JC polyomavirus (JCPyV) is associated with progressive multifocal leukoencephalopathy (PML), but its plausible role in brain cancers is also disputed. One candidate to mediate cell transformation is the Large T antigen (LTAg), which has the capability to bind the Wnt pathway protein β-catenin, thus deregulating the cell cycle. In the current study, we investigated the presence and molecular state of JCPyV in pediatric brain tumors and the effects of virus-positivity on the Wnt pathway. JCPyV DNA was found in 31/101 (30.7%) brain tumors with a viral load of 3.2 copies/cell. The amplified NCCR r

Polyomaviruses (PyVs) are widespread commensals among vertebrates, including humans, where they silently persist lifelong in healthy hosts. Polyomavirus infection in immunocompromised individuals can cause life-threatening diseases. Of the 14 human polyomaviruses discovered to date, resurgent infections by the JC and BK PyVs are responsible for high morbidity and mortality in individuals with certain inherited or acquired immune perturbations. JCPyV causes several brain disorders, the most fully characterized and of highest (albeit rare) incidence being Progressive Multifocal Leukoencephalopat

Progressive multifocal leukoencephalopathy and cerebral toxoplasmosis are serious opportunistic infections in patients with advanced HIV infection. Diagnosis is often challenging due to overlapping clinical and radiological features. We present the case of a 40-year-old woman with a three-week history of expressive dysphasia, headache, and right arm weakness. Imaging revealed multiple enhancing brain lesions, initially raising concern for metastatic cancer. An extensive malignancy workup was unrevealing. Lumbar puncture confirmed JC virus in the CSF, and further testing identified previously u

BACKGROUND: Progressive multifocal leukoencephalopathy-immune reconstitution inflammatory syndrome (PML-IRIS) is a high-mortality disease among patients with AIDS. It is caused by infection with the John Cunningham virus (JCV). Currently, there are no specific antiviral treatments targeting JCV. Thus, immune reconstitution remains the primary therapeutic approach.
CASE PRESENTATION: A 29-year-old male patient diagnosed with AIDS presented for medical evaluation after two months of antiretroviral therapy (ART), reporting symptoms of dizziness and headache. The detection of JC virus was confirm

Progressive multifocal leukoencephalopathy (PML) is a rare but fatal demyelinating disorder resulting from reactivation of latent John Cunningham virus (JCV), which induces demyelinating lytic infection of oligodendrocytes. We report a case of PML in an individual without identifiable immunodeficiency who presented with insidious and progressive deficits in speech and motor function. Diagnostic workup revealed JC viral DNA in cerebrospinal fluid, and magnetic resonance imaging (MRI) demonstrated multiple asymmetric lesions within the subcortical and deep white matter. Despite administration of

Progressive multifocal leukoencephalopathy (PML) is a rare central nervous system disease caused by John Cunningham (J.C.) virus reactivation in the setting of immunosuppression. We describe a young woman with chronic migraines, Factor V Leiden and prior intravenous drug use who presented with headache, fatigue and bilateral leg weakness. Imaging demonstrated stable subcortical T2/fluid-attenuated inversion recovery hyperintensities consistent with prior PML. During evaluation, she was found to have an autoimmune hepatitis flare associated with transient worsening of her neurologic deficits, s

One of the most devastating central nervous system infections, progressive multifocal leukoencephalopathy (PML), is caused by the JC virus (JCV). In immunosuppressed patients, PML is a well-recognized complication recognized by clinicians across specialties, such as neurology, internal medicine, oncology, and rheumatology. It can be encountered in patients receiving hematopoietic or solid-organ transplants, cytotoxic chemotherapy, immune checkpoint inhibitors, or long-term immunosuppressive therapies. In neurological settings, those treated with natalizumab, rituximab, ocrelizumab, fingolimod,

JC virus (JCV) replicates within the nuclei of glial cells in the human brain and causes progressive multifocal leukoencephalopathy. JCV possesses a small, circular, double-stranded DNA genome, divided into early and late protein-coding regions. The non-coding control region (NCCR) functions bidirectionally for both early and late genes, and the agnogene is located downstream of TCR and upstream of three capsid proteins in the late region. Previously, in cell culture systems, we demonstrated that these capsid proteins accumulate in intranuclear domains known as promyelocytic leukemia nuclear b

Progressive multifocal leukoencephalopathy (PML) is a rare and often fatal complication associated with advanced HIV infection, caused by the reactivation of the John Cunningham (JC) virus in immunocompromised individuals. Diagnosis is challenging due to nonspecific symptoms and the need for specialized diagnostic tools, which may not be readily available in resource-limited settings. Early initiation of antiretroviral therapy (ART) is critical for managing the condition, though prognosis remains poor in advanced cases. We present the case of a 30-year-old male with newly diagnosed HIV who pre

Progressive multifocal leukoencephalopathy (PML) is a rare, often fatal demyelinating disease of the central nervous system (CNS) caused by JC virus (JCV) reactivation in the setting of impaired cellular immunity. While commonly associated with human immunodeficiency virus/acquired immunodeficiency syndrome (HIV/AIDS) and immunosuppressive therapies, PML can also arise in primary immunodeficiency disorders. We report a case of PML in a young adult with autosomal recessive hyper-IgE syndrome (AR-HIES) due to DOCK8 deficiency, highlighting the importance of considering genetic immunodeficiencies

We herein report a case of progressive multifocal leukoencephalopathy (PML) in a woman in her 70s with hepatitis B-related liver cirrhosis. She presented with cerebellar onset symptoms, including ataxia and dysarthria. The diagnosis was delayed despite the initial magnetic resonance imaging (MRI) findings of an asymmetric cerebellar white matter lesion. PML was ultimately diagnosed based on a re-evaluation of a brain biopsy specimen and a retrospective JC virus (JCV) PCR analysis of the cerebrospinal fluid. Treatment with mefloquine and mirtazapine resulted in viral clearance from the cerebros

JC Polyomavirus (JCPyV) is the causative agent of progressive multifocal leukoencephalopathy (PML), an often-fatal demyelinating disease. Unfortunately, a diagnosis of PML occurs only after patients have suffered irreversible neuropathologies. One requirement for the development of PML is for JCPyV to enter the brain, but the mechanisms responsible for neuroinvasion have not been well established. The blood-brain barrier (BBB) is a potential site for JCPyV neuroinvasion. JCPyV DNA is found in the vascular endothelium in postmortem brain tissue from PML patients, and demyelinating lesions commo

Progressive Multifocal Leukoencephalopathy (PML) is a rare, often fatal demyelinating disease of the central nervous system caused by reactivation of the John Cunningham virus (JCV) in immunocompromised individuals. Despite an estimated 2.4 million people living with HIV in India, the reported incidence of PML remains lower than in Western countries, likely due to underdiagnosis, underreporting, and distinct host genetic and viral factors. The rising number of individuals on immunosuppressive therapies, including organ transplant recipients and those with autoimmune disorders, further emphasiz

BACKGROUND: Infection-related movement disorders (IRMD) constitute nearly one-fifth of secondary movement disorders, with higher prevalence in endemic regions. Timely recognition is critical, as many are treatable or preventable yet they often mimic primary movement disorders, leading to diagnostic delays.
METHODS: We conducted an observational prospective cohort study at the Movement Disorders Clinic of a tertiary-care centre in India (May 2024-July 2025). All consecutive patients fulfilling the 2024 International Parkinson and Movement Disorder Society consensus criteria for IRMD were inclu

Objective The study aims to evaluate infection-related safety profiles of belimumab, rituximab, and TNF inhibitors using the FDA Adverse Event Reporting System (FAERS) database. Methods A disproportionality analysis combining Reporting Odds Ratios (ROR) and Bayesian Confidence Propagation Neural Network (BCPNN) was conducted on FAERS data from Q1 2014 to Q2 2024. Results Belimumab and rituximab are more likely to cause progressive multifocal leukoencephalopathy (PML), while infliximab show a greater variety tuberculosis (TB)-related AEs than other biological agents. Rituximab exhibited a risk

Progressive multifocal leukoencephalopathy (PML) is a demyelination disease of the central nervous system (CNS) caused by viral infection with John Cunningham polyomavirus (JC virus, JCV). PML affects mainly patients with immunodeficiency. Since the diagnosis is often challenging, it requires the close cooperation of clinicians. Due to the lack of specific treatment of JCV infection leading to PML, the current treatment is based on reversing the immunosuppression. Here we present a case of a 58-year-old woman who was ultimately diagnosed with PML based on neuropathological analysis of stereota
