Upper-Limb Ischemia Due to Distal Embolization from a Dissecting Axillary Artery Pseudoaneurysm in an Elite Overhead Athlete: A Case Report.

In overhead-throwing athletes, repetitive shoulder motion may cause intimal injury and pseudoaneurysm formation, most commonly involving the posterior circumflex humeral artery. An elite cricket fast bowler in his 20s presented with 6 months of left shoulder and hand discomfort, followed by rest pain and numbness of the fourth and fifth fingers. Duplex ultrasonography showed thrombosis of an axillary artery pseudoaneurysm with distal radial and ulnar artery embolization, and magnetic resonance imaging demonstrated a 1.5-cm pseudoaneurysm at the origin of the subscapular artery. Surgical explor

Dedicated Single-Branch Platforms for Totally Endovascular Zone 2 TEVAR with LSA Revascularization: A Comparison of Castor/Cratos and Gore TAG Thoracic Branch Endoprosthesis.

Zone 2 thoracic endovascular aortic repair (TEVAR) frequently requires left subclavian artery (LSA) preservation to maintain vertebrobasilar and upper-extremity perfusion while obtaining a durable proximal seal. Dedicated single-branch endografts were developed to standardize this step and to facilitate a reproducible fully endovascular strategy. Two main device concepts currently shape this field: integrated unibody branch platforms, represented by Castor and the second-generation Cratos, and modular retrograde-branch systems, represented by the Gore TAG Thoracic Branch Endoprosthesis (TBE).

[Stanford Type A Acute Aortic Dissection With Brachiocephalic Artery Malperfusion and Consciousness Disturbance at Onset:Report of a Case].

A 77-year-old woman complained of sudden chest pain and consciousness disturbance (Japan Coma Scale 3). Consciousness disturbance was improved( Japan Coma Scale 1) without paralysis on arrival at our hospital. Right upper-extremity and carotid arterial pulsation was impalpable. Contrast-enhanced computed tomography (CT) revealed dissection of the ascending aorta, and Stanford type A acute aortic dissection was diagnosed. The aortic dissection was extended into the brachiocephalic artery, which was occluded by the expanded false lumen. Head magnetic resonance imaging (MRI) identified no acute c

Impact of arm position on low dose dual-source CT one-step aortic and cerebral-carotid artery angiography image quality and radiation dose.

BACKGROUND: Although one-stop cerebral carotid aortic CTA is useful for diagnosing and treating type A aortic dissection, the radiation dose is increased due to the wider scanning range. The impact of varying arm positions on radiation dose and image quality is unknown when optimizing the scan protocol. This study aims to determine the best scanning protocol to minimize radiation dose while maintaining image quality, as well as how arm position impacts radiation dose and image quality of low dose one-stop cerebral carotid aortic CTA. METHODS: Between January 2022 and August 2023,a total of 18

Percutaneous radial access is safe and effective for subclavian branch treatment during zone 2 endovascular aortic repair with thoracic branch endoprosthesis.

The thoracic branch endoprosthesis (TBE, W. L. Gore & Associates) provides an off-the-shelf endovascular solution for zone 2 aortic pathology that maintains perfusion to the arm. This study evaluates safety and efficacy of percutaneous radial access for subclavian branch treatment during TBE repair. A single-institution retrospective review of consecutive patients who underwent zone 2 aortic repair with TBE between 2022 and 2024 was performed. Treatment indications included acute complicated aortic dissection, aortic dissection with aneurysmal degeneration, penetrating aortic ulcer, blunt thor

Performance of the open-first repair strategy in type A aortic dissection with mesenteric malperfusion syndrome eligible for delayed repair.

OBJECTIVE: For patients with type A aortic dissection complicated by mesenteric malperfusion syndrome, some centers advocate a nontraditional approach based on up-front endovascular intervention and delayed open repair. However, the efficacy of this strategy cannot be understood without first understanding outcomes of the traditional open-first strategy in the same select patient population eligible for delayed repair, applying modern techniques of hybrid aortic surgery. METHODS: Patients with acute type A aortic dissection and mesenteric malperfusion syndrome were queried from a single insti

Right-sided Aortic Arch with Aberrant Left Subclavian Artery arising from Kommerell's Diverticulum: A case report.

Right-sided aortic arch, first documented by Fioratti and Aglietti in 1763, is a rare variant of the thoracic vascular anatomy present in about 0.1% of the adult population. In half of these cases the left subclavian artery is also aberrant. The aberrant left subclavian artery usually originates from a conical dilatation near its origin from the aorta also known as "Kommerell's diverticulum." Fewer than 80 of these cases have been reported in the literature as far as our web search is concerned. It is usually asymptomatic and diagnosed incidentally during adulthood. We are presenting a 56 year

Feasibility and Safety of Endovascular Interventions via Percutaneous Access to the Axillary Artery.

BACKGROUND: Endovascular treatment of peripheral arterial disease requires safe and reliable arterial access. This study evaluates the feasibility and safety of percutaneous axillary artery access for endovascular therapy. A variety of anatomic and logistic obstacles can be overcome with upper extremity access. In this retrospective case review, we describe our experience with percutaneous trans-axillary access for lower extremity peripheral arterial disease intervention. METHODS: Medical records of all patients undergoing axillary artery percutaneous access from December 2021 to August 2024

Mid-Term Results of Treating Kommerell's Diverticulum and Aberrant Subclavian Artery Anomalies Using a Patient-Centered Team Approach.

INTRODUCTION: The management of Kommerell's Diverticulum (KD) has evolved from open surgical resection and graft replacement of the aorta, to endovascular repair in asymptomatic patients due to its recognized possible sequelae - aortic rupture and dissection. Despite these technical advances, standard indications for intervention and treatment algorithms remain unclear. We will present our single-center experience in the treatment of KD, supporting a multidisciplinary endovascular-first approach. METHODS: All patients who underwent thoracic endovascular aortic repair (TEVAR) for KD between 20

Percutaneous Axillary Artery Puncture: An Efficient Approach for Upper Extremity Access.

BACKGROUND: The aim was to analyze the anatomic feasibility of the percutaneous axillary access (PAXA) using cadaverous models and then to analyze the complications associated with PAXA during Fenestrated or Branched Endovascular Aneurysm Repair (F/BEVAR) procedures. METHODS: Cadaverous models were used to analyze axillary pedicle after a PAXA on an initial anatomical investigation. A subclavian approach was performed after puncture to assess the injuries caused by the needle. Then, in an observational study, patients who underwent F/BEVAR using a PAXA between July 2019 and July 2021 were inc

Internal carotid artery dissection with different interventions and outcomes: two case reports.

Spontaneous internal carotid artery dissection (ICAD) is a rare disease and an important cause of stroke in young patients. The presentation of ICAD is variable and atypical. We have used three-dimensional T1-weighted volume isotropic turbo spin-echo acquisition to help diagnose ICAD. ICAD should be considered in young patients presenting with relevant symptoms in an emergency setting. We herein report ICAD with an unexplained onset in two patients. Neither had a history of large or small cervical trauma, but both had a history of hypertension. The first patient was a 33-year-old man who prese

Fingertip Replantation: Surgical Technique, Tips, and Tricks.

Replantation of fingertip amputations restores the original tissue and is the ideal treatment to provide the best aesthetic and functional outcome. However, successful fingertip replantation is considered challenging because it requires supermicrosurgery techniques. This article provides a detailed surgical technique for fingertip replantation and the authors' preferences and recommendations. In the authors' experience, the most important factors for successful fingertip replantation are meticulous vascular dissection, reliable arterial repair, and venous anastomosis to avoid postoperative ven

A child who presented with cerebral infarction: Clipping combined with bypass surgery of a thrombosed giant aneurysm.

BACKGROUND: Cerebral aneurysms are not common among children and most of them are presented with subarachnoid hemorrhage or mass effect. Here, we describe a rare case of a pediatric giant aneurysm presented with cerebral infarction. CASE DESCRIPTION: A 38-month-old boy visited the emergency room due to left hemiparesis and left central type facial palsy. Initial magnetic resonance imaging showed acute cerebral infarction on the right basal ganglia and coronal radiata. Furthermore, a thrombosed aneurysm with a diameter of 30.57 mm at the frontal branch of the right middle cerebral artery was o

Spinal subarachnoid hemorrhage as a consequence of dissection with pseudoaneurysm in a cervical radiculomedullary branch of the anterior spinal artery.

Spinal subarachnoid hemorrhage is a rare condition, and it generally arises as a consequence of arteriovenous malformation, although more rarely can be caused by aneurysm, dissection, or pseudoaneurysm. In the following, we present a case of a 58-year-old male who while undergoing treatment for nephrolithiasis, developed persistent hypertension, refractory to his home medications, along with headache, neck pain, and unilateral ptosis and upper extremity ataxia. Initial CT scan demonstrated acute subarachnoid hemorrhage in the posterior fossa extending to the C7 level, Angiography ultimately re
