Staged total arch repair for acute aortic dissection using physician-modified fenestrated thoracic endovascular aortic repair: a case report.

BACKGROUND: Acute type A aortic dissection is a life-threatening condition requiring prompt surgical intervention. Although extensive repair, such as total arch replacement (TAR) with frozen elephant trunk (FET), may reduce late aortic events, this approach can be technically demanding and may increase procedural invasiveness and patient burden. A staged approach combining initial surgery with subsequent endovascular treatment may represent a practical alternative. CASE DESCRIPTION: We report a case of acute type A aortic dissection managed using a staged treatment strategy. Initial surgery w

Clinical Outcomes and Exploratory Haemodynamic Numerical Simulation of Patients with Multichannelled Aortic Dissection: A Multicentre, Retrospective Cohort Study.

OBJECTIVE: Multichannelled aortic dissection (MCAD) is a rare type of aortic dissection with limited data on its clinical features and outcomes. This multicentre, retrospective cohort study compared clinical outcomes between MCAD and double channelled aortic dissection (DCAD) and explored haemodynamic differences. METHODS: Patients with aortic dissection were enrolled from three tertiary hospitals. Patients with one true lumen and one false lumen were classified as having DCAD, whereas those with one true lumen and multiple false lumina were classified as having MCAD. Propensity score matchin

Cytomegalovirus Enteritis after Surgery for Acute Type A Aortic Dissection Complicated by Malperfusion and Thymoma: A Case Report.

INTRODUCTION: Cytomegalovirus (CMV) enteritis is a potentially life-threatening gastrointestinal infection typically observed in immunocompromised individuals, such as transplant recipients or patients receiving chemotherapy or long-term corticosteroids. However, CMV enteritis can also develop in the absence of overt immunosuppression, particularly in elderly or critically ill patients under physiological stress, nutritional decline, or prolonged hospitalization. Diagnosis in these settings is challenging because clinical symptoms and imaging findings are nonspecific and may overlap with ische

Primary small cell neuroendocrine carcinoma of the tonsil combined with squamous cell carcinoma of the hypopharynx: a case report.

BACKGROUND: Primary small cell neuroendocrine carcinoma (SCNEC) of the tonsils is extremely rare and is often misdiagnosed as squamous cell carcinoma. Cases coexisting with hypopharyngeal squamous cell carcinoma (HPSCC) have not been reported. The presence of concurrent aortic arch aneurysm further complicates treatment. CASE DESCRIPTION: A 71-year-old male presented with pharyngeal pain lasting over one month. Imaging and pathological examinations confirmed right tonsillar SCNEC (with approximately 80% Ki-67 positivity) and left HPSCC, along with concurrent aortic arch aneurysm and stage III

Preoperative Risk-assessment Score for Elective Total Aortic Arch Repair: AAORTA Risk Tool.

OBJECTIVE: Total transverse aortic arch replacement is a complex operation performed to address aortic aneurysm with and without chronic aortic dissection. To predict 90-day survival after this procedure, we created a holistic tool for predicting postoperative mortality in patients who undergo total arch repair (TAR): the Aortic Arch Operation Risk Tool for Assessment (AAORTA) score. METHODS: Univariate and multivariate analyses were performed on data from patients (N=470; 243 with nondissecting aneurysm, 227 with chronic Type A dissection) who underwent TAR at a high-volume practice between

Predicting acute kidney injury in type B aortic dissection: development and validation of a clinical nomogram.

OBJECTIVES: Acute Kidney Injury (AKI) is a frequent and serious complication in patients with Type B Aortic Dissection (TBAD). Existing prediction models are mainly derived from type A or mixed cohorts and do not account for the distinct pathophysiology of TBAD. This study aimed to develop and validate a nomogram for individualized AKI risk prediction in TBAD patients. METHODS: Data were obtained from the eICU Collaborative Research Database (eICU-CRD) and the Medical Information Mart for Intensive Care IV (MIMIC-IV). The eICU-CRD cohort was used for model development and internal validation,

An Anatomical Variant of the Accessory Right Hepatic Artery Arising From the Superior Mesenteric Artery: A Cadaveric Case Report.

The right hepatic artery (RHA) provides blood flow to the right lobe of the liver. In typical anatomy, the RHA branches from the proper hepatic artery, which branches from the common hepatic artery originating from the celiac trunk of the abdominal aorta. During a standard cadaveric dissection, a large accessory right hepatic artery (aRHA) was noted arising directly from the superior mesenteric artery. The lumen of the identified aRHA measured approximately 1 cm in diameter. The artery also appeared darker in color than the surrounding vasculature, with no evidence of prior surgery or procedur

Thoracic endovascular aortic repair in hyperacute type B aortic dissection: a mini review.

Thoracic endovascular aortic repair (TEVAR) is established as the preferred treatment for many patients with complicated acute type B aortic dissection (TBAD). However, its role during the hyperacute phase (within 24 hours of symptom onset) remains controversial, as the acutely dissected aortic wall may be vulnerable to device-related injury. Nevertheless, hyperacute timing should not be regarded as an absolute contraindication to TEVAR. In patients with rupture, malperfusion, impending aortic rupture, persistent pain, refractory hypertension, or progressive true-lumen collapse, prompt entry-t

Spontaneous Spinal Cord Infarction Presenting as Anterior Spinal Artery Syndrome: A Case Report.

Spinal cord infarction is a rare cause of acute myelopathy and may be difficult to recognize when it occurs outside a traumatic or perioperative setting. We report a 68-year-old man with hypertension, type 2 diabetes mellitus, dyslipidemia, and active smoking who presented with sudden severe low back pain followed within one hour by rapidly progressive bilateral lower-limb weakness and sensory loss. Neurologic examination showed severe paraparesis, absent lower-limb reflexes, impaired pain sensation below T10, and relative preservation of proprioception. Aortic computed tomography angiography

Preoperative levels of biomarkers of coagulation to predict massive bleeding after surgical repair for acute type A aortic dissection: a retrospective cohort study.

BACKGROUND: Acute type A aortic dissection is a highly emergent condition with a high mortality rate. The condition poses an imminent risk of serious complications such as coagulopathy with massive bleeding. This increases the risk of mortality even after surgical repair. Procoagulant drugs are routinely administered but some patients still suffer postoperative massive bleeding that can be fatal. The study aimed to investigate whether massive bleeding after surgical treatment for type A aortic dissection can be predicted preoperatively through routine coagulation tests such as APTT, PT (INR),

The risk of not imaging: a three-scenario framework for justification.

A recurring argument in radiological protection holds that the radiation risk of an imaging examination must be weighed against the clinical risk of not performing it. The argument is applied inconsistently, and evidence on that clinical risk remains sparse compared with the extensive literature on radiation risk. This asymmetry masks an important distinction: the clinical risk of not imaging is not uniformly high. We propose a three-scenario framework for medical imaging, illustrated using computed tomography. In Scenario 1, the harm of withholding or delaying imaging is well documented and s
