Heterogeneous cerebral perfusion and network inefficiency associated with cognitive impairment in cerebral microbleeds.

BACKGROUND: Cerebral microbleeds (CMBs), a marker of cerebral small vessel disease and common in older adults, are associated with cognitive impairment. However, the neurovascular mechanisms remain incompletely understood. We investigated whether cerebral blood flow (CBF) abnormalities and CBF network changes were associated with cognitive impairment in patients with CMBs. METHODS: In this cross-sectional study, 111 participants with CMBs and 148 controls underwent neuropsychological assessment and arterial spin labeling MRI. Following propensity score matching, CBF and CBF network analyses w

Application of balloon-expandable stent angioplasty with dIstal support of the stent-retriever (BASIS-Stent) technique for acute intracranial artery atherosclerosis-related occlusion.

UNLABELLED: BackgroundRescue stenting may be needed for intracranial atherosclerosis (ICAS)-related large vessel occlusion (LVO). Balloon-expandable (BE) stent implantation often fails due to its stiffness. Balloon-expandable stent angioplasty with the dIstal support of stent-retriever (BASIS-Stent) technique is a novel technique that increases the success and safety of BE stent implantation. Our study presents our initial experience with the BASIS-Stent technique for ICAS-related LVO that requires rescue stenting and to assess its feasibility. METHOD: The clinical and angiographic data of ac

Mechanical thrombectomy failure in anterior circulation large vessel occlusion: an overview from the ROSSETTI registry.

BACKGROUND: Although mechanical thrombectomy (MT) is an effective treatment for large vessel occlusion (LVO) with a high successful recanalization rate, MT failure (MTF) occurs in 10-15% of cases and is associated with unfavorable outcomes. However, little is known about the clinical, technical, and radiological reasons for MTF. We investigated the technical factors associated with MTF. METHODS: We conducted a retrospective analysis of consecutive patients with anterior LVO prospectively included in the ongoing observational multicenter ROSSETTI registry. Patients were categorized according t

Cangrelor versus GPIIb/IIIa inhibitors as adjunctive therapy in endovascular treatment of large vessel occlusion stroke.

BACKGROUND: Endovascular treatment (EVT) failures and early reocclusions in stroke often result from arterial wall disease, incomplete thrombus withdrawal, or acute endothelial injury. Intracranial and extracranial atherosclerosis, in particular, poses a risk of reocclusion, sometimes requiring tailored interventions (eg, angioplasty, stenting). While glycoprotein (GP) IIb/IIIa inhibitors have been widely studied in ischemic stroke, cangrelor remains less explored. OBJECTIVE: To evaluate the safety and efficacy of cangrelor compared with GPIIb/IIIa inhibitors in large vessel occlusion stroke

Role of biliverdin reductase, a heme degradation pathway enzyme, in the development of vasospasm after subarachnoid hemorrhage.

BACKGROUND: Subarachnoid hemorrhage (SAH) is a life-threatening condition with high mortality and significant neurological morbidity, often complicated by delayed cerebral ischemia (DCI) and vasospasm. Heme metabolites such as biliverdin (BV) are implicated in SAH-induced vascular dysfunction, yet the role of biliverdin reductase-A (BVR-A), an enzyme that reduces BV to bilirubin, remains underexplored. This study investigates the contribution of BV and oxidative post-translational modifications of BVR-A to vasospasm development. METHODS: We used a murine model of BV injection into the subarac

Tenzingplasty for cerebral vasospasm following subarachnoid hemorrhage or meningitis: multicenter initial experience.

BACKGROUND: Cerebral vasospasm is a serious complication of intracranial pathologies. Treatment involves intensive medical therapy that may require augmentation with endovascular therapy, most commonly intra-arterial calcium channel blocker infusion. Angioplasty is historically reserved for refractory cases at risk for infarct. Given the risks of iatrogenic complications with angioplasty, alternative mechanical therapies have been explored. This series examines Dotter technique mechanical dilatation performed with Tenzing devices, referred to as Tenzingplasty. METHODS: Institutional review bo

Dynamic transcranial Doppler monitoring predicts intracranial hemorrhage in patients with anterior circulation large vessel occlusion after endovascular treatment.

BACKGROUND: Endovascular treatment (EVT) is the most effective treatment for large vessel occlusion (LVO) stroke. Intracranial hemorrhage (ICH) is one of the most common complications after EVT, which is closely associated with poor clinical outcome. OBJECTIVE: In this study, we investigated the value of dynamic transcranial Doppler (TCD) monitoring in predicting ICH in patients with anterior circulation LVO. METHODS: Based on a prospective cohort, we consecutively collected details of patients with anterior LVO who received successful recanalization after EVT (modified Thrombolysis in Cereb

Spontaneous Lingual Bleeding and Intracerebral Hemorrhage in a Young Adult with Immune Thrombocytopenia (ITP): A Rare Case Report and Brief Recent Update on Treatment.

INTRODUCTION: Immune thrombocytopenia (ITP) is an autoimmune condition characterized by reduced platelet counts due to increased peripheral destruction and impaired platelet generation. An estimated incidence of ITP is 2 to 5 cases per 100,000 individuals in the general population. While mucocutaneous bleeding is common, life-threatening complications, such as spontaneous lingual hematoma and intracerebral hemorrhage (ICH), are extremely rare. Rapid progression of lingual haematomas might compromise airway function and necessitate immediate medical intervention. It is well established that mos

Altered Amyloid-β42 and BACE-1 Proteins in Plasma Astrocyte-Derived Exosomes in Hypertensive Patients With Cerebral Microbleeds.

BACKGROUND: Cerebral microbleeds (CMBs) have been found to promote Alzheimer's disease (AD) progression. Hypertension (HTN) is one of the major etiological factors for CMBs and an important risk factor for AD. However, the association between HTN-related CMBs and AD pathology remains undetermined. This study aims to identify the relationship between HTN-related CMBs and amyloid-β 42 (Aβ42) and β-site amyloid precursor protein cleaving enzyme 1 (BACE-1) levels in plasma astrocyte-derived exosomes (ADEs). METHODS: In total, 88 HTN participants including 30 with deep/infratentorial (D/I) CMBs, 3

Phase I dose-escalation study of tenecteplase, a third-generation fibrinolytic agent, combined with neuronavigation-assisted stereotactic minimally invasive puncture, in patients with acute spontaneous deep cerebral haemorrhage.

INTRODUCTION: Tenecteplase (TNK) offers logistical advantages in stroke thrombolytic therapy with its single bolus administration compared with alteplase. Moreover, its high specificity for fibrin may contribute to a reduction in haemorrhage complications. However, the safety, tolerability and efficacy of TNK, combined with neuronavigation-assisted stereotactic minimally invasive puncture, in patients with acute spontaneous deep cerebral haemorrhage remain unknown. METHODS: We conducted a prospective, open-label phase I trial in a 3+3 dose escalation design to evaluate the safety and tolerabi

Safety and efficacy of selective and superselective intra-arterial cerebral infusion with blood-brain barrier disruption for glioma: a systematic review and meta-analysis.

BACKGROUND: Selective and superselective intra-arterial cerebral infusion (SIACI/SSIACI) delivers chemotherapy directly to tumor-supplying arteries, achieving high local drug levels with reduced systemic toxicity. Often combined with blood-brain barrier disruption (BBBd) to enhance penetration, these methods have been widely studied, yet a comprehensive evidence synthesis is lacking. This study systematically assesses the safety and efficacy of intra-arterial chemotherapy with BBBd for glioma treatment. METHODS: We searched PubMed, Embase, and Web of Science for studies on SIACI with BBBd in
