Case Fatality of Subarachnoid Hemorrhage by Aneurysm Location: A Population-Based Study From Finland and New Zealand.

BACKGROUND AND OBJECTIVES: According to region-specific hospital-based studies, the location of ruptured intracranial aneurysm (RIA) influences the case fatality rate (CFR) of aneurysmal subarachnoid hemorrhage (SAH). However, little is known about whether CFRs vary by RIA location in population-based studies that include prehospital SAH deaths. We assessed whether CFRs and CFR-trends differ by RIA location using whole population data from Finland and New Zealand. METHODS: We used externally validated administrative databases to identify all nonhospitalized and hospitalized SAH cases in Finla

Vocal fold motion impairment following stroke: SPARCS database analysis.

PURPOSE: Vocal fold motion impairment (VFMI) has been reported as a complication of stroke worldwide. However, the incidence and risk factors are unknown. This study aimed to assess the incidence and associated risk factors of post-stroke VFMI. METHODS: A query of the 2016-2023 New York State SPARCS database was performed using ICD-10 codes for adult patients with cerebral infarction (I63.x), intracerebral hemorrhage (I61.x), or subarachnoid hemorrhage (I60.x). Chi-square with post-hoc column proportion testing was performed for univariate analysis of patient demographics and comorbidities. M

Dementia Risk Due to Traumatic Brain Injury in Subtypes of Dementia in the Welsh Population.

BACKGROUND AND OBJECTIVES: Traumatic brain injury (TBI) can increase vulnerability to neurodegenerative disorders. The association between TBI and dementia has been previously reported, but studies have relied on self-reporting of TBI and often do not appropriately adjust for relevant risk factors or cover enough time to include both individuals at age of highest TBI risk and age of dementia onset. This study uses electronic health records, which include over 20 years of data and 1.7 million individuals with hospital or general practitioner diagnoses of dementia and TBI. Therefore, the aim of

Outcomes of cerebrovascular disease in muscular dystrophies: A propensity-matched nationwide analysis.

BACKGROUND AND PURPOSE: The literature lacks large studies on outcomes of cerebrovascular disease (CVD) in patients with muscular dystrophies (MDs). Previous studies primarily focused on ischemic stroke. CVD can add considerable morbidity in patients with MD. We aimed to study the outcomes of CVD in patients with MDs using a national database. METHODS: Data were abstracted from the 2010-2021 Nationwide Readmission Database. We identified hospitalizations for CVDs including ischemic stroke, intracerebral hemorrhage, subarachnoid hemorrhage, and other unspecified CVDs. We used propensity-score

Increased prevalence of peripheral vestibular disorder among patients with Fabry disease.

BACKGROUND: Although peripheral vestibular disorder is a non-fatal complication of Fabry disease, fatalities have been reported in some case reports and case series. To date, no studies have examined the relative risk of peripheral vestibular disorder in patients with Fabry disease compared to the general population without the condition. Due to the high prevalence of Fabry disease in East Asia and the potential shared pathogenic pathways between Fabry disease and vasculopathy, we conducted a study using a nationwide population-based dataset to compare the prevalence of peripheral vestibular d

Incident dementia in ischaemic stroke patients with early cardiac complications: A propensity-score matched cohort study.

INTRODUCTION: The risk of dementia in patients with stroke-heart syndrome (SHS) remains unexplored. PATIENTS AND METHODS: Retrospective analysis using the TriNetX network, including patients with ischaemic stroke from 2010 to 2020. These patients were categorised into two groups: those with SHS (heart failure, myocardial infarction, ventricular fibrillation, or Takotsubo cardiomyopathy within 30 days post-stroke) and those without SHS. The primary outcome was the 1-year risk of dementia (vascular dementia, dementia in other disease, unspecified dementia, or Alzheimer's disease). The secondary

Stroke in critically ill patients with respiratory failure due to COVID-19: Disparities between low-middle and high-income countries.

PURPOSE: We aimed to compare the incidence of stroke in low-and middle-income countries (LMICs) versus high-income countries (HICs) in critically ill patients with COVID-19 and its impact on in-hospital mortality. METHODS: International observational study conducted in 43 countries. Stroke and mortality incidence rates and rate ratios (IRR) were calculated per admitted days using Poisson regression. Inverse probability weighting (IPW) was used to address the HICs vs. LMICs imbalance for confounders. RESULTS: 23,738 patients [20,511(86.4 %) HICs vs. 3,227(13.6 %) LMICs] were included. The inc

The effect of COVID-19 on treatment and outcomes following ischemic stroke: A national assessment.

INTRODUCTION: COVID-19 has had innumerable impacts on the healthcare system, both by worsening patient illness and impeding effective and efficient care. Further, COVID-19 has been tied to increased rates of ischemic stroke, particularly among young patients. We utilized a national database to assess associations of COVID-19 with thrombectomy rates, mortality, and discharge disposition among stroke patients. METHODS: Patients were identified from the National Inpatient Sample (NIS, 2020). Inclusion criteria selected for adult ischemic stroke patients; those with venous thrombosis or unspecifi

Increased risk of all-cause, Alzheimer's, and vascular dementia in adults with migraine in Korea: a population-based cohort study.

BACKGROUND: Studies investigating the association between migraine and dementia have reported inconsistent findings. This study aimed to evaluate whether patients with migraine have an increased risk of dementia compared to individuals without migraine. METHODS: We obtained data from the 2002-2019 Korean National Health Insurance Health Screening Cohort. Non-migraine controls were selected using a 1:1 risk-set matching with a time-dependent propensity score. The main outcome was the development of all-cause dementia, and the secondary outcome was the development of each cause of dementia (Alz

Garbage codes in the Norwegian Cause of Death Registry 1996-2019.

BACKGROUND: Reliable statistics on the underlying cause of death are essential for monitoring the health in a population. When there is insufficient information to identify the true underlying cause of death, the death will be classified using less informative codes, garbage codes. If many deaths are assigned a garbage code, the information value of the cause-of-death statistics is reduced. The aim of this study was to analyse the use of garbage codes in the Norwegian Cause of Death Registry (NCoDR). METHODS: Data from NCoDR on all deaths among Norwegian residents in the years 1996-2019 were

Access-Site Complications in Mechanical Thrombectomy for Acute Ischemic Stroke: A Review of Prospective Trials.

BACKGROUND: A shift has occurred in interventional cardiology from transfemoral to transradial access due to a 70%-80% decrease in complications. This shift has not yet taken place in other interventional specialties, perhaps owing to the lack of generalizability of findings in the cardiology data. PURPOSE: Our aim was to assess data from the recent mechanical thrombectomy prospective trials to better understand the access-site complication rate. DATA SOURCES: Articles were systematically sourced from the National Center for Biotechnology Information PubMed archive. STUDY SELECTION: Accordi

Antiplatelet and anticoagulant agents for secondary prevention of stroke and other thromboembolic events in people with antiphospholipid syndrome.

BACKGROUND: Antiphospholipid syndrome (APS) is a systemic autoimmune disease characterized by arterial or venous thrombosis (or both), and/or pregnancy morbidity in association with the presence of antiphospholipid antibodies. The prevalence of APS is estimated at 40 to 50 cases per 100,000 people. The most common sites of thrombosis are cerebral arteries and deep veins of the lower limbs. People with a definite APS diagnosis have an increased lifetime risk of recurrent thrombotic events. OBJECTIVES: To assess the effects of antiplatelet (AP) or anticoagulant agents, or both, for the secondar

Neurological and neuropsychiatric complications of COVID-19 in 153 patients: a UK-wide surveillance study.

BACKGROUND: Concerns regarding potential neurological complications of COVID-19 are being increasingly reported, primarily in small series. Larger studies have been limited by both geography and specialty. Comprehensive characterisation of clinical syndromes is crucial to allow rational selection and evaluation of potential therapies. The aim of this study was to investigate the breadth of complications of COVID-19 across the UK that affected the brain. METHODS: During the exponential phase of the pandemic, we developed an online network of secure rapid-response case report notification porta

Markers of perinatal hypoxia-ischaemia and neurological injury: assessing the impact of insult duration.

Hypoxic-ischaemic insults occurring during or after birth can cause both acute and long-term neurological impairment. The duration of the insult is a critical factor, but most published reports of duration have important limitations. After the onset of a persistent bradycardia in 125 term born infants, abnormal outcomes occurred in two by 10 minutes, in 12 out of 47 (26%) delivered between 11 and 20 minutes, and in 55 out of 65 (85%) delivered after 20 minutes. Series with unspecified gestation or including infants born preterm give comparable results in over 500 additional cases. Before 20 mi
