Analysis of discrepancies in hemorrhagic transformation and infarct volume in ischemic stroke patients undergoing endovascular treatment.

OBJECTIVE: After endovascular treatment (EVT) for ischemic stroke (IS), clinical observations have shown that some patients with small infarct volumes develop hemorrhagic transformation (HT), while some patients with large infarct volumes do not. This study aims to analyze the factors contributing to these differences and to assess the impact of HT on neurological outcomes. METHODS: A total of 732 patients were divided into small infarct volume (0-15 mL) and large infarct volume (≥70 mL) groups. The incidence of HT, risk factors, neurological outcomes (NIHSS changes), early neurological deter

Invasive vagus nerve stimulation paired with upper limb motor rehabilitation after intracerebral hemorrhage: A single-arm pilot cohort study.

Invasive vagus nerve stimulation paired with rehabilitation improves upper-limb recovery after ischemic stroke, but clinical evidence after intracerebral hemorrhage remains limited. We evaluated the feasibility, safety, and descriptive functional signal of invasive vagus nerve stimulation-paired rehabilitation in chronic intracerebral hemorrhage. Twelve participants with chronic upper-limb motor dysfunction after intracerebral hemorrhage underwent left cervical invasive vagus nerve stimulation implantation followed by 6 weeks of supervised in-clinic rehabilitation and 90 days of home-based reh

When not to explore: a combat-specific decision framework for selective neck exploration at Role 2.

Current civilian algorithms for penetrating neck trauma assume access to definitive vascular reconstruction, endovascular intervention, advanced imaging, and specialist multidisciplinary care. These assumptions may not apply to Role 2 military facilities, where neck exploration can commit the surgeon to procedures that cannot be completed under optimal conditions. This position paper was developed through a focused narrative review of contemporary recommendations and literature on penetrating neck trauma, vascular and aerodigestive injury, temporary hemorrhage control, and combat casualty care

Transient focal neurological episodes in cerebral amyloid angiopathy-related convexity subarachnoid hemorrhage.

BACKGROUND: Convexity subarachnoid hemorrhage (cSAH) is recognized as a cause of transient focal neurological episodes. These episodes can closely mimic transient ischemic attacks (TIAs). We report a case of cerebral amyloid angiopathy (CAA)-related cSAH presenting with recurrent transient monoparesis. CASE DESCRIPTION: An elderly patient with a history of hypertension was admitted to our hospital because of recurrent episodes of transient left upper limb monoparesis. Initial diffusion-weighted magnetic resonance imaging (DWI) demonstrated a focal hyperintensity over the right frontal convexi

Key Predictors of Cerebral Vasospasm Following Subarachnoid Hemorrhage: A Systematic Review and Meta-Analysis.

BACKGROUND: Subarachnoid hemorrhage (SAH) is a life-threatening condition often leading to complications such as cerebral vasospasm, which causes delayed cerebral ischemia and increases mortality. In intensive care, early identification of vasospasm risk factors by nurses is essential for timely intervention and improved outcomes. OBJECTIVES: We sought to review and synthesize evidence on risk factors associated with cerebral vasospasm after SAH and inform early risk assessment and nursing care strategies in intensive care. METHODS: We conducted a systematic review and meta-analysis of obser

When to Start Oral Anticoagulant Therapy Post-Stroke: From Common to Rare Conundrums.

PURPOSE OF REVIEW: This review summarizes recent evidence on when to start oral anticoagulant therapy following stroke, including both common and rare clinical conundrums. RECENT FINDINGS: While previous recommendations based on Diener's rule suggested waiting up to 12 days based on stroke severity and neuroradiological findings, newer data support earlier initiation or resumption of anticoagulation, particularly for mild and moderate ischemic strokes, generally within 4 days. In patients with a history of intracerebral hemorrhage, anticoagulation resumption involves weighing thromboembolic r

Atropine-Associated Central Anticholinergic Syndrome during Carotid Artery Stenting: A Case Report.

Acute neurobehavioral deterioration during carotid artery stenting (CAS) requires prompt evaluation for serious neurological complications. We report a patient in their 70s who developed profound hyperactive delirium approximately 15 minutes after receiving prophylactic intravenous atropine (0.25 mg) during CAS. The episode was characterized by severe agitation, bilateral mydriasis, and facial flushing without focal neurological deficits. Repeated angiography and subsequent neuroimaging showed no evidence of thromboembolism, intracranial hemorrhage, cerebral edema, or cerebral hyperperfusion.

Experiences, barriers and enablers of stroke end-of-life care: a systematic review with mixed-methods synthesis.

BACKGROUND: Despite advances in stroke care, 12% of patients die within 30 days of stroke highlighting the requirement for high-quality end-of-life care (EoLC) within stroke services. EoLC aims to improve the quality-of-life of patients facing death whilst also supporting their families. EoLC after stroke can be difficult due to complex stroke-specific challenges and little clinical guidance. There is a lack of knowledge and understanding of current provision of poststroke EoLC and the barriers and enablers to quality EoLC. Therefore, the aim of this review was to identify the experiences, exp

Oropouche virus disrupts neurodevelopment and exhibits congenital infection potential.

Oropouche virus (OROV) historically caused a self-limiting disease, yet recent strains have been clinically linked to congenital infection and neurodevelopmental disease. These observations highlight the need to study OROV as a congenital pathogen. Here, using both a historical and currently circulating strain, we show OROV infects neurons across differentiation states in human forebrain organoids. Compared with the neurotropic congenital pathogen Zika virus (ZIKV), OROV exhibits a heightened capacity for neuroinfection and pathology in both forebrain organoids and neonatal mice. The increased

Post-hemorrhagic ventricular dilatation and hydrocephalus in preterm infants: a five-year retrospective cohort study from a tertiary referral center.

BACKGROUND: Post-hemorrhagic hydrocephalus (PHH) is a serious complication of intraventricular hemorrhage (IVH) in preterm infants and is associated with increased morbidity and mortality. Although neonatal risk factors are well described, the contribution of maternal and perinatal factors remains inconsistent across studies. OBJECTIVE: To determine the incidence, clinical characteristics, and independent predictors of PHH among preterm neonates with IVH, and to evaluate early outcomes in a tertiary referral center. METHODS: This retrospective cohort study included preterm neonates (< 34 wee

Antihypertensive treatment non-adherence and spontaneous intracerebral hemorrhage: a prospective observational study from Northern India.

OBJECTIVES: Intracerebral hemorrhage (ICH) is a leading cause of stroke-related mortality and long-term disability, with a disproportionate burden in low- and middle-income countries. Hypertension, and particularly non-adherence to antihypertensive therapy, represents the most important modifiable risk factor. This study aimed to evaluate the epidemiological, clinical, and radiological characteristics of hypertensive ICH and to identify preventable risk factors with public health relevance. METHODS: A prospective observational study was conducted at a tertiary-care referral center in Northern

Association Between Texture-Modified Diet Intake Rate and Functional Outcomes After Acute Stroke: A Retrospective Observational Study.

BACKGROUND/OBJECTIVES: Texture-modified diets (TMDs) are commonly prescribed after stroke, but the clinical significance of the amount actually consumed remains unclear. We examined the association between TMD intake rate and functional status at discharge and 90 days after stroke. METHODS: This retrospective single-center study included 406 patients with ischemic stroke or intracerebral hemorrhage who received TMDs for suspected or confirmed dysphagia between 2012 and 2017. Overall TMD intake was calculated as the meal-count-weighted mean consumption proportion across four TMD levels and ana

Association of Glymphatic Imaging Markers With Clinical Outcomes and Cerebral Small Vessel Disease Burden After Intracerebral Hemorrhage.

OBJECTIVE: The glymphatic system plays a critical role in cerebral metabolite clearance and white matter injury. However, its clinical relevance in intracerebral hemorrhage (ICH) remains poorly understood. This study aimed to investigate imaging-based glymphatic markers in relation to functional outcomes and cerebral small vessel disease (CSVD) burden in patients with ICH. METHODS: We included 70 patients with acute supratentorial ICH who underwent diffusion tensor imaging (DTI). Glymphatic-related imaging parameters were derived using magnetic resonance imaging (MRI), including DTI-ALPS, fre

A Primer on COL4A1/COL4A2-Related Cerebral Small Vessel Disease for Neurologists.

COL4A1 and COL4A2 encode type IV collagen, a principal structural component of basement membranes, and pathogenic variants in these genes cause a multisystem small vessel vasculopathy with prominent cerebrovascular involvement. Despite increasing recognition with the expansion of genetic testing, COL4A1/2-related cerebral small vessel disease remains underrecognized in clinical practice, in part due to its striking phenotypic heterogeneity. Presentations span a broad clinical spectrum-from asymptomatic individuals or incidental imaging findings to recurrent ischemic events, intracerebral hemor

Reversible cerebral vasoconstriction syndrome: a severe neurological complication in early puerperium.

Reversible cerebral vasoconstriction syndrome (RCVS) is an uncommon entity characterized by transient, segmental vasoconstriction of the cerebral arteries, which may be associated with sudden-onset headache and acute neurological complications. The puerperium represents a recognized risk period for its development. We describe the case of a 29-year-old woman who, in the early puerperium, presented with sudden-onset orthostatic headache, followed by severe acute headache and impaired level of consciousness. Brain computed tomography revealed bilateral intraparenchymal hemorrhages. Angiographic

Perioperative ASL-derived CBF changes are associated with complications after Moyamoya bypass surgery.

BACKGROUND: Postoperative hyperperfusion and insufficient perfusion augmentation are important contributors to morbidity after revascularization surgery for Moyamoya disease (MMD). Arterial spin labeling (ASL) enables noninvasive assessment of cerebral blood flow (CBF), but the relationship between perioperative ASL-derived CBF changes and specific postoperative complications after direct bypass remains incompletely defined. This study evaluated whether perioperative ASL-derived CBF changes were associated with postoperative complications after direct bypass surgery in patients with MMD. METH
