Anesthetic Management of Open and Endovascular Thoracoabdominal Aortic Aneurysm Repair: When, How, and Why to Use Intraoperative Transesophageal Echocardiography. A Narrative Review and Practical Framework.

Open surgical or endovascular thoracoabdominal aortic aneurysm (TAAA) repair induces abrupt and competing hemodynamic shifts, exposing patients to significant hemodynamic instability and procedure-specific complications. Although intraoperative transesophageal echocardiography (TEE) is a well-established adjunct for intraprocedural diagnostic and anatomic assessment, its role as a structured decision-making tool to support physiologic interpretation in the dynamic perioperative setting of aortic surgery remains insufficiently explored. This narrative review synthesizes current evidence and pro

Targeting Nupr1-Mediated p53 Mitochondrial Sequestration Alleviates Aortic Dissection by Restoring the Gadd45b-Dependent Survival Program.

Aortic dissection (AD) is a catastrophic cardiovascular emergency triggered by severe pathological stress. While continuous stress drives vascular smooth muscle cell (VSMC) apoptosis via mitochondrial dysfunction, the precise upstream orchestrators linking these stress signals to the mitochondrial apoptotic cascade remain elusive. By integrating clinical observations and single-cell RNA sequencing, we found that Nuclear Protein 1 (Nupr1), a critical stress-responsive factor, is significantly upregulated in aortas from AD patients and murine models, with enrichment in VSMCs. Remarkably, VSMC-sp

Lived experiences of post-traumatic growth in patients with acute type A aortic dissection: A qualitative study.

INTRODUCTION: Acute type A aortic dissection is a life-threatening cardiovascular emergency characterized by sudden onset, severe pain, and high mortality, exerting profound physical and psychological burdens on affected individuals. While negative psychological sequelae predominate in most cases, a subset of patients demonstrates remarkable resilience, manifesting post-traumatic growth-a positive psychological transformation emerging in the aftermath of trauma. This qualitative study aimed to investigate the lived experiences of post-traumatic growth among Acute type A aortic dissection survi

Emergency vascular surgery in space, on the moon, and mars: fundamental principles, technical protocols, and hemostatic methods under different gravitational regimes (0 g, 0.16 g, 0.38 g).

Human exploration of the Moon and Mars presents unprecedented challenges for emergency vascular surgery, as gravitational environments ranging from microgravity (0 g) to partial gravity on the lunar (0.16 g) and Martian (0.38 g) surfaces render conventional Earth-based techniques inadequate. This review synthesizes data from experimental physiology, ground-based analog models, parabolic flight testing, and theoretical hemodynamic modeling to establish a framework for vascular interventions across three gravitational regimes. In microgravity, electrosurgery is entirely inapplicable, necessitati

Unexplained intraoperative cardiac tamponade during thoracoscopic lobectomy: a case report.

BACKGROUND: Cardiac tamponade is a life-threatening emergency characterized by reduced cardiac output and circulatory failure. It most commonly occurs during cardiac surgery, interventional cardiology procedures, pericardiocentesis, or central venous catheterization, as well as in the setting of acute myocardial infarction or aortic dissection. To the best of our knowledge, cardiac tamponade during video-assisted thoracoscopic surgery (VATS) lobectomy has not been reported and may has an insidious presentation, making diagnosis challenging in the absence of obvious pericardial injury. Detection of Post-Stent Medial Dissection and White Thrombus Using Hybrid IVUS OCT Imaging may Help Guide Management.

We present a 66-year-old undergoing ostial-to-mid right coronary artery (RCA) percutaneous coronary intervention (PCI) lesion proximal to an existing stent in the mid-to-distal segment. The patient underwent PCI, with intravascular imaging using the Novasight Hybrid IVUS OCT (Conavi, Toronto, CA) which revealed a proximal edge dissection and distal dissection involving the media on OCT, less discernable on IVUS, which required an additional distal stent. Proximally, based on OCT minimal luminal area (MLA) measurements of the dissection, the size of the dissection flap and lack of involvement o

Identification of perioperative risk factors for retrograde type A dissection after thoracic endovascular aortic repair for type B dissection.

OBJECTIVE: To identify risk factors and develop a predictive model for retrograde type A dissection (RTAD) following thoracic endovascular aortic repair (TEVAR) for type B aortic dissection (TBAD). METHODS: We analyzed TBAD patients who underwent TEVAR from 2014 to 2024 (IRB:No. TJ-IRB20230805). Patients developing RTAD within 1 year were matched with controls by age, sex, operation date, and location. Logistic regression identified risk factors, and a nomogram was constructed. Model performance was assessed using ROC curves. RESULTS: Of 2383 patients, 76 (3.19%) developed RTAD within one ye

Radiographic phenotyping of healthy and diseased aortas.

BACKGROUND: Ascending aortic diameter guides intervention for thoracic aneurysms. Many patients with acute type A aortic dissection (ATAAD) present with ascending aortic diameters below 5.5 cm, rendering diameter an insufficient risk discriminator. This study evaluated computed tomography angiography (CTA) morphology to identify predissection radiographic features associated with ATAAD. METHODS: Control (n = 50, CTA for non-aortic indications), aneurysm (n = 61, thoracic ascending aorta [TAA] ≥ 4.5 cm), and predissection (n = 13, CTA within 3 years before dissection) groups were studied. Staged total arch repair for acute aortic dissection using physician-modified fenestrated thoracic endovascular aortic repair: a case report.

BACKGROUND: Acute type A aortic dissection is a life-threatening condition requiring prompt surgical intervention. Although extensive repair, such as total arch replacement (TAR) with frozen elephant trunk (FET), may reduce late aortic events, this approach can be technically demanding and may increase procedural invasiveness and patient burden. A staged approach combining initial surgery with subsequent endovascular treatment may represent a practical alternative. CASE DESCRIPTION: We report a case of acute type A aortic dissection managed using a staged treatment strategy. Initial surgery w

Protocol-based medical management for inflammatory abdominal aortic aneurysms: a five-year single-center experience.

BACKGROUND: Inflammatory abdominal aortic aneurysm (IAAA), a distinct clinical entity characterized by dense periaortic fibrosis and elevated inflammatory markers, lacks a consensus-driven optimal management strategy. While medical therapy is often utilized, comparative data against surgical or endovascular interventions remain limited. We report our experience with a standardized medical protocol for the management of IAAAs. METHODS: We conducted a retrospective cohort study of 27 consecutive patients diagnosed with IAAA between 2012 and 2019. Predefined diagnostic criteria for IAAA included

Cytomegalovirus Enteritis after Surgery for Acute Type A Aortic Dissection Complicated by Malperfusion and Thymoma: A Case Report.

INTRODUCTION: Cytomegalovirus (CMV) enteritis is a potentially life-threatening gastrointestinal infection typically observed in immunocompromised individuals, such as transplant recipients or patients receiving chemotherapy or long-term corticosteroids. However, CMV enteritis can also develop in the absence of overt immunosuppression, particularly in elderly or critically ill patients under physiological stress, nutritional decline, or prolonged hospitalization. Diagnosis in these settings is challenging because clinical symptoms and imaging findings are nonspecific and may overlap with ische
