Veterans Hospitals vs Non-Veterans Centers in the Prevention of Limb Loss.

IMPORTANCE: US veterans remain an at-risk population for limb loss, given the prevalent rates of coexisting diabetes and peripheral arterial disease (PAD). Moreover, despite the perception that a healed minor amputation might obviate the need for a future major amputation, previous work documented increased longitudinal rates of limb loss among non-veteran Medicare beneficiaries. OBJECTIVE: To document the corresponding incidence of major amputation among veterans after a prior minor amputation and accordingly benchmark Veterans Affairs (VA) health care delivery compared to non-VA civilian ho

Arterial structure and function in children with inflammatory bowel disease.

BACKGROUND AND AIM: People with inflammatory bowel disease (IBD) have an increased risk of cardiovascular disease, including in younger adulthood. This may arise in part from chronic, systemic low-grade inflammation. The process of atherosclerosis may begin in childhood. We sought to determine whether pediatric IBD is associated with adverse changes in arterial structure and function as a marker of early increased cardiovascular risk. METHODS: We performed a case-control study comparing children with IBD for a median disease duration of 2.49 (interquartile range 1.23, 4.38) years with healthy

Comparison of Postoperative Complications in Patients Undergoing Limb Salvage Reconstructive Surgery Based on Estimated Prevalence of Preexisting Peripheral Arterial Disease.

BACKGROUND: Limb salvage after lower extremity (LE) trauma requires optimal blood flow for successful microsurgical reconstruction. Peripheral arterial disease (PAD) decreases LE perfusion, affecting wound healing. Patients who present with LE trauma may have undiagnosed PAD, particularly those with atherosclerotic risk factors. This study assesses outcomes after LE salvage in patients at risk for PAD. METHODS: This retrospective review evaluated patients who underwent LE reconstruction at a level 1 trauma center between 2007 and 2022. Patients with a nontraumatic mechanism of injury, missing

[Clinical efficacy and safety of Ethoxidol in cerebrovascular diseases].

OBJECTIVE: Evaluation of efficacy and clinical safety of ethylmethylhydroxypyridine malate (Ethoxidol) in cerebrovascular disease in out-patient, and in-patient setting. MATERIAL AND METHODS: In the observational study was included of the 70 patients (aged 62.84±6.54 y.o) diagnosed with unspecified cerebrovascular disease, hypertensive encephalopathy, atherosclerosis of cerebral vessels. To access neurological deficit Montreal Scale (MoCA test) was used, MFI-20 test for asthenia. Berg balancing (BBS-test), tinnitus severity score (THI), and general clinical condition score also were recorded.

Acute myocardial infarction: Development and application of an ICD-10-CM-based algorithm to a large U.S. healthcare claims-based database.

BACKGROUND: Healthcare administrative claims data hold value for monitoring drug safety and assessing drug effectiveness. The U.S. Food and Drug Administration Biologics Effectiveness and Safety Initiative (BEST) is expanding its analytical capacity by developing claims-based definitions-referred to as algorithms-for populations and outcomes of interest. Acute myocardial infarction (AMI) was of interest due to its potential association with select biologics and the lack of an externally validated International Classification of Diseases, 10th Revision, Clinical Modification (ICD-10-CM) algorit

Comparative Analysis of Outcomes in Patients Undergoing Femoral Endarterectomy plus Endovascular (Hybrid) or Bypass for Femoropopliteal Occlusive Disease.

INTRODUCTION: The gold-standard for management of combined common femoral artery (CFA) and superficial femoral artery (SFA) atherosclerotic occlusive disease has traditionally been open femoral endarterectomy and femoral-popliteal bypass. Hybrid approaches involving an open and endovascular component are increasingly common. The aim of this study was to compare perioperative outcomes in patients who underwent an open versus hybrid revascularization. METHODS: NSQIP data, years 2012-2017, were queried for patients who underwent nonemergent CFA endarterectomy with either SFA transluminal interve

Postmenopausal hormone therapy and risk of stroke: A pooled analysis of data from population-based cohort studies.

BACKGROUND: Recent research indicates a favourable influence of postmenopausal hormone therapy (HT) if initiated early, but not late, on subclinical atherosclerosis. However, the clinical relevance of timing of HT initiation for hard end points such as stroke remains to be determined. Further, no previous research has considered the timing of initiation of HT in relation to haemorrhagic stroke risk. The importance of the route of administration, type, active ingredient, and duration of HT for stroke risk is also unclear. We aimed to assess the association between HT and risk of stroke, conside

A role for autoantibodies in atherogenesis.

An increased risk of cardiovascular disease (CVD) has long been recognized amongst people with autoimmune disease. It has been unclear whether this is due mainly to the ensuing treatment, particularly steroids, or whether some of this risk is due to the autoimmune process itself with subsequent inflammation. Indeed, a large body of evidence supports a role for chronic inflammation in atherogenesis, and autoantibodies have been identified as mediators in this complex inflammatory environment. Our aim is to carry out a systematic review of existing literature in order to formally establish the s

Evaluating the buffering vs. direct effects hypotheses of emotional social support on inflammatory markers: the multi-ethnic study of atherosclerosis.

Social support is associated with cardiovascular disease mortality, however, the physiologic mechanisms underlying this relationship remains unspecified. This study evaluated the association of social support with inflammatory markers associated with cardiovascular risk: C-reactive protein (CRP), interleukin-6 (IL-6), and fibrinogen. We evaluated two competing models of the support-inflammation relationship: first, that low social support is directly associated with inflammation, and second, that high support acts to buffer the effect of stress on inflammation. Using data from the baseline int

Understanding the coronary heart disease versus total cardiovascular mortality paradox: a method to enhance the comparability of cardiovascular death statistics in the United States.

BACKGROUND: Coronary heart disease (CHD) represents the largest share of cardiovascular disease in the United States, but there are conspicuous discrepancies between CHD and total cardiovascular death rates across the states, possibly due in part to variations in physician assignment of causes of death. Our aim was to identify exogenous individual- and community-level predictors of cause-of-death assignment and variability and to use these predictors to improve the comparability of CHD mortality estimates across states. METHODS AND RESULTS: We performed a multinomial logistic regression analy

The cost of hospitalization for the late complications of diabetes in the United States.

Estimates of the cost of diabetes should take into account the development of complications. Patient records identified from the 1987 National Hospital Discharge Survey were used to evaluate the risk of hospitalization due to late complications. Hospitalization for diabetic nephropathy reached a peak of 6.74/1000 between the ages of 45 and 54 years, compared to 0.14 to 1.80/1000 in controls. Diabetic patients less than or equal to 45 years of age were 46 times more likely to be hospitalized due to neuropathy. The risk of cardiovascular complications is high, with a greater incidence of arteria

Vasectomy and the incidence of hospitalized illness.

To determine the long-term effects of vasectomy on health we studied the incidence of hospitalized illness in 4,385 vasectomized and 13,155 age and race-matched nonvasectomized men. In none of the 16 disease groupings we examined was the incidence of hospitalized illness in the vasectomized men significantly different from that in the nonvasectomized men, considering men with all durations of vasectomy. Neither the incidence of acute myocardial infarction, other ischemic heart disease nor that of all atherosclerotic diseases considered as a group was significantly different between the vasecto
