PD-1 regulates CD4+ T cell-mediated CD8+ T cell responses in the brain to balance viral control and neuroinflammation.

Programmed cell death protein 1 (PD-1) is expressed by T cells during progressive multifocal leukoencephalopathy (PML), a life-threatening brain disease caused by the human-only JC polyomavirus (JCPyV). PD-1 checkpoint immunotherapy has benefited some PML patients, but reasons for its variable outcomes are unclear. Using mouse polyomavirus (MuPyV), we show that PD-1 loss acts in a brain-autonomous manner to increase the magnitude of brain-infiltrating CD4+ and CD8+ T cells and the function of virus-specific CD8+ T cells; in concert, brain virus levels decline and neuroinflammation increases. In recovered COVID-19 patients, the state of mucosal immunity remains understudied. Cytological, functional, and metabolic characteristics of neutrophils and the interleukin status will help to correctly assess the need for immunorehabilitation measures. The study objective is to assess the state of mucosal immunity after COVID-19. A comprehensive study of mucosal immunity included the assessment of nasal mucosal neutrophils with the monitoring of destructive and apoptotic changes as well as examination of the functional and metabolic activity of neutrophils entering the nasal secretions. Ischemia of the Tongue and Scalp as an Uncommon Presentation of Giant Cell Arteritis.

UNLABELLED: Giant cell arteritis (GCA) is a chronic large-vessel vasculitis that may present with systemic, neurological or ophthalmological manifestations. Lingual and scalp ischaemia are rare but can have severe potential complications that pose a diagnostic challenge and require immediate treatment to prevent irreversible outcomes. We report the case of an 87-year-old woman who initially presented with holocranial headaches and tongue pain with partially detachable whitish plaques. She was diagnosed with GCA based on clinical features, elevated inflammatory markers and ultrasonographic find

Silent Progression of Adult-Onset SSPE: From Ocular Onset to Evolving EEG.

BackgroundSubacute sclerosing panencephalitis (SSPE) is a rare progressive encephalitis due to persistent measles infection. While classically a childhood disorder, atypical, adult-onset, and subclinical variants are increasingly reported. Ocular findings may precede neurological involvement by years. We aimed to underline EEG's role in tracking disease evolution from isolated ocular signs to neurological progression.CaseA 31-year-old woman presented with isolated ocular complaints and bilateral optic atrophy. Cerebrospinal fluid revealed measles IgG and IgG index positivity, confirming SSPE.

Adult-Onset Subacute Sclerosing Panencephalitis Presenting With Subacute Cognitive Deficits.

We describe the case of a 41-year-old man diagnosed with adult-onset subacute sclerosing panencephalitis (SSPE). The patient presented with subacute progressive cognitive deficits and a neuropsychological profile indicating predominant frontoparietal dysfunction. MRI showed only mild parietal-predominant cerebral atrophy. The patient later developed periodic myoclonic jerks with time-locked periodic slow wave complexes on electroencephalography. Evidence of intrathecal synthesis of anti-measles IgG was identified in the cerebrospinal fluid. The patient satisfied modified Dyken diagnostic crite

System biology and network-based approach to identify the therapeutic signatures and potential inhibitors against polycystic lipomembranous osteodysplasia with Sclerosing Leukoencephalopathy.

Polycystic Lipomembranous Osteodysplasia with Sclerosing Leukoencephalopathy (PLOSL), often recognized as Nasu-Hakola disease, is an uncommon autosomal recessive systemic condition described by numerous bone lesions that resemble cysts and progressive early-onset dementia. One novel way to find possible drug targets is to use Network Analyst tools for network-based gene expression profiling. Identifying the target hub genes, essential for the initiation and progression of PLOSL, is validated by the significance level (p-score) attained using Cytoscape in the survival analysis of the major cent

"Subacute sclerosing panencephalitis: a case report of atypical long-term survivor".

Subacute sclerosing pan encephalitis (SSPE) is a slow viral disease that follows the wild-type measles infection of early childhood with a subsequent progressive encephalitic illness with seizures and global regression after a long latent period. Mostly considered an invariably progressive and terminal degenerative disorder, the intriguing variability in course with plateaus and spontaneous remissions, while rare, have been recognized [1, 2]. The predictors of disease trajectory and the mechanisms at molecular and immunological levels that determine these differences in outcome remain largely

Subacute sclerosing panencephalitis in a child six years after measles infection.

Measles is a highly contagious viral illness that remains endemic in many parts of the world, posing significant risks of severe and long-term complications. We report the case of an 8-year-old boy hospitalized for epilepsy with myoclonic spasms and cognitive decline. Clinical and paraclinical investigations, including electroencephalography, imaging tests, and cerebrospinal fluid (CSF) analysis, suggested a neurodegenerative process. The patient had contracted measles at the age of two and had not received full vaccination. Elevated measles-specific immunoglobulins and intrathecal focus of sy

Preliminary evidence of cardiac dysfunction in subacute sclerosing panencephalitis: a systematic review and meta-analysis.

PURPOSE: Subacute sclerosing panencephalitis (SSPE) is a catastrophic neurological disorder that typically presents in infancy and early adolescence. Cardiac arrhythmia in SSPE is a significant but often under-recognized complication of the disease. The persistent measles virus infection that characterizes SSPE can have widespread effects on the central nervous system, including areas that regulate autonomic function, which in turn can impact cardiac rhythm. In this article, we aim to comprehensively review articles describing cardiac dysfunction associated with SSPE. METHODS: We conducted a

Clinico-virological investigation of measles complications in immunocompromised pediatric patients.

Measles is a highly contagious yet vaccine-preventable disease that is re-emerging in settings with suboptimal immunization, putting immunocompromised children at particular risk. Although measles complications are well documented in children, the data on measles-related complications in immunocompromised hosts remain scarce. We conducted a clinico-virological and bioinformatics investigation of five immunocompromised pediatric measles cases: four with measles-associated encephalitis consistent with measles inclusion body encephalitis (MIBE) and one with severe pneumonia. Cases were confirmed

Associations between TMEM106B C-terminal fragment aggregation, age, and TDP-43 or tau pathology.

Transmembrane protein 106B (TMEM106B) is a lysosomal glycoprotein whose genetic polymorphisms contribute to the severity of neurodegenerative disorders associated with TDP-43 pathology. Recent studies have revealed that TMEM106B can form amyloid filaments composed of C-terminal fragments (CTFs) in the human brain. In the present study, we explored the relationships between TMEM106B, age, TDP-43, and tau aggregates, and their roles in neurodegeneration. We used immunohistochemistry with an antibody against CTFs of TMEM106B on postmortem human brain fragments (amygdala, hippocampus, temporal cor

The Anti-Vaccine Legacy: Re-Emergence of Subacute Sclerosing Panencephalitis in Children.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a chronic, progressive disease of the central nervous system (CNS) caused by persistent infection at this level with the wild measles virus. Its incidence is negatively correlated with measles vaccination coverage. The pathogenesis isn't fully understood, but infection before the age of 2 is an important risk factor. METHODS: This is a retrospective observational study conducted at the Louis Turcanu Emergency Children's Hospital in Timisoara, Romania, based on the analysis of the medical records of patients diagnosed with SSPE between
