The MV1 subtype of sporadic Creutzfeldt-Jakob disease (sCJD) is one of the least studied. Cases are defined by the presence of the methionine/valine (MV) polymorphism at codon 129 of the PRNP gene and a type 1 immunoblot pattern of pathological prion protein (PrPD), where the unglycosylated PrPD fragment migrates at ~21 kDa (T21). Because the originally described MV1 cases had T21 plus brain pathology indistinguishable from MM1 cases (small vacuoles), the MV1 subtype has historically been grouped with MM1. However, the recent identification of MV1 cases with immunoblot and pathological feature

BACKGROUND AND OBJECTIVES: Early recognition of patients with rapidly progressive dementia (RPD) attributed to autoimmune encephalitis (AE) is important because rapid initiation of immunotherapy improves outcomes. However, ancillary testing is often time-consuming because of the broad differential diagnosis, highlighting the need for accurate patient selection based on early clinical features. We aim to determine the frequency of AE subtypes in patients with RPD (AE-RPD) and characterize presenting subphenotypes (i.e., symptoms in addition to dementia) compared with other diagnoses.
METHODS:

Synapse loss is an early feature of prion disease, yet the underlying drivers are poorly understood. We recently found evidence of neuronal hyperactivity and synaptic loss in prion-infected mice. Herein, we identified increased Arc/Arg3.1 in patients with prion disease, suggesting heightened neuronal activity also occurs in the human prion-affected brain. To determine the signaling events initiated by prion aggregates (PrPSc), we developed a disease model in which human iPSC-derived excitatory neurons are stimulated with a PrPSc-mimetic antibody, POM1, that binds cellular prion protein (PrPC).

RATIONALE: Sporadic Creutzfeldt-Jakob disease (sCJD) is a rare and highly fatal neurodegenerative disorder with heterogeneous clinical presentations, making accurate early diagnosis challenging. Furthermore, there is a critical paucity of epidemiological and clinical data regarding sCJD in the Xinjiang region, contributing to frequent misdiagnoses.
PATIENT CONCERNS: We present 3 patients who exhibited rapidly progressive dementia alongside multifocal neurological signs, including ataxia, extrapyramidal symptoms, and visual disturbances.
DIAGNOSES: Comprehensive evaluations revealed positive

BACKGROUND: The diagnosis of neurodegenerative diseases (NDDs), such as Alzheimer disease (AD), Creutzfeldt-Jakob disease (CJD), Parkinson disease (PD), and other α-synucleinopathies, is traditionally based on clinical signs and symptoms. By the time symptoms emerge, neurodegeneration has already advanced, limiting the window for early treatment and research. Early diagnosis could improve therapeutic outcomes and deepen insights into disease onset, making the search for novel diagnostic tools and early biomarkers a major focus of current research. A key pathological hallmark of these prion-lik

BACKGROUND: Genetic prion diseases caused by octapeptide repeat insertions in the PRNP gene are rare. 5-octapeptide repeat insertion (5-OPRI) is an uncommon subtype that usually manifests as Creutzfeldt-Jakob disease (CJD) but shows marked variability in clinical phenotype and disease course.
DISCUSSION: We report a patient with rapidly progressive gCJD caused by a 5-OPRI mutation. A systematic PubMed review identified genetically confirmed 5-OPRI cases with available clinical data. Demographic characteristics, clinical features, ancillary investigations, disease course, and PRNP codon 129 po

Electroencephalography (EEG) has emerged as a promising non-invasive tool for the diagnosis of neurodegenerative disorders, and artificial intelligence (AI) has shown significant potential in this domain, as demonstrated by recent studies. However, strong inter-subject variability remains a major challenge, limiting the ability of AI-based models to learn disease-specific features that generalize across individuals, thereby hindering the development of clinically deployable subject-independent systems. In this work, we propose a cross-subject, AI-based EEG classification framework to distingui

Sporadic Creutzfeldt-Jakob disease (sCJD) is the most prevalent human prion disease, characterized by rapidly progressive neurodegeneration and an invariably fatal outcome. This review comprehensively examines the epidemiological features, molecular pathogenesis, clinical manifestations, diagnostic approaches-including neuroimaging, laboratory assays, cerebrospinal fluid analysis, and genotyping-and current therapeutic strategies alongside prognosis evaluation of sCJD. Despite advances in understanding prion biology and disease mechanisms, challenges remain in early diagnosis and effective tre

BACKGROUND: Serum neurofilament light and heavy chains (sNfL and sNfH) have been assessed as neuronal markers for amyotrophic lateral sclerosis (ALS) and dementias. Whereas sNfL has robust literature, systematic studies on sNfH are lacking. Here, we aimed to assess the diagnostic value of sNfH in comparison to sNfL in a broad range of neurodegenerative disorders.
METHODS: We measured with immunoassays sNfH and sNfL in patients recruited in the multicenter German Frontotemporal Lobar Degeneration (FTLD) Consortium (n = 340) and in a single-center German cohort (n = 290). We assessed the diagno

Prions are unique infectious agents because different conformational properties of their constituent proteins are responsible for the manifestation of distinct strains. While prion replication in humans with sporadic Creutzfeldt-Jakob disease and cattle with bovine spongiform encephalopathy is primarily limited to the central nervous system (CNS), a seminal feature of chronic wasting disease (CWD) in cervids is high infectious titers in peripheral tissues, including skeletal muscles and the lymphoreticular system. Although extensive studies have assessed prion properties in the CNS, our unders

INTRODUCTION: Early recognition of Creutzfeldt-Jakob disease relies on awareness of its clinical variability, particularly in patients with complex medical comorbidities.
CASE PRESENTATION: A 66-year-old woman presented with generalized weakness, tremor, double vision, and slurred speech. Initial imaging and laboratory studies were unremarkable, except for subtle magnetic resonance imaging findings suggestive of encephalitis. Despite treatment with methylprednisolone, her condition worsened. Repeat magnetic resonance imaging raised suspicion for Creutzfeldt-Jakob disease, which was confirmed

Prions are self-templating assemblies of the host prion protein in which conformational templating encodes heritable "strain" information. Human prion diseases, including Creutzfeldt-Jakob disease (CJD), are rare but uniformly fatal neurodegenerative disorders with established public-health relevance through epidemic and iatrogenic transmission and provide a paradigm for conformational templating in neurodegeneration. Mechanistic analysis of human prion propagation and development of infectivity assays for public health surveillance have been limited by the absence of mammalian cell systems th

Creutzfeldt-Jakob disease (CJD) is an exceedingly rare, fatal, and rapidly progressive neurodegenerative disorder caused by misfolded prion proteins. In the Philippines, CJD is underrecognized and underreported due to limited access to diagnostic modalities and the absence of a national registry. This multicenter case registry aimed to characterize the clinical, demographic, and diagnostic features of Filipino patients with CJD; determine the distribution of subtypes; describe presenting symptoms and disease progression; assess diagnostic modalities; estimate diagnostic delays and survival tim

Creutzfeldt-Jakob disease is a rare prion disorder that may present with rapidly progressive neurological and neuropsychiatric deterioration, making early diagnosis challenging. We report the case of a 55-year-old woman admitted for rapidly progressive balance impairment evolving over three months, associated with dysarthria, upper-limb dystonia, visual and auditory hallucinations, progressive loss of autonomy, mutism, refusal of oral intake, and myoclonus involving the face and limbs. A history of a similar neurological disorder in her mother was reported. Routine cerebrospinal fluid (CSF) an

BACKGROUND: Genetic Creutzfeldt-Jakob Disease (gCJD) is an autosomal dominant prion disease caused by heterozygous pathogenic variants in the PRNP gene. It is relatively prevalent in Israel due to a large cluster of individuals from Libyan Jewish origin harboring the p.Glu200Lys variant. In our experience, increasing awareness, expanded reproductive options, and the emergence of research initiatives have led more asymptomatic relatives to seek presymptomatic testing. Since gCJD is a highly penetrant and fatal condition, the decision to pursue testing involves complex medical, psychological, et

Prion diseases represent a unique biological paradigm with mechanistic parallels to other neurodegenerative conditions like Alzheimer's and Parkinson's diseases. However, the study of human prion pathobiology and the development of effective therapeutics has been severely constrained by the inability to propagate human prions in dividing cells-forcing reliance on costly and slow animal bioassays. Here, we report the generation of EKV cells-a humanized cell model which supports the robust, indefinite propagation of sporadic Creutzfeldt-Jakob disease (sCJD) prions. We demonstrate that these cell

BACKGROUND: Creutzfeldt-Jakob disease (CJD) is a rare, rapidly progressive prion disease with established zoonotic transmission only in its variant form. Chronic wasting disease (CWD), a prion disease affecting cervid populations in North America, continues to expand geographically, raising public health concerns regarding potential interspecies transmission.
METHODS: We describe the clinical course, diagnostic evaluation, and public health investigation of a patient with confirmed sporadic CJD and long-term venison exposure from CWD-endemic regions in Louisiana. Clinical data, neuroimaging,
