Staged Endovascular Treatment With Stent-Assisted Coiling Followed by Flow Diversion for Ruptured Intracranial Dissecting Aneurysms: A Report of Two Cases.

Ruptured intracranial dissecting aneurysms are challenging to treat when parent artery sacrifice is unsuitable or technically difficult. Although flow diverters (FDs) enable definitive arterial reconstruction, their use during acute subarachnoid hemorrhage is complicated by the need for dual antiplatelet therapy. We report two patients with ruptured dissecting aneurysms of the internal carotid artery and vertebral artery who underwent a staged endovascular strategy. Stent-assisted coiling (SAC) was performed in the acute phase to protect against rebleeding while preserving the parent artery. B

Early change and new onset of vascular pathology in cervical artery dissection: implications for surveillance.

INTRODUCTION: Antithrombotic therapy is central to stroke prevention after spontaneous cervical artery dissection (sCAD), but its efficacy may depend on the underlying vascular morphology. We examined the temporal course of vascular changes after sCAD. PATIENTS AND METHODS: Consecutive patients with first-time sCAD underwent prospective serial duplex sonography for up to 10 years. In this retrospective cohort analysis, pathology change was defined as recanalisation of an occluded artery or progression from stenosis to occlusion. New pathology resulting from dissection recurrence was considere

Long-term mental health and quality of life in patients after cervical artery dissection.

INTRODUCTION: Most patients with Cervical Artery Dissection (CeAD) experience good long-term functional outcome. Little is known about long-term psychological sequelae in these patients. PATIENTS AND METHODS: In CeAD-patients, we assessed long-term quality of life (QoL), frequency of increased anxiety levels, post-traumatic stress disorder (PTSD), depression and fatigue and explored if baseline characteristics can identify patients at high risk of developing the aforementioned. We performed long-term follow-up (> = 12 months after baseline) in CeAD-patients with excellent 3-month outcome and

Emergent Total Endovascular Repair Using Retrograde in Situ Branched Stent Grafting for Ruptured Retrograde Aortic Dissection.

An 85-year-old bedridden woman (modified Rankin Scale 5) presented with a contained rupture of an acute retrograde aortic dissection (Type B0,7). Because open surgery was prohibitive due to severe frailty, an emergent total endovascular repair was performed using single-branch, retrograde in situ branched stent grafting (RIBS). Two conformable GORE TAG devices established a Zone 1 seal, and the left common carotid artery was preserved by in situ needle fenestration using a percutaneous transhepatic gallbladder drainage needle and bridged with a VIABAHN VBX stent. To limit embolic risk, cerebra

Point-of-Care Ultrasound for Quantitative Assessment of Pupillary Dysfunction in Acute Internal Carotid Artery Dissection.

BACKGROUND AND PURPOSE: Diagnosing internal carotid artery dissection (ICAD) can be challenging even for experienced clinicians, as one of its most specific signs-Horner syndrome (HS)-is often missed during routine neurological examination (NE). This study aimed to evaluate pupillary reactivity in ICAD patients using ultrasound pupillometry (UP) and compare quantitative UP findings with standard NE. METHODS: We enrolled 33 consecutive patients with acute ICAD and an age- and sex-matched healthy control group without ocular pathology. HS was assessed at admission using standard NE. UP was perf

The "Transdural" Paradox: The Case of a Paraclinoid Internal Carotid Artery Aneurysm With an Intradural Neck and Extradural Dome.

Paraclinoid internal carotid artery (ICA) aneurysms pose a surgical challenge because of their complex relationship with the distal dural ring (DDR), the boundary between the extradural cavernous sinus and the intradural subarachnoid space. The classification of these aneurysms as strictly intradural or extradural is often complicated by the anatomical variability of the DDR, which can form a dural pouch known as the carotid cave. This anatomical paradox can lead to a "transdural" aneurysm, in which the dome is in the extradural or intradural space, which poses a risk for subarachnoid hemorrha

Objective Return-to-Sport (RTS) Decision Frameworks for Adolescent Shoulder and Elbow Injuries: Tests, Thresholds and Exercise Targets.

BACKGROUND: Adolescent shoulder and elbow injuries are increasing due to sports specialization and year-round training, yet return-to-sport decisions typically rely on adult-derived heuristics that fail to address the unique physiology, growth plate vulnerability, and recovery needs of developing athletes aged 10-19 years. OBJECTIVE: This review proposes a novel, objective three-pillar RTS framework integrating: (1) foundational tissue capacity, (2) dynamic movement quality and neuromuscular control, and (3) sport-specific load tolerance. We synthesize current evidence to establish provisiona

Single antiplatelet therapy and tirofiban bridged with surface modified flow diverters for ruptured blood blister-like aneurysms: single center experience and systematic review.

BACKGROUND: Blood blister-like aneurysms (BBAs) of the internal carotid artery are rare but high risk lesions that frequently re-rupture due to their fragile structure and dissecting pathology. Treatment is particularly challenging in ruptured cases, given the risks associated with dual antiplatelet therapy. Recent advancements in flow diverter stents (FDSs) with surface modifications, and the use of single antiplatelet therapy (SAPT), offer a potential alternative strategy. METHODS: We conducted a retrospective review of 17 patients with ruptured internal carotid artery BBAs treated with sur

Risk factors for cervical artery dissection: a systematic review with meta-analysis.

BACKGROUND: Cervical artery dissection (CAD) is a rare cause of stroke. This is an update of an earlier systematic review, which focuses on the risk of CAD in the general population. The objective was to identify the risk factors for CAD. METHODS: A comprehensive search was conducted in MEDLINE, EMBASE and Web of Science on 20 September 2024. Observational studies (cohort, case-control studies and case-crossover studies) that studied patients with CAD and a control group were included. Risk of bias was assessed with the ROBINS-E tool, and certainty of the evidence was assessed using Grading o

Complex cervicocephalic artery dissections with wide intimal inlet: determinants and implications for endovascular recanalization.

BACKGROUND AND PURPOSE: Cervicocephalic artery dissection (CCAD) is a well-recognized cause of ischemic stroke. However, complex forms of CCAD, characterized by a wide intimal inlet without a visible intramural hematoma, pose diagnostic challenges and complicate endovascular access to the true lumen when recanalization is required. We aimed to analyze the clinical presentation, outcomes, and feasibility of endovascular treatment of complex CCAD and to propose a novel morphological classification. METHODS: We retrospectively analyzed consecutive patients treated for acute CCAD at Lariboisière

A Case of Bilateral Renal Artery Fibromuscular Dysplasia in a U.S. Soldier.

Fibromuscular dysplasia (FMD) is a vascular disease characterized by abnormal cellular growth in medium-sized arteries progressing to stenosis, aneurysm, or arterial dissection. Renal and carotid artery involvement is common. FMD is not characterized by inflammation or atherosclerosis and predominantly impacts young women. Refractory hypertension is a common primary clinical manifestation of renal artery FMD. Severe or untreated FMD can lead to stroke, aneurysm rupture, or death. Despite the need for early diagnosis and aggressive management, FMD is frequently overlooked because of nonspecific

Endovascular treatment of pediatric ruptured intracranial dissecting aneurysm: a case report and literature review.

Intracranial aneurysms (IAs) are uncommon in children, with an incidence of 1%-5%. However, intracranial dissecting aneurysms (IDA) account for a higher proportion (20%-50%) of all aneurysms in this age group. Pediatric IDAs typically result from vascular wall injury, potentially associated with genetic predisposition, congenital defects, or trauma. These lesions most commonly present with ischemic stroke, while subarachnoid hemorrhage (SAH) is relatively rare. Early symptoms include headache and vomiting, with severe cases potentially leading to neurological deficits. Digital subtraction angi

Lateral Ventricular Symmetry as a Predictor of Postoperative Neurological Outcomes in Acute Type A Aortic Dissection With Cerebral Malperfusion.

OBJECTIVES: Cerebral malperfusion in acute type A aortic dissection (ATAAD) is a serious condition. Predicting postoperative neurological outcomes is important to decide treatment strategies; however, current prediction methods have limitations. Therefore, this study examined whether the lateral ventricular volume ratio (LVR) on preoperative head computed tomography (CT) can predict postoperative neurological outcomes. METHODS: Among patients who underwent surgery for ATAAD at our institution between January 2007 and August 2024, those with cerebral malperfusion who underwent preoperative hea

Optimizing the dissection of small-diameter pulmonary vessels using vessel-sealing systems.

Although vessel-sealing systems have been shown useful for dissection of small-diameter vessels, no studies to date have evaluated the optimal angle of dissection. Lateral thermal damage to a canine pulmonary artery was assessed microscopically. Burst pressure was compared in swine carotid arteries undergoing orthogonal and oblique dissections to determine the safer dissection technique. Histological analysis of a canine model revealed a thermal spread of approximately 1.3 mm. Burst pressure comparison showed no significant differences between orthogonal and oblique dissection angles (829.2 mm
