Association Between Plasma N-Terminal Tau, Cognitive Decline, and Neurodegeneration in the ADNI Cohort.

BACKGROUND AND OBJECTIVES: Blood-based biomarkers offer a cost-effective, minimally invasive alternative to neuroimaging and CSF biomarkers. Newly developed fluid biomarkers, including the N-terminal tau fragment (NT1), show promise for identifying individuals at risk for Alzheimer disease (AD) and for monitoring disease progression. Here, we determine whether NT1 levels are associated with key clinical and biomarker measures in a large, well-characterized cohort. METHODS: Participants from the Alzheimer's Disease Neuroimaging Initiative (ADNI) cohort with available plasma NT1, β-amyloid (Aβ)

Low Uptake of [18F]Flutemetamol in the Mouse Brain Prevents Non-invasive Quantification of Myelin.

Direct in vivo imaging of myelin would greatly benefit research on demyelinating diseases such as multiple sclerosis but remains technically challenging. This study evaluated whether the amyloid-β radiotracer [18F]Flutemetamol is suitable for PET/CT imaging of myelin in the murine cuprizone model of demyelination. Male C57BL/6 mice were fed 0.3% cuprizone for five weeks (n = 5) or control chow (n = 5) and subsequently underwent a 90 min dynamic [18F]Flutemetamol PET/CT scan with a retest scan two days later. PET data were analyzed using region-of-interest-based quantification with an image-der

Cognitive outcomes of low-dose whole-brain radiation therapy in mild Alzheimer's disease: A multicenter, randomized, sham-controlled phase II clinical trial.

BACKGROUND AND PURPOSE: Low-dose radiotherapy (LDRT) has shown potential in preclinical Alzheimer's disease (AD) models, but controlled clinical evidence is limited. We evaluated two whole-brain LDRT regimens in mild AD. MATERIALS AND METHODS: In this multicenter, participant-blinded, three-arm phase II trial, amyloid-positive participants were randomized 1:1:1 to sham irradiation, 0.04 Gy per fraction (0.24 Gy total), or 0.5 Gy per fraction (3 Gy total), delivered twice weekly for 3 weeks. The primary endpoint was change in Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-cog) s

A latent genetic factor underlies multiple component phenotypes of cerebral small vessel disease.

BACKGROUND: Cerebral small vessel disease (cSVD) encompasses white matter hyperintensities (WMH), enlarged perivascular spaces (PVS), intracerebral hemorrhage (ICH), small vessel stroke (SVS), and cognitive impairment-phenotypes that frequently co-occur yet have been studied as independent genetic endpoints. Whether a shared genetic architecture underlies these co-occurring cSVD-related phenotypes remains unclear. METHODS: We applied Genomic SEM to GWAS summary statistics from six phenotypes (WMH, PVS, ICH, SVS, cognitive performance, systolic blood pressure), followed by multivariate associa

Systemic oxidative stress in patients with Alzheimer's disease.

BACKGROUND: Alzheimer's disease (AD) is a neurodegenerative disorder and oxidative stress (OS) is believed to play a role in disease pathogenesis. However, reliable OS markers are lacking. In this study we aim to assess OS in AD by evaluating free thiols, which serve as key indicators of the systemic redox status. METHODS: SCD (subjective cognitive decline), MCI (mild cognitive impairment), AD, and other dementias (Non-AD) were recruited. Comorbidity data were collected and free thiols were measured in plasma. Additionally, phosphorylated-Tau (pTau217), amyloid-beta ratio (Aβ42/40), Glial Fib

Brain health in older adults: At the crossroads of frailty and resilience.

Brain health is a state of physiological functioning of the organ that allows individuals to fully achieve their age-related potential. Throughout life, a wide range of endogenous and exogenous factors can influence positively and negatively brain health. Among them, age is undoubtedly one of the most important, particularly for its effect on brain's cognitive functions. Pathological cognitive decline is characterized by anatomical changes in areas involved in memory, execution, and reasoning, which may result from various mechanisms such as mitochondrial dysfunction, altered proteostasis, and

The single cell origin of sporadic Alzheimer's disease: A developmental biallelic somatic mutation model.

Alzheimer's disease (AD) is a neurodegenerative disorder beginning decades prior to diagnosis and characterized by aggregates of amyloid-β (Aβ)42 and of hyperphosphorylated tau (ptau) proteins. The etiology of sporadic AD (sAD) is unclear and there are problems in our understanding of dominantly inherited familial AD (fAD). In carriers the dominant mutation is expressed throughout the brain in every neuron in utero; why then is the appearance of Aβ42 plaques focal? To explain this, a single cell origin model for sAD is proposed based on Knudson's biallelic mutation model for neoplasia. Familia

An Anti-Amyloidogenic Approach to Specifically Block Memory Consolidation in Mice for Therapeutic Intervention.

Post-traumatic stress disorder (PTSD) is a mental health disorder triggered by exposure to a traumatic event, which manifests with anguish, intrusive memories, and negative mood changes. So far, there is no efficient treatment for PTSD other than symptomatic palliative care. However, the cytoplasmic polyadenylation element binding protein-3 (CPEB3) has been recently associated with a potential risk gene for PTSD. Considering that the CPEB3 protein is a functional amyloid whose importance for long-term memory consolidation in mammals is well established, we propose the active (amyloid) state of

Locus coeruleus degeneration impairs forebrain arteriole physiology and perfusion in a rat model of Alzheimer's disease.

The locus coeruleus (LC) provides forebrain norepinephrine (NE) and degenerates early in Alzheimer's disease (AD). We have shown that experimental LC lesions in the Tg344-19 AD rat model result in parenchymal arteriole (PA) stenosis, blood-brain barrier permeability, amyloid angiopathy, and cognitive impairment. Here, we show that PAs from LC-lesioned Tg344-19 AD rats displayed increased myogenic tone and decreased responsiveness to TRPV4 channel agonists, which was reversible by NE replacement. MRI studies indicated reduced forebrain perfusion and fractional anisotropy in LC-lesioned animals

Melatonin ameliorates behavioral deficits in 5xFAD mice through modulation of neurogenesis, neuroimmune responses, neurotransmission, and metabolic pathways.

Alzheimer's disease (AD) is a neurodegenerative disorder with limited disease-modifying treatment options. Melatonin, an endogenous hormone, has been shown to have therapeutic effects on amyloid deposition, oxidative stress, and neuroinflammation in AD. However, the mechanisms of its neuroprotective effects in the hippocampus remain largely unknown. This study aimed to investigate the multi-mechanistic neuroprotective effects of melatonin in AD. Eight-month-old female 5xFAD transgenic mice were treated with melatonin (10 mg/kg) for 4 weeks. Temozolomide and bromodeoxyuridine were used to block

zDHHC11 is a common palmitoyltransferase regulating microglia-mediated neuroinflammation across neurodegenerative diseases.

PURPOSE: Microglia-mediated neuroinflammation is a shared feature across Alzheimer's disease (AD), Parkinson's disease (PD), and diabetic cognitive impairment (DCI), but the convergent molecular mechanisms remain unclear. This study investigated whether protein S-palmitoylation serves as a common regulator of microglial neuroinflammation across distinct neurodegenerative diseases etiologies and aimed to identify the key palmitoyltransferase involved. METHODS: In vivo ND models (APP/PS1 for AD, MPTP for PD, STZ/high-fat diet for DCI) and in vitro BV2 microglial models stimulated with amyloid-β

Comparison of brain-derived versus conventional pTau biomarkers in CSF and plasma for detecting Alzheimer's disease.

Brain-derived phosphorylated tau species are postulated to improve the detection of Alzheimer's disease pathology by mitigating non-specific increases. However, their true diagnostic performance, relative to conventional CSF and plasma phosphorylated tau assays, remains uncharacterized. The objective was to compare the diagnostic performance of brain-derived and conventional phosphorylated tau assays across multiple analytes (phosphorylated tau 181, phosphorylated tau 217, phosphorylated tau 231) in CSF and plasma, and to examine biomarker differences across amyloid-β status and cognitive grou

Cerebrospinal fluid outflow pathways in neurodegeneration: nasopharyngeal lymphatics and the ocular glymphatic-like axis.

The glymphatic system supports cerebrospinal fluid (CSF)-interstitial fluid (ISF) exchange and has been implicated, to different extents, in the transport or clearance of amyloid-β (Aβ), tau, lactate and other solutes. Although most studies have focused on CSF influx, astrocytic aquaporin 4 (AQP4) expression, perivascular localisation and polarisation, AQP4-mediated transmembrane water transport, and perivascular exchange, brain clearance also depends on efficient extracranial drainage of waste-containing fluid. In this narrative Review, we examine this 'outflow end', including meningeal lymph

Contrastive learning and advanced bundle metrics outperform fractional anisotropy for Alzheimer's disease diffusion MRI prediction: findings from the Beyond FA challenge.

PURPOSE: The inclusion of diffusion-weighted magnetic resonance imaging (DW-MRI) in major national studies has allowed for quantification and characterization of white matter macrostructure and microstructure in cases of disease and disorder. Diffusion tensor imaging (DTI) yields 3D scalar maps to summarize the DW-MRI signal with quantitative metrics. Fractional anisotropy (FA) is a DTI metric that describes the degree of anisotropy within diffusion and is frequently used to interpret white matter integrity, but it exhibits high sensitivity in pathological interpretation. These properties of F

Safety of lecanemab in real-world patients with Alzheimer's disease: a prospective, observational case series supported by FAERS signal analysis.

OBJECTIVE: Lecanemab, a humanized immunoglobulin monoclonal antibody, has been approved for the treatment of Alzheimer's disease (AD). Although clinical trials have demonstrated its favorable safety profile, real-world studies are crucial for identifying rare adverse events (AEs) and evaluating post-marketing safety. This study aimed to evaluate the safety of lecanemab by combining a clinical prospective, observational case cohort with data from the FAERS database, thereby providing a robust basis for the identification of potential safety signals. METHODS: A prospective and observational stu

Basal Forebrain Volume is Associated With Cortical Amyloid Burden in Cognitively Unimpaired Older Adults at Varying Genetic Risk for Alzheimer's Disease.

BACKGROUND: In mild cognitive impairment and dementia due to Alzheimer's disease (AD), postmortem and in vivo neuroimaging studies have demonstrated significant neuronal loss in the basal forebrain cholinergic system (BFCS), which provides the primary cholinergic input to the cerebral cortex. Within the BFCS, atrophy is most prominent in the nucleus basalis of Meynert (nbM), a posteriorly located cluster of magnocellular neurons. However, less is known surrounding the relationship between amyloid deposition, BFCS atrophy, and medial temporal lobe (MTL) volume loss in preclinical AD. This study

Cryo-EM advances in TDP-43 amyloid fibrils structure determination.

TAR DNA-binding protein 43 (TDP-43) is a DNA/RNA-binding protein whose pathological aggregation characterizes a spectrum of neurodegenerative disorders known as TDP-43 proteinopathies, including amyotrophic lateral sclerosis (ALS), frontotemporal lobar degeneration (FTLD), and chronic traumatic encephalopathy (CTE). Recent breakthroughs in cryo-electron microscopy (cryo-EM) have revolutionized our understanding of these diseases by resolving the atomic structures of TDP-43 amyloid fibrils. These structural insights have been pivotal in elucidating the transition from functional liquid-liquid p
