Leveraging in-silico deep learning and computational analyses to predict the pathogenicity of ROBO4 variants of uncertain significance in aortic aneurysm and dissection patients.

Variants of uncertain significance (VUS) in genes implicated in thoracic aortic aneurysm (TAA) present clinical challenges due to ambiguous pathogenicity and low patient representation. This study investigates the pathogenic potential of missense VUS in the ROBO4 gene, previously associated with vascular integrity and ascending aortic aneurysm. Clinical and genetic data from five patients with heterozygous ROBO4 VUS and thoracic aortic aneurysms or dissections were analyzed. Computational tools including AlphaFold2, AlphaMissense, REVEL, PolyPhen-2, SIFT, FATHMM, MutationTaster2, GranthamMatri

Mortality Due to Aortic Dissection in Adults With Primary Hypertension: A Nationwide Analysis Over Two Decades.

OBJECTIVE: Hypertension is a key risk factor for aortic dissection (AD). AD, if left untreated, carries significant mortality rates. Our aim is to analyse trends in mortality due to AD in adults with primary hypertension in the United States (US). METHODS: We used the CDC WONDER database to extract mortality data for patients with primary hypertension who died due to AD. Age-adjusted mortality rates (AAMRs) and crude mortality rates (CMRs) were extracted per 100 000 persons. Annual percentage changes (APCs) and average APCs (AAPCs) in AAMRs and CMRs were calculated using Joinpoint regression.

Epigenetic Programming of Macrophage Phenotypes by STING-IRF3 Drives Inflammation in Ascending Thoracic Aortic Dissection.

BACKGROUND: Ascending thoracic aortic dissection (ATAD) is characterized by extensive macrophage (MΦ) accumulation and profound inflammation; however, the mechanisms sustaining pro-inflammatory MΦ activation remain incompletely defined. Emerging evidence indicates that epigenetically generated immune memory drives innate immune cells toward persistent inflammatory states. In this study, we investigated whether epigenetic reprogramming governs MΦ phenotypic fate and contributes to ATAD pathogenesis. METHODS: We performed single-cell RNA sequencing of human ascending aortic tissues from control

The National Association of Medical Examiners (NAME) Position Paper: Postmortem Genetic Testing in Forensic Pathology Cases.

Individuals who die suddenly and unexpectedly will often fall under the jurisdiction of a Medical Examiner or Coroner (MEC). Forensic pathologists may therefore be the first physicians to diagnose a genetic disease. Identifying these conditions at autopsy improves the accuracy of death certification and provides biological relatives the opportunity to seek diagnosis and intervention. Accessibility and diagnostic capabilities of postmortem genetic testing (PMGT) have rapidly expanded over the last decade since the previous National Association of Medical Examiners position paper on this topic.

Return to Work After Type A Aortic Dissection.

Objective Acute type A aortic dissection (TAAD) frequently affects patients during working years, but patient-centered employment outcomes after operative repair are not well characterized. The aim of this study was to describe return-to-work and self-reported income loss among 30-day survivors using a voluntary survey within a dual-center, single health system. Methods After Institutional Review Board (IRB) approval, we conducted a retrospective review of a prospectively maintained database and included any patient who suffered an acute type A aortic dissection that was treated operatively. [Challenges and strategies in managing distal aneurysms after endovascular repair of aortic dissection].

Aortic dissection is a life-threatening cardiovascular condition, with thoracic endovascular aortic repair (TEVAR) emerging as the preferred treatment for Stanford type B aortic dissection. TEVAR facilitates thrombosis of the false lumen and remodeling of the true lumen by sealing the initial entry tear. While distal re-entry tears are often left untreated, which can result in the formation and progression of distal aneurysms. Current surgical techniques for managing distal aneurysms each possess distinct advantages and limitations, while their complex anatomical characteristics present signif

CircRNA-TMEM71 Inhibits Human Aortic Smooth Muscle Cell Ferroptosis and Prevents Aortic Dissection Formation Through IGF2BP3-Mediated Stability of FSP1.

BACKGROUND: Aortic dissection (AD) is a severe cardiovascular disease with high mortality and limited treatment options. Previous studies have shown that circular RNA and ferroptosis play significant roles in various cardiovascular diseases, regulating disease progression. However, there is little research on how circular RNA regulates ferroptosis in vascular smooth muscle cells during AD progression, and the specific molecular mechanisms remain a mystery. METHODS AND RESULTS: In this study, we found that circTMEM71 is downregulated in AD and angiotensin II-induced human aortic smooth muscle

Fibroblast Activation Protein Promotes Thoracic Aortic Dissection via PLAUR/ITGB1-Mediated Pro-inflammatory Macrophage Polarization.

BACKGROUND: Thoracic aortic dissection (TAD) is a lethal vascular emergency lacking targeted therapies. Fibroblast activation protein (FAP), a protease implicated in tissue remodeling, exhibits unknown roles in TAD pathogenesis. METHODS: Human TAD specimens and β-aminopropionitrile-induced TAD models were used to assess FAP expression. Global and fibroblast-specific Fap knockout mice were generated to evaluate biological effects of FAP. Bulk and single-cell RNA sequencing were used to map cellular crosstalk and dysregulated signaling pathways. A combination of pharmacological inhibition (Ac-G

Complex aortic reconstruction using double-barrel frozen elephant trunks.

We present a complex aortic reconstruction for a 60-year-old patient presented with chronic type B aortic dissection, with a large proximal entry tear in zone 3, aneurysmal degeneration in the distal arch, and a small true lumen with a calcified septum. Endovascular options were limited by a poor proximal landing zone, difficulty excluding the arch pseudoaneurysm while maintaining true and false lumen flow, lack of septal fenestrations, and a calcified septum. Open extent 2 thoracoabdominal repair carried a significantly higher risk compared with sternotomy and ascending aortic and arch replac

STF083010 Mitigates Thoracic Aortic Dissection by Modulating the NLRP3 Inflammasome-Dependent Pyroptosis.

Thoracic aortic dissection (TAD) is a life-threatening vascular pathology with limited therapeutic options. The role of the small molecule STF083010, a novel IRE1α RNase-specific inhibitor, in TAD has not yet been defined. We aimed to target the IRE1α-XBP1s axis pharmacologically using STF083010, and evaluate its therapeutic potential in the occurrence and progression of TAD. Using a β-aminopropionitrile monofumarate (BAPN)-induced murine TAD model combined with interleukin 1β (IL1β)-stimulated vascular smooth muscle cells (VSMCs), we systematically evaluated the effects of STF083010 on the de

Modified bentall surgery using a graft insertion technique for prosthetic valve endocarditis with left ventricular-aortic discontinuity: a case report.

BACKGROUND: Infective prosthetic valve endocarditis is a life-threatening condition with high morbidity and mortality; it is often complicated by periannular abscesses and left ventricular–aortic (LV–Ao) discontinuity. These situations make standard valve or root replacement technically challenging and require complex reconstructive procedures. CASE PRESENTATION: We report a 59-year-old man with a history of hemiarch replacement and aortic valve replacement for acute Stanford type A dissection. He presented with fever and heart failure symptoms. Imaging revealed severe prosthetic valve regurgi

Dissected Thoracic Aorta Masked as Seizures: A Case Report.

Background: Aortic dissection (AoD) is a life-threatening medical emergency characterized by the separation of the layers of the aortic wall. The typical clinical presentation of AoD includes intense thoracic pain in the anterior chest or interscapular region, often described as migratory and tearing in nature. However, in rare cases, AoD can present without classic signs but with neurological symptoms, including seizures. Case Presentation: A 60-year-old patient experienced a sudden loss of consciousness followed by a tonic-clonic seizure and subsequently developed right-sided weakness. He ha

From Endothelial Barrier Dysfunction to Circulating Biomarker: Clinical Potential of Claudin-5 in Thoracic Aortic Aneurysm and Dissection.

Background and Objectives: Thoracic aortic aneurysm and dissection (TAAD) is a life-threatening vascular disease with limited effective diagnostic and therapeutic strategies. Although endothelial barrier dysfunction represents an early event in TAAD pathogenesis, the role of endothelial tight junction proteins remains largely undefined. In this study, we systematically explored the function of claudin-5 (CLDN5), an endothelial-specific tight junction sealing protein, in TAAD through integrated bioinformatic, clinical, and experimental approaches. Materials and Methods: In the study, we combine

Could the combination of fibrinogen and D-dimer improve risk assessment for acute type A aortic dissection in the emergency department?

D-dimer has been proven to be an excellent rule-out marker for acute type A aortic dissection (ATAAD) with a sensitivity exceeding 95% while specificity remains low. We hypothesized that the addition of fibrinogen, which is decreased in ATAAD patients due to consumption coagulopathy, would increase predictive ability. This was an exploratory, retrospective, single-region, observational study comparing ATAAD patients with available preoperative D-dimer and fibrinogen values who underwent surgery at our department between 2008 and 2023 (93 patients) to all patients who visited an emergency depar

Notch1 gene promoter methylation promotes phenotypic transformation and functional alteration of VSMCs to regulate acute type A aortic dissection.

Acute type A aortic dissection (ATAAD) is a life-threatening cardiovascular emergency. A key driver of disease progression is dysfunction of vascular smooth muscle cells (VSMCs), whose loss of contractile phenotype and increased matrix degradation contribute to aortic wall weakening. Aberrant DNA methylation patterns have been observed in thoracic aortic disease and linked to VSMCs dysfunction and extracellular matrix degradation. This study aimed to investigate the change of DNA methylation in ATAAD and the mechanism underlying notch receptor 1 (Notch1) promoter methylation regulating VSMCs t

Machine-learning-guided transcriptomic integration identifies GFM1 as a lactylation-related candidate biomarker in aortic dissection.

Aortic dissection (AD) is a life-threatening aortic disease with limited disease-modifying pharmacologic options. Lysine lactylation is a metabolism-linked post-translational modification implicated in vascular and inflammatory biology, but its relationship to AD has not been well characterized. Public AD transcriptomic datasets were integrated for differential expression analysis and WGCNA. Lactylation-related DEGs were defined by intersecting DEGs with a curated lactylation-related gene set. Candidate genes were prioritized using complementary machine-learning models (LASSO, Random Forest, a
