Cerebral venous sinus thrombosis in pregnancy and the postpartum period: epidemiology, management, and maternal mortality-a systematic review and meta-analysis.

BACKGROUND: Cerebral venous sinus thrombosis is a rare cerebrovascular disease of importance both during pregnancy and in the puerperium. However, the existing cohort literature relates to two distinct clinical populations: women with acute onset of pregnancy-/puerperium-associated cerebral venous thrombosis (CVST), and women with known previous CVST who were assessed in connection with a subsequent pregnancy. As these populations address distinct clinical questions and have distinct denominators and follow-up structures, this review has distinguished outcomes from the two populations and synt

Barriers to immediate corticosteroid treatment in suspected giant cell arteritis with visual symptoms: insights from a tertiary eye care unit.

BACKGROUND: Visual symptoms in Giant cell arteritis (GCA) represent a sight-threatening condition; immediate treatment with systemic corticosteroids is important. Treatment delays may occur due to several issues such as diagnostic uncertainty, unfamiliarity with managing the condition or logistical medication issues. Current literature has not assessed local factors that might impact timely management of suspected GCA patients with visual features; we explored this and created an innovative infographic to enhance current practice. METHODS: Following an index case, we formed a multi-disciplinar

Artificial intelligence models in cerebral infarction: performance and applications in diagnosis, classification, grading, treatment, and disease course prediction-a systematic review.

Artificial intelligence (AI) holds significant promise for transforming cerebral infarction care, yet its real-world performance across the entire disease management continuum remains inadequately synthesized, with heterogeneous evidence and unclear clinical translation pathways. To address this gap, this systematic review and meta-analysis comprehensively evaluates the performance and application value of AI models in diagnosis, subtype classification, severity grading, treatment guidance, and prognosis prediction for cerebral infarction. Adhering to PRISMA 2020 guidelines, we identified 76 e

Role of Low-Field MRI in Acute Stroke.

Portable, low-field (LF) magnetic resonance imaging (MRI) is emerging as a clinically relevant adjunct in acute stroke care, enabling MRI in environments where conventional neuroimaging access is delayed, unavailable, or impractical. Advances in permanent magnet design, compact gradient and radiofrequency hardware, and the use of contemporary reconstruction methods have improved LF image quality and operational feasibility, supporting deployment at the point-of-care in emergency departments, intensive care units, and resource-limited settings. This review summarizes the evolving role of LF-MRI

The Impact of VEGFA rs2010963 on Bilaterality and Posterior Artery Stenosis in Patients with Moyamoya Disease.

VEGFA plays a pivotal role in angiogenesis and is known to affect clinical phenotypes related to the vasculature in various diseases, such as cancer or stroke. However, it is still unclear how the VEGFA genotype influences the clinical phenotype of moyamoya disease (MMD). This study included 137 Japanese MMD patients (84 adult and 53 pediatric patients) carrying either the heterozygous RNF213 p.R4810K or the RNF213 wild-type genotypes. Patients with homozygous RNF213 p.R4810K genotype or the other rare RNF213 variants were excluded because these genotypes are already known to affect the clinic

Post-endovascular thrombectomy perfusion imaging (CT and MR) as a prognostic biomarker and patient-selection tool for adjuvant therapy: A systematic review, meta-analysis, and clinical threshold framework.

BACKGROUND AND PURPOSE: Despite high rates of macrovascular recanalization, approximately half of patients with large vessel occlusion stroke fail to achieve functional independence after endovascular thrombectomy (EVT). Residual tissue-level perfusion abnormalities - detectable on post-procedural CT perfusion (CTP) - have been proposed as a biomarker of futile recanalization. We aimed to establish post-EVT perfusion imaging (by CT or MR perfusion) as a surrogate marker predictive of functional independence on the modified Rankin Scale - by synthesizing evidence-based perfusion thresholds and

From disease-specific control to system-embedded governance: a multidimensional policy design analysis of stroke prevention and stroke center development in China.

BACKGROUND: Stroke remains a major cause of death, disability, and long-term care burden in China. National policy has expanded from disease-specific prevention and treatment to broader arrangements involving chronic disease governance, emergency care, hierarchical services, rehabilitation, and medical quality improvement. However, the policy design and governance structure remain underexamined. METHODS: We conducted a multidimensional national policy text analysis of stroke prevention and stroke center development in China. The final corpus comprised 33 national policies issued between 2009

Unsupervised Phenotyping of Atrial Fibrillation Using Variational Autoencoders in a Critical Care Cohort: The ATLAS-AF Study.

OBJECTIVE: Atrial fibrillation (AF) is a heterogeneous disorder associated with substantial morbidity and mortality, yet management remains largely guided by traditional classifications and risk scores. We aimed to identify distinct AF phenotypes using variational autoencoder (VAE)-based unsupervised learning and to evaluate differences in mortality, healthcare utilization, and treatment patterns across them. METHODS: Retrospective observational cohort study in MIMIC-IV v3.1, a single-center critical care database, restricted to the 2014 to 2016, 2017 to 2019, and 2020 to 2022 anchor year gro

Left atrial appendage closure versus direct oral anticoagulants after pulmonary vein isolation for atrial fibrillation: protocol for a multicentre, prospective, randomised, non-inferiority trial (PROMOTE study).

INTRODUCTION: Atrial fibrillation (AF), with a prevalence of 1-2%, is the most common cardiac arrhythmia. AF is associated with a fivefold increased risk of cardioembolic events; approximately 20% of all strokes are caused by AF. Pulmonary vein isolation (PVI) has become the first-line treatment for AF. However, PVI cannot eliminate the residual stroke risk. Current guidelines recommend that anticoagulation be continued in this specific group of patients, regardless of the presence or absence of AF. In this large AF population post-PVI, who are considered to be in an earlier stage of AF, it is

Oxygen Therapy for Intracranial Hemorrhage.

AIMS: Intracranial hemorrhage (ICH) is a severe cerebrovascular disease with a high mortality rate that impairs patient well-being and quality of life. Oxygen therapies, including hyperbaric hyperoxia (HBO) and normobaric hyperoxia (NBO), have attracted widespread attention as potential adjuvant treatments because of their neuroprotective effects. This review aimed to summarize the current literature addressing the neuroprotective mechanisms of oxygen therapy in ICH, as well as its effectiveness and safety in patients with ICH. METHODS: We systematically searched multiple literature databases

Clinical characteristics and outcomes of intracerebral haemorrhage in young vs older adults: insights from the INTERACT3 trial.

INTRODUCTION: ICH is a severe form of stroke with increasing global burden. Although more common in older adults, ICH in younger individuals (≤50 years) is a clinically distinct but understudied subgroup. This secondary analysis of the INTERACT3 trial compared baseline characteristics, management and outcomes between younger and older ICH patients. PATIENTS AND METHODS: INTERACT3 was a stepped-wedge, cluster-randomised trial conducted in 122 hospitals across 10 countries (2017-2021), evaluating a bundled care intervention for acute ICH. This sub-study analysed 7031 patients and compared demog

Mechanism of activation of an ancestral TEC kinase by PIP3.

BACKGROUND: The introduction of second-generation antipsychotics (SGAs) over the past 2 decades generated considerable optimism that better antipsychotic treatments for schizophrenia and bipolar disorder were possible. SGAs offer several tolerability benefits over first-generation antipsychotics (FGAs), particularly with respect to extrapyramidal symptoms. However, SGAs can induce serious metabolic dysregulations, especially in drug-naive, first-episode, and child and adolescent populations, with olanzapine and clozapine having the highest propensity to cause these abnormalities. In this conte

Stroke etiology was associated with tirofiban efficacy in acute ischemic stroke without endovascular treatment: A pre-specified subgroup analysis of the TREND trial.

BACKGROUND: Different stroke etiologies are associated with varied incidences of early neurological deterioration (END) in patients with acute ischemic stroke (AIS). The Tirofiban for the Prevention of Neurological Deterioration in Acute Ischemic Stroke (TREND) trial demonstrated the efficacy of tirofiban in preventing END in patients with AIS. Herein, we conducted a pre-specified subgroup analysis of this trial data to investigate whether stroke etiologies influenced the effects of tirofiban. METHODS: We performed a pre-specified subgroup analysis of the TREND trial, including 413 patients w

Agreement across different measurements for internal carotid artery stenosis in patients with TIA or stroke in the CONVINCE trial.

OBJECTIVES: In patients with atherosclerosis of the internal carotid artery (ICA), stenosis grading is commonly performed using the NASCET (North American Symptomatic Carotid Endarterectomy Trial) method. Semi-automated software techniques for computed tomography angiography (CTA) offer alternative methods. We compared the NASCET method (based on the absolute minimal diameter) with three different stenosis measurements. MATERIALS AND METHODS: We analysed 519 baseline CTA scans from patients in the CONVINCE (Colchicine for prevention of vascular inflammation in Non-CardioEmbolic stroke) trial.

Risk of Recurrent Stroke Among Patients With Patent Foramen Ovale vs Pulmonary Arteriovenous Malformation: A Nationwide Study.

OBJECTIVES: Pulmonary arteriovenous malformation (pAVM) is an anatomical intrapulmonary left-to-right vascular shunt, and it may be a cause of paradoxical stroke. However, the risk of stroke associated with pAVMs is unknown. In this study, we seek to quantify the risk of stroke recurrence among cryptogenic stroke (CS) patients with pAVM compared with those with patent foramen ovale (PFO). METHODS: This was a retrospective analysis of the 2016-2022 Nationwide Readmissions Database. Patients with acute ischemic stroke were identified, and those with embolic, thrombotic, lacunar, or other specif
