Blunt abdominal aortic trauma in paediatric patients.

BACKGROUND: Blunt abdominal aortic trauma (BAAT) is a very rare occurrence in children, with significant morbidity and mortality. Varied clinical presentations and sparse literature evidence make it difficult to define the proper management policy for paediatric patients. METHOD: We report our centre's data on three consecutive children with BAAT managed between 2006 and 2010. A Medline search was also performed for relevant publications since 1966, together with a review of references in retrieved publications. RESULTS: Forty children (range 1-16 years) were included in our final analysis.

Triple-barrel graft as a novel strategy to preserve supra-aortic branches in arch-TEVAR procedures: clinical study and systematic review.

OBJECTIVE: To report our early experience with total endovascular repair of aortic-arch aneurysm using double chimney-grafts and present a literature overview. PATIENTS AND METHODS: The double chimney-graft technique was performed in six male patients with contained ruptured aneurysm, dissecting aneurysm, pseudoaneurysm, penetrating aortic ulcer and proximal endoleak after TEVAR. Furthermore, a systematic electronic health database search of available articles was conducted according to PRISMA Guidelines. RESULTS: In all cases, all supra-aortic vessels had to be covered with aortic stent-gra

Focal common carotid artery intramural haematoma.

Focal common carotid artery intramural haematomas are rare. A 50-year-old man with a spontaneous onset of tenderness in the region of the right common carotid artery presented to our hospital (Christchurch, New Zealand). Ultrasound and CT imaging were consistent for an intramural haematoma. He was treated with antiplatelet therapy. A follow-up MRA showed resolution of the haematoma. Focal common carotid artery intramural haematomas are rare but not to be overlooked. This case presents a cause for intractable spontaneous neck pain in the region of the common carotid artery. Given the risk of ru

Retroperitoneal and aortic manifestations of immunoglobulin G4-related disease.

Retroperitoneal fibrosis is one of the prototypic manifestations of immunoglobulin G4 (IgG4)-related disease (IgG4-RD), but there is growing evidence that the aorta is also involved. These 2 conditions are closely linked, and based on the epicenter of the disease, the clinical manifestations can be classified as retroperitoneal fibrosis, inflammatory abdominal aortic aneurysm (including a combination of the 2), and thoracic aortitis. IgG4-RD is responsible for only a subset (∼50%) of cases of retroperitoneal fibrosis and inflammatory aortic aneurysms. Histological features include an extensive

Dolichoectatic vertebrobasilar dissecting aneurysm originating from atherosclerosis: an autopsy case.

We herein report an unusual case of profound brain infarction of the posterior circulation due to a dolichoectatic vertebrobasilar dissecting aneurysm (DVDA) originating from atherosclerosis. On autopsy, diffuse atherosclerosis was observed with a multi-fusiform aneurysm measuring 1 to 2 cm in diameter ranging from the left vertebral artery to the basilar artery. The microscopic findings of the aneurysm revealed severe stenosis of the artery caused by intimal thickening, intimal flap formation and thrombosis, indicating the presence of a dissecting aneurysm originating from atherosclerosis. A combined proteomic and transcriptomic approach shows diverging molecular mechanisms in thoracic aortic aneurysm development in patients with tricuspid- and bicuspid aortic valve.

Thoracic aortic aneurysm is a pathological local dilatation of the aorta, potentially leading to aortic rupture or dissection. The disease is a common complication of patients with bicuspid aortic valve, a congenital disorder present in 1-2% of the population. Using two dimensional fluorescence difference gel electrophoresis proteomics followed by mRNA expression, and alternative splicing analysis of the identified proteins, differences in dilated and nondilated aorta tissues between 44 patients with bicuspid and tricuspid valves was examined. The pattern of protein expression was successfully

Long-term prognosis of ascending aortic aneurysm after aortic valve replacement for bicuspid versus tricuspid aortic valve stenosis.

OBJECTIVE: The bicuspid aorta is thought to have a higher risk of progressive dilation after aortic valve replacement with a subsequently increased risk of adverse aortic events. Our aim was to compare the risk of late aortic events after isolated aortic valve replacement surgery for bicuspid versus tricuspid aortic valve stenosis with concomitant mild to moderate dilatation of the proximal aorta. METHODS: A total of 325 consecutive patients (60% males; mean age, 59.5 ± 10 years) with aortic valve stenosis and concomitant ascending aortic dilatation of 40 to 50 mm underwent isolated aortic va

Type II DeBakey dissection with complete aortic rupture visualized by unenhanced postmortem imaging.

We present postmortem computed tomography (pmCT) as well as postmortem magnetic resonance (pmMR) imaging findings in a case of type II DeBakey aortic dissection with a complete rupture of the ascending aorta compared to the findings obtained at forensic autopsy. PmCT only allowed a presumptive diagnosis of aortic dissection based on an anterior mediastinal enlargement. However, at pmMR the dissection including the aortic rupture was clearly visible. Visualization was realized in an unenhanced manner without the need for postmortem angiography.

Large aneurysm of left coronary sinus of Valsalva presenting with effort-related ventricular fibrillation.

Sinus of Valsalva aneurysms are very rare and are often asymptomatic. Clinical manifestations depend on associated complications, most commonly rupture or dissection. We describe the unusual case of a 46-year old presenting with exercise-induced ventricular fibrillation due to extrinsic compression of the left coronary artery. We also describe the surgical correction by valve-sparing aortic root replacement.

Diagnostic dilemma: Saccular aneurysm or pseudoaneurysm of the ascending aorta with dissection above level of leaflets.

BACKGROUND: In true aneurysm, the wall of aneurysm is composed of the normal histological components of aorta. A false aneurysm (pseudoaneurysm) represents a rupture which does not contain the normal histological components of aorta. It is a fibrous peel that has formed from a small perforation of aorta. We describe an unusual presentation that has signs which some of them are only manifested in true aneurysm and some others only in pseudoaneurysm. CASE REPORT: An 85-year-old man underwent elective coronary angiography for chest pain work-up. Our evaluation by invasive angiography and CT angi

[Rupture of the right ventricular wall closed with Daflon patching and biological glue].

In this paper we present a rare case of Dacron patch closure of tear in right ventricular myocardial at outflow level, caused by stroke of hypoplastic right coronary artery occlusion, in patient with non-obstructive hypertrophic cardiomyopathy caused by hypertension and aortic stenosis, surgically treated by implantation of mechanical prostheses, and pericarditis postcardiotomy. Our purpose is to trace evidence from postmortem identification of pathogenic mechanisms occurring in causing fatal event. The severe myocardial hypertrophy observed is supposedly based on valvular or hypertensive, is

Aortic dissection in the outer third of the media: what is the role of the vasa vasorum in the triggering process?

OBJECTIVES: Extensive clinicopathological analyses of aortic dissection have implicated hypertension and genetic abnormalities as the major pathogenic mechanisms. However, previous findings from pathological examinations have often been inconsistent with these mechanisms. In this paper, we suggest a significant role for the vasa vasorum in the aetiology and pathogenesis of aortic dissection. METHODS: We reviewed records of patients who underwent thoracic aortic dissection repair at our institution between June 2008 and August 2011. Twenty-one patients (10 men, 11 women; mean age, 65.0 ± 12.0

A Mycobacterium bovis mycotic abdominal aortic aneurysm resulting from bladder cancer treatment, resection, and reconstruction with a cryopreserved aortic graft.

Mycotic abdominal aortic aneurysms (AAAs) are a clinical challenge for vascular surgeons due to their critical location, surrounding inflammation, risk of rupture, and danger of reinfection following treatment. We present a case of Mycobacterium bovis AAA in a 69-year-old male after treatment with intravesicular bacillus Calmette-Guérin (BCG) therapy for bladder carcinoma. The classical approach for mycotic AAA entails extra-anatomic reconstruction followed by resection with oversewing of the proximal and distal aortic stumps. Alternative in-line reconstruction options have also been advocated

Chronic contained aortic rupture presenting as anterior thigh pain.

An 83-year-old man presented with severe left anterolateral thigh and knee pain for several days. He was completely unable to ambulate owing to the pain. The patient reported no history of acute back pain prior to presentation. His history initially confounded diagnosis. Subsequent investigations demonstrated a chronic contained rupture of an infrarenal aortic aneurysm with significant vertebral body erosion and obliteration of the ipsilateral psoas muscle. Proposed symptomatology was compression of the lateral femoral cutaneous nerve and its roots. The patient had multiple medical comorbiditi

Current role for endovascular treatment of ruptured abdominal aortic aneurysms.

Endovascular repair of ruptured abdominal aortic aneurysms seems to have better outcomes than open repair if certain strategies, techniques, and adjuncts are employed. These include a standard approach or protocol; use of fluid restriction (hypotensive hemostasis), performance of the procedure in a site equipped for excellent fluoroscopic imaging and open surgery, use of percutaneous approaches and local anesthesia for initial guide wire and catheter placement, placement of a large supraceliac aortic sheath, and obtaining balloon control only when absolutely necessary. Details of obtaining thi

Endovascular treatment of a chronic contained ruptured abdominal aortic aneurysm.

We report a 69-year-old Caucasian male who presented with irreversible ischemia to the left foot. CT-scan showed, besides occlusion of both superficial femoral arteries, a chronic contained rupture of an abdominal aortic aneurysm. The aneurysm was excluded with a stent graft placed through a femoral approach in a semi-urgent procedure. CT-scans at 1, 3 and 6 months demonstrated continued exclusion of the aneurysm. Chronic ruptures are a rare but important subset of ruptured abdominal aortic aneurysms. CT-scan with contrast enhancement is the gold standard diagnosing technique. Treatment consis

Mycotic aneurysms of the ascending aorta in the absence of endocarditis.

Mycotic aneurysm formation is a rare and potentially fatal sequela of bacteremia. We present the cases of 2 octogenarians who had surgically confirmed mycotic aneurysms that involved the ascending aorta, with contained rupture (pseudoaneurysm). Neither patient had evidence of valvular endocarditis. Patient 1, an 82-year-old man, had streptococcal bacteremia. Imaging confirmed a mycotic aneurysm of the ascending aorta, and resection was successful. Patient 2, an 83-year-old woman, had recurrent staphylococcal bacteremia and progressive widening of the mediastinum, and imaging revealed a mycotic
