Structural characterization of human neutralizing antibodies against JC and BK polyomaviruses.

The human JC polyomavirus (JCPyV) causes the fatal demyelinating disease progressive multifocal leukoencephalopathy (PML) in immunocompromised individuals. JCPyV frequently undergoes mutations in PML patients, and these variants are thought to establish "antibody recognition holes" that enable the virus to circumvent the antibody response of infected individuals. Many of these PML-associated mutations cluster in the glycan receptor-binding site of the virus. Using X-ray crystallography, we investigated the binding modes of JCPyV VP1-specific human monoclonal antibodies (mAbs) that were isolate

Infectious Complications of Antibody-Drug Conjugates: A Review of Safety Data from FDA-Approved Agents.

PURPOSE OF REVIEW: With fifteen agents now FDA-approved and indications expanding into earlier treatment lines, antibody-drug conjugates (ADCs) represent a rapidly growing class of targeted cancer therapeutics. Despite their selective mechanism of action, infectious complications are clinically significant and have been flagged as a disproportionate safety signal in post-marketing surveillance. This review characterizes the infection risk profiles of all fifteen FDA-approved ADCs through systematic extraction of prescribing information and pivotal trial safety data, and examines the pathophysi

The Challenges of Compassion Bias in Clinical Decision-Making: A Case-Based Ethical Analysis.

Bias in clinical decision-making is commonly understood as arising from negative stereotypes or cognitive shortcuts that undermine patient-centered care. Less attention has been paid to forms of bias that emerge from clinicians' well-intentioned efforts to alleviate suffering. This article introduces the concept of compassion bias, defined as a distortion in clinical judgment that occurs when empathy and a desire to avoid harm unintentionally constrain objectivity, patient autonomy, or the range of treatment options offered. We present the case of a young woman with advanced neurologic decline

A case of progressive multifocal leukoencephalopathy in an adult patient with ADA-SCID.

This case report describes progressive multifocal leukoencephalopathy in a patient with ADA-SCID status after gene therapy. Persistent metabolic dysfunction likely contributed to JC virus reactivation, highlighting this rare complication of ADA-SCID and incomplete reconstitution seen with early protocols.

Characteristics of progressive multifocal leukoencephalopathy in hematological malignancies, With reference to HIV-infected patients.

Progressive multifocal leukoencephalopathy (PML) is a rare opportunistic infection of the central nervous system (CNS) observed in immunocompromised individuals. We retrospectively evaluated the clinical courses of PML in patients with hematological malignancies at North Japan Hematology Study Group institutions, using patients with human immunodeficiency virus (HIV) infection as a reference. Eight PML patients had hematological malignancies and 9 had HIV infection. CD4 + T cell counts were markedly low in patients with hematological malignancies (median 168/μl, range 48.4-330/μl) and did not

Current Perspectives on JC Polyomavirus Transmission and Associated Diseases: Implications for Prevention in Risk Populations.

JC polyomavirus (JCPyV) is a worldwide-distributed human virus. Primary infection with JCPyV is usually asymptomatic and followed by a lifelong persistent infection. In situations of profound immunosuppression or prolonged treatment with specific immunomodulatory drugs, such as natalizumab, viral reactivation can occur and lead to the development of JCPyV-associated diseases. Progressive multifocal leukoencephalopathy (PML), a severe demyelinating disease of the central nervous system, is the most common clinical manifestation of JCPyV reactivation. Less frequently, viral reactivation may be a

Bispecific Antibodies Are Associated With Progressive Multifocal Leukoencephalopathy.

BACKGROUND AND OBJECTIVES: Bispecific antibodies (BisAbs) have transformed the management of relapsed and refractory multiple myeloma (MM), achieving high response rates in heavily pretreated patients. However, these therapies induce profound immune perturbation, including plasma cell aplasia, hypogammaglobulinemia, and T-cell exhaustion, predisposing patients to serious infections. Progressive multifocal leukoencephalopathy (PML) has rarely been reported in this setting. METHODS: We report on 5 patients with MM who developed PML following BisAbs therapy. Clinical presentation, prior treatmen

Pembrolizumab in people with HIV-associated progressive multifocal leukoencephalopathy: a retrospective case series.

Progressive multifocal leukoencephalopathy (PML) in people with HIV (PWH) carries high mortality, and no specific antiviral treatment is available. In this retrospective single-centre case series of six patients treated with pembrolizumab, four showed clinical improvement or stabilisation. Pembrolizumab might represent a salvage option in this population.

[Progressive multifocal leukoencephalopathy associated with rituximab treatment].

Progressive multifocal leukoencephalopathy (PML) is a rare and life-threatening demyelinating disease of the central nervous system that occurs almost exclusively in the setting of impaired cell-mediated immunity. An 86-year-old woman with a history of PR3-ANCA-associated vasculitis treated with rituximab presented to the emergency department with progressive left-sided weakness and gait instability. Brain MRI demonstrated an enlarging white matter lesion without contrast enhancement or mass effect. JC virus was detected in the cerebrospinal fluid, thereby confirming the diagnosis of PML. Ritu

Progressive multifocal leukoencephalopathy in an untreated chronic lymphocytic leukemia patient: Case report and review of the literature.

Progressive multifocal leukoencephalopathy (PML), caused by John Cunningham virus (JCV), is a rare opportunistic infection in patients with chronic lymphocytic leukemia (CLL) after immunosuppressive therapy, with only a handful of cases reported in treatment-naïve patients. Herein we present a 71-year-old man with treatment-naïve CLL who developed rapidly progressive left facial weakness, hemiparesis, dysarthria, and urinary incontinence. Brain magnetic resonance imaging demonstrated characteristic imaging findings of PML, including multifocal white matter lesions involving the subcortical U-f

Tenofovir in Progressive Multifocal Leukoencephalopathy: Virus Suppression Without Clinical Benefit in 2 Cases.

OBJECTIVES: Progressive multifocal leukoencephalopathy (PML) is a rare, often fatal demyelinating disease of the CNS caused by reactivation of the JC polyomavirus (JCV), typically in immunocompromised individuals. As no specific therapy against JCV exists, current management focuses on immune reconstitution. Recently, a potential therapeutic effect of tenofovir was reported. METHODS: We report 2 patients with PML who received tenofovir under a compassionate use protocol. Treatment was accompanied by MRI, clinical, and virologic monitoring. RESULTS: In both patients (serum and CSF), viral loa
