Discovery of a BBB-Penetrant Lanosterol Derivative as an Aβ Aggregation Inhibitor for Alzheimer's Disease.

The abnormal aggregation of amyloid-beta (Aβ) peptides is a core driver of Alzheimer's disease (AD). Developing central nervous system (CNS)-penetrant small-molecule Aβ inhibitors offers a complementary strategy to overcome the high costs and severe side effects of antibodies. Lanosterol has demonstrated excellent potential in inhibiting abnormal protein aggregation. However, poor physicochemical and pharmacokinetic profiles strictly hinder its direct therapeutic application. Herein, rational modifications on lanosterol were conducted to modulate the hydrophilic-lipophilic balance around the 3

Blood-Based Biomarkers as Predictors of All-Cause Dementia: A 13-Year Follow-Up in the Whitehall II Cohort Study.

BACKGROUND AND OBJECTIVES: Blood-based biomarkers are strongly correlated with amyloid positivity in research settings, but their long-term performance in heterogeneous populations remains unclear. Our aim was to determine their long-term prediction accuracy for incident all-cause dementia on their own and when used alongside known risk factors in community-dwelling older adults. METHODS: This was a retrospective study based on the UK Whitehall II longitudinal, population-based cohort study established in 1985 among civil servants that includes a clinical examination every 4-5 years. Blood sa

Case report: Meningitis of a 66-year-old dog owner caused by Capnocytophaga canimorsus.

Capnocytophaga canimorsus is a fastidious Gram-negative rod transmitted from dog saliva and typically associated with immunocompromised hosts. We report meningitis in a 66-year-old immunocompetent dog owner with prior cerebral amyloid angiopathy who presented with acute global aphasia, fever and inflammatory cerebrospinal fluid (CSF) consistent with bacterial meningitis, but without meningism or history of any dog bite. Fusiform Gram-negative rods in CSF prompted extended culture in thioglycolate broth, enabling growth and subsequent species identification by MALDI-TOF MS as C. canimorsus with

Regional tau burden tracks with lower microglial HLA class II transcription in the human brain.

BACKGROUND: Human leukocyte antigen class II (HLA-II) genes mark several microglial states in Alzheimer's disease, but it remains unclear how their average transcription relates to tau measured in the same brain region. METHODS: We used a locked 13-gene HLA-II panel selected by biological function, cross-cohort detection, and variance, without considering effect estimates. The primary analysis was conducted using the Seattle Alzheimer's Disease Brain Cell Atlas (SEA-AD): 361 donor-region observations from 79 donors across 6 cortical regions were modeled with within-donor pathology decompositi

Differential atrophy along the longitudinal axis of the hippocampus in Alzheimer's disease and suspected non-Alzheimer's disease pathophysiology (SNAP).

PURPOSE: Cerebrospinal fluid (CSF) biomarkers are increasingly used to support Alzheimer's disease (AD) diagnosis: Alzheimer's Disease continuum (ADc) is defined by abnormal amyloid-beta (Aβ), while Suspected Non-AD Pathophysiology (SNAP) refers to normal Aβ with elevated tau. Given the differential properties along the hippocampal longitudinal axis, this study aimed to evaluate anterior-posterior subregional hippocampal volume differences among ADc, SNAP, and biomarker-negative participants, and their associations with CSF biomarkers and clinical presentation. METHODS: Data from 1242 partici

Stability of plasma pTau217/Aβ42 ratio with delayed blood processing.

INTRODUCTION: Blood-based biomarkers may improve access to Alzheimer's disease diagnosis, but use may be limited in clinics without on-site processing capability. We evaluated the stability of the plasma phosphorylated tau 217 (pTau217)/amyloid beta 42 (Aβ42) ratio after delayed whole-blood processing. METHODS: We compared blood processed within 2 h or held at 4°C for approximately 24 or 48 h before centrifugation and analysis using the Fujirebio Lumipulse G1200. Stability was assessed using ordinary least squares regression, error index, Bland-Altman analysis, and Deming regression. RESULTS

[Intravenous Thrombolysis for Acute Ischemic Stroke: Current Evidence and Perspectives].

Since its introduction, intravenous thrombolysis (IVT) with alteplase has remained a mainstay of reperfusion therapy in the hyperacute management of acute ischemic stroke (AIS). Recent clinical evidence has driven a major paradigm shift from a strictly "time-based" approach to a "tissue-based" approach. Advanced neuroimaging techniques, including diffusion-weighted imaging-fluid-attenuated inversion recovery (DWI-FLAIR mismatch) and perfusion mismatch analyses, have enabled extension of the treatment window beyond 4.5 h and treatment of patients with strokes of unknown onset, particularly bene

Direction-specific biomarker profiles and clinical significance of plasma p-tau217/amyloid PET discordance in cognitively normal adults.

INTRODUCTION: Plasma phosphorylated tau (p-tau)217 is increasingly used to triage amyloid testing, but the biological and clinical meaning of discordant plasma p-tau217 and amyloid positron emission tomography (PET) results in cognitively normal adults remains uncertain. METHODS: We analyzed cognitively normal participants from the Chinese Preclinical Alzheimer's Disease Study (CPAS; n = 594) and the Alzheimer's Disease Neuroimaging Initiative (ADNI; n = 475). Amyloid PET positivity was defined as Centiloid ≥ 20. Cohort-specific p-tau217 positivity was defined using PET-anchored cutoffs with

EXPRESS: Synaptic density PET mediates the relationship between Alzheimer's disease pathology and cognitive performance.

INTRODUCTION: Synaptic loss is one of the strongest post-mortem predictors of Alzheimer's disease (AD) severity and may impact how pathology affects cognition. This study compared regional [11C]UCB-J synaptic density across the AD continuum and evaluated the extent to which synaptic density mediates relationships between AD biomarkers. METHODS: Seventy-two participants completed PET scans, MRIs, and cognitive testing. Participants were categorized into subgroups representing typical AD progression. Regional synaptic density was compared across subgroups and pathological variables. Standard me

Skeletal muscle fatigue resistance is differentially affected in 8-month-old male and female hAβ-KI mice.

Approximately 95% of Alzheimer's disease (AD) diagnoses are sporadic onset, present after the age of 65, and arising from a combination of non-modifiable and modifiable factors. This underscores the potential to identify early systemic changes that may contribute to disease development. One factor may be skeletal muscle atrophy, which has been associated with cognitive decline, although the exact relationship between skeletal muscle and the brain is unknown. Furthermore, whether muscle atrophy begins prior to or after the onset of AD remains elusive. The humanized amyloid-beta knock-in mouse (

Transcriptomic analysis identifies candidate molecular mechanisms underlying SCF+G-CSF-reduced neuropathology in aged APP/PS1 mice.

BACKGROUND: Alzheimer's Disease (AD) is a neurodegenerative disease characterized by amyloid plaque deposition, tau hyperphosphorylation, neuroinflammation, and cognitive decline. Our previous studies demonstrated that combined treatment with stem cell factor (SCF) and granulocyte colony-stimulating factor (G-CSF) reduces AD pathology in APP/PS1 mice. The present study aimed to explore the molecular mechanisms underlying the effects of SCF+G-CSF using transcriptomic analysis. METHODS: Aged APP/PS1 mice received daily subcutaneous injections of SCF+G-CSF or vehicle for 12 days. RNA was extract

Epigenetic regulation of the transcriptional activity of genes mediating the neuroprotective activity of ceftriaxone in the brain in a rat model of sporadic Alzheimer disease: the involvement of histone deacetylase 1.

Histone deacetylases (HDACs) are enzymes that provide transcriptional repression, while HDAC inhibitors are considered to be potential drugs in the treatment of Alzheimer disease (AD). Earlier we found that ceftriaxone (CEF), an antibiotic with neuroprotective activity, augmented mRNA levels of Ide and Mme, encoding enzymes involved in the catabolism of amyloid beta in the amygdala of OXYS rats, a model of sporadic AD. On the other hand, disturbances of neurotrophic factors (BDNF in particular) play an important role in AD pathogenesis. Moreover, HDACs of class I (HDAC1) are involved in the re

Beyond Alzheimer's story: How an engineered molecule gained an endogenous essence.

A hallmark of Alzheimer's disease (AD) is extra-neuronal protein aggregates formed mainly by the amyloid-β peptide (Aβ), that are deposited inside the brain as amyloid plaques. The formation of amyloid plaques requires participation of zinc ions. The 11-Glu-Val-His-His-14 region of Aβ is critical for interaction between Aβ and zinc ions. The engineered peptide Ac-His-Ala-Glu-Glu-NH2 (HAEE) acts as competitive inhibitor of such interactions and suppresses formation of Aβ aggregates in animal models of AD. We sought to develop a reliable method for studying the behavior of the synthetic HAEE pep

Porphyromonas gingivalis links Alzheimer's disease and periodontal disease: Neuroinflammatory mechanisms and therapeutic implications.

The global increase in the aging population has significantly increased the prevalence of Alzheimer's disease (AD), placing substantial burdens on healthcare systems. Emerging evidence highlights a potential link between AD and periodontal disease (PD), implicating Porphyromonas gingivalis (Pg) as a key microbial driver. In this review, we explore the bidirectional relationship between AD and PD through epidemiological evidence, Pg dissemination along the oral-brain axis, virulence factor-driven neuroinflammation and blood-brain barrier dysfunction, as well as aging-mediated susceptibility and

Aβ-42 Side Chain Deamidation at Q15 or N27 Modulates Protein Aggregation and Alters Microglial Cytokines and CD68.

The progressive aggregation of amyloid beta (Aβ) monomers into oligomers is a critical factor in Alzheimer's disease (AD) pathogenesis. Although mutated forms of Aβ have been shown to display altered aggregation dynamics, the specific effects of deamidated Aβ on microglial function remain understudied. Our research group previously found that the deamidated variant Aβ-42-N27D modified Aβ aggregation, reduced neurotoxicity, and reduced microglial reactivity, but the impact of Aβ-42 side chain deamidation in general on such parameters remained unclear. Here, we expanded on our prior work by inve

Indolizine-Based Small-Molecule Fluorescent Probes for Ex Vivo Imaging of Amyloid-β Plaques.

Fluorescence probes for visualizing amyloid-β (Aβ) plaques are important tools in Alzheimer's disease (AD) research. We synthesized a library of 49 indolizine derivatives using a modular synthetic approach and evaluated their fluorescence labeling behavior directly in ex vivo brain tissue sections from 5XFAD transgenic mice. Through an ex vivo tissue-based screening workflow using anti-Aβ antibody 6E10 as a reference standard for plaque-containing regions, five hit compounds (YIZ-20, YIZ-32, YIZ-36, YIZ-45, and YIZ-47) displaying plaque-associated fluorescence patterns were identified. Serum β-synuclein predicts dementia conversion and mortality beyond amyloid-β and phospho-tau.

Synaptic dysfunction is among the earliest pathological events in Alzheimer's disease and among the strongest neuropathological correlates of cognitive impairment, yet blood-based synaptic biomarkers are lacking. Whether serum β-synuclein provides prognostic information independent of established amyloid-β (A; CSF Aβ42/Aβ40 ratio) and tau (T1; CSF pTau181) biomarkers has not been determined. We studied 259 non-demented individuals with subjective or mild cognitive impairment from a single-centre memory clinic, classified by CSF biomarkers using externally validated cut-offs into A-T1- controls

Osteoporosis and Alzheimer's Disease: The Role of Sclerostin.

PURPOSE OF REVIEW: Little is known about the underlying cause contributing to the association between Osteoporosis (OP) and Alzheimer's disease (AD). This review highlights the emerging role of sclerostin, a protein mainly produced by osteocytes, in the newly discovered crosstalk between bone and brain, with a focus on its potential contribution to the relationship between OP and AD. RECENT FINDINGS: Circulating sclerostin levels increase with aging and are positively associated with brain amyloid-β burden in older adults. Studies demonstrate that elevated sclerostin can cross the blood-brain

Clinical performance and decision thresholds of an automated plasma p-tau217 assay for amyloid pathology in a Taiwanese memory clinic.

BACKGROUND: Fully automated plasma p-tau217 assays may facilitate clinical implementation of blood-based testing for Alzheimer's disease (AD) amyloid pathology. However, data on assay-specific decision thresholds and clinical utility in East Asian memory-clinic populations remain limited. METHODS: We evaluated plasma Roche p-tau217 in 139 participants from a Taiwanese memory clinic using amyloid PET or CSF biomarkers as reference standards. Diagnostic performance and single- and dual-cutoff decision thresholds were assessed. Head-to-head comparisons with Simoa p-tau217, Roche p-tau181, and ne
