURPOSE: Moyamoya vasculopathy is a progressive cerebrovascular disease characterized by stenosis of the intracranial internal carotid arteries, leading to chronic cerebral ischemia. Indirect revascularization techniques, such as multiple burr hole surgery, aim to promote collateral vessel formation and improve cerebral perfusion. This study evaluates the clinical and radiological outcomes of multiple burr hole surgery in a diverse Belgian patient population with Moyamoya vasculopathy. METHODS: A retrospective single-center cohort study was conducted, including 15 patients (22 hemispheres) who

Hybrid surgical and endovascular approach for bilateral symptomatic moyamoya disease: a single-center preliminary experience.

BACKGROUND: Bilateral ischemic manifestations in Moyamoya disease (MMD) pose substantial therapeutic challenges, particularly in cases characterized by heterogeneous cerebrovascular involvement where one hemisphere presents with complete occlusion while the contralateral side exhibits severe stenosis. Contemporary treatment paradigms frequently prove inadequate in addressing this complex clinical scenario, often failing to achieve optimal revascularization outcomes and long-term symptom management. OBJECTIVE: To perform a preliminary evaluation of the efficacy and safety of a hybrid surgical

Focal Cerebral Arteriopathy Severity Score Validation and Temporal Dynamics in Korean Pediatric Stroke: Distinguishing Inflammatory Arteriopathy From Unilateral Moyamoya Disease.

BACKGROUND: Focal cerebral arteriopathy-inflammatory type (FCA-i) is a leading cause of pediatric arterial ischemic stroke, but diagnostic challenges persist, particularly in East Asian populations where moyamoya disease (MMD) prevalence is high. The focal cerebral arteriopathy severity score quantifies arteriopathy severity but has not been validated in East Asian cohorts. We aimed to validate the focal cerebral arteriopathy severity score in Korean pediatric patients with FCA-i and compare temporal progression patterns with unilateral MMD. METHODS: We conducted a retrospective cohort study

Establishment and external validation of prediction models for risk factors of cerebral hyperperfusion syndrome after combined revascularization in moyamoya disease based on meta-analysis and retrospective cohort studies.

OBJECTIVE: Cerebral hyperperfusion syndrome (CHS) is a complication affecting up to 50% of moyamoya disease (MMD) patients after combined revascularization. The aim of this study was to identify reliable CHS risk factors and develop predictive models. METHODS: The authors performed a meta-analysis including studies on both combined and direct revascularization to capture potential risk factors, followed by a sensitivity analysis on the combined bypass group. They also performed a retrospective cohort analysis. Then three models were developed using the derivation cohort and externally validat

The research of subdural effusion collection complicating cerebral revascularization following ventriculoperitoneal shunting in adults with hemorrhagic moyamoya disease: A retrospective case series.

Ventriculoperitoneal shunting (VPS) helps reduce intracranial pressure and alleviate clinical symptoms caused by hydrocephalus in hemorrhagic Moyamoya disease (MMD). To date, no literature describes the occurrence of subdural fluid collection (SDFC) in hemorrhagic MMD patients undergoing VPS prior to cerebral revascularization. This report aims to explore the potential pathological mechanisms underlying SDFC following cerebral revascularization after prior VPS, and to provide effective strategies for future prevention. Clinical data of hemorrhagic MMD patients undergoing VPS prior to bypass ad

Extracranial-to-intracranial conversion: postoperative vascular remodeling in moyamoya disease after revascularization.

OBJECTIVE: Effective revascularization in moyamoya disease (MMD) entails not only the formation of extracranial-intracranial (EC-IC) collaterals but also a postoperative shift in perfusion responsibilities-from intracranial arteries to newly established extracranial pathways. This dynamic process, characterized by extracranial collateral ingrowth (EC-Ingrowth) and regression of intracranial arterial supply (IC-Regression), represents a redistribution of cerebral perfusion we define as EC-IC conversion. However, its extent and determining factors remain poorly understood. This study aimed to qu

Salvage STA-MMA anastomosis for intraoperative middle meningeal artery in moyamoya disease: a technical note.

BACKGROUND: In numerous cases of Moyamoya disease (MMD), the middle meningeal artery (MMA) develops as a spontaneous collateral prior to surgery, providing an extracranial-intracranial compensatory pathway. However, the MMA is vulnerable to iatrogenic injury during surgery. There is a lack of literature regarding managing iatrogenic injuries to the MMA which has collateral supply to the cerebral hemisphere. CASE DESCRIPTION: We report a case of a 61-year-old woman with MMD with collateral flows from the MMA. During a right-sided direct revascularization surgery, the MMA was inadvertently rupt

Long-term outcomes in the pre-moyamoya stage of RNF213-related vasculopathy presenting with ischaemic stroke or transient ischaemic attack: insights from the NCVC Genome Registry.

BACKGROUND: RNF213 is a susceptibility gene for moyamoya disease, with the p.R4810K variant being the founder mutation. However, long-term outcomes in non-moyamoya stage of RNF213-related vasculopathy remain poorly characterised in longitudinal studies. METHODS: This multicentre, prospective, longitudinal study was conducted from March 2017 to March 2022. It included patients with ischaemic stroke (IS) or transient ischaemic attack (TIA) within one week of onset. Patients with moyamoya disease were excluded. The primary outcome was recurrent symptomatic IS or TIA. Subgroup analyses with Essen

Superficial temporal artery dilation and the risk of transient neurological events after revascularization for moyamoya disease: Impact of preoperative PCA stenosis.

OBJECTIVE: The superficial temporal artery (STA)-to-middle cerebral artery (MCA) anastomosis is an established revascularization procedure for moyamoya disease (MMD), although transient neurological events (TNEs) are occasionally observed postoperatively. Postoperative dilation of the STA, reflecting the development of bypass blood flow, has been proposed as one of the potential causes of TNEs. However, no definitive biomarker has yet been identified to predict which cases will exhibit marked STA enlargement following revascularization for MMD. METHODS: This study analyzed 40 hemispheres in 2

Critical Care Nursing in Moyamoya Disease: Complexities and Considerations.

Moyamoya disease is a rare progressive cerebrovascular disorder involving stenosis of major intracranial arteries and the formation of fragile collateral vessels. Patients face lifelong risks of ischemic and hemorrhagic stroke. Surgical revascularization is the definitive treatment, aimed at restoring cerebral perfusion and reducing neurologic injury. Postoperative care is among the most complex in neuroscience nursing. Critical care nurses play a vital role in managing cerebral perfusion, conducting frequent neuro assessments, regulating blood pressure, and preventing seizures. Success requir

Non-Atherosclerotic and Genetic Intracranial Stenoses: Diagnostic Challenges and Emerging Genetic Insights.

Intracranial arterial stenosis is a major cause of ischemic stroke worldwide. It is particularly prevalent among Asian populations. Intracranial arterial stenosis was traditionally considered as a manifestation of atherosclerotic disease. Subsequent research has documented it as a heterogeneous group of vasculopathies with diverse underlying mechanisms. Advanced high-resolution vessel wall imaging and genetic studies have revealed that non-atherosclerotic intracranial arteriopathies occur more commonly than previously recognized. In addition, these arteriopathies often exhibit overlapping clin

Dystonia as the Initial Manifestation of Moyamoya Disease: A Case Report and Literature Review.

INTRODUCTION: Moyamoya disease (MMD) is a progressive cerebrovascular disorder characterized by progressive narrowing of large intracranial arteries around the circle of Willis and the secondary development of collateral "moyamoya vessels." It typically presents with ischemic or hemorrhagic events, whereas movement disorders, particularly dystonia, are exceedingly rare initial manifestations. CASE REPORTS: We report two adult cases of MMD initially presenting with hemidystonia. The first case involved a 27-year-old man with recurrent involuntary movements of the left limbs. Imaging revealed b

Association between posterior cerebral artery involvement and arterial wall enhancement in moyamoya disease: a cross-sectional study with vessel wall imaging.

PURPOSE: Early detection and activity assessment of posterior cerebral artery (PCA) involvement in moyamoya disease (MMD) are challenging. We aimed to assess the cross-sectional association between PCA involvement and PCA wall enhancement on vessel wall MRI (DANTE T1-SPACE). METHODS: We analyzed consecutive patients with MMD who underwent both non-contrast-enhanced and contrast-enhanced DANTE T1-SPACE. Positive wall enhancement was defined as location-appropriate arterial wall enhancement with an enhancement ratio ≥ 1.5. PCA involvement was graded 0-2 on MRA obtained during the same period.

Direct Versus Indirect Bypass in Early-Stage Moyamoya (Suzuki I-III): A Propensity Score-Weighted Study.

Moyamoya angiopathy (MMA) lacks stage-specific comparative evidence for surgical strategy. Because many studies mix Suzuki grades, potential technique effects may be obscured. We compared direct revascularization (DR) with indirect revascularization (IR) in a stage-restricted cohort (Suzuki I-III), using propensity score weighting (PSW). We conducted a multicenter retrospective cohort study across 13 academic centers. Adults with confirmed MMA (Suzuki I-III) who underwent DR or IR were included. Patients < 16 years and combined procedures were excluded. Outcomes were symptomatic stroke, overal

Surgical revascularization versus nonsurgical management in children with symptomatic moyamoya arteriopathy: a North American multicenter cohort study.

OBJECTIVE: Moyamoya arteriopathy is a leading cause of cerebral ischemia in children. In pediatric patients with symptomatic or progressive moyamoya disease, surgical revascularization is the mainstay of treatment; however, there is limited comparative evidence demonstrating an advantage of surgical revascularization over nonsurgical management in North American cohorts. The authors aimed to determine whether surgery reduces the risk of subsequent ischemic events in North American children with symptomatic ischemic moyamoya arteriopathy compared to nonsurgical management alone. METHODS: Patie
