Management of intraoperative malperfusion syndrome using femoral artery cannulation for repair of acute type A aortic dissection.

BACKGROUND: The site of cannulation for repair of ascending aortic dissection remains controversial. The objective of this study was to evaluate the incidence and management of intraoperative malperfusion syndrome using femoral cannulation for repair of acute type A aortic dissection. METHODS: Between May 2002 and February 2007, 107 patients with initial femoral artery cannulation for the repair of type A aortic dissection were enrolled in this study. The mean age was 63.7 +/- 11.8 years; 51 patients were female. Preoperative findings indicated malperfusion in 16 patients. Intraoperative malp

Left subclavian artery coverage during thoracic endovascular aortic repair: a single-center experience.

OBJECTIVE: This study was conducted to determine the results of left subclavian artery (LSA) coverage during thoracic endovascular aortic repair (TEVAR). METHODS: We retrospectively reviewed the results of 308 patients who underwent TEVAR from 1999 to 2007. The LSA was completely covered in 70 patients (53 men, 13 women), with a mean age of 67 years (range 41-89). Elective revascularization of the LSA was performed in 42 cases, consisting of transposition (n = 5), bypass and ligation (n = 3), or bypass and coil embolization (n = 34). Mean follow-up was 11 months (range, 1-48 months). The chi(

Spontaneous subclavian artery dissection causing ischemia of the arm: diagnosis and endovascular management.

Spontaneous subclavian artery dissections are rare, with very few cases described in the literature. We report an unusual case of a 62-year-old female who presented with ischemia of the left arm secondary to spontaneous dissection of the first part of the left subclavian artery. We describe the imaging findings on both aortic arch angiogram and CT angiogram and discuss management by endovascular means.

Brachial artery aneurysm rupture in a patient with neurofibromatosis: a case report.

Peripheral vascular manifestations of neurofibromatosis are rare but may result in fatal haemorrhaging when they rupture. Surgeons should be aware of this life-threatening condition. We report a case of 35-year-old woman with neurofibromatosis who presented with a swollen and tender mass around her right arm. Angiography revealed 2 aneurysms in the brachial artery. Surgical occlusion revealed a large amount of clotted blood within the subfascial space, and the bleeding point was identified as a pinpoint opening in the aneurysm. The brachial artery abutting the aneurysm and the surrounding soft

[Organic or non Organic?].

Every symptom and complaint can be of psychogenic origin. In order to tell apart an organic disease from a psychogenic disturbance one has to look for objective findings, which correlate with the symptoms. Cases are presented, which illustrate psychogenic disorders, but also cases, which seem to be psychogenic but in fact are of organic origin.

Painless aortic dissection with initial symptom of right upper extremity weakness: a case report.

Thoracic aortic dissection is a dangerous disease. It usually presents as severe chest or back pain. Symptoms resulting from aortic branch involvement may also be involved. Sometimes, it presents with atypical symptoms. Here, we report a patient who came to the emergency department (ED) because of acute onset of right upper limb weakness and numbness. Brain computed tomography (CT) was performed initially because cerebral vascular disease was suspected. Subsequently, angiography was performed as artery occlusion of the limb was found. The patient suddenly collapsed in the ED. Stanford type A a

Unusual presentation of ulnar artery aneurysm and dissection with associated anomalous radial artery: appearances on multi-detector row CT angiography.

We present a case of ulnar artery aneurysm and dissection associated with a congenitally hypoplastic ipsilateral radial artery. We postulate that the aetiology is due to increased blood flow through the ulnar artery as a consequence of the radial artery anomaly, therefore making the ulnar artery more vulnerable to aneurysm formation and traumatic damage. This might represent a variant of the hypothenar hammer syndrome with associated ulnar artery dissection and recurrent distal embolisation manifesting as Raynaud's phenomenon. Forearm arterial injuries, treatment and the importance of upper li

Innovative technique to treat acute cerebral and peripheral malperfusion during type A aortic dissection repair.

Cerebral and/or visceral malperfusion during CPB is a potentially dramatic situation which can be easily misunderstood if a complete monitoring of arterial pressure and cerebral saturation is not available. Here we present a case in which we could promptly diagnose cerebral and visceral malperfusion just after starting cardiopulmonary bypass. Use of an original and unusual method for distal perfusion allowed us to treat malperfusion, uneventful cooling of the patient down to deep hypothermia and to complete the procedure in circulatory arrest as planned.

Ask the doctor. I was recently admitted to the hospital in the middle of the night with chest pain. When the doctors took my blood pressure, they found that it was much lower in my right arm than in my left. They rushed me in for a CT scan, saying they were looking for a "tear." Fortunately, there wasn't one, and the next day I underwent angioplasty. What was this tear they were worried about?

[Emergency small-sized stent placement following aortic true lumen obliteration of Stanford type B acute aortic dissection].

Acute aortic obstruction induced by acute aortic dissection frequently causes life-threatening organ ischemia. Although early reperfusion of lower extremities, renal and mesenteric artery is necessary, surgical treatment such as graft replacement is invasive and may result in myonephropathic-metabolic syndrome (MNMS), which leads to loss of limb and life. We herein report a case of stent placement as a less invasive treatment for true lumen obliteration associated with Stanford type B aortic dissection in a patient with chronic renal failure on hemodialysis. Ten mm metallic stents were placed

Resection of the descending thoracic aorta: outcomes after use of hypothermic circulatory arrest.

BACKGROUND: Use of hypothermic circulatory arrest (HCA) for operations on the descending thoracic aorta is controversial. While deep hypothermia may provide better end-organ and spinal cord function, prolonged cardiopulmonary bypass and circulatory arrest may increase morbidity. This study assessed outcomes after use of HCA for descending thoracic aortic resection in a large cohort of consecutive patients. METHODS: Hypothermic circulatory arrest was utilized if arch or extensive descending thoracic aortic resection was required, or if aortic pathology precluded cross-clamping. One hundred thi

[A case of common carotid artery dissection showing remission in short-term].

We report a case of left common carotid artery dissection showing complete remission in the short term. A 48-year-old man presented a 1-week history of weakness in the right arm. Neurological findings revealed mild motor weakness of the right upper extremity. Magnetic resonance imaging (MRI) showed sporadic cerebral infarction and hemorrhagic infarction in the area of the left middle cerebral artery (MCA) and the posterior border zone. Computed tomographic angiography (CTA) revealed irregular stenosis with a smooth surface in the left common carotid artery and ultrasonography indicated an iso-

Experience with arteriovenous fistulas for chronic hemodialysis in children: technical details and refinements.

PURPOSE: The aim of this paper is to report our experience in the creation of arteriovenous fistulas in children by using microsurgical vascular techniques, with emphasis on the details of the surgical technique. METHODS: The children underwent surgery from July 1997 to March 2004. Operating loupes (magnification: 3.5X) were used by the entire surgical team. After dissection and adequate mobilization, the vein was anastomosed to the artery in an end-to-lateral fashion by using 4 separate 7/0 or 8/0 prolene running sutures. RESULTS: Twenty nine children underwent 33 fistula creations - 21 rad

Surgical outcome of elephant-trunk anastomosis between the left carotid and subclavian arteries in aortic arch repair including the postoperative physical status of the left arm.

We studied the outcome of our modified aortic arch replacement procedure in which the distal end of the graft is anastomosed between the left common carotid artery and the left subclavian artery including the postoperative physical status of the left arm in comparison with the conventional technique. We assessed the surgical outcome of 26 patients who underwent our new technique and 11 patients who underwent the conventional one. Postoperative clinical symptoms and physical status of the left arm were also evaluated. No operative deaths were observed in the new technique group and one operativ

[Stanford type A aortic dissection with occlusion of the brachiocephalic artery; report of a case].

A 73-year-old female was referred to our hospital because of pain in the right upper extremity, left hemiparesis and syncope. Computed tomography (CT) of the head showed no active lesion, but chest CT showed Stanford type A aortic dissection with occlusion of the brachiocephalic artery. Carotid ultrasonography showed occlusion of the right common carotid artery. Emergent graft replacement of the ascending aorta and aortic arch was performed. The right common carotid artery was opened but no thrombus was found. In order to restore and maintain the cerebral circulation, the right carotid artery
