Mechanical Thrombectomy in Patients With Cervical Artery Dissection: A Multicenter Analysis on Technical, Safety, and Functional Outcomes.

BACKGROUND AND AIMS: The safety and effectiveness of mechanical thrombectomy (MT) in patients with cervical artery dissection (CeAD) remain uncertain. This study aimed to evaluate the safety, recanalization rates, and functional outcomes of MT in LVO-AIS patients with and without CeAD. METHODS: Retrospective multicenter study based on data prospectively collected from June 2021 to June 2024. CeAD-related LVO-AIS patients treated with MT were compared with matched anterior and posterior circulation AIS patients without CeAD. Primary outcomes included procedural adverse events, recanalization r

Isolated Brachiocephalic Artery Dissection after Two-Debranching Thoracic Endovascular Aortic Repair: A Case Report.

An 87-year-old man with a distal aortic arch aneurysm underwent 2-debranching thoracic endovascular aortic repair (TEVAR). Five days postoperatively, he developed sudden chest pain and syncope, with severe upper-limb hypoperfusion. Contrast-enhanced computed tomography (CECT) revealed brachiocephalic artery (BCA) dissection compressing the true lumen. Although conservative management was initially selected, percutaneous transluminal angioplasty with kissing bare-metal stents from the BCA to the right subclavian and right common carotid arteries was performed for recurrent hypotension. Postproc

Sex differences in aortopathy, arteriopathy, and mortality in vascular Ehlers-Danlos syndrome.

OBJECTIVE: Vascular Ehlers-Danlos syndrome (VEDS) is a rare autosomal dominant disorder due to pathogenic alterations in type III collagen structure and production, which results in increased risk of aortopathy and arteriopathy. Given the documented sex-related differences in aortic and other arterial aneurysms and dissections in non-VEDS populations, the potential influence of sex on the incidence of aortopathy, arteriopathy, and mortality in individuals with VEDS was examined. METHODS: A cross-sectional analysis on a cohort of 557 individuals with VEDS with COL3A1 pathogenic variants was pe

Case Report: Hybrid retrograde transcarotid stenting for common carotid artery dissection secondary to acute type A aortic dissection.

BACKGROUND: Endovascular repair is an established option for symptomatic carotid artery dissection, particularly when hemodynamic compromise or embolic risk persists. In patients undergoing repair for acute type A aortic dissection (ATAAD), conventional transfemoral or transradial access may be prohibitive because of altered arch anatomy after graft reconstruction and stent-grafting. We report a postoperative left common carotid artery (CCA) dissection with focal aneurysmal dilatation and cerebral malperfusion following ATAAD repair. The lesion was treated using a hybrid strategy: direct cervi

Baseline normalized wall index predicts delayed recanalization in internal carotid artery dissection with intramural hematoma: a high-resolution vessel wall imaging study.

OBJECTIVE: This study aims to investigate the evolution of intramural hematoma (IMH) in internal carotid artery dissection (ICAD) and identify predictors of favorable recanalization. METHODS: This study cohort comprised 53 patients diagnosed with IMH due to ICAD on the basis of high-resolution magnetic resonance vessel wall imaging (HR-VWI) between June 2018 and June 2025. Thirty-eight medically managed patients underwent follow-up imaging. Neuroradiologists blinded to clinical data independently reviewed imaging. The primary outcome was favorable recanalization (<50% luminal stenosis) at fol

Circumferential Distal Dural Ring Opening Through a Natural Microsurgical Dissection Corridor: A Plane-Oriented Strategy for Paraclinoid Aneurysm Surgery.

BACKGROUND: Safe circumferential distal dural ring (DDR) opening remains one of the most technically demanding steps in microsurgical clipping of paraclinoid aneurysms. This study aimed to characterize the anatomical conditions associated with successful circumferential distal dural ring opening and to present a plane-oriented microsurgical strategy for paraclinoid aneurysm surgery. METHODS: We retrospectively reviewed 51 consecutive patients who underwent microsurgical clipping for paraclinoid aneurysms requiring distal dural ring (DDR) opening. DDR opening was defined as any incision of the

A Technique to Avoid Endograft Lumen Narrowing during Endovascular Aortic Arch Repair in Chronic Dissections.

INTRODUCTION: The use of retrograde branches during endovascular aortic arch repair in chronic dissections has been associated with endograft lumen compression in the distal arch true lumen. This results in a functional pseudocoarctation, which carries a risk of cardiac failure. TECHNIQUE: This exemplar case presents a patient treated with a three-branch arch endograft for an anastomotic false aneurysm and a residual chronic arch dissection extending to the supra-aortic trunks. To overcome device compression, a kissing angioplasty of the endograft and the left common carotid artery branch was

Internal carotid artery dissection in a young healthy male patient.

PRESENTATION: A 41-year-old male patient presented to the emergency department (ED) with left sided headache, radiating to the left ear while also complaining of dizziness. A non-contrast CT brain, performed during a previous presentation five days earlier, was normal. On his second presentation, the patient displayed clinical signs of carotid artery dissection including left sided partial Horner's syndrome. DIAGNOSIS: A CT Intracranial angiogram revealed a likely left internal carotid artery (ICA) dissection. TREATMENT: The patient was commenced on dual antiplatelet therapy. DISCUSSION: As

Raeder's paratrigeminal neuralgia secondary to internal carotid artery dissection mimicking trigeminal autonomic cephalalgia: a case report.

Raeder's paratrigeminal neuralgia is a rare clinical syndrome characterized by ipsilateral periorbital pain, partial Horner syndrome (ptosis and miosis without anhidrosis), and sensory disturbances in the ophthalmic division (V1) of the trigeminal nerve1. The syndrome results from involvement of sympathetic fibers traveling along the internal carotid artery (ICA), often in association with vascular or parasellar pathologies2. Among its etiologies, ICA dissection is particularly important, as it may present with unilateral headache and cranial autonomic features that closely mimic trigeminal au

Spontaneous Disappearance of Intracranial Unruptured Saccular Aneurysms: Two Case Reports.

Spontaneous disappearance of small unruptured intracranial aneurysms is exceedingly rare, as most reported cases describe resolution in the context of rupture, giant aneurysms, or dissecting lesions. We report two cases of small saccular aneurysms that vanished during long-term magnetic resonance angiography follow-up. In Case 1, a 3.6 × 3.4-mm right internal carotid artery aneurysm demonstrated gradual reduction followed by complete disappearance, coinciding with proximal hemodynamic alteration. In Case 2, a 3.6 × 2.9 mm aneurysm of the right middle cerebral artery remained stable for 7 years

The aberrant right subclavian artery and non-recurrent laryngeal nerve: a cadaveric study.

BACKGROUND: The aberrant right subclavian artery (ARSA), or arteria lusoria, is an aortic arch variant in which the right subclavian artery arises distal to the left subclavian artery and typically follows a retroesophageal course. Although often asymptomatic, ARSA can present with clinical symptoms and complicate surgical procedures, particularly when associated with other anatomical variants such as a non-recurrent laryngeal nerve (non-RLN). MATERIALS AND METHODS: During routine dissection of a donated cadaver, variations in the aortic arch and laryngeal nerves were identified. Arterial dim

Fatal Delayed Stent-edge Rupture Confirmed by Autopsy after Stent-assisted Coil Embolization for a Blood Blister-like Aneurysm.

Blood blister-like aneurysms of the internal carotid artery are uncommon lesions that often present with subarachnoid hemorrhage and are considered part of the spectrum of intracranial arterial dissections. Although endovascular treatment options have expanded in recent years, serious complications remain a clinical concern. We report the case of a 42-year-old woman with a ruptured blood blister-like aneurysm arising from the anterior wall of the internal carotid artery who was treated with stent-assisted coil embolization. The initial postoperative course was uneventful and follow-up imaging

A rare case report of vascular Ehlers-Danlos syndrome as a pulsatile neck mass with an internal carotid artery aneurysm.

Vascular Ehlers-Danlos syndrome, also known as Ehlers-Danlos syndrome type IV, is a rare inherited connective tissue disorder caused by autosomal-dominant pathogenic variants in the COL3A1 gene. Defective type III collagen results in marked fragility of arteries and hollow organs, predisposing affected individuals to spontaneous aneurysms, dissections, and rupture. We report the case of a 35-year-old Bangladeshi woman who presented with a gradually enlarging right-sided pulsatile neck mass accompanied by intermittent headache. Physical examination revealed a soft pulsatile cervical swelling wi

Cerebrovascular injury following manual strangulation and subsequent cervical manipulation: challenges in attribution and imaging decisions.

Cervical artery dissection (CAD) causes approximately 2% of ischaemic strokes overall but up to 25% in adults younger than 50 years. Isolated head or neck pain without infarction occurs in 8%-12% of cases and may contribute to delayed diagnosis. CAD has been described following manual strangulation and chiropractic manipulation. A woman in her 30s who presented with persistent headache following repeated intimate partner violence-related strangulation was initially discharged after a negative head CT. After subsequent chiropractic cervical manipulation, CT angiography demonstrated distal intra

Takayasu Arteritis Complicated with Vertebral Artery Dissection Aneurysm Treated Endovascularly: Report of One Case.

Takayasu arteritis (TA) is a chronic granulomatous large-vessel vasculitis that primarily affects the aorta and its major branches, such as the subclavian, carotid, and renal arteries. Pathologically, it is characterized by transmural infiltration of lymphocytes and macrophages with granulomatous inflammation, leading to intimal fibrosis and thickening, destruction of medial elastic fibers, and subsequent luminal stenosis or occlusion. Aneurysmal dilation may occur in some cases. We here report a case of TA complicated by a vertebral artery dissecting aneurysm. Our patient was a 47-year-old ma

Feasibility and predictors of failure in a distal transradial-first access strategy for endovascular treatment of intracranial aneurysms: A single-center study of 208 cases.

PURPOSE: To assess the feasibility, procedural success, predictors of failure, and complications associated with the distal transradial approach (dTRA) as first-line endovascular treatment for intracranial aneurysms. METHODS: We retrospectively reviewed 208 patients (211 aneurysms) treated between April 2024 and April 2025 in whom dTRA was the default access. Success rates, factors associated with failure, and access-related complications were analyzed. RESULTS: Transradial approach (TRA) puncture success was achieved in 97.1% (202/208) of cases, with an overall success rate of 92.8% (193/20

Early single-centre experience with physician-modified and Najuta fenestrated endografts for aortic arch disease: technical considerations and bailout strategies.

BACKGROUND: Thoracic endovascular aortic repair (TEVAR) for aortic arch disease involving the supra-aortic branches remains technically challenging because of the need to preserve branch perfusion while achieving an adequate proximal sealing zone. The Najuta stent graft is a semi-custom-made fenestrated device available in Japan, whereas physician-modified endograft TEVAR (PMEG-TEVAR) allows intraoperative device modification. This study aimed to evaluate our early experience with these two fenestrated TEVAR strategies and to describe their technical characteristics and pitfalls. METHODS: We

[Technologies for maintaining blood flow through the left subclavian artery during thoracic aortic replacement in the Z-2 zone].

BACKGROUND: In modern clinical practice thoracic endovascular aortic repair is the dominant treatment method for patients with diseases of the descending thoracic aorta. In patients with a shorter proximal neck, there is a need to implant the stent graft in Ishimaru zone 2, which involves covering the left subclavian artery. In modern practice, both surgical and endovascular techniques for LSA preservation are available. Objective the aim of the study was to evaluate the efficacy and safety of endovascular aortic repair with coverage of the left subclavian artery using various preservation str

Association between spinal manipulative therapy and cervical artery dissection in patients with neck pain or headache: a retrospective cohort study.

OBJECTIVES: Cervical artery dissection (CeAD) has been proposed as a sequela of spinal manipulative therapy (SMT), however, protopathic bias arising from care-seeking for CeAD symptoms (e.g., neck pain/headache) may overestimate observed associations. Among adults experiencing new neck pain/headache, we tested the null hypothesis of no significant CeAD risk within 30 days following SMT compared to matched ibuprofen-prescribed controls. METHODS: We searched United States TriNetX Linked health records/claims data (2010-2025) for adults seeking care for new episodes of neck pain/headache receivi
