Diagnostic Accuracy of Transcranial Doppler and Electroencephalogram for Delayed Cerebral Ischemia Following Aneurysmal Subarachnoid Hemorrhage: A Systematic Review and Meta-Analysis.

This review was done to assess the diagnostic accuracy of transcranial Doppler (TCD) or electroencephalogram (EEG) accuracy for delayed cerebral ischemia (DCI) in adults with aneurysmal subarachnoid hemorrhage. MEDLINE, Embase, Cochrane CENTRAL, Scopus, and Web of Science were searched from inception to April 2025. Two reviewers independently screened studies, extracted 2 × 2 diagnostic data, and assessed the risk of bias using QUADAS-2. Pooled sensitivity, specificity, positive and negative likelihood ratios (LR+, LR-), diagnostic odds ratios (DOR), and area under the hierarchical summary rec

Clinical and diagnostic characteristics of autoimmune, infectious, and cryptogenic central nervous system vasculitis at a tertiary care center.

BACKGROUND: Central nervous system (CNS) vasculitis is a rare, diagnostically challenging disorder involving inflammation of cerebral arteries. Differentiating infectious from autoimmune etiologies is difficult due to nonspecific presentations and limited tools. We characterized CNS vasculitis cases at a tertiary care center to identify distinguishing features by etiology. METHODS: Patients admitted to Columbia University Irving Medical Center between 2020-2024 with an ICD diagnosis of CNS vasculitis were reviewed. Cases were classified as infectious, autoimmune, or cryptogenic based on clini

Prognostic impact of chronic kidney disease in patients with atrial fibrillation after percutaneous coronary intervention.

BACKGROUND: Patients with chronic kidney disease (CKD) have an elevated risk of adverse cardiovascular events. However, the impact of different stages of CKD on the prognosis of patients with atrial fibrillation (AF) after percutaneous coronary intervention (PCI) remains uncertain. METHODS: This multicenter registry included 902 patients with AF undergoing PCI. Patients were divided into 3 groups based on renal function: normal [estimated glomerular filtration rate (eGFR) ≥ 60 mL/min/1.73m2], early CKD (30 ≤ eGFR <60 mL/min/1.73m2), and advanced CKD (eGFR <30 mL/min/1.73m2 or patients on hemo

Early Tracheostomy in Patients with Nontraumatic Intracerebral Hemorrhage is Associated with Lower in-Hospital Complications and Reduced Resource Use Without Increased Mortality.

BACKGROUND: The appropriate timing of tracheostomy in patients with brain injury is debated, with differing opinions on how to improve patient outcomes and limit complications. This study aims to evaluate the association between timing of tracheostomy and resource use (length of stay, hospitalization cost), in-hospital complications (sepsis, acute kidney injury, deep vein thrombosis, pulmonary embolism, ventilator-associated pneumonia, and acute respiratory distress syndrome), and mortality in mechanically ventilated patients with nontraumatic intracerebral hemorrhage (ICH). METHODS: Using th

Causal cues and mediating pathways of potential genetic mechanisms of subarachnoid hemorrhage and intracerebral hemorrhage.

Subarachnoid hemorrhage (SAH) and intracerebral hemorrhage (ICH) are the primary subtypes of hemorrhagic stroke. Obesity and dietary factors may play significant roles in their pathogenesis, yet the genetic causal relationships and potential mediating pathways remain unclear. This study utilized publicly available genome-wide association study databases and applied Mendelian randomization (MR) methods to systematically evaluate the causal associations between obesity, nutritional intake (fats, proteins, and carbohydrates), alcohol intake, and both SAH and ICH. Antiglomerular basement membrane disease (Goodpasture syndrome) associated with ANCAnegative central nervous system vasculitis.

Anti-glomerular basement membrane disease is a rare pediatric autoimmune disease characterized by anti-glomerular basement membrane antibodies that cause rapidly progressive glomerulonephritis and alveolar hemorrhage. Central nervous system (CNS) involvement is sporadic. We present the case of a 13-year-old female patient with cerebral hemorrhage, septic shock, and multiorgan involvement, in whom the diagnosis of anti-glomerular basement membrane disease and CNS vasculitis in the absence of antineutrophil cytoplasmic antibodies (ANCA-negative) was confirmed. Combined treatment with glucocortic

Liberal Versus Restrictive Blood Transfusion Strategies in Neurocritical Care: A Systematic Review and Meta-Analysis of Randomized Controlled Trials.

INTRODUCTION: Neurocritical care patients, including those with traumatic brain injury, subarachnoid hemorrhage, and intracerebral hemorrhage, often develop anemia, compromising brain oxygen delivery and increasing morbidity and mortality. Blood transfusion strategies, either liberal or restrictive, are commonly used to manage anemia in these patients, but the optimal approach remains unclear due to mixed results in existing studies. METHODS: A systematic search of PubMed, Cochrane Library, ScienceDirect, and Google Scholar from inception to December 2024 for randomized controlled trials (RCT

Dyslipidemia management in stroke prevention: An individualized approach.

Dyslipidemia remains a major, modifiable determinant of global stroke burden, accounting for more than one-fifth of ischemic strokes (IS) worldwide. Recent evidence has shifted emphasis from conventional lipid fractions to apolipoprotein B (ApoB)-containing lipoproteins, including lipoprotein(a) [Lp(a)], which more accurately reflect atherogenic particle burden than low-density lipoprotein cholesterol (LDL-C) alone and are increasingly used for stroke risk stratification. While the principle "the faster and the lower, the better" underpins dyslipidemia management, evidence-based, subtype-speci

Cerebral Microbleeds and Long-Term Affective Symptoms in Survivors of Spontaneous Intracerebral Hemorrhage.

BACKGROUND: Spontaneous intracerebral hemorrhage (ICH) is strongly associated with the development of long-term cognitive impairment and neuropsychiatric symptoms (NPS), particularly affective disturbances including depression and anxiety. Although small vessel disease (SVD) underlies most cases of ICH, the contribution of SVD to persistent affective symptoms and the effects of cognitive dysfunction on NPS remain unclear. METHODS: We prospectively recruited 95 survivors of spontaneous ICH (median, 5.7 years post-ICH) between 2021 and 2025 from stroke clinics using standardized clinical protoc

Does adding decompressive craniectomy to hematoma evacuation improve the outcome for spontaneous supratentorial intracerebral hematoma? A GRADE-assessed systematic review and meta-analysis.

To synthesize the data about whether decompressive craniectomy (DC) combined with hematoma evacuation (HE) improves outcomes compared to HE alone in patients with spontaneous supratentorial intracerebral hematoma (SICH). A thorough search was conducted in four databases until July 19, 2024. We searched PubMed, Scopus, Web of Science, and Cochrane CENTRAL for all studies comparing the outcome of DC with HE versus HE alone in SICH patients. Two reviewers independently screened studies, extracted data, and assessed the risk of bias using the Cochrane and Newcastle-Ottawa tools. Statistical analys

FDCA Attenuates Neuronal Ferroptosis and Reduces Hemorrhagic Transformation After Ischemic Stroke with High Glucose.

Hemorrhagic transformation (HT) is a common complication of ischemic stroke that significantly increases the rates of disability and mortality, which could be promoted by hyperglycemia as a risk factor for HT after stroke. Neuronal ferroptosis was implicated as a key contributor to neuronal death during the development of HT which currently has few effective therapies in clinic. The compound fasudil dichloroacetate (FDCA), synthesized with ROCK inhibitor fasudil (F) and PDK inhibitor dichloroacetate (DCA) was previously reported to exhibit neuroprotective effects in ischemic stroke, but whethe

CCR2 knockdown attenuates post-hemorrhagic hydrocephalus and improves glymphatic function after intraventricular hemorrhage.

Post-hemorrhagic hydrocephalus (PHH) is a severe complication of intraventricular hemorrhage (IVH), yet its underlying mechanisms remain unclear. The glymphatic system (GS), a key pathway involved in cerebrospinal fluid (CSF) circulation and metabolic waste clearance, has recently been implicated in the pathogenesis of PHH. In this study, we employed a mouse model of IVH (n = 6 per group, assessed from 6 h to 28 days post-IVH) to investigate the role of the CCL2/CCR2 signaling axis in GS dysfunction and PHH progression. Behavioral tests, CSF tracer imaging, immunofluorescence, and Western blot

Spatiotemporal multi-omics profiling of secondary brain injury after intracerebral hemorrhage in an optimized autologous blood-induced mouse model with human tissue validation.

BACKGROUND: Secondary brain injury (SBI) following intracerebral hemorrhage (ICH) involves complex molecular events such as oxidative stress, inflammation, and immune cell infiltration. Understanding their spatiotemporal patterns is essential for identifying therapeutic targets and optimal intervention windows. METHODS: We optimized an autologous blood-induced ICH mouse model and performed neuroimaging, behavioral testing, histological evaluation, and transcriptomic and proteomic analyses across multiple time points (Days 1, 3, 5, 8, and 12). Time-resolved analyses focused on dynamic changes
