Manual volumetric segmentation of brain hyperintensities on T2-weighted imaging (T2WI) and fluid-attenuated inversion recovery (FLAIR) sequences in children with neurofibromatosis type 1: inter-rater reliability and clinical correlates.

BACKGROUND: Brain hyperintensities are frequently observed on T2-weighted imaging (T2WI) and fluid-attenuated inversion recovery (FLAIR) sequences of children with neurofibromatosis type 1 (NF1). This study aimed to quantitatively analyze the burden of brain hyperintensities in a large pediatric cohort of patients with NF1 using T2WI and FLAIR sequences, assess the inter-rater reliability of manual volumetric segmentation, and explore the relationship between hyperintensity volume and clinical correlates. METHODS: A retrospective cross-sectional study was performed on the clinical and whole-b

Ischemic territorial mismatch due to posterior cerebral artery occlusion illustrated by the FLAIR ivy sign in moyamoya disease.

Cerebral infarctions in moyamoya disease frequently do not conform to anatomical arterial territories. This phenomenon is closely associated with delayed steno-occlusive involvement of the posterior cerebral artery (PCA) following progression of internal carotid artery disease, which compromises PCA-derived leptomeningeal collateral circulation that had previously supported the anterior circulation. Although PCA involvement is common in moyamoya disease, the hierarchical vulnerability of PCA-derived collateral pathways and their relationship to the fluid-attenuated inversion recovery (FLAIR) i

Clinical importance of the persistent primitive hypoglossal artery in vascular lesions and endovascular treatment: a narrative review.

The persistent primitive hypoglossal artery (PPHA) is a rare and anatomically complex cerebrovascular variant. Although typically asymptomatic, its presence is associated with various cerebrovascular pathologies, including intracranial aneurysms, carotid artery stenosis, acute large vessel occlusion, moyamoya disease, brain arteriovenous malformations, and other vascular anomalies. When such lesions involve the PPHA, therapeutic intervention may be necessitated. Surgical management is particularly challenging due to the artery's deep anatomical location and intricate surrounding vasculature. C

Acetazolamide-challenged ASL shows comparable performance to CT perfusion for angiographic outcomes in moyamoya angiopathy.

BACKGROUND: Moyamoya angiopathy is a chronic steno-occlusive cerebrovascular disorder in which assessment of cerebral hemodynamics, particularly cerebrovascular reserve (CVR), is essential for risk stratification and surgical planning. Because PET is not widely available and CT perfusion (CTP) involves radiation and contrast exposure, arterial spin labeling (ASL) MRI with an acetazolamide challenge may offer a noninvasive alternative. We evaluated the ability of ASL-derived CBF and CVR to predict angiographic severity and compared ASL- and CTP-based predictive models. METHODS: In this single-

Experimental Approach to Moyamoya Angiopathy: Insights into Vascular Cell Crosstalk.

BACKGROUND: The pathophysiological mechanisms of Moyamoya angiopathy (MA) are still largely unknown, although a dysfunctional vasculogenesis has been hypothesized to contribute to it. The association between this rare cerebrovascular condition and variants of Ring Finger Protein 213 (RNF213) strengthens the role of genetic factors in MA pathogenesis. METHODS: To investigate the molecular mechanisms of MA, we carried out RNA interference (RNAi) targeting RNF213 in human endothelial cells (ECs) and vascular smooth muscle cells (VSMCs). The combined effect of RNAi and/or hypoxia on expression of

An Overview of Genetics of Moyamoya: Beyond RNF213 Gene.

Moyamoya angiopathy (MMA) is a rare, chronic progressive cerebrovascular condition characterized by bilateral stenosis or occlusion of the terminal internal carotid arteries and their major branches. This progressive occlusion triggers the development of telangiectatic and fragile vessels at the base of the brain, creating the characteristic angiographic appearance of a "puff of smoke." Depending on the etiology, MMA is classified as Moyamoya Disease (MMD) when idiopathic and primary or Moyamoya Syndrome (MMS) when associated with underlying systemic conditions. While the RNF213 gene, particul

Surgical Treatment of an Aneurysm in the Subacute Stage of Hemorrhagic Moyamoya Disease: Aneurysm Resection Combined With STA-MCA Bypass: A Case Report.

INTRODUCTION: There is no consensus on the optimal management of intracranial aneurysms in patients with moyamoya disease. Here, we report a case of a patient in the subacute hemorrhagic stage of moyamoya disease with a presumed pseudoaneurysm detected on interval imaging, who was treated with aneurysm resection combined with superficial temporal artery (STA)-middle cerebral artery (MCA) bypass. This report illustrates a feasible individualized surgical approach for similar cases. CASE REPORT: A 27-year-old male without a prior history of cerebrovascular disease was admitted for a right tempo

Encephaloduroarteriosynangiosis effectively reduces rebleeding risk in adults with hemorrhagic moyamoya disease: A long-term ambidirectional cohort study.

This cohort study was conducted using prospective and retrospective data to investigate whether indirect encephaloduroarteriosynangiosis (EDAS) revascularization can reduce the risk of rebleeding in adult patients with hemorrhagic moyamoya disease (hMMD). We enrolled adult patients diagnosed with hMMD prospectively and retrospectively. All patients voluntarily chose to undergo EDAS or conservative treatment. The clinical features, angiographic findings, and follow-up outcomes were investigated and analyzed using the Kaplan-Meier survival analysis and Cox proportional hazards regression models

Investigating the Genetic and Inflammatory Mechanisms Behind Moyamoya Disease: A Literature Review.

INTRODUCTION: While a plethora of studies exist exploring the diverse components of MMD, a multifaceted approach is needed to target new therapeutic interventions and provide a holistic understanding of the genetic and inflammatory components of MMD etiology. METHODS: A literature review was conducted using PubMed, Embase, and the Cochrane Library, ranging from genetic predispositions in specific populations, the p.R4810K variant in the RNF213 gene, animal models, other genetic variants, and inflammatory pathways such as cytokines and biomarkers. The search encompassed studies up to January 2

A parameterized gamma-variate function can describe the temporal response of cerebral blood flow after Acetazolamide injection in Moyamoya patients.

BACKGROUND: Quantitative modelling of the temporal response of cerebral blood flow (CBF) following Acetazolamide (ACZ) may be performed using a parameterized gamma-variate function with physiologically interpretable parameters. When combined with sequential CBF measurement before and after administration of ACZ, such a model could enable assessment of cerebrovascular reserve (CVR) while accounting for regional differences in the timing of the vascular response between steno-occlusive and unaffected territories. To evaluate this approach, we applied the model to patients with moyamoya disease (

Intracranial Arterial Stenosis beyond Atherosclerosis: Insights from RNF213.

Intracranial arterial stenosis is a major cause of ischemic stroke worldwide, particularly in the Asian population. Although traditionally regarded as a manifestation of atherosclerotic disease, increasing evidence suggests that non-atherosclerotic arteriopathies also contribute substantially to its pathogenesis. These conditions, including arterial dissection, inflammatory vasculopathies, and moyamoya disease, often share overlapping clinical and imaging features, thus making precise differentiation challenging in routine practice. This phenotypic overlap highlights the heterogeneity of intrac

Harry J. Buncke to M. Gazi Yaşargil: a historical lesson in cross-specialty microsurgical innovation.

This historical review examines Dr. Harry J. Buncke's development of microsurgical instrumentation and anastomotic techniques for submillimeter vessels, discusses the 1966 Vermont Microvascular Surgery Conference where technical knowledge was exchanged between plastic surgeons and neurosurgeons, and highlights how Buncke's methods directly informed Dr. M. Gazi Yaşargil's cerebral revascularization procedures. Buncke's innovations in microsuture technology, jeweler's tool adaptation, vasospasm control protocols, and vessel repair techniques for arteries < 1 mm in diameter, demonstrated through

Anatomy of the bony canal containing the middle meningeal artery for a vessel-sparing craniotomy in moyamoya disease.

OBJECTIVE: The middle meningeal artery (MMA) often traverses a bony canal, rendering it vulnerable during craniotomy. In moyamoya disease (MMD), it serves as a collateral pathway for cerebral perfusion, making vessel preservation essential during bypass surgery. This study examined the anatomy of MMA bony canals in dry skulls and compared findings in patients with MMD and atherosclerotic cerebrovascular disease (ACVD). METHODS: A total of 175 adult and pediatric dry skulls and cranial CT scans of MMD (n = 100) and ACVD (n = 100) patients were analyzed retrospectively. The relationship of bony

Neuroplastic and Augmented Reality Integration in Moyamoya Revascularization: A Reconstructive Microsurgical Paradigm.

Cerebrovascular revascularization for moyamoya disease remains one of the most technically demanding procedures in neurosurgery, requiring meticulous microvascular anastomosis and precise intraoperative execution. The success of these operations depends on advanced microsurgical skill, careful teamwork, and refined perioperative planning. This review outlines the emerging neuroplastic approach-a collaborative model between neurosurgeons and plastic microsurgeons-and its integration with augmented reality (AR) technologies to enhance surgical precision, planning, and education. Extracranial vascular association of moyamoya disease: a systematic review and machine learning analysis.

BACKGROUND: Intracranial steno-occlusive lesions are characteristic of moyamoya disease (MMD), but increasing reports of extracranial vascular involvement suggest a possible systemic arteriopathy. METHODS: We conducted a PRISMA-guided systematic review (1974-May 2025) to identify extracranial vascular involvement in classical MMD. Seventy-four studies were included. Because the search was limited to English-language publications, language bias may be present. A subset of 85 patients from 46 studies was used for exploratory multilabel machine-learning and deep-learning modeling based on clinic
