BACKGROUND: Cluster headache is a primary trigeminal autonomic cephalalgia characterized by severe, strictly unilateral orbital/periorbital pain with ipsilateral autonomic features. It is rare for a patient with cluster headache to have coexisting multiple headache disorders and experience unusual triggers.
CASE PRESENTATION: A 53-year-old male with a 14-year history of episodic cluster headache presented with his third cluster episode. He exhibited strictly unilateral left orbital/periorbital/temporal drilling pain [Visual Analogue Scale (VAS) 9-10/10], ipsilateral conjunctival injection, la

Takayasu arteritis (TA) is a chronic, granulomatous large-vessel vasculitis affecting the aorta and its branches. This report describes a 33-year-old woman with longstanding, unsupervised disease who developed extensive collateral revascularization, including rare coronary-carotid and coronary-vertebral pathways. Coronary angiography revealed normal epicardial coronary arteries but demonstrated collateral filling of the carotid system from the left coronary circulation. Computed tomography angiography identified complete occlusion of all aortic arch branches and extensive systemic, thoracic, p

Takayasu arteritis (TAK) is a chronic, refractory large-vessel vasculitis designated as a Designated Intractable Disease in Japan. The Japanese diagnostic criteria for TAK, last revised in 2017, have been updated to the 2026 version as part of the Japanese Circulation Society 2026 guidelines on the management of large vessel vasculitis, and are presented here in English for the first time. The revision was conducted through a 3-round modified Delphi process involving a panel of 21 experts, supplemented by email discussion and an in-person meeting. The updated criteria incorporate 3 major chang

Takayasu arteritis (TA) is a rare chronic inflammatory disease affecting large vessels that can result in stenosis, occlusion, and aneurysm formation. Although not a contraindication, TA poses significant anesthetic challenges in pregnancy. We describe the successful anesthetic management of a patient with TA complicated by aortic root dilation, abdominal aortic aneurysm, bilateral subclavian artery, and bilateral carotid artery stenosis undergoing elective cesarean delivery at 37 weeks. To minimize hemodynamic fluctuations, we used a slowly titrated dural puncture epidural technique along wit

OBJECTIVE: The natural history of intracranial arterial stenosis caused by giant cell arteritis (GCA) and the effects of therapeutic interventions remain unclear. We report a case of rapidly progressive intracranial arterial stenosis associated with GCA and discuss its clinical course and the implications for endovascular management.
CASE PRESENTATION: A 75-year-old man developed multiple intracranial arterial stenoses and occlusions over several months, resulting in cerebral infarction. Percutaneous transluminal angioplasty was performed for severe stenosis of the cavernous segment of the ri

BACKGROUND Giant cell arteritis (GCA) is the most common large-vessel vasculitis in individuals over age 50 years. Although it typically affects extracranial branches of the carotid artery, central nervous system involvement is rare and can manifest with ischemic stroke. Links between GCA and malignancy have been reported, especially hematologic cancers, but paraneoplastic GCA associated with solid tumors remains exceptional and poorly understood. CASE REPORT We describe a 59-year-old man presenting with temporal headaches, right-hand paresthesia, monocular visual loss, and gait instability. I

Within the population over age 50, non-arteritic anterior ischemic optic neuropathy (NAION) represents a primary etiology for acute, unilateral visual impairment. It shares common features with conditions like optic neuritis and papilledema, making accurate diagnosis difficult. A 61-year-old woman came to AKFA Medline University Hospital with sudden, painless vision loss in her left eye that began four days prior. Her medical history included high blood pressure. An eye examination revealed swelling of the optic disc with blurred edges and decreased vision in her left eye. Evaluation via optic

Concurrent headache and visual symptoms pose a diagnostic challenge across specialties, ranging from benign to life-threatening etiologies. Overlapping manifestations and terminological confusion often lead to misdiagnosis and delayed care. This narrative review searched eight databases from inception to May 5th, 2026, for articles on headache and visual symptoms. Extracted data were synthesized into a structured diagnostic framework. Key benchmarks were clarified across seven categories. Retinal migraine and migraine with aura are distinguished by monocular/binocular involvement, duration, an

Acute retinal artery occlusions present with sudden painless monocular vision loss and are homologous to cerebral ischemic strokes. The diagnosis is made on ocular fundus examination facilitated by fundus photography and optical coherence tomography. In the acute setting, patients should undergo a standard stroke evaluation, in addition to workup for possible giant cell arteritis in patients aged 50 years and older. Recent meta-analyses of observational studies have suggested a potential positive treatment effect for intravenous tPA when delivered within 4.5 hours of symptom onset. Analyses of

OBJECTIVES: Acute ischemic events (AIE) constitute life-threatening complications of Takayasu arteritis (TAK). This study aimed to identify vascular, inflammatory and immune factors associated with concurrent AIE in TAK patients.
METHODS: We conducted a 10-year retrospective 1:2 matched case-control study including TAK patients without exposure to glucocorticoids, immunosuppressants, or antithrombotic agents for at least 3 months. Clinical characteristics, disease activity, inflammatory markers, and lymphocyte subsets were compared between patients with and without AIE. Three sequential multi

Retinal artery occlusions (RAOs) constitute a rare but serious ophthalmological emergency with a poor prognosis for visual recovery, for which there is currently no adequate therapeutic strategy. However, there is substantial evidence highlighting the importance of systemic (non-ophthalmological) management of patients with RAO. Since RAOs are the ocular analogous of a cerebral stroke, they require a similar urgent, multidisciplinary approach involving laboratory tests and imaging studies that go beyond the typical scope of ophthalmologists. This calls for cooperation and understanding from al

A 77-year-old woman with a history of palindromic rheumatism and a previous episode of steroid-induced psychosis was treated with high-dose prednisolone for suspected giant cell arteritis because of the risk of irreversible visual loss. Within days, she developed recurrent psychotic symptoms requiring hospital admission. During her admission, she experienced profound hypovolaemic hyponatraemia complicated by a generalized tonic-clonic seizure. Following correction of her sodium level, brain magnetic resonance imaging unexpectedly demonstrated an acute left basal ganglia lacunar infarction desp

This case report describes a fatal case of rhinocerebral mucormycosis and Aspergillus flavus coinfection in a 63-year-old Saudi male with poorly controlled Type II diabetes and hypertension. The patient presented with a 2-day history of rapid right-sided facial swelling, blurred vision, and proptosis. Clinical examination revealed necrotic nasal mucosa and ophthalmoplegia, while laboratory tests showed critical hyperglycemia (RBS 500 mg/dL) and severe leukocytosis. CT and MRI imaging confirmed extensive invasive fungal sinusitis with spread into the right cavernous sinus and involvement of the

A diabetic woman in her 40s presented with a subacute onset of a left temporal headache with no other symptoms, especially no visual or nasal symptoms. An initial diagnosis of giant cell arteritis was made due to a raised erythrocyte sedimentation rate (ESR) and the presence of jaw claudication; however, the temporal artery biopsy was negative. Corticosteroid therapy was commenced, but subsequent computed tomography (CT) imaging of the face revealed extensive left maxillary sinus disease with bony erosion of the greater palatine and vidian nerve canals. The ear, nose, and throat (ENT) examinat

BACKGROUND: Disseminated mucormycosis is a rare, angioinvasive fungal infection predominantly affecting immunocompromised individuals, especially those with poorly controlled diabetes.
CASE PRESENTATION: A 57-year-old Palestinian woman presented with sudden unilateral blindness, ophthalmoplegia, facial numbness, and ptosis, with hyperglycemia. Early brain MRI/MRA/MRV findings were normal, suggesting that giant cell arteritis, or other autoimmune vasculitis, was suspected; thus, IV steroids were administered. Subsequently, her condition progressed to bilateral fixed pupils, complete ophthalmop

BACKGROUND: Prosthetic valve-related complications are well recognized in Takayasu arteritis (TAK). However, migration of a prosthetic aortic valve into the left ventricular outflow tract (LVOT) is exceedingly rare.
CASE SUMMARY: We report 2 patients with TAK who experienced downward migration of the aortic prosthetic valve into the LVOT after aortic valve replacement, without any evident paravalvular leak. In patient 2, progressive valve migration led to mechanical interference with the anterior mitral leaflet, resulting in hemodynamics similar to those of mitral stenosis. In both cases, val

BACKGROUND: Giant cell arteritis (GCA) is a systemic vasculitis of medium- and large-sized arteries, most commonly presenting with headache, scalp tenderness, and, if untreated, can lead to visual loss from ischemic optic neuropathy. Although the most common ophthalmic manifestations of Giant Cell Arteritis are related to ocular ischemia, orbital involvement can occur and remains underrecognized. Fewer than 10 cases in the literature describe perioptic or retrobulbar infiltration, underscoring the uniqueness and diagnostic challenge of such presentations.

INTRODUCTION: Ischemic stroke in young adults is uncommon and is frequently associated with rare etiologies, including autoimmune diseases and vasculitis. Takayasu arteritis (TA) is a chronic inflammatory large-vessel arteriopathy involving the aorta and its major branches and may result in cerebral ischemia due to arterial stenosis or thrombosis.
CASE PRESENTATION: We report the case of a 26-year-old woman with a history of suspected rheumatoid arthritis and Lyme disease who presented with acute left-sided hemiparesis and dysarthria. At admission, large-vessel vasculitis had not yet been sus
