Monoallelic TYROBP deletion is a novel risk factor for Alzheimer's disease.

Biallelic loss-of-function variants in TYROBP and TREM2 cause autosomal recessive presenile dementia with bone cysts known as Nasu-Hakola disease (NHD, alternatively polycystic lipomembranous osteodysplasia with sclerosing leukoencephalopathy, PLOSL). Some other TREM2 variants contribute to the risk of Alzheimer's disease (AD) and frontotemporal dementia, while deleterious TYROBP variants are globally extremely rare and their role in neurodegenerative diseases remains unclear. The population history of Finns has favored the enrichment of deleterious founder mutations, including a 5.2 kb deleti

Diagnostic Dilemma Between Autoimmune Encephalitis and SSPE: Case Report of Two Pediatric Patients.

Subacute sclerosing panencephalitis (SSPE) and autoimmune encephalitis (AE) are distinct clinical entities, each characterized by unique management strategies and prognosis. SSPE represents a late sequela of persistent measles infection, with progressive cognitive decline, behavioral alterations, motor function deterioration and myoclonus. In contrast, AE is an inflammatory disorder of central nervous system, typically presenting with constellation of neurological and psychiatric symptoms. In pediatric AE, seizures, movement disorders, and focal neurological deficits are more prominent than be

Subacute sclerosing panencephalitis as a re-emerging condition due to low vaccination coverage: a case-series.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a chronic progressive disease affecting the central nervous system (CNS) because of persistent measles virus (MeV) infection. The disease presents with a range of symptoms, starting with gradual cognitive and motor decline, progressing to coma, vegetative state, and typically resulting in death within 3-4 years of clinical onset. The incidence of SSPE is inversely related to vaccination coverage and its pathogenesis remains poorly understood. The primary risk factor is infection by the wild type of measles virus, particularly in individ

Subacute sclerosing panencephalitis incidence following measles elimination efforts in the Republic of Korea.

BACKGROUND: Subacute sclerosing panencephalitis (SSPE) is a rare but fatal neurodegenerative disease caused by persistent measles virus infection. After a significant measles outbreak in 2000-2001, the Republic of Korea implemented a nationwide measles elimination program, which led to a dramatic reduction in measles incidence. This study aimed to evaluate the impact of these measles elimination efforts on the incidence of SSPE in the Republic of Korea. METHODS: This nationwide, population-based retrospective cohort study identified patients newly diagnosed with measles and SSPE between 2007

Subacute Sclerosing Panencephalitis in a North African Cohort: Findings From the Post-Measles Epidemic Era.

Subacute sclerosing panencephalitis (SSPE) is a rare, fatal encephalitis caused by chronic measles virus infection. Its recent resurgence, particularly after measles epidemics, underscores the need for vigilance in children. This study describes the clinical, paraclinical, therapeutic features, and prognosis of SSPE in 15 patients. Initial symptoms included gait disturbances, psychomotor regression, and behavioral changes. Neurological examination revealed cognitive decline, visual impairment, hypotonia, pyramidal signs, ataxia, and movement disorders. Electroencephalogram showed periodic delt

Subacute Sclerosing Encephalitis in an Adult with Congenital HIV Infection - Case Report.

Subacute sclerosing panencephalitis (SSPE) is the result of a chronic infection of the central nervous system caused by a mutated measles virus. We present a case of SSPE in a 19-year-old female with congenital HIV-infection. The patient has been using antiretroviral therapy regularly since she was 4 years old. At the age of 15, she contracted measles of moderate severity. At the age 17, she had the HIV viral load < 20 copies/ml and the CD4 count 420 cells/μl. Three years after measles, bilateral necrotizing retinitis developed, and five months later, myoclonic seizures of the left limbs. S

Spasms and not Myoclonus in Subacute Sclerosing Panencephalitis. A Case Report and Review of the Literature.

Subacute sclerosing panencephalitis (SSPE) is a neurodegenerative disease caused by the measles virus. An affected child typically presents with cognitive decline and abnormal movements, described as myoclonia. Early diagnosis is crucial for prognosis, but can be challenging because early symptoms may be subtle, and EEG findings are not always typical. We propose that better description and documentation of motor symptoms may facilitate earlier recognition of SSPE.A 4-year-old boy presented with cognitive decline and motor symptoms evolving over 6 months. The patient had a history of measles a

Subacute sclerosing panencephalitis.

Subacute sclerosing panencephalitis (SSPE) is a devastating neurodegenerative disorder due to a persistent mutated, wild-measles virus infection of the nervous system. While the acute infection is generally self-limiting, defective viral clearance can lead to the emergence of neurovirulent strains that undergo mutations within the host, evade immune surveillance, establishing chronic central nervous system (CNS) infection. It predominantly affects children and young adults although no age, including infants, is immune to SSPE. The characteristic neurological manifestations include progressive

Measles Matters: A Clinical Overview and Update.

Measles is a highly transmissible viral disease that is preventable through vaccination. Despite global initiatives, outbreaks persist, demonstrating the need for high immunization coverage. Spread occurs via respiratory droplets and airborne particles, reinforcing the need for herd immunity. Symptoms include fever, cough, and a distinctive rash. Complications may include pneumonia, encephalitis, and in rare cases, acute disseminated encephalomyelitis and subacute sclerosing panencephalitis. Diagnosis relies on reverse transcription-polymerase chain reaction and serology tests. Two doses of th

Beyond Myoclonus-Seizures, Epilepsy Syndromes and Atypical Electroencephalographic (EEG) Findings in Children With Subacute Sclerosing Panencephalitis.

BackgroundSubacute sclerosing panencephalitis is typically characterized by myoclonic jerks, cognitive decline, movement disorders, and periodic complexes on electroencephalography (EEG). Although myoclonus is a hallmark feature, other seizure types including generalized/focal seizures are less commonly described in subacute sclerosing panencephalitis. We aimed to study seizure frequency, types, spectrum of epilepsy syndromes, and atypical EEG findings among children with subacute sclerosing panencephalitis.Materials and MethodsA retrospective chart review of 100 children (aged 1-18 years) dia

Understanding the structure of measles virus and its implications for novel drug discovery.

INTRODUCTION: Despite having a stably effectively vaccine for decades, the Measles virus (MV) still causes periodic outbreaks given its highly contagious nature and a consistent decline in immunization coverage, which was further exacerbated during the COVID-19 pandemic, leading to reduced immunization rates. Equally concerning, there are also no approved treatments for measles. AREAS COVERED: Herein, the authors explore the current challenges of MV therapy discovery. Firstly, the article will provide an overview of the potential drug-targeted steps in the MV infection process, followed by di
