[Progressive multifocal leukoencephalopathy developed 3 years after related HLA-haploidentical peripheral blood stem cell transplantation].

The patient was a 22-year-old man. 8 years ago, he developed T-cell lymphoblastic lymphoma and relapsed during treatment. He underwent HLA-haploidentical peripheral blood stem cell transplantation (PBSCT) from a related donor and achieved remission. After transplantation, he developed severe chronic graft-versus-host disease (GVHD) with systemic involvement, requiring long-term administration of several immunosuppressive drugs. About 3 years and 9 months after the transplant, he was experiencing depression, anorexia, and weight loss. Brain MRI showed hyperintense lesions in T2-weighted imaging

Portable ultra-low-field MRI for progressive multifocal leukoencephalopathy: Case studies, sensitivity, and potential applications.

BACKGROUND AND OBJECTIVE: Progressive multifocal leukoencephalopathy (PML) is a severe, disabling infection caused by JC virus reactivation. PML-related disability complicates the MRI monitoring needed to assess treatment interventions in clinical trial or compassionate use settings. Portable ultra-low-field MRI (pULF-MRI) offers a convenient approach when such frequent imaging is needed. We evaluated the potential utility of pULF-MRI as an adjunctive tool for decreasing the burden of clinical study participation and clinical management in PML. METHODS: We examined paired high-field (HF) and

Prolonged survival in HIV-associated Progressive Multifocal Leukoencephalopathy treated with Pembrolizumab: a case series on treatment and long-term follow-up.

Progressive Multifocal Leukoencephalopathy (PML) is a rare opportunistic infection of the central nervous system (CNS) caused by human polyomavirus JC virus, with high mortality rate in people living with HIV (PLWH), without an effective specific treatment beside combined antiretroviral therapy (cART). The use of Pembrolizumab, an inhibitor of the Programmed cell death protein 1 (PD-1) receptor on T cells, has been associated with decreased viral clearance. Aim of this study is to evaluate clinical course of PLWH affected by PML treated with pembrolizumab. We report four consecutive PLWH with

MRI Shrimp Sign and Parietooccipital Lesion in Progressive Multifocal Leukoencephalopathy.

Recognizing the MRI characteristics of progressive multifocal leukoencephalopathy is essential for early diagnosis. This report describes the oldest known case of an 83-year-old man with rapidly progressive dementia and HIV-associated classic PML. Brain MRI revealed hallmark findings, including asymmetric, continuous, non-enhancing subcortical lesions in the parietooccipital regions and the "shrimp sign" in the cerebellar white matter. These characteristic imaging patterns, in the appropriate clinical context, are clues for the early identification of PML.

The microvascular endothelium of the blood-brain barrier is highly restrictive to JC Polyomavirus neuroinvasion.

JC Polyomavirus is the causative agent of progressive multifocal leukoencephalopathy (PML), an often-fatal demyelinating disease. Unfortunately, a diagnosis of PML occurs only after patients have suffered irreversible neuropathologies. The first step in the initiation of PML is viral entry to the brain, but the route and mechanisms responsible for neuroinvasion have not been well established. To gain a better understanding of this, we asked whether purified virus or virus associated with extracellular vesicles (EVs) could penetrate two different cell culture models of the blood-brain barrier.

Polyomaviruses After Allogeneic Hematopoietic Stem Cell Transplantation.

Polyomaviruses (PyVs) are non-enveloped double-stranded DNA viruses that can cause significant morbidity in allogeneic hematopoietic stem cell transplant (allo-HSCT) recipients, particularly BK polyomavirus (BKPyV) and JC polyomavirus (JCPyV). BKPyV is primarily associated with hemorrhagic cystitis (HC), while JCPyV causes progressive multifocal leukoencephalopathy (PML). The pathogenesis of these diseases involves viral reactivation under immunosuppressive conditions, leading to replication in tissues such as the kidney, bladder, and central nervous system. BKPyV-HC presents as hematuria and

A first report of progressive multifocal leukoencephalopathy in childhood-onset NMOSD.

BACKGROUND: Progressive multifocal leukoencephalopathy (PML) has not been reported in pediatric neuromyelitis optica spectrum disorder (NMOSD) and rarely described in children. OBJECTIVES: To report a case of PML in childhood-onset NMOSD. METHODS: A single retrospective case report. RESULTS: Nine years after diagnosis of aquaporin-4 antibody positive NMOSD, a 17-year-old boy treated with rituximab presented with painless right visual loss over 6 weeks coinciding with CD19 repopulation. Acute relapse treatment was given but he continued to deteriorate, and JC virus was detected in cerebrospi

Progressive multifocal leukoencephalopathy during 4 years of Palbociclib for advanced breast cancer with a history of follicular lymphoma patient.

A 67-year-old woman with a history of follicular lymphoma and recurrent breast cancer with multiple metastasis developed PML during receiving fluvestrant plus palbociclib. Rituximab and bendamustine had finished 6.5 years ago. Discontinuing palbociclib and using mirtazapine, she is alive a year after diagnosis of PML. It is rare for PML to be diagnosed long after the last dose of rituximab and bendamustine. We will discuss the possible involvement of palbociclib in the course of PML.

Early diagnosis of progressive multifocal leukoencephalopathy in untreated HIV infection via ultrasensitive PCR testing for JC virus: A case report.

Progressive multifocal leukoencephalopathy (PML) is a fatal demyelinating disease triggered by the reactivation of JC virus (JCV) in individuals with immunodeficiency, particularly those with untreated human immunodeficiency virus (HIV) infection. This case report describes a 46-year-old HIV-positive man who initially presented with neurological symptoms and was incorrectly diagnosed as cerebral infarction. Although standard real-time polymerase chain reaction (PCR) testing for JCV in cerebrospinal fluid (CSF) at a commercial laboratory was negative, neuroimaging and clinical suspicion prompte

The role of the immune system in progressive multifocal leukoencephalopathy: a comparative analysis of two cases following autologous and allogeneic hematopoietic stem cell transplantation.

Progressive multifocal leukoencephalopathy (PML) is a rare, subacute demyelinating disorder of the central nervous system (CNS) caused by the JCV. In immunosuppressed hosts, PML is caused by reactivation of a latent infection rather than primary exposure. Hematological patients, particularly post-transplant, presenting with worsening neurological symptoms should promptly consider PML in the differential diagnosis. The rarity of PML after autologous or allogeneic hematopoietic stem cell transplantation (HSCT) and the absence of a universally effective therapy represents a clinical challenge. He

I saw the "Milky Way sign" and the "Barbell sign" in the same patient.

The Milky Way sign is characterized by multiple punctate lesions, with hyperintense signal on T2-weighted imaging, with no restricted diffusion and usually no gadolinium-enhancement. The Barbell sign is seen as a lesion extending across the splenium of the corpus callosum, with hyperintense signal on T2-weighted imaging with no or minimal mass effect, no gadolinium-enhancement, and no restricted diffusion or only a thin rim of peripheral restricted diffusion. Although these signs have differential diagnoses, in the correct clinical setting, they are suggestive of progressive multifocal leukoen

Histomorphological variations in progressive multifocal leukoencephalopathy correlated with JCV replication in brain lesions: insights from 91 patients.

Progressive multifocal leukoencephalopathy (PML) is a demyelinating disease caused by JC polyomavirus (JCV). The histopathology of PML is morphologically diverse and characterized by the classical triad of demyelination, enlarged oligodendroglial nuclei, and bizarre astrocytes. Pathological diagnostic criteria for PML require both the classical triad and viral detection in brain tissue. However, the frequency of this triad in surgical pathology specimens and its correlation with disease progression and viral loads remain unclear. In this study, 117 brain tissues from 91 pathologically confirme

A Systematic Review of BK and JC Polyomaviruses in Asia: Molecular Insights and Prevalence.

The Polyomaviridae family consists of small circular double-stranded DNA viruses. Two members of this family, BK polyomavirus (BKPyV) and JC polyomavirus (JCPyV) have been studied extensively since their discovery in 1971. These viruses are present in 80%-90% of populations worldwide. Although they usually do not cause any symptoms and remain inactive, they can become active when the immune system is weak. This activation can result in BK Virus-Associated Nephropathy and Progressive Multifocal Leukoencephalopathy for BKPyV and JCPyV, respectively. While there has been significant progress in u

CRISPR antiviral inhibits neurotrophic JC polyomavirus in 2D and 3D culture models through dual-gRNA excision by SaCas9.

Without an effective antiviral, JC virus (JCV) has persisted throughout multiple epochs of immunosuppression, causing the opportunistic demyelinating disease, progressive multifocal leukoencephalopathy (PML). This study proposes a novel therapy using a dual-gRNA, SaCas9, CRISPR antiviral targeting JCV transcription factor, large tumor antigen (LT-Ag), and capsid protein, viral protein 1 (VP1). This treatment was validated using traditional two-dimensional cell culture. A recombinant cell line was produced from SVG astrocytes (SVGA) via lentiviral inoculation and puromycin selection. Is Cauda Equina Involvement Related to BK Virus in Patients with Combined Immunodeficiency?

The BK virus, an unenveloped double-stranded DNA virus, infects up to 90% of the population, The virus often remains dormant but can reactivate and cause illness under conditions of impaired cellular immunity. It can cause progressive multifocal leukoencephalopathy and has been implicated in a variety of conditions, including encephalitis, nephritis, cystitis, and retinitis. This report explores neurological symptoms linked to the BK virus, focusing on its potential role in spinal cord pathology in patients with combined immunodeficiency by presenting and analyzing two distinct case studies. Efficacy of filgrastim therapy for patients with progressive multifocal leukoencephalopathy: Two case reports.

Progressive multifocal leukoencephalopathy (PML) is a life-threatening central nervous system condition caused by reactivation of JC virus in individuals with immunosuppression. Although PML typically develops in patients with human immunodeficiency virus (HIV), cases in individuals without HIV have been increasing in recent years. Non-HIV-associated PML has a particularly poor prognosis, high mortality rates, and few therapeutic options. Here, we present the cases of two patients with non-HIV-, nondrug-associated PML who achieved good clinical outcomes after treatment with filgrastim (granulo
