Trends in aortic aneurysm- and dissection-related mortality in the state of São Paulo, Brazil, 1985-2009: multiple-cause-of-death analysis.

BACKGROUND: Aortic aneurysm and dissection are important causes of death in older people. Ruptured aneurysms show catastrophic fatality rates reaching near 80%. Few population-based mortality studies have been published in the world and none in Brazil. The objective of the present study was to use multiple-cause-of-death methodology in the analysis of mortality trends related to aortic aneurysm and dissection in the state of Sao Paulo, between 1985 and 2009. METHODS: We analyzed mortality data from the Sao Paulo State Data Analysis System, selecting all death certificates on which aortic aneu

Causes of late mortality after endovascular and open surgical repair of infrarenal abdominal aortic aneurysms.

INTRODUCTION: Several reports suggest unexpectedly high rates of late abdominal aortic aneurysm (AAA) rupture occur after endovascular AAA repair (EVAR). However, a population-based study examining causes of late death after EVAR vs open surgical repair has not been performed. METHODS: We performed a retrospective cohort study of patients undergoing infrarenal AAA repair using information from the Medicare inpatient hospital discharge records (MedPAR files), physician claim files (Part B files, 20% sample), and Medicare Denominator Files for the years 2001 to 2004. Using the Social Security D

Secondary rupture of abdominal aortic aneurysm following endovascular repair.

AIM: The aim of this study was to analyze the Literature covering the characteristics of secondary ruptured abdominal aortic aneurysm (AAA) following endovascular repair with aortic stent graft (ASG). METHODS: The study based on a Pubmed search of articles describing the characteristics of secondary ruptured AAA after ASG between January 1995 and May 2008. A total of 105 articles were selected, including the following characteristics: aneurysm diameter evolution, delay between implantation and rupture, average follow-up time, mechanisms implicated, results following further repair. RESULTS:

Sealed rupture of abdominal aortic aneurysms: CT features in 6 patients and a review of the literature.

PURPOSE: To assess the CT features of sealed rupture of abdominal aortic aneurysm. PATIENTS AND METHODS: We reviewed the CT scans of six index cases obtained over a 3 year period with a sealed rupture of an abdominal aortic aneurysm and those reported in the literature over a 21 year period. CT scans were reviewed for aneurysm size, the presence of a draped aorta and adjacent vertebral erosion. A group of consecutive patients with non-ruptured abdominal aortic aneurysm, referred for endovascular aneurysm repair during the same 3 year period constituted the control group. RESULTS: In the stud

Chyloperitoneum following abdominal aortic surgery.

Chyloperitoneum is an uncommon but serious complication following abdominal aortic surgery. As there seems to be no uniformity concerning the treatment, we performed a literature search to evaluate the best treatment results. All Medline-cited articles on chyloperitoneum after abdominal aortic surgery were assessed and analyzed. Thirty-eight cases of chyloperitoneum following abdominal aortic surgery were found. Patients developed symptoms after a median of 16 days (range 2-120 days) and were asymptomatic after 68 days (mean, range 27-93 years). All patients underwent diagnostic paracentesis,

Influence of pulmonary status and diabetes mellitus on aortic neck dilatation following endovascular repair of abdominal aortic aneurysms: a EUROSTAR report.

PURPOSE: To elucidate the association of impaired pulmonary status (IPS) and diabetes mellitus (DM) with clinical outcome and the incidences of aortic neck dilatation and type I endoleak after elective endovascular infrarenal aortic aneurysm repair (EVAR). METHODS: In 164 European institutions participating in the EUROSTAR registry, 6383 patients (5985 men; mean age 72.4+/-7.6 years) underwent EVAR. Patients were divided into patients without versus with IPS or with/without DM. Clinical assessment and contrast-enhanced computed tomography (CT) were performed at 1, 3, 6, 12, 18, and 24 months

Hybrid open-endovascular repair for thoracoabdominal aortic aneurysms: current status and level of evidence.

PURPOSE: To report the results of a systematic review of the literature and to provide evidence for the hybrid open-endovascular repair (HOER) in patients with thoracoabdominal aortic aneurysms (TAAAs). METHODS: A comprehensive literature review was performed and all studies identified that reported the results of HOER in patients with TAAA and information about primary technical and clinical success in evaluating the immediate and long-term complications such as neurological, renal and respiratory morbidity. All studies were reviewed by two independent observers for the above mentioned param

Incidental aneurysms of aorta and basilar artery in patients with coronary artery ectasia. A magnetic resonance angiography study.

OBJECTIVE: Incidental intracranial aneurysms have been revealed in 0.5-1% of adult patients undergoing cerebral angiography, while only 8% of those aneurysms are located in the basilar artery. Those aneurysms running usually symptomless, may lead to life-threatening situations due to rupture. Intracranial aneurysms could co-exist with abdominal aneurysms. Another dilating arterial lesion, coronary artery ectasia was linked in previous studies with aneurysms of the abdominal aorta. The aim of the present study is to investigate the coexistence of coronary artery ectasia with other aneurysms sin

[Spontaneous rupture of an abdominal aortic aneurysm into the inferior vena cava--case report].

A 76-year-old man was admitted to the surgical department with acute abdominal pain and impaired sensation of the lower extremities. An aneurysm of the abdominal aorta (AAA) was diagnosed already in the past. There were no signs of cardiovascular failure. Examination (sonography, CT) did not show intraabdominal bleeding. Nevertheless AAA rupture was suspected. A decision on an urgent operation was taken. Despite permanent resuscitation the patient died on the table before the operation began. In the discussion four type of AAA rupture are mentioned: into the open abdominal cavity, into the ret

Five-year results of elastin and collagen markers as predictive tools in the management of small abdominal aortic aneurysms.

OBJECTIVE: small abdominal aortic aneurysms (AAAs) do rupture and only half of AAAs above 5 cm would have ruptured unoperated. Furthermore, conservative treatment of AAAs may cause psychological side effects and impaired quality of life. To optimise the indication and time for operation for AAAs, we analysed whether serum elastin peptides (EP), procollagen-IIIN-terminal propeptide (PIIINP), and the initial AAA size could predict operation for AAAs in initially conservatively treated AAA. MATERIAL AND METHODS: in 1994, 4404 65-73 year old males were invited to hospital-based screening for AAAs

Ruptured mycotic abdominal aortic aneurysm in a patient with systemic lupus erythematosus.

Abdominal pain, a common condition, has been reported in up to 37|X% of-patients with systemic lupus erythematosus (SLE) (1). There are many possible causes including peritonitis, pancreatitis, mesenteric vasculitis, thrombosis of the mesenteric vessels, intra-abdominal infections, or side effects of the medications used in the treatment of the disease, especially non-steroidal anti-inflammatory drugs (1-3). However, ruptured mycotic aneurysm of the abdominal aorta has rarely been mentioned as a cause of abdominal pain in SLE (1-3). We recently saw a patient with SLE who had an acute surgical

Primary care diagnosis of acute abdominal pain.

The abdomen, as the largest cavity in the body, holds both fixed as well as relatively mobile organs, which when either diseased, traumatized, malfunctioning, or infected may present a wide and diverse range of signs and symptoms. Clues to the origin of abdominal pain can be well-localized or referred and quite obtuse. This article reviews the surface anatomy of the abdomen, the types of abdominal pain, approach to the patient with abdominal pain, and history-taking and physical examination. Adjunctive studies, which might help to reduce the differential diagnosis, are mentioned. The goal of t

[Imaging of aneurysms and dissections of the aorta. Part 1--Aneurysms of the abdominal aorta].

The authors review the main imaging findings of aneurysms and dissections of the aorta. Based on the experience and imaging techniques available in their institution (Conventional Radiology, Sonography and Computed Tomography), they try to make a decision tree of radiologic procedures in ambulatory and emergency patients. Nowadays, in the majority of situations, these techniques allow the Radiologist to make the correct diagnosis, detect the complications and evaluate the patients in the pre and post-surgical phases. Other imaging techniques not available in their institution, such as Magnetic

[Management of hemoperitoneum due to rupture of visceral branches of the aorta with a cylinder-type balloon catheter].

We have designed a cylinder-type balloon catheter and devised a new procedure for the management of the aneurysm in a condition difficult for treatment. This balloon catheter consists of a balloon located at the tip of a catheter. The balloon has a central lumen for maintaining the distal aortic bloodflow and can be inflated at various intended sites. The catheter with a deflated balloon was inserted into the abdominal aorta upward through a vascular graft sutured at the side of the aorta. In the case of a ruptured visceral artery, e.g., celiac axis or superior mesenteric artery, controlling o
