Reversible visual loss in biopsy-proven giant cell arteritis.

A late 60s man presented with a 1-week history of recurrent transient monocular visual loss in his left eye, followed by sudden, severe visual loss and eyelid pain. Examination revealed a left relative afferent pupillary defect and segmental chalky-white oedema of the superior half of the optic disc. During admission, he experienced repeated episodes of no light perception in the left eye. A systematic review revealed jaw claudication, low-grade fever and scalp tenderness. Inflammatory markers were elevated and temporal artery ultrasound showed vessel wall thickening and stenosis. Extensive wo

Tabetic Arthropathy of the Knee: A Case Series of Three Patients.

Tabetic arthropathy is a destructive neurogenic joint disorder occurring as a late manifestation of tertiary syphilis. Although it has become exceedingly rare owing to early and appropriate treatment of syphilis, it remains relevant in developing countries and poses significant management challenges. We report three cases of tabetic arthropathy diagnosed in our department. A retrospective analysis was conducted on three cases of tabetic arthropathy diagnosed in our department between January 2022 and December 2025. Our series included three patients (n=3) with a mean age of 66 years and a male

Lyell´s syndrome associated with the use of topical agents: case report.

UNLABELLED: Pseudomonas aeruginosa AG1 (PaeAG1) is a Costa Rican strain that was isolated in 2010 in a major Hospital. This strain has resistance to multiple antibiotics such as β-lactams (including carbapenems), aminoglycosides, and fluoroquinolones. PaeAG1 is considered critical (Priority 1) due to its resistance to carbapenems, and it was the first report of a P. aeruginosa isolate carrying both VIM-2 and IMP-18 genes encoding for metallo-β-lactamases (MBL) enzymes (both with carbapenemase activity). Owing to these traits, we have studied this model for 10 years using diverse approaches inc

Cerebral Arteritis Caused by a Streptococcus Anginosus Group Infection Leading to Sequential Cerebral Infarction and Intracerebral Hemorrhage.

A 55-year-old man was admitted with a right frontal headache and low-grade fever. The initial neuroimaging and cerebrospinal fluid findings were normal, and giant cell arteritis was suspected. After administering prednisolone therapy, he developed a cerebral infarction and multiple cerebral arterial stenoses. Blood cultures were positive for the Streptococcus anginosus group, and the cerebrospinal fluid findings were consistent with bacterial meningitis. Echocardiography revealed no valvular vegetations. Although antibiotic therapy improved the patient's symptoms, intracerebral hemorrhage deve

Fulminant bilateral blindness despite initially normal neuroimaging in rhinocerebral mucormycosis.

BACKGROUND: Rhinocerebral mucormycosis is a rapidly progressive and frequently fatal invasive fungal infection that most commonly occurs in patients with uncontrolled diabetes mellitus in the setting of diabetic ketoacidosis. CASE PRESENTATION: We present a woman in her 70 s with diabetic ketoacidosis, who developed right-sided facial numbness and ptosis, followed by acute, complete bilateral vision loss within hours. Initial cranial CT and diffusion MRI were unremarkable despite rapidly evolving cranial neuropathies. This created significant diagnostic uncertainty and prompted consideration

Balamuthia mandrillaris presenting as central nervous system vasculitis in a young child.

Balamuthia mandrillaris is a rare and difficult-to-diagnose infection with high mortality. We present the case of an immunocompetent toddler presenting with central nervous system (CNS) vasculitis. He was initially diagnosed with Takayasu arteritis and had an initial period of improvement following immunosuppressive therapy. Subsequently, he had rapid decompensation with diffuse intracranial lesions progressing to severe neurological injury and compassionate extubation. Metagenomic sequencing of the CSF and pathology and PCR from brain biopsy were positive for B. mandrillaris To our knowledge,

Mapping historical diaries into knowledge graphs: a multi-granular approach.

PLAIN ENGLISH SUMMARY: There are new ways to engage people with science and research but many patient support groups and charitable organisations still hold traditional meetings to provide updates on their activities and to report new developments in their field of interest. These meetings often feature presentations given by medical doctors or, in the case of research-focussed organisations, by research scientists.Receiving feedback from people who are confused and sometimes upset by some types of information, and the way it is presented at meetings, made us think about better ways for resear

High-resolution magnetic resonance imaging visualizes intracranial large artery involvement in giant cell arteritis.

OBJECTIVE: Giant cell arteritis (GCA) is a large vessel vasculitis, typically involving the aorta and its branches with predilection for the scalp arteries. Intracranial involvement is still part of ongoing research. We assessed inflammation of the intracranial arteries on 3D compressed sensing black-blood MRI (3D-CS-BB-MRI) in patients with GCA and age-matched controls. METHODS: One hundred and five patients with 3D-CS-BB-MRI of the brain were included in this retrospective dual-centre case-control study, 55 with diagnosed GCA and 50 age-matched controls. High-resolution 3D-CS-BB-MRI was per

A 51-Year-Old Man With Dyspnea and a Pulmonary Nodule.

A 51-year-old man presented with chest tightness, exertional dyspnea, and occasional chest pain for 2 years. The patient visited his local hospital initially, and CT scan revealed a ground glass opacity (GGO) located in the right upper lobe (Fig 1A). He was diagnosed as having pulmonary infection and treated with levofloxacin for 12 days. A repeated chest CT scan 14 days later demonstrated a progressed solid nodule with surrounding ground glass opacity (Fig 1B). With a suspicion of carcinoma in situ, right upper lobectomy was performed via video-assisted thoracoscopic surgery at the local hosp

Risk Factors and Pharmacological Interventions Impacting Cerebrovascular Ischemic Events in Giant Cell Arteritis: A Narrative Review.

INTRODUCTION: Giant cell arteritis (GCA) is a common vasculitis predominantly affecting larger vessels, especially in individuals aged 70-79. Cerebrovascular ischemic events (CIE), such as stroke and transient ischemic attacks, are serious but rare complications of GCA, with a pooled prevalence of 4%. Some studies found that within 2 weeks of GCA diagnosis, 74% and 34% of patients experience transient or severe ischemic events, respectively. AIMS: Our study aims to help physicians better manage GCA patients to reduce GCA-related CIE by indicating important risk factors and pharmacological int

Age, anticoagulants, hypertension and cardiovascular genetic traits predict cranial ischaemic complications in patients with giant cell arteritis.

OBJECTIVES: This project aimed to determine whether cranial ischaemic complications at the presentation of giant cell arteritis (GCA) were associated with pre-existing cardiovascular (CV) risk factors, CV disease or genetic risk of CV-related traits. METHODS: 1946 GCA patients with clinicodemographic data at GCA presentation were included. Associations between pre-existing CV-related traits (including Polygenic Risk Scores (PRS) for CV traits) and cranial ischaemic complications were tested. A model for cranial ischaemic complications was optimised using an elastic net approach. Positional ge

Vertebral artery involvement in giant cell arteritis: Symptoms, treatment and outcome.

BACKGROUND: Giant cell arteritis (GCA) of the vertebral artery (VA) is a rare but serious cause of ischemic stroke, however, the long-term clinical and sonographic course of GCA patients with VA involvement (VA+) is poorly understood. METHODS: All patients with suspected GCA who were consecutively referred to our ultrasound (US) lab over a 12-year-period were analyzed. US examination (GCA-specific) of the cranial and cervical arteries was performed. Patients with a positive US diagnosis of GCA were identified, and further analysis was restricted to VA+ patients. Follow-up data were extracted

Giant cell arteritis: Role of surgery in the management of vascular complications.

Apart from life-threatening and/or functional emergencies, treatment of vascular lesions in giant cell arteritis (GCA) is medical. Revascularization may be considered if the lesion remains symptomatic or progressive despite optimal medical treatment, provided that there is no disease-related inflammation, and always managed by a team of trained experts. The main risk associated with aortic involvement (aortitis) is the development of an aneurysm, most often in the thoracic aorta, after several years of progression. Indications and surgical techniques used to manage these aneurysms follow the r

Aseptic meningitis in the setting of giant cell arteritis (GCA): a case report.

BACKGROUND: Giant cell arteritis (GCA) is a vasculitis primarily affecting medium- and large-sized arteries. The diagnosis may be challenging and lead to delays in treatment. Cerebrospinal fluid (CSF) pleocytosis is an uncommon association but may occur due to central nervous system (CNS) vasculitis or pachymeningitis. We describe a case fulfilling the criteria for diagnosing GCA, associated with CSF pleocytosis and normal neuroimaging. CASE PRESENTATION: A 76-year-old woman presented to our regional hospital with three weeks of fever, confusion and fatigue. Two days later, she developed a ri

Unilateral Arteritic Anterior Ischaemic Optic Neuropathy (AAION) as a Presenting Manifestation of Fatal Giant Cell Arteritis (GCA): A Case Report.

Giant cell arteritis (GCA), also known as temporal arteritis, is a form of vasculitis that primarily affects the large and medium-sized arteries, which can lead to narrowing or blockage of the arteries. When GCA involves vital vessels such as the coronary arteries, aorta, or cerebral arteries, it can pose a serious risk to life. We present a case of a 61-year-old Malay woman with hypertension and chronic kidney disease who presented with a two-day history of sudden-onset visual loss in the right eye preceded by an inferior visual field defect for four days associated with both lower limb pain.
