Thoracic endovascular aortic repair with the provisional extension to induce complete attachment (PETTICOAT) technique can cause persistent false-lumen perfusion through distal re-entry tears, leading to late aneurysmal degeneration and challenging reinterventions. We report the use of renal artery covered stent placement to treat persistent false-lumen perfusion after PETTICOAT caused by a re-entry tear in a 57-year-old man with chronic type B aortic dissection. Preoperative assessment of the relationship between the re-entry tear and bare-stent struts enabled device delivery and false-lumen

Fibromuscular dysplasia (FMD) is a vascular disease characterized by abnormal cellular growth in medium-sized arteries progressing to stenosis, aneurysm, or arterial dissection. Renal and carotid artery involvement is common. FMD is not characterized by inflammation or atherosclerosis and predominantly impacts young women. Refractory hypertension is a common primary clinical manifestation of renal artery FMD. Severe or untreated FMD can lead to stroke, aneurysm rupture, or death. Despite the need for early diagnosis and aggressive management, FMD is frequently overlooked because of nonspecific

BACKGROUND: Autologous blood transfusion (ABT), as a blood conservation strategy, is widely used in surgeries for acute Stanford-A aortic dissection(ATAAD). However, investigations into the correlation between ABT and postoperative 24-hour serum creatinine(Scr) are limited. This study evaluates the relationship between ABT and Scr in patients who underwent ATAAD surgery. METHODS: This retrospective study included 157 ATAAD patients from our center between February 2020 and November 2022. Patients were categorized into a death group(n = 12) and a survival group (n = 145) based on individual pro

BACKGROUND: Peripheral arterial involvement is increasingly recognized in patients with Marfan syndrome and related heritable thoracic aortic diseases (HTAD), yet its prevalence and clinical relevance remain poorly defined. We aimed to assess the prevalence, anatomical distribution, and association with aortic events of primary peripheral arterial lesions (PPAL) in a genetically characterized HTAD cohort.
METHODS: We performed a cross-sectional, single-center study including 61 patients with genetically confirmed HTAD who underwent comprehensive pan-arterial vascular ultrasound. PPAL were cla

BACKGROUND: Renal artery aneurysm (RAA) is a rare but potentially life-threatening vascular pathology. Surgical management depends on the aneurysm’s location and morphology. In cases with extensive aortic calcification or anatomical complexity, standard clamping techniques may be infeasible, requiring alternative hemostatic strategies. We present a unique case of RAA in a patient with Takayasu’s arteritis and diffuse aortic calcification, in whom intra-aortic balloon occlusion and patch aortoplasty were used as a salvage technique during surgical repair. CASE PRESENTATION: An 80-year-old woman

IntroductionFollowing thoracic endovascular aortic repair (TEVAR) for aortic dissection, visceral arteries arising from the false lumen may require revascularization when communication between the true and false lumens is naturally occluded or iatrogenically sealed. This study describes an endovascular technique using a Wingman catheter for intimal flap fenestration to establish access for revascularization of abdominal visceral arteries originating from the false lumen.Materials and MethodsA coaxial system was developed by inserting an LRK-shaped catheter within a RE-SS-shaped guiding cathete

Aneurysms are permanent, focal dilations of vessels that are ≥150% of normal diameter. While single aneurysms are relatively common, the presence of multiple aneurysms in different vascular territories is uncommon, and consolidated discussions of such cases remain limited. We report the case of a 96-year-old male cadaver with extensive but clinically silent aortic aneurysms identified during routine anatomical dissection. A large abdominal tumor was also noted compressing the right renal artery. Additionally, we review published reports of multi-territory aneurysms, which highlight the heterog

BACKGROUND: The impact of a narrow true lumen (NTL) on the outcomes of fenestrated-branched endovascular repair in patients with postdissection thoracoabdominal aortic aneurysms (PD-TAAAs) is underreported.
METHODS: Data from an international, multicenter registry were analyzed, to identify patients treated for PD-TAAAs (2015-2025) at 23 centers. All patients underwent fenestrated-branched endovascular repair using custom or off-the-shelf endografts. NTL was defined by a true lumen diameter <25 mm identified at any aortic level on preoperative computed tomography angiogram. Short-term endpoin

BACKGROUND: Spontaneous coronary artery dissection (SCAD) is frequently associated with systemic arteriopathies, particularly fibromuscular dysplasia. Although extracoronary arterial screening is recommended, abnormalities of the internal mammary artery (IMA), the usual preferred conduit for coronary artery bypass grafting, is rarely described.
CASE SUMMARY: We report 2 women with systemic arteriopathy and recurrent SCAD who had structural abnormalities of the IMA. The first had Loeys-Dietz syndrome and prior cervical artery dissections, presented with neck pain, and was found to have vertebr

Loeys-Dietz syndrome (LDS) is an autosomal dominant connective tissue disorder characterized by aggressive aortic pathology, primarily caused by pathogenic variants in genes such as TGFBR1. We report a 44-year-old female with a known LDS diagnosis who presented with a symptomatic, rapidly expanding abdominal aortic aneurysm (44.8 mm) with concurrent iliac and renal artery involvement. Given the high rupture risk, she underwent open abdominal aortic reconstruction. However, she subsequently developed a Type B aortic dissection nearly 2 months after the initial repair, necessitating a second ope

The field of endovascular surgery continues to evolve rapidly with the introduction of novel techniques and devices. Among the most significant advances in recent decades is the development of branched and fenestrated endografts, which have become the cornerstone of treatment for complex aortic pathologies, including thoracoabdominal aortic aneurysms. These devices enable the preservation of visceral vessel perfusion while allowing for effective aneurysm exclusion. However, their use introduces technical challenges that demand a high level of expertise, particularly during the catheterization

Takayasu arteritis (TA) is a chronic granulomatous large-vessel vasculitis that primarily affects the aorta and its major branches, such as the subclavian, carotid, and renal arteries. Pathologically, it is characterized by transmural infiltration of lymphocytes and macrophages with granulomatous inflammation, leading to intimal fibrosis and thickening, destruction of medial elastic fibers, and subsequent luminal stenosis or occlusion. Aneurysmal dilation may occur in some cases. We here report a case of TA complicated by a vertebral artery dissecting aneurysm. Our patient was a 47-year-old ma
