Neuronal-surface antibodies are uncommon in patients with a final diagnosis of sporadic Creutzfeldt-Jakob disease: a systematic review and meta-analysis.

BACKGROUND: Neuronal antibodies detected during evaluation of rapidly progressive dementia may indicate treatable autoimmune encephalitis or incidental seroreactivity in prion disease. We estimated their frequency in sporadic Creutzfeldt-Jakob disease (sCJD), separately from diagnostic yield in suspected CJD. METHODS: Under a registered protocol (PROSPERO CRD420261458659), MEDLINE/PubMed, Embase and CENTRAL were searched on 12 July 2026 and with expanded antibody terms on 13 September 2026. Screening, extraction and JBI appraisal were performed independently in duplicate with third-reviewer a

Proportionate and disproportionate reporting of different infectious complications of disease modifying therapy in multiple sclerosis.

INTRODUCTION: Progessive multifocal leukoencephalopathy (PML) is a complication of immunoincompetence due to disease or immunosuppressive therapy and in particular due to disease modifying therapy (DMT) in multiple sclerosis. It is not clear however, whether there is a specific vulnerability to PML or whether this is due to immunosuppression per se. The same holds for the combination of cryptococcal disease and Fingolimod. METHODS: We used the reporting odds ratio (ROR) as calculated from entries to Vigibase for a sample of drugs used for DMT (Alemtuzumab, Cladribine, Fingolimod, Dimethyl fum

Subacute Sclerosing Panencephalitis after Measles Outbreaks, the Philippines, 2007-2022.

Subacute sclerosing panencephalitis (SSPE) is a delayed, almost uniformly fatal complication of measles. The Philippines had measles outbreaks in 2010-2011, 2013, and 2018-2019; the last was the largest in the World Health Organization Western Pacific Region. Published SSPE data from the Philippines end in 2015. We reviewed all 20 children admitted with SSPE to a tertiary pediatric hospital in Quezon City during 2007-2022 and estimated year of measles infection. Median patient age was 6.2 years at hospital admission and 0.7 years at reported measles illness. Five (25.0%) of 20 admissions occur

When Psychosis Masks Neurodegeneration: A Case of Sporadic Creutzfeldt-Jakob Disease.

We present a diagnostically challenging case of a 57-year-old male with new-onset persecutory delusions, hallucinations, functional decline, and gait instability following recent travel. While psychiatric features are recognised in Creutzfeldt-Jakob disease (CJD), they are often underappreciated as initial manifestations, leading to misdiagnosis and delayed recognition. Initially suspected to have a primary psychotic disorder, the patient's rapidly progressive course, neurological signs, characteristic MRI findings, and positive cerebrospinal fluid (CSF) real-time quaking-induced conversion (R

Astrocytic expression of interferon-induced myxovirus resistance protein A and protein kinase R in prion diseases: Distinct plaque-associated myxovirus resistance protein A localization in Gerstmann-Sträussler-Scheinker disease.

Human prion diseases, including sporadic Creutzfeldt-Jakob disease (sCJD) and Gerstmann-Sträussler-Scheinker disease (GSS), are fatal neurodegenerative disorders characterized by misfolded prion protein (PrP) accumulation. Although type I interferon (IFN) signaling and downstream IFN-stimulated genes are well characterized as essential components of host defense in experimental animal models, their roles in human prion pathology remain poorly defined. Myxovirus resistance protein A (MxA) and protein kinase R (PKR) have immunomodulatory functions, yet their histopathological expression in prion

Cranial MRI Findings in Rheumatoid Arthritis Patients Treated with Rituximab: Prevalence, Patterns, and Clinical Correlates.

Background/Objectives: Neurological manifestations in rheumatoid arthritis (RA) are common, yet the clinical relevance of central nervous system (CNS) abnormalities detected on neuroimaging remains uncertain. We aimed to determine the prevalence and patterns of cranial magnetic resonance imaging (MRI) findings in RA patients treated with rituximab (RTX) and to assess whether these abnormalities are associated with disease-specific mechanisms or with age- and comorbidity-related factors. Methods: In this retrospective observational study, 84 RA patients who received RTX and had available pre-tr

Ubiquitin C-hydrolase L1 as a cerebrospinal fluid and blood biomarker for prion diseases.

Digital enzyme-linked immunosorbent assays are based on single-molecule array technology. In this study, ubiquitin C-terminal hydrolase L1 (UCH-L1) levels were measured in cerebrospinal fluid (CSF) and blood samples (serum or plasma) obtained from patients with sporadic Creutzfeldt-Jakob disease and fatal familial insomnia using a commercially available UCH-L1 assay. We determined the inter- and intra-assay variability as well as the stability of UCH-L1 detection to ensure a reliable assay performance. Cut-off values were calculated to differentiate prion disease patients from neurodegenerativ

Chronic traumatic encephalopathy in a neuropathologically confirmed Creutzfeldt-Jakob disease cohort.

Chronic traumatic encephalopathy (CTE) is a tauopathy associated with repetitive head impacts, experienced most commonly in contact sports; however, its relationship with prion diseases remains unclear. Postmortem CTE screening using phosphorylated tau staining was conducted at the Neurological Tissue Bank of Hospital Clínic-IDIBAPS on hippocampal and frontal cortex samples from patients with neuropathologically confirmed Creutzfeldt-Jakob disease (CJD), blinded to patients' clinical histories. In positive and suggestive cases, the analysis was expanded in accordance with the NINDS/NIBIB conse

Implications of cellular prion protein in tumor biology.

Prion proteins (PrP) are primarily known for their role in neurodegenerative conditions like Creutzfeldt-Jakob Disease, where the misfolded, pathogenic isoform PrPSc propagates by using the normal cellular prion protein, PrPc, as a template, leading to aggregation and neuronal death. PrPc is a widely expressed glycoprotein that, beyond its role in prion disease, has emerged in specific contexts as a modulator of several hallmark cancer processes, including proliferation, apoptosis resistance, cellular signaling, adhesion, survival, and motility. This paper explores current evidence on how PrPc

Expanded EP-QuIC Testing for Sporadic CJD in a Local Geographic Area of Canada Results in Improved Surveillance.

BACKGROUND: Diagnosis of sporadic Creutzfeldt-Jakob disease (sCJD) has traditionally relied on non-specific cerebrospinal fluid (CSF) biomarkers such as 14-3-3γ and total tau. The development of quaking-induced conversion (QuIC) assays has enabled direct detection of prion seeds, improving antemortem diagnostic accuracy. We previously noted a high CSF submission rate from the Canadian province of New Brunswick. We leveraged this observation to evaluate how expanded regional testing influences prion disease surveillance and the diagnostic performance of EP-QuIC. METHODS: CSF submissions betwee

PRNP D178N-129MM prion disease: Creutzfeldt-Jakob disease-like cortical ribboning without prominent early insomnia.

BACKGROUND: The PRNP p.Asp178Asn (D178N) mutation on a methionine-bearing codon-129 allele is classically associated with fatal familial insomnia (FFI), but some carriers develop a Creutzfeldt-Jakob disease (CJD)-like phenotype. CASE PRESENTATION AND FINDINGS: We report a 59-year-old woman with two months of rapidly progressive cognitive-language decline, caregiver-observed nocturnal sitting-up and sleep talking, weight loss, extensive cortical diffusion-weighted imaging (DWI) ribboning, positive cerebrospinal fluid 14-3-3, and positive multisite skin real-time quaking-induced conversion. Real-world effectiveness and safety of Ocrelizumab in a gulf multiple sclerosis cohort: extension of the Q-OCRE study to 451 patients.

Real-world evidence on ocrelizumab in multiple sclerosis (MS) from the Middle East and North Africa (MENA) is limited. We extended our original Q-OCRE cohort (n = 60) to evaluate effectiveness and safety in an ethnically diverse Gulf population. In a single-center retrospective cohort of all patients starting ocrelizumab at Hamad General Hospital, Doha, Qatar (October 2017-May 2025), the primary outcome was No Evidence of Disease Activity (NEDA-3) at Year 1; secondary outcomes were annualized relapse rate (ARR), Expanded Disability Status Scale (EDSS) trajectory, 24-week confirmed disability w

Safety and effectiveness of carotegrast methyl in patients with ulcerative colitis: an interim report of post-marketing surveillance in Japan.

BACKGROUND: The first multicenter, prospective post-marketing surveillance (PMS) in Japan was conducted to confirm the safety and effectiveness of carotegrast methyl (hereafter referred to as carotegrast) in the routine clinical practice for ulcerative colitis (UC) in Japan. Here, we report the results of the interim analysis of PMS. METHODS: The ongoing PMS was initiated in June 2022 and will be continued until May 2028. Study patients are individuals with moderately active UC who were refractory or intolerant to 5-aminosalicylic acid, and who started carotegrast therapy for the first time.

SPORADIC CREUTZFELDT-JAKOB DISEASE: CLINICAL FEATURES, DIAGNOSIS AND THERAPEUTIC HORIZONS.

Sporadic Creutzfeldt-Jakob disease (sCJD) is a rare, rapidly progressive, and fatal prion disease that remains a major diagnostic and therapeutic challenge. Its presentation often overlaps with other causes of rapidly progressive dementia, making early recognition essential, particularly to exclude potentially reversible conditions. This review provides an updated overview of the diagnostic tools, cerebrospinal fluid (CSF) biomarkers, prognostic factors, clinical progression, differential diagnosis, and current management strategies in sCJD. A literature-based review was conducted using PubMed

Potential misclassification of probable sporadic Creutzfeldt-Jakob disease diagnosed without PRNP testing.

BACKGROUND: Although probable sporadic Creutzfeldt-Jakob Disease (CJD) can be diagnosed without prion protein gene (PRNP) testing, some patients with genetic prion disease may meet the diagnostic criteria for sporadic CJD. We aimed to assess the potential for such misclassification in the absence of genetic information. METHODS: We analyzed data from the nationwide prospective registry of prion diseases in Japan. For patients diagnosed with probable sporadic CJD, clinical characteristics were compared according to whether PRNP testing had been performed. In addition, patients with genetic pri

Beyond Wilson Disease: a comprehensive review of the Non-Wilsonian Causes and differentiating imaging features of the "Face of the Giant Panda" sign.

The "Face of the Giant Panda" sign is a characteristic magnetic resonance imaging (MRI) appearance classically associated with Wilson disease. Although traditionally considered highly suggestive of Wilson disease, increasing evidence shows that this pattern is not pathognomonic and may occur across a wide range of neurological disorders. This review provides a comprehensive account of non-Wilsonian causes of the giant panda sign, with emphasis on pathophysiological mechanisms, characteristic imaging features, and, critically, the additional MRI findings beyond the panda configuration itself th

Whole-brain cortical morphometry and structural covariance network disruption in subacute sclerosing panencephalitis.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a progressive, fatal encephalitis caused by persistent measles virus infection. Quantitative cortical imaging in SSPE remains poorly characterised, and the topology of the cortical structural covariance network (SCN) has not been examined. OBJECTIVE: To characterise whole-brain cortical structural alterations in SSPE using surface-based morphometry (SBM) and voxel-based morphometry (VBM), evaluate their clinical correlates, and describe the topology of the cortical SCN. METHODS: Twenty-eight patients with SSPE and 48 age- and sex-matc
