Hybrid repair of Kommerell's diverticulum with aberrant subclavian artery.

Kommerell's diverticulum (KD) is a rare aortic arch anomaly associated with aberrant subclavian arteries and risk of rupture. We present an 80-year-old woman with a right-sided aortic arch and aberrant left subclavian artery arising from a 5.0-cm KD. A staged hybrid repair was performed: right carotid-subclavian bypass, thoracic endovascular aortic repair, and embolization of the aberrant subclavian artery. Postoperative imaging confirmed complete exclusion of the diverticulum and a patent bypass graft. This case illustrates the safety and effectiveness of hybrid repair for KD in complex arch

Bentall procedure for giant unruptured right sinus of Valsalva aneurysm treated.

A sinus of Valsalva aneurysm (SVA) is a rare cardiac anomaly that may remain silent until rupture, often leading to acute heart failure. Surgical intervention is advised for unruptured SVAs when large, progressive, or associated with aortic regurgitation (AR). We report a 72-year-old woman with a 25-mm right coronary SVA and severe symptomatic AR. Transthoracic echocardiography confirmed severe AR with preserved ventricular function. A Bentall procedure using a bioprosthetic valve and Valsalva graft was performed, with coronary button reimplantation with the Carrel patch technique. The postope

Towards Personalised Assessment of Abdominal Aortic Aneurysm Structural Integrity.

Abdominal aortic aneurysm (AAA) is a life-threatening condition characterized by the progressive dilation of the aorta, which can lead to rupture if undetected or untreated. Stress-based rupture risk estimation using computational biomechanics has been widely studied; however, it requires wall strength data that cannot be measured in humans in vivo. To overcome this limitation, the goal of this study is to present a new method for biomechanical assessment of AAA via simultaneous consideration of tension and strain in AAA wall. We present a patient-specific, non-invasive method for assessing th

Generation of an induced pluripotent stem cell line, JHUi006-A, from a Marfan Syndrome patient harboring a pathogenic c.5225-2A > C intronic splicing variant.

Marfan Syndrome, a heritable connective tissue disorder caused by mutations within the fibrillin-1 (FBN1) gene, can have deleterious effects on heart and aorta, eyes, the skeletal system and bone. FBN1 mutations that result in increased aortic vulnerability to rupture are associated with high mortality rates. Here, we describe an induced pluripotent stem cell line (JHUi006-A) generated from patient-derived human dermal fibroblasts harboring a heterozygous c.5225-2A > C intronic splice acceptor site variant preceding Exon 43 of FBN1 that results in exon skipping. The clonal line has a normal ka

Imminent Rupture of an Infected Aortic Aneurysm Presenting as Lower Back Pain in an Elderly Patient.

An infected abdominal aortic aneurysm (IAAA) is a rare and potentially life-threatening condition that can mimic common spinal disorders, leading to delayed diagnosis. We report a case of a 63-year-old male who presented with lower back pain and left leg numbness. Initially suspected to have a spinal pathology, further investigation revealed an IAAA complicated by a psoas abscess and septic shock. Despite initial antibiotic therapy, the aneurysm ruptured, necessitating emergency surgical intervention. Unfortunately, the patient died from circulatory failure on postoperative day four. This case

Ventricular Septal Perforation Associated with Takotsubo Syndrome and Its Pathogenic Mechanisms.

INTRODUCTION: Takotsubo syndrome (TTS), once considered a benign self-limiting disease, is now recognized to have substantial short-term morbidity and mortality. During the acute phase, it can result in severe heart failure, cardiogenic shock, arrhythmias, cardiac rupture, ventricular septal perforation (VSP), and thromboembolic events, with in-hospital mortality rates comparable to those observed in acute coronary syndrome. Although VSP is rare, it represents a potentially lethal complication of TTS. Herein, we present a case of VSP associated with TTS and discuss a mechanism for the developm

Physician-modified four-fenestration endograft: The first case in Türkiye.

Endovascular repair has become a standardized approach for aortic diseases, but visceral branch involvement presents major technical challenges. Fenestrated repair allows aneurysm exclusion while maintaining organ perfusion, though custom-made devices are costly and often unavailable in urgent settings. We report the first physician-modified four-fenestration endograft (PMEG) procedure performed in Türkiye for a contained rupture of the visceral aorta. All four visceral branches were successfully revascularized with full exclusion of the aneurysm. The patient had an uneventful recovery, and on

Morphological Predictors and Preventive Strategies for Type IA and II Endoleaks After Endovascular Aneurysm Repair: A 20-Year Single-Centre Experience.

BACKGROUND: Type IA and type II endoleaks remain among the most frequent and clinically relevant complications following endovascular aneurysm repair (EVAR). These events compromise aneurysm exclusion, may cause sac enlargement, and are associated with an increased risk of late rupture. The ability to identify morphological features predisposing to endoleak formation and to apply targeted preventive strategies could substantially improve the long-term durability of EVAR. DESIGN: Single-center retrospective cohort study. METHODS: We analyzed 250 consecutive patients who underwent EVAR for inf

A case report: Acute presentation of chronic aortic dissection.

Chronic aortic dissection is a rare variant of aortic dissection where dissection in a weakened aortic wall and subsequent blood flow occur over a period of 3 months or more. The underlying mechanism for chronicity and global burden of the disease is not well documented or understood. Further contributed by the overall rarity of aortic dissection, literature published on the topic is lacking. We present a case of a 83-year-old lady who presented with features of Cor Pulmonale with coexisting right hypochondrium tenderness which when further evaluated with a USG revealed an aortic aneurysm with

Feasibility of Zero-Dimensional-Model-Based Pulse Waveform Analysis as a Tool to Detect Ascending Thoracic Aortic Aneurysm Growth.

Patients with ascending thoracic aortic aneurysms (ATAA) only have a 10-20% chance of survival upon aneurysm rupture. If aneurysm growth is detected, however, surgical repair can mitigate the rupture risk. Current computed tomography (CT)- and magnetic resonance imaging (MRI)-based surveillance methods require frequent hospital visits, increasing healthcare costs and reducing patient adherence. Pulse-based measurements, which could eventually be performed at home, are an attractive but poorly explored alternative. In this study, we investigated the feasibility of using frequency analysis of th

Kruppel-Like Factor 5 Modulates the Nuclear Factor Erythroid-2-Related Factor 2/Heme Oxygenase 1 Signalling Pathway to Regulate Vascular Smooth Muscle Cell Ferroptosis in Abdominal Aortic Aneurysm.

OBJECTIVE: Abdominal aortic aneurysm (AAA) refers to a disease where the abdominal aorta progressively dilates to 3.0 cm or more, making it prone to rupture. The etiologic and pathophysiological mechanisms underlying the formation and development of AAA are not yet fully understood. A preliminary investigation was conducted into the effects of Kruppel-like factor 5 (KLF5) regulation of the nuclear factor erythroid-2-related factor 2/heme oxygenase 1 (NRF2/HO-1) signalling pathway on ferroptosis in AAA vascular smooth muscle cells (VSMCs). METHODS: ApoE-/- mice or primary VSMCs were induced wi

The role of IVUS in BEVAR as additional intraoperative tool: procedure guidance and quality control imaging.

BACKGROUND: Intravascular ultrasound (IVUS) is a catheter-based imaging technique that utilizes high-frequency sound waves to produce real-time, cross-sectional images of the vessel lumen. IVUS is a well-known endovascular imaging tool for peripheral and aortic procedures, with potential benefit also during branched endovascular aortic repair (BEVAR). METHODS: We conducted a single-center retrospective observational study, including 47 patients undergoing elective BEVAR procedure from 2020 to 2024. Exclusion criteria were acute dissection, frank rupture. Two groups were compared: an IVUS-guid

Thromboembolism related to heparin-induced thrombocytopenia during Impella support after cardiac surgery for free wall rupture: a case report.

BACKGROUND: Left ventricular free wall rupture (LVFWR) is a severe complication of acute myocardial infarction that frequently leads to cardiac tamponade and circulatory collapse. Prompt surgical repair and mechanical circulatory support are essential for patient survival. The Impella device provides left ventricular unloading and is increasingly used after cardiac surgeries. However, it carries the risks of bleeding and thromboembolism. Heparin-induced thrombocytopenia (HIT) may be an important contributor to thromboembolic risk. Diagnosing HIT in an acute postoperative setting using the Impe

Early IKKβ-Dependent Anabolic Signature Governs Vascular Smooth Muscle Cells Fate and Abdominal Aortic Aneurysm Development.

Abdominal aortic aneurysm (AAA) is a serious disease with no effective pharmacological therapy. Although inflammation is recognized as a key regulator of AAA, targeting inflammatory pathways once the disease is established does not improve outcomes. Understanding the earliest molecular indicators could clarify precise biological targets and prognostic markers for AAA. Using ApoE-deficient mice, we performed RNA-Seq on suprarenal abdominal aortas (SRAs) from Ang II- and saline-treated mice 24 h after infusion. We further developed a unique model of hyperlipidemic mice in which the expression of

Predicting risk of ascending thoracic aortic aneurysm in asymptomatic adults using machine learning.

Most patients with ascending thoracic aortic aneurysms (ATAA) remain asymptomatic until they develop fatal complications, including aortic dissection and rupture. We aimed to develop and validate machine-learning models for predicting ATAA risk. We developed a predictive model for the risk of ATAA based on data from 18,382 participants from the Kangbuk Samsung Health Study between January 1, 2010, and December 31, 2018. In the screening context, an ATAA was defined as an ascending thoracic aorta with a diameter ≥ 3.7 cm. For the model inputs, we used 16 variables from medical records, includin

Urgent and Emergent Endovascular Treatment of the Downstream Aorta Soon After Open Surgical Repair in Acute Type A Aortic Dissection: Analyzing Indications and Outcomes of an Institutional Case Series.

Objectives: Thoracic endovascular aortic repair (TEVAR) is rarely indicated on an urgent or emergent basis soon after open surgical repair of type A aortic dissection (TAAD), and systematic data on clinical outcomes are therefore missing. In the present study, we analyze a contemporary case series regarding the outcome after urgent and emergent endovascular treatment of the downstream thoracic aorta, following open surgery for TAAD. Methods: The study was conducted as a retrospective observational analysis. From January 2024 until April 2025, seven patients (four male, aged 56.8 ± 5.6 years) w
