Primary stenting of the extracranial internal carotid artery in a patient with multiple cervical dissections: technical case report.

OBJECTIVE AND IMPORTANCE: Spontaneous dissection of the extracranial internal carotid artery (ICA) and vertebral artery (VA) is a well-documented cause of stroke in young, previously healthy patients. The majority of patients with spontaneous dissection are successfully treated with antiplatelet or anticoagulation therapy, but a significant proportion of patients progress to suffer devastating morbidity and mortality. Surgical intervention has primarily consisted of proximal ligation, extracranial-intracranial bypass, or endarterectomy. Generally, these procedures are technically demanding and

[Cervical deficit radiculopathy in 3 cases of vertebral artery dissection].

Three cases of extracranial vertebral artery dissections with upper limb peripheral motor deficit (C5-C6) are reported. Six similar cases were also found in the literature. Central neurological symptoms occurred in five of these nine cases, suggesting the diagnosis of dissection. The peripheral motor or sensorial deficit was strictly isolated in the four other cases, simulating radicular neuralgia due to discopathy or foraminal compression. In case of dissections, a precise analysis of pain is helpful to guide diagnosis; sharp, unbearable, continuous and extended neck pain without nocturnal pa

Delayed presentation of 50 years after a World War II vascular injury with intraoperative localization by duplex ultrasound of a traumatic false aneurysm.

A case of delayed presentation of a traumatic false aneurysm in the left arm 50 years after penetrating injury sustained during World War II is described. The original injury resulted in brachial artery occlusion and complete median nerve palsy. The false aneurysm presented with a spontaneous, contained rupture. Surgical repair was performed after duplex ultrasound localization of the lesion to a small collateral artery lateral to the elbow, thereby avoiding dissection in the densely scarred tissue plains in the antecubital fossa. Duplex ultrasound was also used intraoperatively to facilitate

Upper and lower extremity somatosensory evoked potential recording during surgery for aneurysms of the descending thoracic aorta.

Since tibial nerve somatosensory evoked potentials (SEPs) recording is influenced by hemodynamic changes and anesthetics, alterations cannot always be attributed to spinal cord ischemia, so causing false positive results. Additional recording of median nerve SEPs facilitates interpretation. From January 1988 to July 1993, 60 consecutive patients (44 men, 16 women, mean age 66 years, ranging from 26 to 83 years) underwent surgery for an aneurysm of the descending thoracic aorta using a non-heparinized left heart bypass (Biomedicus pump). In 40 patients recording of the tibial and median nerve S

Percutaneous transluminal stent placement to treat subclavian steal syndrome.

PURPOSE: To describe nonsurgical stent placement to treat subclavian steal syndrome after failure of traditional balloon angioplasty. PATIENTS AND METHODS: Seven patients with subclavian steal syndrome underwent diagnostic angiography, which showed four stenoses and three occlusions of the proximal left subclavian artery. Most patients presented with vertigo, left arm claudication, or syncope. Initial balloon angioplasty failed; there was elastic recoil with more than 30% residual stenosis or subintimal dissection. The three occlusions were treated with urokinase pulse-spray thrombolysis. All

Subclavian artery dissection during diagnostic cardiac catheterization: the role of conservative management.

Dissection of the subclavian artery during routine cardiac catheterization while obtaining cannulation to the left internal mammary artery is an unusual complication and to our knowledge has never been reported. Conservative management of this vascular injury can avoid the sequelae of high-risk surgical repairs made difficult by a complex operative exposure. We describe a case in which dissection of the left subclavian artery was treated conservatively with an excellent outcome.

"The antero-axillary thoracotomy for operations of the distal aortic arch and the proximal descending aorta".

Antero-axillary thoracotomy in a 45 degrees position has become the most frequent approach for lung resection in our country. This approach also offers an ideal view of the aortic arch with the supraaortic vessels being closer to the incision site than in sternotomy or standard thoracotomy. We have therefore used this approach in our last 14 patients with lesions of the distal aortic arch and proximal descending aorta. Operative diagnoses included three arteriosclerotic aortic arch aneurysms, one post-traumatic aneurysm and two acute traumatic transections, as well as four acute type B dissect

Aberrant subclavian artery: surgical treatment in thirty-three adult patients.

PURPOSE: Because of the scarcity of large series in the literature, our experience with surgery for aberrant subclavian arteries (aSA) in adults was reviewed. METHODS: During the last 16 years we have surgically treated 33 adult patients with aSA. Twenty-eight patients had a left-sided aortic arch with a right aSA whereas five had a right-sided aortic arch with a left aSA. Eleven patients (group 1) had dysphagia caused by esophageal compression by a nonaneurysmal aSA; five patients (group 2) had ischemic symptoms caused by occlusive disease of a nonaneurysmal aSA; 10 patients (group 3) had an

Internal carotid artery dissection after remote surgery. Iatrogenic complications of anesthesia.

BACKGROUND: Subintimal dissection with acute occlusion of the internal carotid artery resulting in acute cerebral infarction has not been reported as an iatrogenic complication of general anesthesia. CASE DESCRIPTIONS: An anesthetist stretched the neck of a 44-year-old man by anchoring an anesthesia mask posterior to the angle of the jaw with overlying fingers as the patient struggled during an insufficient inhalational mask anesthetic. A 33-year-old man received an endotracheal anesthetic without struggle, but as he was turned from a supine to a prone position, his head and neck were not imm

Brachiocephalic aneurysm: the case for early recognition and repair.

Over a 40 year period (1950-1990) only 73 patients were treated surgically for brachiocephalic aneurysms. An operation was performed for 38 subclavian, 25 extracranial carotid, six innominate, three aberrant right subclavian, and one vertebral artery aneurysm. Twenty-three other associated aneurysms occurred in 14 patients. Five patients had an additional untreated brachiocephalic aneurysm, and nine patients had 18 aneurysms located in different anatomic regions. There were 40 men and 33 women with a mean age of 50.5 years (range 16 to 82 years). Forty patients (54.8%) presented with potential

[A mechanism of cheiro-oral syndrome due to brainstem lesions, a case of a dissecting aneurysm of the basilar artery].

A 42-year-old woman developed an abrupt onset of severe headache, nausea, vomiting, unstable gait and numbness around the right side of her mouth and in her right hand. Neurological examination revealed bilateral pyramidal tract signs and hypesthesia of her right palmar tip and the right side of her mouth. However, pain and temperature sensibility was preserved. Cerebrospinal fluid was clear and colorless. CT scan showed an enhancing mass in the prepontine cistern compressing the pontine base. Vertebral angiography revealed irregular narrowing of bilateral vertebral arteries (string sign) prox

Proximal clipping and bypass between bilateral vertebral arteries using a radial arterial graft for the treatment of a dissecting aneurysm of the vertebral artery.

Proximal clipping has been performed recently as the main surgical treatment for a dissecting aneurysm of the vertebral artery. When there is a contralateral vertebral artery of a smaller size with arteriosclerotic changes, some form of bypass to prevent cerebellar and brain stem infarction is needed in addition to proximal clipping. We treated a 50-year-old man with a ruptured dissecting aneurysm of the left vertebral artery and stenosis at the V3 segment of the right vertebral artery. The caliber of the right vertebral artery was smaller than the left. After an anastomosis between bilateral

Bilateral brachial artery emboli presenting as aortic dissection.

Loss of peripheral pulses in a patient with chest pain suggests the diagnosis of aortic dissection. An 80-year-old woman presented with an episode of chest pain and acute bilateral loss of upper extremity pulses that was initially treated as aortic dissection. Findings of physical examination and echocardiography were consistent with mitral stenosis. Angiography revealed bilateral brachial artery emboli, which were treated by embolectomy. To our knowledge, this case represents the first report of simultaneous brachial artery emboli in association with mitral stenosis.

Surgical management of acute aortic dissection complicated by stroke.

Although patients with acute type A aortic dissection are best managed by emergency surgical intervention, preoperative stroke is known to be an independent predictor of late mortality and is considered by some to be a contraindication to operation because of the risk of precipitating hemorrhagic cerebral infarction and poor long-term outcome. In a series of 272 consecutive, unselected patients with aortic dissection undergoing surgical treatment during a 25-year span (1963-1987), 128 (47 +/- 3% [+/- 70% confidence level (CL)]) had an acute type A dissection. A total of seven patients with acu

Vascular complications associated with spontaneous aortic dissection.

Three hundred twenty-five cases of spontaneous aortic dissection seen at two institutions between 1965 and 1986 were reviewed to assess the incidence, morbid sequelae, and specific management of aortic branch compromise. Noncardiac vascular complications occurred in 33% of the study group, and in these patients the overall mortality rate (51%) was significantly (p less than 0.001) higher than in patients without (29%) such complications. Although aortic rupture was the strongest correlate of mortality (90%), death specifically related to vascular occlusion was common when such occlusion occurr
