Bacterial aortic endarteritis with sepsis as a rare complication of aortic dissection in a cat.

A 14-year-old domestic shorthair cat was examined because of respiratory distress. Thoracic radiography showed a prominent aortic arch and no other abnormalities. Echocardiography documented a mobile flap within the ascending aorta creating 2 lumens, consistent with aortic dissection (AD), mild pericardial effusion with tamponade, left ventricular concentric hypertrophy, and normal left atrial size. Systemic hypertension (200 mmHg) was the presumed cause of AD, prompting administration of telmisartan, furosemide, and amlodipine. The cat clinically improved, became normotensive, and was dischar

The 'R sign' as a potential risk factor for progression of uncomplicated Type B aortic dissection: a case series.

BACKGROUND: Complicated and uncomplicated Type B aortic dissection (TBAD) need distinct therapeutic pathways. While TBAD is a complex and dynamic clinical entity, ∼75% of cases present as uncomplicated initially. However, up to half of these may progress to complicated TBAD, highlighting the need for better prognostic tools. In this case series, we report a novel potential radiological risk factor for progression, the 'R sign', to contribute to a deeper understanding of this disease's behaviour. CASE SUMMARY: We present two male patients with TBAD. Both cases exhibited a localized dissection

TRPC1 Promotes Thoracic Aortic Dissection Through Inducing Proinflammatory Phenotype of Vascular Smooth Muscle Cells.

BACKGROUND: Thoracic aortic dissection (TAD) is a life-threatening vascular disease that lacks effective pharmacological treatment. Canonical transient receptor potential (TRPC) channels are involved in regulating various cardiovascular functions, yet their roles in TAD have not been elucidated. METHODS: Three-week-old male mice were administered β-aminopropionitrile dissolved in drinking water for 28 days to induce TAD formation. The effects of TRPCs on TAD were explored in vitro and in vivo. RNA sequencing of thoracic aortas was performed to investigate the mechanism by which TRPCs regulate

Mouse Abdominal Aortic Aneurysm Model Induced by Perivascular Sheath Microinjection of Chymotrypsin.

Abdominal aortic aneurysm (AAA) is a life-threatening degenerative vascular disease for which no effective pharmacological therapy currently exists. Reliable and technically accessible animal models are essential for mechanistic and preclinical investigations. However, existing murine platforms are limited by technical complexity, prolonged operative time, or incomplete recapitulation of human AAA pathology. Here, this study presents a novel murine AAA model established by direct perivascular sheath microinjection of chymotrypsin. After midline laparotomy and identification of the infrarenal a

Isolation and Culture of Primary Murine Aortic Smooth Muscle Cells for In vitro Studies Relevant to Vascular Remodeling and Aortic Aneurysm Biology.

Isolation and cultivation of primary aortic smooth muscle cells (SMCs) provide a valuable approach for investigating cellular mechanisms underlying vascular diseases, including aortic aneurysms. The present article describes a detailed and optimized protocol for the isolation and cultivation of murine aortic SMCs from thoracic and abdominal aortic segments. The current procedure includes careful aortic dissection, removal of surrounding tissue, and a two-step enzymatic digestion using collagenase, elastase, and soybean trypsin inhibitor, followed by mechanical dissociation and cell expansion u

Performance and incremental value of preoperative multimodal imaging and perioperative clinical characteristics in predicting postoperative major adverse events in type A aortic dissection.

BACKGROUND: Postoperative major adverse events (MAEs) can occur after surgical repair of acute Stanford type A aortic dissection (ATAAD), and conventional clinical assessment may not fully capture the prognostic information provided by preoperative multimodal imaging. This study aimed to develop and externally validate a staged risk prediction model for postoperative MAEs and to evaluate the incremental value of preoperative multimodal imaging relative to conventional clinical and laboratory assessment in the prediction of MAEs. METHODS: In this retrospective, two-center cohort study, patient

Meta-analysis of the histopathological features of surgically removed aortas classified according to the SCVP and AECVP consensus documents.

The Society for Cardiovascular Pathology (SCVP) and the Association for European Cardiovascular Pathology (AECVP) published consensus statements on the surgical pathology of the aorta to unify nomenclature and provide a new classification scheme to systematically evaluate the degenerative and inflammatory lesions of the thoracic aorta. The aim of this meta-analysis was to determine the histopathological features of thoracic aneurysmatic aortas, thoracic dissected aortas, and thoracic aortas in patients with a bicuspid aortic valve (BAV). Additionally, the aim was to map how consensus documents

Labetalol-induced hyperkalemia in acute aortic dissection: A rare but clinically relevant adverse effect.

BACKGROUND: Hyperkalemia is a potentially life-threatening electrolyte abnormality often associated with renin-angiotensin-aldosterone system (RAAS) inhibitors, renal dysfunction, or potassium-sparing agents. Non-selective beta-blockers, including labetalol, may also precipitate hyperkalemia through beta-2 (β-2)-adrenergic blockade, although this adverse effect is underrecognized, particularly in patients without underlying renal disease. CASE PRESENTATION: A 59-year-old man with hypertension, diabetes, asthma, and polysubstance use disorder presented with severe chest pain and hypertensive e

Towards a regional and national cardiothoracic biobank in India: translating a Dutch model to address a poorly characterised disease burden.

Cardiothoracic disease in India presents earlier and against a different biological background than in the high-income populations from which most evidence and guidelines derive. Coronary disease occurs about a decade earlier in South Asians; rheumatic heart disease remains endemic, with India carrying a quarter to a half of the global burden; and acute type A aortic dissection presents in the mid-to-late forties, with a higher reported proportion of connective-tissue disease than Western registries. The biological substrate of these conditions in Indian patients is largely uncharacterised, an

Admission blood glucose and neutrophil-to-lymphocyte ratio provide independent and complementary prognostic value for in-hospital mortality in acute type A aortic dissection.

Acute type A aortic dissection (ATAAD) remains a critical cardiovascular emergency with substantial in-hospital mortality. A rapid and interpretable preoperative risk stratification using routinely available biomarkers is clinically desirable. We investigated whether combining blood glucose and the neutrophil-to-lymphocyte ratio (NLR) provides incremental prognostic information beyond either marker alone and developed a bootstrap-validated nomogram for early risk estimation. In this study, 609 patients who underwent Stanford type A acute aortic dissection surgery were enrolled. Logistic regres

ST elevation in lead aVR - a marker of critical left main coronary artery stenosis: A series of case reports and literature review.

RATIONALE: Lead augmented vector right (aVR) has historically been overlooked in acute coronary syndromes. ST-segment elevation (STE) in lead aVR with diffuse ST depression in multiple leads has emerged as a critical marker for left main coronary artery (LMCA) stenosis and triple-vessel disease (3VD). Recognition of this high-risk electrocardiogram (ECG) pattern is essential for timely diagnosis and improved outcomes. PATIENT CONCERNS: In this retrospective case series, 5 consecutive patients presented to the emergency department between January 2023 and June 2024 with acute chest pain or car

Whole-Genome Sequencing and Single-Cell Transcriptomics Identify Human Leukocyte Antigen Loci and a COG5 Variant in Aortic Dissection.

BACKGROUND: Aortic dissection (AD) is a lethal cardiovascular emergency with a complex and underexplored genetic basis. METHODS: We performed whole-genome sequencing on 151 patients with sporadic Stanford type B AD and 965 healthy controls in a Chinese population as a discovery phase analysis to assess both common and rare variants. Cross-ancestry meta-analyses incorporating 4 external genome-wide association data sets were used to identify population-specific risk loci. To pinpoint functional candidates, we integrated epigenomic profiles and single-cell transcriptomes from human aortic tissu

Suspected Mesenteric Malperfusion in Acute Type A Aortic Dissection During Pregnancy: A Case Report and Review of Reperfusion-First Strategies.

Aortic dissection during pregnancy is rare but potentially catastrophic. Acute Type A aortic dissection generally requires urgent surgical repair. When visceral malperfusion is present, however, the optimal sequence of interventions may be complex, particularly when pregnancy and recent cesarean delivery increase operative risk. Endovascular reperfusion followed by delayed aortic repair has been proposed in selected patients, although evidence supporting this strategy in pregnancy remains limited. A 37-year-old woman at 31 weeks' gestation presented with chest and back pain. Initial CT pulmona

Curative-intent lung resection after recovery from fulminant immune checkpoint inhibitor-associated myocarditis during neoadjuvant therapy: a case report.

BACKGROUND: Immune checkpoint inhibitor (ICI)-associated myocarditis is a rare but potentially fatal immune-related adverse event (irAE) that may interrupt planned curative treatment for resectable non-small-cell lung cancer. However, the suitability of definitive surgery after recovery from fulminant myocarditis remains unclear. CASE PRESENTATION: A 53-year-old man with left upper lobe adenocarcinoma and ipsilateral hilar nodal disease was administered four cycles of neoadjuvant cisplatin, pemetrexed, and pembrolizumab, resulting in a partial response. Single-stage hybrid repair of extensive aortic dissection with traumatic diaphragmatic hernia.

We report a rare case of a 70-year-old male patient who developed a left diaphragmatic hernia combined with extensive aortic dissection and exhibited signs of lower limb ischemia following a motor vehicle accident. An urgent, single-stage surgical intervention was performed, consisting of endovascular aortic repair first to stabilize circulation, followed by diaphragmatic repair. The patient recovered smoothly postoperatively, with no abnormalities observed at 1-month and 6-month follow-ups. This case highlights the necessity of accurate diagnosis and multidisciplinary team collaboration in em

Marfan Syndrome versus Loeys-Dietz Syndrome after Aortic Surgery: A Systematic Review and Meta-analysis of Reconstructed Time-to-Event Data.

OBJECTIVE: Marfan syndrome (MFS) and Loeys-Dietz syndrome (LDS) are hereditary connective tissue diseases, both characterized by aortic root and vessel involvement with an increased risk of aneurysms and dissections. Despite advances in surgical techniques and improved postoperative follow-up, the impact of surgery on long-term outcomes in these patient groups is still unknown. METHODS: We performed a systematic review and a meta-analysis to compare the postoperative long-term events. The main outcome was long-term survival. Additional outcomes were freedom from dissection and reoperation. Af
