Association of Location-Specific Perihematomal Edema with Clinical Outcomes in Deep Intracerebral Hemorrhage.

OBJECTIVE: Perihematomal edema (PHE) represents a major secondary injury mechanism following intracerebral hemorrhage (ICH). This study aims to evaluate the impact of early PHE expansion on functional outcomes in patients with deep ICH. METHODS: Patients with deep intracerebral hemorrhage who underwent baseline computed tomography (CT) within 24 h of symptom onset and a follow-up CT within 72 h were included. Absolute PHE increase was defined as the difference in PHE volume between the follow-up and baseline CT scans. Poor outcome was defined as a modified Rankin Scale score > 3 at 3 months.

Remote Ischemic Conditioning After Subarachnoid Hemorrhage: A Systematic Review and Meta-analysis.

BACKGROUND: Remote ischemic conditioning (RIC), an intervention in which brief episodes of ischemia are applied to an organ or tissue to induce remote organs' tolerance of subsequent ischemic injury, has shown potential in reducing the incidence of delayed cerebral ischemia (DCI) secondary an aneurysmal subarachnoid hemorrhage (aSAH). This review aimed to evaluate the effectiveness of RIC in patients with SAH. METHODS: A comprehensive search was conducted to identify peer-reviewed randomized controlled trials (RCT) and non-randomized studies comparing RIC with no conditioning or sham procedur

Cerebral Vasospasm Detection and Delayed Cerebral Ischemia Prediction after Aneurysmal Subarachnoid Hemorrhage: A Scoping Review.

BACKGROUND/OBJECTIVE: Aneurysmal subarachnoid hemorrhage (aSAH) is complicated by angiographic cerebral vasospasm and delayed cerebral ischemia (DCI), distinct entities with different reference standards and surveillance targets. We mapped the evidence on (1) diagnostic accuracy of vasospasm monitoring tools using digital subtraction angiography (DSA) as the reference standard and (2) detection and prediction of DCI using the 2010 consensus definition. METHODS: We performed a Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews (PRISMA-ScR)-guided s

Use of Desmopressin in Spontaneous Nontraumatic Intracerebral Hemorrhage Among Patients Taking Antiplatelet Therapy: Systematic Review and Bayesian Meta-analysis.

Antiplatelet therapy-associated intracerebral hemorrhage (APT-ICH) remains a significant neurological concern due to high morbidity and mortality among these patients. Early trials of desmopressin (DDAVP) suggested clinical benefit; however, the number of studies available at the time was limited. This systematic review and Bayesian meta-analysis aim to answer whether administration of DDAVP among patients with APT-ICH improves clinical course. Five databases were searched for studies comparing DDAVP and placebo/control/usual care in patients with APT-ICH. Bayesian meta-analysis was performed

Preterm Birth Causes Decreased Postnatal Neurogenesis: A Novel Mechanism Underlying Adverse Neurodevelopmental Outcomes.

Although the survival rates of preterm infants have markedly improved, their neurodevelopmental outcomes remain poor. Historically, brain injury in preterm infants has been recognized as intraventricular hemorrhage or white matter injury, often resulting in cerebral palsy. More recently, increasing attention has focused on the elevated risk of neurodevelopmental disorders among preterm infants across a wide range of gestational ages. Furthermore, preterm birth is also associated with an increased risk of psychiatric disorders in adulthood, including depression and schizophrenia. However, the u

Incidental Finding of an Asymptomatic Perielectrode Cyst After Deep Brain Stimulation for Parkinson's Disease.

Implanted intracerebral devices can lead to infection, hemorrhage, or the formation of fluid-filled cysts. Formation of a perielectrode cyst after deep brain stimulation (DBS) has been reported in a limited number of patients. Additionally, diverse causes and clinical presentations have been associated with these cysts. We present a case of perielectrode cyst formation three months after implantation of a subthalamic nucleus DBS (STN-DBS) for Parkinson's disease (PD). A 59-year-old woman was diagnosed with PD in 2011. In November 2023, the patient underwent staged placement of an STN-DBS witho

Impact of FXIII Levels on Hematoma Expansion and Outcomes in Patients with Spontaneous Intracranial Hemorrhage: A Prospective Observational Cohort Study of 132 Patients.

BACKGROUND: Diminished levels of FXIII are significantly correlated with hematoma expansion (HE) and unfavorable clinical outcomes. Although the crucial function of FXIII in coagulation is well-recognized, the connection between FXIII deficiency and either HE or negative prognosis in patients with spontaneous intracerebral hemorrhage (sICH) is not well understood. METHODS: Patients with sICH were confirmed through baseline computed tomography scans conducted within 6 h of symptom onset. Plasma FXIII levels were assessed at the time of admission using a standardized enzyme-linked immunosorbent

Predominantly mild thrombocytopenia, haematoma expansion and clinical outcomes in intracerebral haemorrhage.

INTRODUCTION: Evidence regarding the prognostic significance of thrombocytopenia in spontaneous intracerebral haemorrhage (ICH) remains limited and inconsistent. We investigated its associations with haematoma expansion (HE), functional outcomes and mortality. PATIENTS AND METHODS: We retrospectively analysed prospectively collected data from consecutive patients with spontaneous ICH in a single-centre registry between August 2022 and March 2025. Thrombocytopenia was defined as an admission platelet count <150 × 109/L. HE was defined as a haematoma-volume increase >6 mL or >33% on follow-up n

Discontinuation of intravenous thrombolysis in acute ischemic stroke.

BACKGROUND: Alteplase (rt-PA) was the standard thrombolytic in acute ischemic stroke (AIS) for decades. Due to its short half-life it must be administered over 1 h after an initial bolus, but the administration can be stopped when serious side effects occur. However, there are limited data about the frequency and reasons for interrupted or stopped intravenous thrombolysis (IVT) in AIS. METHODS: We retrospectively analyzed our stroke registry over a time of more than six years (January 1, 2018 - March 31, 2024) to determine the proportion of discontinued IVT. We compared the baseline, stroke a

Association of the systemic immune-inflammation index with consciousness disturbance in cerebral venous sinus thrombosis: a retrospective cohort study.

BACKGROUND: Consciousness disturbance (CD) is a severe neurological manifestation of cerebral venous sinus thrombosis (CVST), but the association between the systemic immune-inflammation index (SII) and CD has not been fully characterised. METHODS: This retrospective single-centre cohort included 213 adults with imaging-confirmed CVST, of whom 55 (25.8%) had CD during the acute admission. We applied a four-step variable-selection procedure, a ten-seed LASSO-consensus approach, fixed-feature 3 × 10-fold repeated cross-validation, and full-pipeline nested internal validation as a sensitivity an

Effects of Blood Pressure Lowering by Hematoma Location in Acute Intracerebral Hemorrhage.

BACKGROUND AND OBJECTIVES: Early intensive blood pressure (BP) lowering is now recognized as an effective approach to improve outcome after acute intracerebral hemorrhage (ICH). However, whether the effect varies by hematoma location is uncertain. We aimed to determine the heterogeneity of the treatment effect of intensive BP lowering on functional outcome by hematoma location. METHODS: We conducted a pooled analysis of individual patient-level data (IPD) from all 4 Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trials (INTERACT) and the second Antihypertensive Treatment of A
