Circulating microRNAs reflect hemorrhagic MRI markers in cerebral amyloid angiopathy.

BACKGROUND: Cerebral amyloid angiopathy is a major cause of intracerebral hemorrhage in older individuals. Although cerebral microbleeds (CMBs) and cortical superficial siderosis (cSS) aid MRI diagnosis they offer limited pathophysiological insight. MicroRNAs (miRs) regulate microvascular function and may serve as noninvasive biomarkers for distinguishing CAA subtypes and assessing hemorrhagic risk. This study investigated circulating miR profiles in probable CAA and Dutch-type hereditary CAA and examined their relationship with hemorrhagic MRI markers. METHODS: Small RNA sequencing was perfo

Discontinuation of intravenous thrombolysis in acute ischemic stroke.

BACKGROUND: Alteplase (rt-PA) was the standard thrombolytic in acute ischemic stroke (AIS) for decades. Due to its short half-life it must be administered over 1 h after an initial bolus, but the administration can be stopped when serious side effects occur. However, there are limited data about the frequency and reasons for interrupted or stopped intravenous thrombolysis (IVT) in AIS. METHODS: We retrospectively analyzed our stroke registry over a time of more than six years (January 1, 2018 - March 31, 2024) to determine the proportion of discontinued IVT. We compared the baseline, stroke a

Association of the systemic immune-inflammation index with consciousness disturbance in cerebral venous sinus thrombosis: a retrospective cohort study.

BACKGROUND: Consciousness disturbance (CD) is a severe neurological manifestation of cerebral venous sinus thrombosis (CVST), but the association between the systemic immune-inflammation index (SII) and CD has not been fully characterised. METHODS: This retrospective single-centre cohort included 213 adults with imaging-confirmed CVST, of whom 55 (25.8%) had CD during the acute admission. We applied a four-step variable-selection procedure, a ten-seed LASSO-consensus approach, fixed-feature 3 × 10-fold repeated cross-validation, and full-pipeline nested internal validation as a sensitivity an

Case Report: Botulinum Toxin Type A Injection for Bruxism Following Left Basal Ganglia Hemorrhage.

BACKGROUND Post‑stroke bruxism is an underrecognized complication that may impair oral function, cause orofacial pain, and negatively impact rehabilitation outcomes. Botulinum toxin type A (BoNT‑A) has been reported for bruxism, but its application in post‑stroke populations remains sparsely documented. This case describes the assessment and treatment of bruxism following cerebral hemorrhage, utilizing the Standardized Tool for the Assessment of Bruxism (STAB) as a structured multidimensional framework. CASE REPORT A 40-year-old man developed bruxism during recovery from a massive left hemisph

Mast cell degranulation damages glymphatic system via PKC/p38MAPK/AQP4 to exacerbate hydrocephalus post intraventricular hemorrhage.

Hydrocephalus following intraventricular hemorrhage (IVH) remains a serious neurological condition with limited treatment options beyond invasive surgical interventions, underscoring the urgent need to elucidate its underlying mechanisms. Emerging evidence implicates impaired glymphatic system function in the pathogenesis of hydrocephalus, particularly through dysregulation of aquaporin-4 (AQP4) expression in astrocytes, which is essential for cerebral waste clearance and fluid homeostasis. Since mast cells are key mediators of neuroinflammation and can influence cerebrospinal fluid (CSF) dyna

Effects of Blood Pressure Lowering by Hematoma Location in Acute Intracerebral Hemorrhage.

BACKGROUND AND OBJECTIVES: Early intensive blood pressure (BP) lowering is now recognized as an effective approach to improve outcome after acute intracerebral hemorrhage (ICH). However, whether the effect varies by hematoma location is uncertain. We aimed to determine the heterogeneity of the treatment effect of intensive BP lowering on functional outcome by hematoma location. METHODS: We conducted a pooled analysis of individual patient-level data (IPD) from all 4 Intensive Blood Pressure Reduction in Acute Cerebral Hemorrhage Trials (INTERACT) and the second Antihypertensive Treatment of A

RAGE in neutrophils triggers cerebral vasospasm in the hyperacute phase after subarachnoid hemorrhage.

Subarachnoid hemorrhage (SAH) due to the rupture of an intracranial aneurysm is a highly fatal type of stroke. Cerebral vasospasm (CVS) is a major post-SAH complication leading to delayed ischemic neurological deficits, thereby worsening patient outcomes. Previously, we found that lower plasma levels of the soluble receptor for advanced glycation end products (RAGE) predict symptomatic CVS in patients with SAH. However, the molecular mechanisms underlying CVS remain unclear. Here, using an SAH mouse model with endovascular perforation, we found that neurological deficits, CVS in the circle of

CXCR4 targeting by TPCA‑1 attenuates neuroinflammation in intracerebral hemorrhage through inhibiting NF‑κB and AP‑1 activation.

Intracerebral hemorrhage (ICH)-induced secondary brain injury (SBI) is a critical contributor to poor neurological outcomes. In this study, we aimed to identify potential small‑molecule therapeutic agents and systematically validate their neuroprotective efficacy. Bioinformatics analysis was performed to screen ICH‑related hub genes, and Connectivity Map (CMap) was used to predict candidate compounds, followed by molecular docking to verify their binding affinity. The therapeutic effect of TPCA‑1 was further evaluated in a mouse ICH model using neurological scoring, cerebral edema assessment,

Evidence Update for Minimally Invasive Surgical Evacuation of Supratentorial Spontaneous Intracerebral Hemorrhage: A Science Advisory From the American Heart Association.

Recent evidence has reignited interest in the role of surgical intervention for improving outcomes after spontaneous intracerebral hemorrhage. Historically, the surgical management of intracerebral hemorrhage has been limited due to concerns regarding procedural risks and lack of functional improvement. However, recent advances in minimally invasive surgical techniques suggest improvements in functional recovery, broadening consideration for surgical treatment of certain patients, such as those with lobar hemorrhage after spontaneous intracerebral hemorrhage. This science advisory summarizes r

Recurrent Subarachnoid Hemorrhage and Multifocal Cerebral Infarction Associated with Amphetamine Abuse.

Illicit stimulants, such as amphetamines and methamphetamines, can induce profound sympathomimetic effects, including significant cerebrovascular complications. We describe a 48-year-old man who presented with persistent consciousness disturbance and repeated strokes. Imaging findings revealed a subarachnoid hemorrhage, multiple infarctions in the globus pallidus, pons, corpus callosum, and lateral ventricles, and a transient basilar artery pseudoaneurysm. Although meningoencephalitis was initially considered, further tests were negative for infectious causes of the disease. A positive ampheta
