Aortic dissection is a major aortic catastrophe associated with significant morbidity and mortality. While Thoracic Endovascular Aortic Repair (TEVAR) is the standard treatment for chronic type B aortic dissection cases, a persistent fibrotic septum often prevents complete false lumen exclusion, leading to continued pressurization and treatment failure. Endovascular septostomy is increasingly explored as an adjunctive therapy to modify this septum and promote aortic remodeling. However, current techniques carry risks such as embolic debris generation, thermal injury, and unpredictable tearing,

INTRODUCTION: Internal hernia involving the stomach associated with a sigmoid colostomy is extremely rare. The hernia defect is typically located between the colostomy limb and the left abdominal wall. Because only a few cases have been reported, no standardized management strategy has been established, particularly in patients in whom immediate colostomy closure is not appropriate.
CASE PRESENTATION: A woman in her 70s underwent a laparoscopic Hartmann's procedure with sigmoid colostomy for obstructive sigmoid colon cancer with para-aortic lymph node metastasis. On POD 9, the patient develop

BACKGROUND: Prosthetic aortic infection involving composite roots and frozen elephant trunk components presents an extraordinary challenge with high mortality. We present a complex aortic reconstruction using custom bovine pericardial conduits.
CASE SUMMARY: A 55-year-old man with a history of a redo Bentall procedure and frozen elephant trunk implantation presented with extensive pan-aortic prosthetic infection, systemic septic emboli, and acute intracranial hemorrhage. Surgery focused on radical debridement and complete extrication of all synthetic material. The aortic root, ascending aorta

Chronic Type B aortic dissection (cTBAD) often requires aortic septostomy to depressurize the false lumen and enable effective endovascular repair. However, current "off-label" techniques, such as mechanical "cheese-wire" or electrosurgical cutting, lack precision and carry significant risks of uncontrolled tearing and collateral thermal injury. To address this unmet need, we developed the Aquablade catheter, which utilizes a dual-guidewire system and a contained, athermal, high-velocity fluid jet to precisely cut the aortic septum longitudinally. This study reports the design and preliminary

Infective endocarditis following transcatheter aortic valve implantation represents an increasingly recognized and devastating clinical entity characterized by severe morbidity, high mortality, and formidable surgical complexity. As indications for transcatheter aortic valve procedures expand toward younger and lower-risk patient cohorts, late-onset endocarditis poses significant therapeutic dilemmas. Surgical management is technically challenging due to dense tissue integration, neointimal proliferation, and complete endothelialization of the expanded metallic frame into the aortic wall and s

Marfan syndrome, caused primarily by mutations in the fibrillin-1 (FBN1) gene, is associated with progressive dilation of the proximal aorta, with dissection and rupture representing the leading causes of early mortality. A characteristic feature of Marfan-associated aneurysm is fragmentation of elastic fibers; however, the extent to which elastin loss alone can account for the observed geometric and mechanical changes remains unclear. In this study, we use a multiscale computational growth and remodeling framework to determine which geometric and mechanical features of the Marfan aorta can be

Accurate identification of aortic dissection (AD) in emergencies is clinically critical. Non-contrast CT (NC-CT), the standard emergency imaging for chest pain, has limited AD diagnostic sensitivity. We developed an anatomy prior-driven multimodal contrastive learning framework, extracting discriminative features from contrast-enhanced CT (CE-CT) and radiological reports to enhance NC-CT-based AD detection. Aortic segmentation and straightening focus analysis on relevant anatomy, while our tri-encoder architecture leverages Fourier cross-attention (core innovation) to co-extract spatial and fr

Aortic dissection is a life-threatening cardiovascular emergency characterized by rapid progression and high mortality. Although medial degeneration is a key pathological feature of aortic dissection, the mechanisms that drive aortic wall instability remain incompletely understood. Aortic calcification is a pathological mineralization process that can alter vascular structure and function, but its role in aortic dissection has received limited attention. Here we show that tissue nonspecific alkaline phosphatase associated calcium phosphate microdeposition is linked to aortic medial degeneratio

Iatrogenic aortic dissection (IAD) is a rare but life-threatening complication of open cardiac surgical procedures and catheter-based interventions, associated with high morbidity and mortality. With an ageing population and increasing procedural complexity, the absolute burden of IAD is expected to rise. Unlike spontaneous aortic dissection, IAD typically results from direct intimal injury during cannulation, clamping, catheter manipulation, or device deployment-while its clinical course is strongly influenced by the procedural context and the speed of recognition. Evidence to guide the manag

Severe perivisceral aortic thrombus is a rare cause of embolic organ dysfunction with limited treatment guidance. We report a 70-year-old woman with progressive renal failure due to extensive perivisceral and renal artery thrombus. Given high embolic risk, endovascular therapy was deferred, and open aortic endarterectomy was performed. Persistent renal artery compromise necessitated intraoperative conversion to extra-anatomic thoracobirenal bypass. Postoperatively, renal function normalized with durable graft patency at 1 year. A concomitant descending thoracic aortic dissection was managed co

Short common iliac artery anatomy may preclude iliac branch device use during fenestrated or branched aortic repair. We describe two patients treated with a low-profile, short-tip distal branched custom-made device based on an aortouni-iliac configuration and combined with fenestrated or branched repair. The device incorporated iliac side branches to preserve bilateral internal iliac artery perfusion. In aneurysmal anatomy, complete release caused loss of ipsilateral branch access, requiring through-and-through control and balloon-assisted luminal modulation. In chronic dissection, staggered r

We report the case of a patient who developed an acute Stanford type B aortic dissection 4 days after thoracoabdominal branch endoprosthesis repair for a ruptured thoracoabdominal aortic aneurysm. The dissection caused complete endograft collapse and profound malperfusion. Emergent salvage endovascular repair successfully re-expanded the graft; however, the patient ultimately died because of the severity of the systemic injuries. This case highlights the potential for acute type B aortic dissection to occur in a recently stented thoracic aorta segment following thoracoabdominal branch endopros

BACKGROUND: Primary malignant melanoma of the uterine cervix (PMMC) is an exceptionally rare mucosal malignancy with an aggressive clinical behavior and no established standard of care. Distinguishing a primary cervical melanoma from metastatic disease is particularly challenging and requires integration of pathological findings with comprehensive evaluation for an alternative primary site.
CASE PRESENTATION: We report a 67-year-old postmenopausal woman presenting with vaginal bleeding and an apparently benign endocervical polyp. Histopathological and immunohistochemical examination establish

Aortic dissection (AD) is a life-threatening cardiovascular disease with an extremely high mortality rate. Excessive reactive oxygen species (ROS) production and inflammatory responses in diseased vascular regions are central drivers of AD pathogenesis, leading to destruction of the aortic wall structure and fatal hemorrhage. However, conventional clinical therapies for AD cannot effectively eliminate excessive ROS at lesion sites while simultaneously exerting anti-inflammatory effects. To address these limitations, we developed a biomimetic, inflammation-targeting nanoparticle, ZIF-8@Res@PEG-

BACKGROUND: Iatrogenic crescentic hematoma of the right coronary sinus is a rare complication after coronary angiography. It mimics aortic root dissection, easily triggering diagnostic confusion and unnecessary invasive interventions.
CASE SUMMARY: A 60-year-old man developed this hematoma during coronary angiography. Multimodal imaging excluded aortic extension and secondary cardiac complications. The patient remained hemodynamically stable and received conservative management, with complete hematoma absorption seen at the 3-month follow-up.
DISCUSSION: The 2 lesions share similar imaging m
