Leveraging Large Language Models for Automated Extraction of Abdominal Aortic Aneurysm Features from Radiology Reports.

Background/Objectives. Abdominal computed tomography (CT) radiology reports contain critical information for abdominal aortic aneurysm (AAA) management, including aneurysm presence, size, rupture status, and prior repair. However, this information is often embedded within lengthy, heterogeneous reports, making manual extraction inefficient. We evaluated the performance of multiple large language models (LLMs) for automated extraction of AAA-related findings from radiology reports. Methods. We retrospectively analyzed 500 abdominal CT reports mentioning AAA from an urban academic health system

Harnessing Artificial Intelligence to Restore Standards in Medical and Dental Education in Pakistan.

The month of March is dedicated to Colon Cancer Awareness. Worldwide, colorectal cancer (CRC) incidence has been on the rise. It is currently the third most common cancer in men (746,000 cases, 10.0% of the total) and the second in women (614,000 cases, 9.2% of the total).1 Arecent meta-analysis reported a 61% risk reduction in CRC incidence with colonoscopy.2 Unlike screening programs for breast and prostate cancers, not only has CRC screening reduced mortality from colon cancer and detected early CRC, it has also decreased the incidence of CRC through detection and removal of pre-cancerous l

Kidney Volume after Endovascular Exclusion of Abdominal Aortic Aneurysms by EVAR and FEVAR.

BACKGROUND: Decreased kidney volume is a sign of renal aging and/or decreased vascularization. The aim of this study was to determine whether renal volume changes 24 months after exclusion of an abdominal aortic aneurysm (AAA) and to compare fenestrated (FEVAR) and infrarenal (EVAR) endovascular repair. METHODS: Retrospective single-center study from a prospective registry, including patients between 60 and 80 years with normal preoperative renal function (estimated glomerular filtration rate [eGFR] ≥ 60 ml/min/1.73 m-2) who underwent fenestrated (FEVAR) or infrarenal (EVAR) stent grafts betw

Analysis of High-Risk Factors and Mortality Prediction of Ruptured Abdominal Aortic Aneurysm.

INTRODUCTION: Ruptured abdominal aortic aneurysms (RAAAs) are among the most dangerous emergencies in vascular surgery, with a high death rate and numerous risk factors influencing perioperative death. Therefore, identifying the critical risk factors for RAAAs is crucial to increasing their survival rate. Our aim was to identify those risk factors from a wide range of parameters. METHODS: Retrospective analysis of hospitalized RAAA patients treated at this center between May 2004 and January 2023. After comparing the preoperative data of patients who survived and those who died, high-risk cha

Multicenter Study on Physician-Modified Endografts for Thoracoabdominal and Complex Abdominal Aortic Aneurysm Repair.

BACKGROUND: Physician modified endografts (PMEGs) have been widely used in the treatment of complex abdominal aortic aneurysm and thoracoabdominal aortic aneurysm, however, previous data are limited to small single center studies and robust data on safety and effectiveness of PMEGs are lacking. We aimed to perform an international multicenter study analyzing the outcomes of PMEGs in complex abdominal aortic aneurysms and thoracoabdominal aortic aneurysms. METHODS: An international multicenter single-arm cohort study was performed analyzing the outcomes of PMEGs in the treatment of elective, s

Impact of posthospital syndrome on outcomes of elective endovascular repair of abdominal aortic aneurysm.

OBJECTIVE: Posthospital syndrome (PHS) is an acquired, transient period of health vulnerability after a hospital admission for acute illness. It is characterized by physiologic deconditioning secondary to stressors from disruption in circadian rhythm, depletion of nutritional and physiologic reserve as well as the pain and discomfort associated with hospitalization. PHS is reported as an independent risk factor for readmission and adverse postoperative outcomes. The aim of this study is to investigate whether preoperative hospitalization affects outcomes of elective endovascular repair of abdo

Biomechanical Properties of the Periaortic Abdominal Tissue: It is Not as Fragile as It Seems.

BACKGROUND: The perivascular adipose tissue has been studied as a critical element that could influence physiological and disease processes of the vessel covered by it. In terms of anatomy, during the abdominal aorta's dissection, it is possible to identify the periaortic adipose tissue and the periaortic parietal peritoneum lying over it, sealing the retroperitoneal space. They seem to be fragile layers, with apparently no biomechanical role in the abdomen. However, it is well known that most cases of ruptured abdominal aortic aneurysms (AAAs) that reach the emergency department still alive p

Cost-effectiveness of Screening for Abdominal Aortic Aneurysm in Combination with Medical Intervention in Patients with Small Aneurysms.

OBJECTIVES: Screening for abdominal aortic aneurysm (AAA) among 65 year old men has been proven cost-effective, but nowadays is conducted partly under new conditions. The prevalence of AAA has decreased, and endovascular aneurysm repair (EVAR) has become the predominant surgical method for AAA repair in many centers. At the Malmö Vascular Center pharmacological secondary prevention with statins, antiplatelet therapy, and blood pressure reduction is initiated and given to all patients with AAA. This study evaluates the cost-effectiveness of AAA screening under the above mentioned conditions. M

Shifting abdominal aortic aneurysm mortality trends in The Netherlands.

OBJECTIVE: Mortality from abdominal aortic aneurysm (AAA) in developed countries has decreased since the late 1990s. Our objective was to get an insight of mortality trends in The Netherlands for AAA disease. METHODS: Data of all AAA deaths (1980 to 2010) were collected from the Dutch cause of death register. Cause of death was divided in two groups: with the mention of rupture and without the mention of rupture. Data were standardized and divided into three age groups (55-69, 70-84, and ≥85 years). Mortality rates per 100,000 were analyzed for both sexes and for each age group. Significant p

Protease inhibitor 15, a candidate gene for abdominal aortic internal elastic lamina ruptures in the rat.

The inbred Brown Norway (BN) rat develops spontaneous ruptures of the internal elastic lamina (RIEL) of the abdominal aorta (AA) and iliac arteries. Prior studies with crosses of the BN/Orl RJ (susceptible) and LOU/M (resistant) showed the presence of a significant QTL on chromosome 5 and the production of congenic rats proved the involvement of this locus. In this study, we further dissected the above-mentioned QTL by creating a new panel of LOU.BN(chr5) congenic and subcongenic lines and reduced the locus to 5.2 Mb. Then we studied 1,002 heterogeneous stock (HS) rats, whose phenotyping revea

Putative targeting of matrix metalloproteinase-8 in atherosclerosis.

There is compelling evidence indicating that some members of the matrix metalloproteinase (MMP) family play important roles in the pathogenesis of atherosclerosis and related vascular and cardiac conditions such as atherosclerotic plaque rupture leading to myocardial infarction, heart failure after myocardial infarction, neointima formation following angioplasty, and abdominal aortic aneurysm. Studies have shown that administration of MMP inhibitors can deter some of these conditions in experimental animal models, but few pertinent human clinical trials have been reported to date. Clinical stu

[Gender-specific influences on the results of vascular surgery].

BACKGROUND: This overview comments on gender-specific differences in prevalence, treatment and outcome in patients with peripheral arterial disease (PAD), carotid stenosis, and abdominal aortic aneurysm (AAA). METHOD: For the literature review, the Medline database (PubMed) was searched under the key words "peripheral arterial disease AND gender", "carotid stenosis AND gender" and "abdominal aortic aneurysm AND gender". RESULTS: 1.) Women (preferably black women) with PAD experience rather than men bypass failure or amputation. Nevertheless, gender should not constitute a selection criterion

The use of heparin in patients with ruptured abdominal aortic aneurysms.

The use of systemic heparin in patients with ruptured abdominal aortic aneurysms (rAAAs) remains a contentious issue with no clear guidelines. This review reports the current understanding, at a molecular and clinical level, of the possible benefits and risks of heparin in emergency aneurysm repair (both open and endovascular). MEDLINE, EMBASE, AMED, SCOPUS, CINAHL and Cochrane Library were searched for all articles containing the keywords 'rupture', 'abdominal', 'aneurysm' and 'heparin'. Current experience, indications and outcomes were analyzed. Articles were searched for both endovascular a

[Endovascular treatment of subclavian artery pseudoaneurysm as a delayed complication after surgery for aorto-bifemoral graft infection].

CASE REPORT: In this article we present a relatively rare vascular surgical complication and an uncommon treatment of it. In this case we used an aorto-bifemoral bypass on a patient with Leriche syndrome. The implanted Y-graft got infected and we were forced to remove it. Having inserted the abdominal aortic graft, an axillobifemoral bypass was also applied to secure the circulation of the lower limbs. However, the graft occluded later on, and 37 months after the inital surgery a rather large pseudoaneurysm developed at the origin of the graft in the right subclavian artery. Another surgical i
